Associating liver partition and portal vein ligation for staged hepatectomy (ALPPS)—pushing the envelope in modulation of future liver remnant before major hepatectomy

Insufficient future liver remnant (FLR) is an important factor that precludes patient from upfront major liver resection as it predisposes to post-hepatectomy liver failure and mortality. As such, augmentation of FLR by portal vein embolization (PVE) was the conventional approach to improve the safety profile of major hepatectomy. In recent years, associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) has been popularized as an alternative approach for FLR augmentation (1,2). However, the initial inception of the ALPPS procedure was met by criticism for its high incidence of procedure-related morbidity, such as bile leakage and sepsis (3). In light of this issue, an international registry was set up for collection of cases and experience around the world and soon after that, the first International Expert Meeting was held in Hamburg in 2015 that led to the publication of eight recommendations on terminology of the procedures, indications and techniques for ALPPS (4). Two years later, a pre-congress meeting was held during the EHPBA (European Hepatopancreatobiliary Association) 12th Annual Congress in Mainz, Germany to celebrate the 10th Anniversary of ALPPS (5) since the first case was performed by Prof. Hans Schlitt a decade ago (1). During this meeting, ten different aspects on ALPPS were covered including surgical anatomy, underlying mechanisms, perioperative assessment of liver function, technical approach, indications and its effectiveness in comparison to two-stage hepatectomy.

AIM： To combine ultrasound-guided fine-needle aspiration （US-FNA） and Liu （Riu） stain to make a quick study on liver tumor lesions. METHODS： Two hundred and twenty-eight aspirations from 232 patients were completely studied. The operator himself made the quick cytodiagnosis of US-FNA smear stained by Liu method within thirty minutes. The US-FNA specimen was also sent to the pathological department for cytological study and cellblock histology. The result of our Uu-stain quick cytodiagnosis in each patient was confirmed by the final cytopathological diagnosis from pathological report. RESULTS： Among 228 samples, the quick cytodiagnosis revealed 146 malignancies, 81 benign lesions and one inadequate specimen. Cytopathological diagnosis from the pathological department revealed 150 malignancies, and 78 benign lesions. Four well-differentiated hepatocellular carcinomas （HCCs） were under-diagnosed by quick cytodiagnosis as benign and 3 benign lesions were over-diagnosed as well-differentiated HCCs. Compared with cytopathological diagnosis, quick cytodiagnosis correctly diagnosed 143 malignancies and 77 benign lesions. Except for the one inadequate specimen in quick cytodiagnosis, the accuracy of quick cytodiagnosis was 96.9% （220/227）, and its sensitivity, specificity and positive and negative predictive values were 97.9%, 95.1%, 97.3% and 96.3%, respectively. CONCLUSION： Liu-stain quick cytodiagnosis is a fast, convenient, safe and effective method for hepatologists in clinic practice to diagnose liver tumor. In few cases of well-differentiated HCC, Liu-stain quick cytodiagnosis has its limitation.

To the Editor： Hepatocellular carcinoma （HCC） remains one of the commonest cancers worldwide especially in hepatitis B endemic regions. Its aggressive behavior is characterised by the natural history of increasing size, a tendency for vascular invasion into the hepatic veins and portal veins. Further growth into the inferior vena cava （IVC） and right atrium （RA） is an infrequent finding but signifies a pre-terminal event with a dismal prognosis.

Seamless handover between unicast and multicast multimedia streams

With the deployment of heterogeneous networks, mobile users are expecting ubiquitous connectivity when using applications. For bandwidth-intensive applications such as Internet Protocol Television(IPTV), multimedia contents are typically transmitted using a multicast delivery method due to its bandwidth efficiency. However, not all networks support multicasting. Multicasting alone could lead to service disruption when the users move from a multicast-capable network to a non-multicast network. In this paper, we propose a handover scheme called application layer seamless switching(ALSS) to provide smooth real-time multimedia delivery across unicast and multicast networks. ALSS adopts a soft handover to achieve seamless playback during the handover period. A real-time streaming testbed is implemented to investigate the overall handover performance, especially the overlapping period where both network interfaces are receiving audio and video packets. Both the quality of service(QoS) and objective-mapped quality of experience(QoE) metrics are measured. Experimental results show that the overlapping period takes a minimum of 56 and 4 ms for multicast-to-unicast(M2U) and unicast-to-multicast(U2M) handover, respectively. The measured peak signal-to-noise ratio(PSNR) confirms that the frame-by-frame quality of the streamed video during the handover is at least 33 dB, which is categorized as good based on ITU-T recommendations. The estimated mean opinion score(MOS) in terms of video playback smoothness is also at a satisfactory level.

