Novel Formula for Calculation of the Optimal Insertion Depth for Cuffed Endotracheal Tubes in Pediatric Major Surgery

Background: Accurate determination of the optimal insertion depth of a pediatric endotracheal tube (ETT) is quite important. The aim of this study was to create an easily available formula that can be used to determine the optimal insertion depth for a cuffed ETT even without depth marking with clear definitions of the upper and lower limits for the tip of ETT in the trachea in clinical practice. Methods: Eighty children under 12 years of age were enrolled. The depth marking of the cuffed ETT was placed at the vocal cords and both lungs were then auscultated using a stethoscope. The upper limit was radiographically defined as the position of the tip of the cuffed ETT being between the clavicles. The lower limit was defined as a distance of 5 mm above the carina. The relationship between the insertion depth and patient characteristics was analyzed to create a formula for optimal ETT insertion depth. Results: Sixty-nine ETTs were optimally placed in the trachea. There were good correlations between the optimal insertion depth of ETTs and patients characteristics (height (R = 0.92);BSA (R = 0.92);weight (R = 0.91);age (R = 0.88)). Using these patient characteristics, we created the following three formulas for calculation of the optimal insertion depth for pediatric cuffed ETTs: insertion depth (cm) = height (cm)/11 + 5.5, weight (kg)/3 + 9.5 or 11 + 3/4 × age (years). The rates of appropriate tube placement of both pediatric cuffed ETTs were 87.5% (Hi-Contour) and 85.0% (Microcuff). Conclusions: Our formula and graphs may be easy to determine the optimal insertion depth of cuffed ETT even without depth marking in clinical practice.

Background: Surveys of pediatric endotracheal tube (ETT) management previously reported that specialists in pediatric anesthesia and intensive care medicine preferred to use uncuffed ETTs for children younger than 8 to 10 years of age. The aim of this study was to reveal the most recent attitudes and clinical practices of pediatric ETT management in Japan. Methods: The attitudes and clinical practices of pediatric ETT management were investigated using the data sheets of each institution and each patient. The data sheets contained information on patient characteristics and type of hospital, surgical procedures, devices used for intubation, and ETT information including types, size, depth, intracuff pressure (ICP), interval of ICP measurement, laryngeal packing, ETT exchange, airway complications, and reintubations. Results: The response rate of this survey was 66.7%. More than half of children older than 2 years of age were intubated with cuffed ETTs;83.5% of cuffed ETTs were used with the cuffs inflated, and ICP was measured in 80.7% of cuffed ETTs. More than half of ICP measurements were only taken at the time of intubation. Post-extubation stridor was rarely observed in cuffed (0.4%) or uncuffed ETTs (1.2%). The pediatric ETT management questionnaire revealed age-based size selection, differences in pressure of air leakage between cuffed (15 - 20 cmH2O) and uncuffed ETTs (20 - 30 cmH2O) of different sizes, the depthmarking method of insertion length. Continuous measurement of ICP was not common. Conclusion: This study revealed widespread use of cuffed ETTs in children older than 2 years of age, rarely occurrence of post-extubation stridor, inflation of cuffs, and practice of ICP measurement.

An integrated model to simulate sown area changes for major crops at a global scale

Dynamics of land use systems have attracted much attention from scientists around the world due to their ecological and socio-economic implications. An integrated model to dynamically simulate future changes in sown areas of four major crops (rice, maize, wheat and soybean) on a global scale is pre- sented. To do so, a crop choice model was developed on the basis of Multinomial Logit (Logit) model to model land users' decisions on crop choices among a set of available alternatives with using a crop utility function. A GIS-based Environmental Policy Integrated Climate (EPIC) model was adopted to simulate the crop yields under a given geophysical environment and farming management conditions, while the International Food Policy and Agricultural Simulation (IFPSIM) model was utilized to estimate crop price in the international market. The crop choice model was linked with the GIS-based EPIC model and the IFPSIM model through data exchange. This integrated model was then validated against the FAO statistical data in 2001-2003 and the Moderate Resolution Imaging Spectroradiometer (MODIS) global land cover product in 2001. Both validation approaches indicated reliability of the model for ad- dressing the dynamics in agricultural land use and its capability for long-term scenario analysis. Finally, the model application was designed to run over a time period of 30 a, taking the year 2000 as baseline. The model outcomes can help understand and explain the causes, locations and consequences of land use changes, and provide support for land use planning and policy making.

Fully automatic DOM generation method based on optical flow field dense image matching

Automatic Digital Orthophoto Map(DOM)generation plays an important role in many downstream works such as land use and cover detection,urban planning,and disaster *** DOM generation methods can generate promising results but always need ground object filtered DEM generation before otho-rectification;this can consume much time and produce building facade contained *** address this problem,a pixel-by-pixel digital differential rectification-based automatic DOM generation method is proposed in this ***,3D point clouds with texture are generated by dense image matching based on an optical flow field for a stereo pair of images,***,the grayscale of the digital differential rectification image is extracted directly from the point clouds element by element according to the nearest neighbor method for matched ***,the elevation is repaired grid-by-grid using the multi-layer Locally Refined B-spline(LR-B)interpolation method with triangular mesh constraint for the point clouds void area,and the grayscale is obtained by the indirect scheme of digital differential rectification to generate the pixel-by-pixel digital differentially rectified image of a single image ***,a seamline network is automatically searched using a disparity map optimization algorithm,and DOM is smartly *** qualitative and quantitative experimental results on three datasets were produced and evaluated,which confirmed the feasibility of the proposed method,and the DOM accuracy can reach 1 Ground Sample Distance(GSD)*** comparison experiment with the state-of-the-art commercial softwares showed that the proposed method generated DOM has a better visual effect on building boundaries and roof completeness with comparable accuracy and computational efficiency.

