Efficacy and safety of liquid nitrogen cryotherapy for treatment of Barrett's esophagus

AIM to evaluate the efficacy and safety of liquid nitrogen cryotherapy as a primary or rescue treatment for BE,with and without dysplasia,or intramucosal adenocarcinoma (IMC).MEtHODS this was a retrospective,single-center study carried out in a tertiary care center including 45 patients with BE who was treatment-na?ve or who had persistent intestinal metaplasia(IM),dysplasia,or IMC despite prior ***'s mucosa was resected via EMR when clinically appropriate,then patients underwent cryotherapy until eradication or until deemed to have failed *** biopsies were taken at standard *** From 2010 through 2014,33 patients were studied regarding the efficacy of ***,29 patients (88%) responded to cryotherapy,with 84% having complete regression of all dysplasia and *** eradication of cancer and dysplasia was seen in 75% of subjects with IMC; the remaining two subjects did not respond to *** cryotherapy,15 patients with high-grade dysplasia (HGD) had 30% complete regression,50% IM,and 7% low-grade dysplasia (LGD); one subject had persistent *** eradication of dysplasia occurred in all 5 patients with *** 5 patients with IM,complete regression occurred in 4,and IM persisted in *** 136 cryotherapy sessions amongst 45 patients,adverse events included chest pain (1%),stricture (4%),and one gastrointestinal bleed in a patient on dual antiplatelet therapy who had previously undergone *** Cryotherapy is an efficacious and safe treatment modality for Barrett's esophagus with and without dysplasia or intramucosal adenocarcinoma.

Mechanical and Dielectric Properties of Inte Crystals

the mechanical properties of indium telluride (Inte) crystals grown by the Bridgman technique were investigated at room temperature using a Vickers hardness tester. the microhardness is observed to vary nonlinearly with the applied load, 10 - 100 g. the cleaved ingots are found to have high value of microhardness (222.44 kg/mm2 at a load of25 g), which reflects an appreciable degree of strength due to their covalent bonding and homogeneity. the studies revealed that the dislocations in the grown crystals offered a resistance to fresh dislocations due to interaction. At higher loads, plastic deformation induces by slip, exhibiting a decrease in hardness from the peak value. the dielectric constant and dielectric loss of indium telluride crystals were evaluated in the frequency range, 1 kHz - 1 MHz for different temperatures (35oC - 140oC). the frequency dependence of AC conductivity was analyzed as a function of temperature. the effect of temperature and frequency on the dielectric response of Inte crystals has been assessed on their cleavage faces and the obtained results are discussed.

Host nucleotide polymorphism in hepatitis B virusassociated hepatocellular carcinoma

Hepatocellular carcinoma(HCC) is etiologically linked with hepatitis B virus(HBV) and is the leading cause of death amongst 80% of HBV patients. Among HBV affected patients, genetic factors are also involved in modifying the risk factors of HCC. However, the genetic factors that regulate progression to HCC still remain to be determined. In this review, we discuss several single nucleotide polymorphisms(SNPs) which were reportedly associated with increased or reduced risk of HCC occurrence in patients with chronic HBV infection such as cyclooxygenase(COX)-2 expression specifically at COX-2-1195G/A in Chinese, turkish and Egyptian populations, tumor necrosis factor α and the three most commonly studied SNPs: PAt-/+, Lys939Gln(A33512C, rs2228001) and Ala499Val(C21151t, rs2228000). In genome-wide association studies, strong associations have also been found at loci 1p36.22, 11q22.3, 6p21(rs1419881, rs3997872, rs7453920 and rs7768538), 8p12(rs2275959 and rs37821974) and 22q11.21. the genes implicated in these studies include HLA-DQB2, HLA-DQA1, tCF19, HLA-C, UBE2L3, LtL, FDX1, MICA, UBE4 B and PG. the SNPs found to be associated with the above-mentioned genes still require validation in association studies in order to be considered good prognostic candidates for HCC. Screening of these polymorphisms is very beneficial in clinical experiments to stratify the higher or lower risk for HCC and may help in designing effective and efficient HCC surveillance programs for chronic HBV-infected patients if further genetic vulnerabilities are detected.

