Hormone-Naïve Metastatic Prostate Cancer: A Presentation of 110 Cases in a Urology Center in the City of Douala, Cameroon

Aim: According to World Health Organization, prostate cancer is one of the increasing malignancies in men worldwide. This paper aims to describe the epidemiological, clinical, diagnostic, therapeutic, and evolutionary aspects of patients with early metastatic prostate cancer in a urology center in the city of Douala in Cameroon. Materials and Methods: It is a retrospective and descriptive study of 110 patients with prostate cancer that was immediately metastatic at diagnosis over a period of six years (from January 2014 to December 2020). Results: The average age of patients at diagnosis was 67.5 years (range: 45 years to 88 years) and 53.63% of patients had body mass indexes greater than 25. Disorders of the lower urinary tract were the main presenting complaint in 55.45% of cases, followed by bone and joint pain in 46.36% of cases. Digital rectal examination was suggestive of prostate cancer in 96.36% of cases with an average total prostatic specific antigen (PSAT) level of 676.9 ng/ml (range: 21.8 to 8832 ng/ml). The diagnosis was made through prostate biopsy in 57 (51.81%) patients or after palliative endoscopic resection of the prostate indicated for lower urinary tract symptoms or even acute urinary retention in 53 (48.18%) patients. Adenocarcinoma of the prostate was the main histologic type, and in 47.27% of cases, the tumor was poorly differentiated with a Gleason’s score of greater than 7. The sites of metastasis were mainly the lymph node (87.27%), bone (56.36%), and both (44.54%). The treatment was palliative and dominated by bilateral pulpectomy in 60% of cases and luteinizing hormone-releasing hormone agonists (Triptorelin 11.25 mg every 3 months) in 44 (40%) of cases. Conclusion: Prostate cancer is a real public health problem in developed countries but also in Africa, especially in Cameroon. Background: Prostate cancer, which is the second most frequent cancer diagnosis made in men, more commonly occurs in the elderly. This disease is often diagnosed late in resource-limited settings, which results in people having advanced forms of the disease and a poor prognosis. This study aimed to identify factors indicative of prostate cancer aggressivity and a poor prognosis in patients with prostate cancer at a single center in Douala, Cameroon. Methods: We performed a retrospective study from 2015 to 2020 at the Centre medico-chirugical d’urologie in Douala, Cameroon, in which we included 203 patients aged 41 years to 85 years who had prostate cancer diagnosed via histopathology after either prostate biopsyor laparoscopic prostatectomy. Epi-info 7 was used for data analysis and logistic regression analyses were performed to identify factors associated with prostate cancer aggressivity and patients’ outcomes (survival or mortality). Results: The mean age of our study participants was 64.76 ± 7.48 years. Ten patients had a contributive family history of prostate cancer. The patients presented with lower urinary tract symptoms in 61.58% of cases. All patients had serum prostate-specific antigen (PSA) levels of >4 ng/ml, 100 patients were anemic, and 36 patients had aggressive forms of the disease. Eighty-eight patients had remarkable digital rectal examination (DRE) findings. The median prostate volume, as determined via transrectal ultrasonography (TRUS), was 59 [43 - 80] ml. Fifty-nine patients had abnormal prostate echostructures, and 33 patients died during follow-up. The presence of paraplegia and the practice of professions requiring unskilled labor were significantly associated with aggressive prostate cancer. The presence of lymphoedema, abnormal DRE findings, anemia, enlarged prostate glands (prostate volume >50 ml), and abnormal prostatic echostructures were significantly associated with both prostate cancer aggressivity and patients’ outcomes. Background and Aim: Ever since the first-ever laparoscopic nephrectomy performed in 1991 by Clayman, laparoscopy has become the technique of choice for benign renal pathologies and also for cancerous lesions. In this paper, we present and evaluate the results of laparoscopic nephrectomy carried out on patients with non-functional kidneys at the Centre medico-chirugical d’urologie in Douala, Cameroon. Materials and Methods: This was a retrospective study carried out from 2016 to 2020. We included 25 patients with non-functional kidneys who underwent transperitoneal laparoscopic nephrectomy. Results: We included 25 patients (15 males and 10 females) with a mean age of 32.80 ± 9.76 years. Twenty (80%) patients presented with low back pain, four (16%) presented with acute pyelonephritis, and one (4%) presented with both low back pain and hematuria. The right kidney was damaged in 10 (40%) patients and the left kidney in 15 (60%) patients. The kidney failure requiring nephrectomy was due to stones in 16 (64%) patients and upper pyeloureteric junction obstruction in nine (36%) patients. The mean surgery duration was 111.08 ± 31.95 minutes. The median perioperative blood loss was 100 [70 - 120] ml. Percutaneous drainage was required in 13 (52%) patients. Only two (8%) patients developed postoperative complications. The mean follow-up duration was 64 ± 24.48 days. All patients survived the surgical intervention. Conclusion: Laparoscopic nephrectomy is a mini-invasive technique that is suitable for the surgical removal of non-functional kidneys due to either ureteropelvic junction obstruction or massive kidney stones.