Web-based interactive visualization of 3D video mosaics using X3D standard

We present a method of 3D image mosaicing for real 3D representation of roadside buildings, and implement a Web-based interactive visualization environment for the 3D video mosaics created by 3D image mosaicing. The 3D image mo- saicing technique developed in our previous work is a very powerful method for creating textured 3D-GIS data without excessive data processing like the laser or stereo system. For the Web-based open access to the 3D video mosaics, we build an interactive visualization environment using X3D, the emerging standard of Web 3D. We conduct the data preprocessing for 3D video mosaics and the X3D modeling for textured 3D data. The data preprocessing includes the conversion of each frame of 3D video mosaics into concatenated image files that can be hyperlinked on the Web. The X3D modeling handles the representation of concatenated images using necessary X3D nodes. By employing X3D as the data format for 3D image mosaics, the real 3D representation of roadside buildings is extended to the Web and mobile service systems.

Methane dehydroaromatization over Mo-modified H-MFI for gas to liquid catalysts

For direct gas to liquid（GTL）,a novel process producing energy sources for methane dehydroaromatization is *** MoO3 on H-MFI zeolite shows the high catalytic capacity and a selective activity for dehydroaromatization of methane to benzene at 973 K in a fixed bed *** the other hand,deactivation by coke on the active sites in all the catalysts is formed during the reaction.H2 co-feed suppressed the deactivation,which is probably due to the decrease in coking *** K-edge X-ray absorption fine structure（XAFS） results showed the formation of dispersed Mo2C species with low crystallinity after *** LIII-edge XANES（X-ray absorption near-edge structure） indicated the formation of active Mo species including Mo2C and Mo-oxycarbide（MoOxCy）,where the redox state should be independent in the absence/presence of *** is concluded that Mo-oxycarbide species act as highly active species,and their stability affected the durable activity in the presence of H2.

Integrating GPS trajectory and topics from Twitter stream for human mobility estimation

Understanding urban dynamics and large-scale human mobility will play a vital role in building smart cities and sustainable urbanization. Existing research in this domain mainly focuses on a single data source (e.g., GPS data, CDR data, etc.). In this study, we collect big and heterogeneous data and aim to investigate and discover the relationship between spatiotemporal topics found in geo-tagged tweets and GPS traces from smartphones. We employ Latent Dirichlet Allocation-based topic modeling on geo-tagged tweets to extract and classify the topics. Then the extracted topics from tweets and temporal population distribution from GPS traces are jointly used to model urban dynamics and human crowd flow. The experimental results and validations demonstrate the efficiency of our approach and suggest that the fusion of cross-domain data for urban dynamics modeling is more practical than previously thought.

Interoperability for Global Observation Data by Ontological Information

The Ontology registry system is developed to collect, manage, and compare ontological information for integrating global observation data. Data sharing and data service such as support of metadata deign, structuring of data contents, support of text mining are applied for better use of data as data interoperability. Semantic network dictionary and gazetteers are constructed as a trans-disciplinary dictionary. Ontological information is added to the system by digitalizing text based dictionaries, developing 'knowledge writing tool' for experts, and extracting semantic relations from authoritative documents with natural language processing technique. The system is developed to collect lexicographic ontology and geographic ontology.

Development of a GIS-based Decision Support System for Assessing Land Use Status

The objectives of this study are to assess land s ui tability and to predict the spatial and temporal changes in land use types (LUTs ) by using GIS-based land use management decision support system. A GIS databas e with data on climate, topography, soil characteristic, irrigation condition, f ertilizer application, and special socioeconomic activities has been developed a nd used for the evaluation of land productivity for different crops by integrati ng with a crop growth model-the erosion productivity impact calculator (EPIC). International food policy simulation model (IFPSIM) is also embedded into GIS fo r the predictions of how crop demands and crop market prices will change under a lternative policy scenarios. An inference engine (IE) including land use choice model is developed to illustrate land use choice behavior based on logit models , which allows to analyze how diversified factors ranging from climate changes, crop price changes to land management changes can affect the distribution of agr icultural land use. A test for integrated simulation is taken in each 0.1° by 0.1° grid cell to predict the change of agricultural land use types at globa l level. Global land use changes are simulated from 1992 to 2050.

Features of hepatocellular carcinoma in cases with autoimmune hepatitis and primary biliary cirrhosis

AIM: To characterize the clinical features of hepatocellular carcinoma (HCC) associated with autoimmune liver disease, we critically evaluated the literature on HCC associated with autoimmune hepatitis (AIH) and primary biliary cirrhosis (PBC). METHODS: A systematic review of the literature was conducted using the Japana Centra Revuo Medicina database which produced 38 cases of HCC with AIH (AIH-series) and 50 cases of HCC with PBC (PBC-series). We compared the clinical features of these two sets of patients with the general Japanese HCC population. RESULTS: On average, HCC was more common in men than in women with AIH or PBC. While many patients underwent chemolipiodolization (CL) or transcatheter arterial embolization (TAE) (AIH-series: P = 0.048 (vs operation), P = 0.018 (vs RFA, PEIT); PBC-series: P = 0.027 (vs RFA, PEIT), others refused therapeutic interventions [AIH-series: P = 0.038 (vs RFA, PEIT); PBC-series: P = 0.003 (vs RFA, PEIT)].Liver failure was the primary cause of death among patients in this study, followed by tumor rupture. The survival interval between diagnosis and death was fairly short, averaging 14 ± 12 mo in AIH patients and 8.4 ± 14 mo in PBC patients. CONCLUSION: We demonstrated common clinical features among Japanese cases of HCC arising from AIH and PBC.