trifecta Outcomes of Screening Detected and Patients with Lower Urinary tract Symptoms after Open Radical Prostatectomy for Localized Prostate Cancer

Background: Many studies have reported on trifecta outcomes after radical prostatectomy. there is however paucity of studies that compares the trifecta outcome between screen detected and patients presenting with lower urinary symptoms with localized prostate cancer after radical prostatectomy. this study compares the trifecta outcomes between these two groups after an open retropubic radical prostatectomy. Methodology: this is a retrospective study, on the trifecta outcomes (urinary continence, erectile function, and cancer control) of consecutive patients that had open radical retropubic prostatectomy for localized prostate cancer by a single surgeon. Patients were grouped into screen detected and presentation with lower urinary symptoms or retention of urine. the parameters considered were the age of the patients, the total prostate specific antigen (tPSA) at presentation, the clinical t stage, the Gleason score of prostate biopsies, the risk categories using the D’Amico risk groups and the trifecta outcomes after the procedure. Results: In all, 119 patients met the criteria for inclusion. the median follow up was 63.5 months (range 12 - 156 months). Of these 40.3% of the patients were diagnosed through screening with elevated PSA while 59.7% had presented with symptoms of lower urinary tract obstruction. the mean age for the patients was 60.8 ± 6.5 years, median PSA 12.6 ng/ml (IQR 8.6 - 19.7) and median prostate weight of 50.0 (IQR 40.0 - 60 g). the urinary continence rate after the procedure was 93.3%, erection rate of 81.5%, cancer control rate of 71.4% and trifecta achieved in 57.1%. Comparing the screening and the symptomatic cases, the urinary continence rate was 91.7% vrs 94.3%;erectile function rate was 79.2% vrs 83.1%;cancer control 68.8% vrs 73.2% and trifecta achieved in 58.3% vrs 56.3%. there was no statistically significant difference between the two groups in terms of urinary continence p = 0.564, erection function p = 0.588, cancer control p = 0.5

Expedited Discharge of Patients with Diabetic Ketoacidosis in an Emergency Department Short Stay Unit

Aim: Review of presentation, management and outcome of patients admitted with Diabetic Ketoacidosis (DKA) to an emergency department short stay unit with expedited discharge. Methods: All admitted patients with a discharge diagnosis of “Diabetic Ketoacidosis”, were identified by the file auditing section in the Emergency Department. Data obtained from the medical records were collected using an explicit chart review from January 2012 to June 2013. Data included clinical monitoring, investigations performed, the type and amount of intravenous fluids given, the insulin regime, potassium supplementation and outcome. Results: Out of a total of 120 patients labelled as DKA or hyperglycaemia on arrival, hundred patients fulfilled the criteria for DKA. In the population studied the mean age was 25 years with a male predominance. Eighty-two patients suffered from type 1 Diabetes Mellitus (t1DM) whilst eighteen patients had type 2 Diabetes Mellitus (t2DM). Sixteen patients were newly diagnosed during the present admission. Seventy-six (76%) patients were on insulin. the insulin regimen and potassium supplementation were followed as per protocol in all patients. All the patients except one were given intravenous fluid according to protocol. Parameters were monitored adequately except fluid input and output monitoring. the median length of stay in the short stay unit was 1.5833 days. there was no return visit within one week of discharge. Conclusion: the median length of stay was short and there were no documented complications or deaths during the stay. there was poor compliance with documentation of fluid input and output. In this population, the short stay model of care appeared to be safe and efficient.