Evolution of Floristic Composition and Structural Parameters between Two Management Inventories in a Tropical Forest in the East Cameroon Region

The realization of two management inventories in 2003 and 2020 on the same forest concession made it possible to characterize the floristic composition and to describe some structural parameters on three sites established according to the period of exploitation. Due to the change in the orientation of the lines, it was noted that respectively 82 and 85 species from the first inventory are not found in the second and inversely. A total of 311 species were identified in 2003, compared to 314 in 2020. The density varies from 111 to 140 stems/ha for all the individuals, 19 to 25 for the main species and 15 to 20 for the most exploited species. The decrease in the basal area between the two phases of 12% for all the species and 13% for the main species on the site exploited before the first inventory indicates a persistence of the disturbance for years after logging. It is proposed to supplement the management plans decisions with annual measures based on more complete inventories and to ensure the application of reduced impact logging measures.

Endoscopic Management of Bladder Stones: Initial Experience at a Single Center in Cameroon

Purpose: This study aimed to evaluate the efficacy and safety of endoscopic lithotripsy with the lithoclast (EMS, Switzerland) and laser Holmium YAG in the management of bladder calculi. Materials and methods: This was a retrospective study carried out from January 2013 to December 2019 on 32 patients with bladder calculi. All the patients underwent either Lithoclast or Laser lithotripsy using a 22F Storz cystoscope at the Centre medico-chirugicale d’urologie in Douala, Cameroon. Data on patients’ ages, clinical symptoms, stone sizes, type of lithotripsy, surgery duration, and results of lithotripsy were collected and analyzed using Epi info 7. Results: We recruited 32 participants (24 men and 8 women) with a median age of 41.28 [22 - 68] years into this study. In 9 (28.12%) participants, macroscopic hematuria was the main presenting complaint, followed by lower urinary tract symptoms in 8 (25%) patients. A cystoscopy was performed in 17 (53.12%) patients to confirm the diagnosis of a bladder stone, and ultrasonography of the upper urinary tract was performed in 29 (90.6%) cases to certify the absence of another stone. Lithoclast EMS was used to manage the stones in 23 (71.87%) patients while laser lithotripsy was used in 9 (28.13%). A dormia basket was used to remove stone fragments in 10 (31.25%) patients. The mean surgery duration was 33.59 ± 14.2 minutes, and the bladder stones were successfully managed in all the participants of this study. Minor complications such as pain during micturition were found in 28 (87.37%) patients, with complete resolution occurring two weeks after surgery. Conclusion: Endoscopy with lithotripsy is a safe and effective method of management of bladder stones. This technique is also associated with short surgical procedures and postoperative hospitalization periods. We believe that it is an excellent treatment modality in the management of bladder stones.

Background and Objective: Laparoscopy can be an alternative modality in the management of ureteral stones. We herein present our experience with laparoscopic ureterolithotomy although most ureteral stones are managed using endourologic techniques, open surgery, or shockwave lithotripsy. Materials and Methods: This retrospective study was performed from January 2014 to December 2019 on 20 patients with ureteral stones who were treated using transperitoneal laparoscopic access. We collected data on patients’ ages, genders, clinical profiles, relevant medical history, sizes of the calculi, localisation of the calculi as confirmed by imaging, and outcome of lithotripsy. Continuous data were presented as mean values and standard deviations (for normally distributed data) and medians with interquartile ranges (for skewed data). Categorical data were presented as frequencies and percentages. Results: We included 20 patients (13 males and 7 females) with a mean age of 40.40 ± 13.25 years. The mean stone size was 18.5 ± 3.05 mm and all procedures were completed laparoscopically. The mean operative time was 96 ± 22.34 minutes. The mean estimated blood loss was less than 150 ml, and none of the patients received a blood transfusion. There was no intraoperative complication or postoperative complications, except for leakage of urine in the suture area. The mean hospital stay was 2.05 ± 0.69 days and the double J stent was removed after an average of 20 days post-operatively. The stone-free rate was 100% and after a mean follow-up period of 3 months, there was no stone recurrence. Conclusion: Laparoscopic ureterolithotomy is an effective and safe technique in the management of ureteric stones. The benefits of this technique include minimal postoperative morbidity, short postoperative hospitalization, a short convalescence period, and remarkable cosmetic results.

