Laparoscopic Surgery of Incisional Hernia: Technique and Short-Term Results in Three Surgical Units in Cameroon

Introduction: Incisional hernia is a common medical situation and its treatment has always been a challenge for general surgeons. If laparoscopic repair has become a gold standard elsewhere, it is increasingly done in our milieu, but no data has been found. The aim of this study was to review this surgery done in our country so far, to describe the operative technique and to evaluate the postoperative outcomes. Patients and Methods: We conducted an observational descriptive study with retrospective collection of data from the 1st of July 2018 to 30th of June 2022, in three hospitals in Cameroon. Data on socio-demographic and clinical characteristics, surgical technique and postoperative outcomes of patients who had a laparoscopic repair of their incisional hernia were collected. Results: We reviewed 20 files. There were 14 women with an average age of 54.6 years. The average BMI was 32.8 kg/m2. Thirteen patients had a moderate size hernia and the hernia was localized at the midline in 18 cases. Only composite biface meshes were used with dimensions chosen to obtain at least 5 cm overlap. Spiral tacks were the only fixation means used. We registered 3 complications. One case of generalized peritonitis secondary to missed digestive perforation, one case of seroma and one case of postoperative ileus. We registered no death. Conclusion: Laparoscopic repair of incisional hernias is feasible in a resource-limited setting like ours. Compliance with the operative technique and recommendations is important to obtain short-term results closed to that put forward by literature.

Steatosis as a co-factor in chronic liver diseases

The finding of lipid accumulation in the liver, so-called hepatic steatosis or non-alcoholic fatty liver disease, is a common condition frequently found in healthy subjects. Its prevalence, in fact, has been estimated by magnetic resonance studies to be about 35% in the general population and 75% in obese persons. Nevertheless, its presence generates liver damage only in a small percentage of subjects not affected by other liver diseases. It should be defined as a "co-factor" capable of affecting severity and progression, and also therapeutic perspectives, of liver diseases to which it is associated. Herein we will evaluate the impact of hepatic steatosis and obesity on the most common liver diseases: chronic viral hepatitis C and B, and alcoholic liver disease.

Evaluation of the Quality of Micturition in Patients Who Underwent Anastomotic Urethroplasty at the Yaounde Central Hospital, Cameroon

Introduction: Radiological investigation which is the gold standard to perform following anastomotic urethroplasty in order to evaluate the quality of micturition is costly. In our context, due to financial limitations, reconstructive urologists prefer to use the uroflowmetry in order to assess the micturition post-surgery. Therefore, the objective of the study was to assess the quality of micturition using the uroflowmetry after anastomotic urethroplasty. Methodology: We conducted an 11-year retrospective review (1st January 2006-31st December 2017) and a cross-sectional descriptive observational study for a period of 8 months (November 2017-June 2018) at the Urology and Andrology Department of the Yaounde Central Hospital (YCH) of patients who were diagnosed with urethral stenosis and underwent an anastomotic urethroplasty at the YCH. We excluded patients who had incomplete files, patients lost to follow-up and did not do pre-operative uroflowmetry. Quality of micturition was evaluated using a uroflowmetry. Data was analyzed using EPI info 7.0. Parametric variables were reported as means and standard deviations and percentages and counts were used to report categorical variables. Results: We had a sample of 60 patients. The mean age was 42 ± 5 years with extremes ranging from 20 to 76 years. Twenty-seven (27) patients, or 45%, had no post-operative complications, and those who did had a urinary tract infection (26.70%). In our series, we had 82% excellent results (patient satisfied with his urination with bell-shaped urinary stream and urinary flow greater than or equal to 15 ml/sec);15% good results (patient with moderate dysuria with average urinary stream and urinary flow between 10 and 14.9 ml/sec) and 3% poor results (severe dysuria with urinary flow less than 10 ml/sec, urinary retention or urinary incontinence). Based on these results we can say that the success rate in our series was 97%. 96.70% of patients were satisfied against 3.3%

A Markov-switching dynamic regression analysis of the asymmetries related to the determinants of US crude oil production between 1982 and 2019

The structural changes brought about by shale oil revolution have inspired this paper of which the aim is to analyze the potential asymmetries related to the determinants of crude oil production in the ***,using a Markov-switching dynamic regression model in which parameters change when oil production moves from one regime to the other,it is found that for both oil production and oil relative importance,the regime that was dominant during the 1980s and the early 1990s when oil production in the USA was substantially high is the same regime that has once again become dominant in the decade corresponding to the shale oil ***,the study reveals the existence of asymmetries in the relationship between US crude oil production and both manufacturing production and the consumer price *** are also found in the relationship between the relative importance US crude oil and manufacturing ***,it is found that the intercept and the variance parameter also vary from one regime to the other,thus justifying the use of regime-dependent models.