thermodynamic dissociation constants pKa of 2,2-bis(hydroxymethyl)-2,2’,2”-nitrilotriethanol have been determined at 12 temperatures from (278.15 to 328.15) K including the body temperature 310.15 K by the electromotive-force measurements (emf) of hydrogen-silver chloride cells without liquid junction of the type: Pt(s), H2(g), 101.325 kPa|BIS-tRIS (m) + BIS-tRIS·HCl (m)| AgCl(s), Ag(s), where m denotes molality. the pKa values for the dissociation process of BIS-tRIS·H++ H2O = H3O+ + BIS-tRIS given as a function of t in Kelvin (K) by the equation pKa = 921.66 (K/t) + 14.0007-1.86197 ln(t/K). At 298.15 and 310.15 K, the values of pKa for BIS-tRIS were found to be 6.4828 ± 0.0005 and 6.2906 ± 0.0006 respectively. thus buffer solutions composed of BIS-tRIS and its hydrochloride would be useful as secondary pH buffer standards and for control of acidity in the pH range 6 to 8. At 298.15 K the thermodynamic functions G°, H°, S° and Cp° for the dissociation process of BIS-tRIS·H+ are G°=37,005 J·mol-1, H° = 28,273 J·mol-1, S°= 29.3 J·K-1·mol-1 and Cp° = 36 J·K-1·mol-1. these results are compared with the dissociation of protonated bases structurally related to BIS-tRIS·H+.

背景门诊患者中，脉压（pulsepressure，PP）增大是公认的增加血管事件的危险因素，但对于冠状动脉搭桥（CABG）手术患者，脉压与急性围手术期血管事件的关系尚不明确。方法我们进行了一项前瞻性观察性研究，涉及5436例择期体外循环冠状动脉搭桥术患者，其中4801例符合标准并最终入选。综合分析数据包括病史、手术中、手术后生理和实验室检查、诊断性检查、临床事件。使用多因素logistic回归对手术前高血压病（收缩压、舒张压、PP）与缺血性心脏病、脑血管事件、死亡率进行分析，P〈0．05认为差异有显著性。结果917例（19．1％）发生致命性或非致命性血管并发症，包括146例（3．0％）脑血管意外和715例（14．9％）心血管并发症，院内死亡147例（3．1％）。手术前测量的所有血压变量中，脉压是增加手术后并发症风险的重要因素。脉压增加10mmHg（高于40mmHg阈值），脑血管并发症发生率就随之增加（校准比值比：1．12；95％可信区间『1．002～1．28]；P=0．026）。与PP≤80mmHg者比较，PP〉80mmHg的患者，脑血管事件和（或）神经源性死亡的发生率几乎翻倍（5．5％：2．8％；P=0．004）。同时发现，PP〉80mmHg与心脏并发症相关，可导致充血性心力衰竭的发生率升高52％，心源性死亡率升高近100％（P值分别为0．003和0．006）。结论PP增大是导致CABG术患者致命性和非致命性的心、脑血管并发症增加的一项重要、独立的因素。这些发现突显了手术前脉压与手术后急性心脑血管并发症之间的联系。

Dissociation Constant of N-(2-Acetamido)-Iminodiacetic Acid Monosodium (ADA) from (278.15 to 328.15) K

the acidic dissociation constant of N-(2-acetamido)-iminodiacetic acid monosodium (ADA) has been determined at 12 temperatures from 278.15 to 328.15 K by electromotive-force (emf) measurements of hydrogen-silver chloride cells without liquid junction. At 298.15 K, the value of the dissociation constant (pK2) is 6.8416 ± 0.0004. In response to the need for new physiological pH standards, buffer solutions of NaADA and its disodium salt, Na2ADA would be useful for pH control in the biological region of pH 6.5 to 7.5. the pK2 values over the experimental temperature range are given as a function of the thermodynamic temperature (t) by the equation pK2 = 2943.784/t - 47.05762 + 7.72703 ln t. At 298.15 K, standard thermodynamic quantities for the dissociation process have been derived from the temperature coefficients;ΔH° = 12,252 J·mol-1, ΔS° = -89.9 J·K-1·mol-1 and = -148 J·K-1·mol-1. the results are interpreted and compared with those of structurally related derivatives of GLYCINE.