Hydrodynamic Performance of a Newly-Designed Pelagic and Demersal Trawls Using Physical Modeling and Analytical Methods for Cameroonian Industrial Fisheries

This study proposed the newly-designed Pelagic and demersal trawls for the fishing vessels operating in Cameroonian waters in pelagic and demersal fishing grounds. The engineering performances of both trawls were investigated using physical modelling method and analytical method based on the predicted equations. In a flume tank, a series of physical model tests based on Tauti’s law were performed to investigate the hydrodynamic and geometrical performances of both trawls and to assess the applicability of the analytical methods based on predicted equations. The results showed that in model scale, the working towing speed and door spread for the pelagic trawl were 3.5 knots and 1.85 m, respectively, and for the bottom trawl net they were 4.0 knots and 1.8 m. At that speed and door spread, the drag force, net opening height, and wing-end spread of the pelagic model trawl were 36.73 N, 0.89 m, and 0.86 m, respectively, and the swept area was 0.76 m2. Bottom trawl speed and door spread were 30.43 N, 0.38 m, and 0.45 m, respectively, and the swept area was 0.25 m2. The maximum difference between the experimental and analytical results of hydrodynamic performances was less than 56.22% and 41.45%, respectively, for pelagic and bottom trawls, the results of the geometrical performances obtained using predicted equations were close to the experimental results in the flume tank with a maximum relative error less than 12.85%. The newly developed pelagic and bottom trawls had advanced engineering performance for high catch efficiency and selectivity and could be used in commercial fishing operations in Cameroonian waters.

The Knowledge of turbulent flow developing inside and around the bottom trawl net is of great importance not only for improving the hydrodynamic performance of the gear but also for the selectivity via the fish response,such as the herding response or escape *** 3-D Electromagnetic Current Velocity Meter(ECVM)measurements were performed to investigate the effect of turbulent flow on the bottom trawl net performance and to analyze the turbulence intensity and velocity ratio inside and around different parts of the trawl *** orthogonal decomposition(POD)method was applied in order to extract the phase averaged mean velocity field of turbulent flow from each available ECVM instantaneous *** results demonstrated the existence of turbulence flow,consisting of turbulent boundary layer flow and the turbulence due to the trawl wake developing all inside and around the bottom trawl *** input streamwise velocity results in faster trawl movement and a significant turbulent *** maximum turbulence intensity inside and around trawl wing,square part,first belly,second belly,third belly,cod-end is 0.95%,1.34%,3.40%,4.10%,4.25%and 3.80%,*** was found that the mean velocity field in a turbulent flow inside and around trawl net cod-end recovered on the average was~77.58%of the input streamwise *** is~12.92%,~13.07%,~11.40%,~13.00%and~0.45%less than that inside and around trawl wing,square part,first belly,second belly,and third belly of the bottom trawl net,*** turbulent flow behavior depends strongly on the structure oscillation,input streamwise velocity and,porosity of the net *** is necessary to take into account the velocity reduction inside and around a different part of the trawl net to improve the entire drag force determination,cod-end design,and further selectivity control of the fishing gear.

Campylobacter species belong to the most important foodborne bacteria which cause gastroenteritis in humans in both developed and developing countries. Applied hygienic measures result only partially efficient, as demonstrated by the high number of reported cases in the human population. Alternative strategies to prevent the disease though a direct action at primary production level have been explored. Preliminary data showed that cattle and pigs fed with bamboo and olive by-products, respectively, were free from Campylobacter, suggesting potential sustainable prevention strategy. In addition, biochemical analysis showed high levels of unsaturated fatty acids in red meat and meat products, adequate for the application of health claims in line with EU food law.