Vegetation structure,dominance patterns and height growth in an Afromontane forest,Southern Africa

Information on forest structure is fundamentally important to track successional vegetation dynamics for efficient forest *** study reports on vegetation characteristics,dominance patterns and species height growth in a northern mistbelt forest type in South *** alpha-diversity indices(species richness and Shannon-Weiner diversity),structural vegetation parameters(tree density and basal area),and species importance value index were *** class distribution and height-diameter allometry were further examined for the overall stand and most important *** densities(472.0±43.5 and 605.3±28.1 trees ha^-1 for C 5 cm to\10 cm and C 10 cm dbh(diameter at breast height)classes,respectively)and basal area values(1.99±0.19 and 48.07±3.46 m^2 ha^-1,respectively)are comparable to other Afromontane forests in East *** overall stand showed an inverted-J shaped distribution pattern which is a typical feature of stand size class distribution in most natural *** ecologically important species also exhibited an inverted-J shaped distribution pattern,suggesting good regeneration and recruitment *** were significant differences in species on height,reflecting species-specific height growth patterns,possibly a result of intrinsic growth potential and competitive *** present study suggests that conservation and management policies,including protection of surrounding land uses against fire,contribute to maintaining a successful recovery of these ***,it should be noted that these forests may be experiencing relatively slow dynamic flux as a result of the overmature state of some trees with several years under relatively strict protection.

Application of site-specific biomass models to quantify spatial distribution of stocks and historical emissions from deforestation in a tropical forest ecosystem

Allometric equations developed for the Lama forest, located in southern Benin, West Africa, were applied to estimate carbon stocks of three vegetation types：undisturbed forest, degraded forest, and fallow. Carbon stock of the undisturbed forest was 2.7 times higher than that in the degraded forest and 3.4 times higher than that in fallow. The structure of the forest suggests that the individual species were generally concentrated in lower diameter classes. Carbon stock was positively correlated to basal area and negatively related to tree density, suggesting that trees in higher diameter classes contributed significantly to the total carbon stock. The study demonstrated that large trees constitute an important component to include in the sampling approach to achieve accurate carbon quantification in forestry. Historical emissions from deforestation that converted more than 30% of the Lama forest into cropland between the years 1946 and 1987 amounted to 260,563.17 tons of carbon per year（t CO2/year） for the biomass pool only. The study explained the application of biomass models and ground truth data to estimate reference carbon stock of forests.

通信是生活中非常重要的一方面。随着时间的推移和需求的增加,通信领域已经有了飞速的发展,为了更好的进行传输,以模拟方式传输的信号,现在越来越多地以数字方式来进行传输,单载波技术也正在被多载波技术所取代。CDMA和OFDM等系统现在正被广泛地应用。正交频分复用(OFDM)是一种将信道划分为多个相等频段的技术,每个载波通过占用一个频段来传输一部分用户信息,并且保持任何一副载波都与其他副载波是正交的(即与其他副载波相互独立)。这项技术于40年前发明,并在其后的时间里被广泛地应用于多项技术中,如非对称数字用户环线(ADSL) services、IEEE802.11a/g, IEEE802.16a、数字视频广播(DVB)、数码地面电视与音频广播、4G、IEEE802.11n、IEEE802.16和IEEE802.20.与其他调制技术不同,OFDM是一种多载波的调制技术,载波之间可以有大幅的重叠。对于每一个高频载波,OFDM同时使用多个副载波来传输多个高速信号,因为副载波之间相互正交,所以不会相互产生干扰。因为能够通过重叠频谱来提供高带宽、对抗多径衰落等传输条件,OFDM被认为是下一代移动通信的极佳选择。但是,OFDM系统的主要缺点在于其峰均功率比(PAPR)过高,这会导致大功率放大器利用率过低,从而导致传输效率的降低。OFDM由大量独立的副载波组成,这就导致了信号的振幅可能会过大。在本文中,我们提出了一种融合了DCT和SLM技术的有效减幅策略。此策略由DCT及随后的SLM组成,它使用Riemann矩阵的来获取SLM的阶段序列。仿真的结果显示所提出的策略能大幅地减少PAPR。在与OFDM的比较中,OFDM有较高的PAPR值(大约10.4dB),而我们所提出的方法在低复杂度的计算中能将PAPR降低4.7dB。同时,此方法也能避免阶段的随机选择,使得接收器的解码更简单。另一个优点是,接收端生成矩阵来获取数据信号,所以不需要传输附加信息(SI)。