AIM： To evaluate the outcome of transarterial chemoembolization （TACE）in patients with unresectable hepatocellular cardnoma （HCC）〈5 cm in diameter eligible for radiofrequency ablation （RFA）. METHODS： The treatment-related mortality, morbidity, long-term survival, and prognostic factors of HCC patients who had TACE and fulfilled the present inclusion criteria for RFA were evaluated. RESULTS： Of the 748 patients treated with TACE between January 1990 and December 2002, 114 patients were also eligible for RFA. The treatment-related mortality and morbidity were 1% and 19%, respectively. Survival at 1, 3, and 5 years was 80%, 43%, and 23%, respectively. Older age and a high albumin level were associated with a better survival, whereas a high α-fetoprotein level （AFP） and the size of the largest tumor 〉3 cm in diameter were adverse prognostic factors in multivariate analysis. CONCLUSION： The morbidity, mortality, and survival data after TACE for small HCCs eligible for RFA are comparable to those reported after RFA in the literature. Our data suggest the need for a randomized comparison of the two treatment modalities for small HCCs.

BACKGROUND:Controversy remains over whether the middle hepatic vein should be included in the liver graft in right liver living donor liver *** in the anterior sector of a right liver graft can cause graft malfunction,which is especially devastating in the case of a graft with marginal size in relation to recipient body size on top of poor pre-transplant recipient *** case we report here highlighted the importance of the middle hepatic vein in right liver living donor liver ***:We illustrated the rectification of outflow obstruction of the middle hepatic vein in the anterior sector of right liver graft caused by technical error during *** rectification was performed with emergency re-routing using an artificial ***:Congestion in the anterior sector of the graft improved immediately and the patient’s postoperative liver function test results improved ***:The middle hepatic vein is important for effective drainage of the anterior sector of a right liver *** re-routing technique described in the report can also be applied to cases in which the middle hepatic vein is injured during hepatectomy requiring immediate reconstruction.

Origin and tectonic evolution of the Qilian Precambrian basement on NW China were investigated using zircon U-Pb ages with collaborating stratigraphic and paleontological evidence. Zircon grains were separated from two schists, two granitic gneisses and one mylonized gneiss and dated with SHRIMP. Seventy percent of sixty-one detrital zircon ages from two schists ranges from 0.88 Ga to 3.09 Ga, mostly within 1.0 Ga to 1.8 Ga with a peak at 1.6 Ga to 1.8 Ga, and twenty percent varies from 2.0 Ga to 2.5 Ga. A few falls in the Archean and Neoproterozoic periods. The two granitic gneisses were dated 930±8 Ma and 918±14 Ma, whereas the mylonized granitic gneiss was dated 790±12 Ma. These ages represent two periods of magmatisms, which can be correlated with the early and late stages of magmatisms associated with the Jinningian movement on the Yangtze Blocks. The results from this and previous studies indicate that the ages of the Precambrian detrital zircons from the Qilian Block are widely distributed in the Proterozoic era, distinct from the North China Block which was stable in the Neo-Mesoproterozoic era. By contrast, the age histograms of the detrital zircons from the Qilian Block is similar to those from Precambrian basement of the Yangtze Craton. Therefore, it is suggested that the Qilian Block had a strong affinity toward the Yangtze Craton and might belong to the supercontinent Gondwana in the Neoproterozoic time. This inference is supported by Nd model age (TDM), stratigraphic, and paleontological evidence. It is further considered that the Qilian Block was rifted from the supercontinent Gondwana during late Sinian to form an isolated continent in the Proto-Tethyan Ocean, moving towards the Alaxa Block in the North China Craton. The part of Proto-Tethyan Ocean between the Qilian and Alaxa Blocks should correspond to the so-called Paleo-Qilian Ocean.