The Knowledge of turbulent flow developing inside and around the bottom trawl net is of great importance not only for improving the hydrodynamic performance of the gear but also for the selectivity via the fish response,such as the herding response or escape *** 3-D Electromagnetic Current Velocity Meter(ECVM)measurements were performed to investigate the effect of turbulent flow on the bottom trawl net performance and to analyze the turbulence intensity and velocity ratio inside and around different parts of the trawl *** orthogonal decomposition(POD)method was applied in order to extract the phase averaged mean velocity field of turbulent flow from each available ECVM instantaneous *** results demonstrated the existence of turbulence flow,consisting of turbulent boundary layer flow and the turbulence due to the trawl wake developing all inside and around the bottom trawl *** input streamwise velocity results in faster trawl movement and a significant turbulent *** maximum turbulence intensity inside and around trawl wing,square part,first belly,second belly,third belly,cod-end is 0.95%,1.34%,3.40%,4.10%,4.25%and 3.80%,*** was found that the mean velocity field in a turbulent flow inside and around trawl net cod-end recovered on the average was~77.58%of the input streamwise *** is~12.92%,~13.07%,~11.40%,~13.00%and~0.45%less than that inside and around trawl wing,square part,first belly,second belly,and third belly of the bottom trawl net,*** turbulent flow behavior depends strongly on the structure oscillation,input streamwise velocity and,porosity of the net *** is necessary to take into account the velocity reduction inside and around a different part of the trawl net to improve the entire drag force determination,cod-end design,and further selectivity control of the fishing gear.

Cancer stem cells in colorectal cancer from pathogenesis to therapy: Controversies and perspectives

Colorectal cancer remains one of the most common and lethal malignancies worldwide despite the use of various therapeutic strategies. A better understanding of the mechanisms responsible for tumor initiation and progression is essential for the development of novel, more powerful therapies. The traditional, so-called “stochastic model” of tumor development, which assumes that each cancer cell is tumorigenic, has been deeply challenged during the past decade by the identification of cancer stem cells (CSCs), a biologically distinct subset of cells within the bulk of tumor mass. This discovery led to the development of the hierarchical model of tumorigenesis which assumes that only CSCs have the ability to initiate tumor growth, both at primary and metastatic sites. This model implies that the elimination of all CSCs is fundamental to eradicate tumors and that failure to do so might be responsible for the occurrence of relapses and/or metastases frequently observed in the clinical management of colorectal cancer patients. Identification and isolation of CSCs is essential for a better understanding of their role in the tumorigenetic process and for the development of CSC-specific therapies. Several methods have been used for this purpose and many efforts have been focused on the identification of specific CSC-surface markers. This review provides an overview of the proposed roles of CSC in human colorectal tumorigenesis focusing on the most important molecules identified as CSC-specific markers in colorectal cancer and on the potential strategies for the development of CSC-targeted therapy.

Background:The relationship between diet and intestinal microbiota and mucin composition appears to be fundamental for poultry gut *** effects of insect meal(whose role as alternative feed ingredient is now well recognized)on gut microbiota and mucin composition have recently been reported in Tenebrio molitor-fed free-range and broiler chickens,but no data are currently available for Hermetia illucens(HI)-fed *** present study evaluated the effects of dietary HI meal inclusion on cecal microbiota and intestinal mucin composition of broiler ***:A total of 256 male broiler chickens were allotted to 4 dietary treatments(control diet[C]and 5%,10%and 15%HI meal inclusion,with 8 replicate pens/treatment and 8 birds/pen)and slaughtered at 35 d of age(2 animals/pen,16 birds/diet).The cecal microbiota assessment by 16S rRNA amplicon based sequencing showed lower alpha diversity in HI15 chickens(Shannon,P<0.05)and higher beta diversity(Adonis and ANOSIM,P<0.001)in birds fed HI diets than ***,HI15 birds displayed significant increase of the relative abundance of Proteobacteria phylum(False Discovery Rate[FDR]<0.05)when compared to HI10.L-Ruminococcus(Ruminococcus from Lachnospiraceae family),Faecalibacterium,Blautia and Clostridium genera were found to be characteristic of HI5 cecal microbiota(FDR<0.05),while broiler chickens fed HI10 and HI15 diets were characterized(FDR<0.05)by Lactobacillus and Ruminococcus(HI10)and Bacteroides,Roseburia and Helicobacter genera(HI15).Periodic-acid Schiff,Alcian Blue pH 2.5 and high iron diamine staining on small and large intestine also demonstrated lower mucin staining intensity in the intestinal villi of HI10 and HI15 birds than C(P<0.05).Conclusions:Dietary HI meal utilization at low inclusion levels(i.e.,5%)positively influenced either the cecal microbiota or the gut mucin dynamics in terms of selection of potentially beneficial bacteria and increase in villi ***,high inclusion levels(in
