AIM: To compare the utility of single-balloon colonoscopy (SBC) or double-balloon colonoscopy (DBC) for difficult colonoscopies. METHODS: Between August 2008 and June 2010, patients in whom total colonoscopy failed within 30 min of insertion were assigned randomly to undergo either SBC or DBC. No sedatives were used. After the endoscopy, all patients were asked to evaluate pain during the procedure on a 10-point analog scale (1 = no pain; 10 = worst imaginable pain) with a questionnaire. The study outcomes were the cecal intubation rate and time, endoscopic findings, complications, and pain score. RESULTS: The SBC and DBC groups included 11 and 10 patients, respectively. All but one SBC patient achieved total colonoscopy successfully. The cecal intubation times were 18 min (range: 10-85 min) and 12.8 min (range: 9.5-42 min) in the SBC and DBC groups, respectively (P= 0.17). No difference was observed in the prevalence of colon polyps between the SBC and DBC groups (45% vs 30%, P = 0.66). SBC showed advanced colon cancer in the ascending colon, which was inaccessible using conventional colonoscopy. The respective pain scores were 5 (1-10) [median (range)] and 5 (1-6) in the SBC and DBC groups (P = 0.64). No complications were noted in any patient. CONCLUSION: The utility of singleand double-balloon endoscopy for colonoscopy seems comparable in patients with incomplete colonoscopy using a conventional colonoscope.

AIM: To elucidate the mechanisms of hepatocyte preconditioning by H2O2 to better understand the pathophysiology of ischemic preconditioning. METHODS: The in vitro effect of H2O2 pretreatment was investigated in rat isolated hepatocytes subjected to anoxia/reoxygenation. Cell viability was assessed with propidium iodide fluorometry. In other experiments, rat livers were excised and subjected to warm ischemia/ reperfusion in an isolated perfused liver system to determine leakage of liver enzymes. Preconditioning was performed by H2O2 perfusion, or by stopping the perfusion for 10 min followed by 10 min of reperfusion. To inhibit Kupffer cell function or reduced nicotinamide adenine dinucleotide phosphate (NADPH) oxidase, gadolinium chloride was injected prior to liver excision, or diphenyleneiodonium, an inhibitor of NADPH oxidase, was added to the perfusate, respectively. Histological detection of oxygen radical formation in Kupffer cells was performed by perfusion with nitro blue tetrazolium. RESULTS: Anoxia/reoxygenation decreased hepatocyte viability compared to the controls. Pretreatment with H2O2 did not improve such hepatocyte injury. In liver perfusion experiments, however, H2O2 preconditioning reduced warm ischemia/reperfusion injury, which wasreversed by inhibition of Kupffer cell function or NADPH oxidase. Histological examination revealed that H2O2 preconditioning induced oxygen radical formation in Kupffer cells. NADPH oxidase inhibition also reversed hepatoprotection by ischemic preconditioning. CONCLUSION: H2O2 preconditioning protects hepato- cytes against warm ischemia/reperfusion injury via NADPH oxidase in Kupffer cells, and not directly. NADPH oxidase also mediates hepatoprotection by ischemic preconditioning.

Evolution History of Gassan Volcano, Northeast Japan Arc

Hepatitis B and C virus infection is a major cause of hepatocellular carcinoma (HCC) in Japan. Here, I will present our data how we have tried to prevent HCC by treating hepatitis. Primary Prevention of HCC Acute Hepatitis Treatment of acute hepatitis C by interferon was so successful. Chronic Hepatitis The effectiveness of interferon therapy against chronic hepatitis C virus infection was shown in many studies including ours. In 1994, we set up a national surveillance program for HCC development among chronic hepatitis C patients and enrolled about 2 900 biopsy-proven cases. 2 400 of them received interferon treatment, showing a sustained virologic response rate of 33% on average. The risk of HCC was reduced by half among the interferon-treated patients

Evolution history of the volcano is essential not only to characterize the volcano, but also consider magma genesis beneath the volcano. Most of the stratovolcanoes in northeast Japan follow a general evolutional course: cone building, horse-shoe shaped caldera forming collapse, and post-caldera stages. However, the detailed history of each stage is not well investigated. We investigated evolution history of young edifice of Gassan volcano, representative stratovolcano in rear side of northeast Japan arc. Most of the products are lavas, which are divided into two groups by geomorphologic and geologic features. The former (Gassan lower lavas) is composed of relatively thin and fluidal lavas, whose original geomorphology remains a little, while the latter (Gassan upper lavas) is composed of relatively thick and viscous lavas, whose original geomorphology is moderately preserved. Based on geologic features, the upper lavas can be further divided into Gassan upper north lavas and upper summit lavas in ascending order. After the formation of the thick lavas, horse-shoe shaped caldera was formed by the instability of the edifice, probably triggered by fault activity. No evidence of post caldera activity inner part of it is observed. Based on K-Ar data, estimated age of Gassan lower lavas is ca. 0.75 to ca. 0.6 Ma, those of Gassan upper north and upper summit lavas are ca. 0.60 to ca. 0.55 Ma and ca. 0.55 to ca. 0.45 Ma. The eruption rate is estimated to be ca. 0.0004 km3/1000 years in Gassan lower lavas and ca. 0.02 km3/1000 years in Gassan upper summit lavas. These values are lower than the eruption rate of representative Japanese stratovolcanoes.

Differential evolution （DE） is a global optimizer for continuous design variables. To enhance DE, it is necessary to handle discrete design variables. In this paper, a discrete differential evolution （DDE） algorithm is proposed to handle discrete design variables The proposed DDE is based on the DE/l/rand/bin method. In the proposed DDE, the mutation ratio is regarded as the exchange probability, and thus, no modifications of DE/l/rand/bin are required. In addition, in order to maintain diversity through the search process, we initialize all search points. By introducing the initialization of all search points, global or quasi-optimum solution can be found. We validate the proposed DDE by applying it to several benchmark problems.

Since the mechanisms of the developmental processes of tumors remain unclear, early detection and early treatment of the tumors is necessary to save patients with malignant tumors. Therapies currently available to patients are surgery, chemotherapy, and radiotherapy. However, there are many patients who cannot be saved by such therapies. In this study, we found the common features of various tumor tissues, and we demonstrated the effect of therapeutics that target them by using experimental rats with spontaneous tumors. 26 kinds of human tumors (epithelial or mesenchymal origin, and malignant or benign) and Sprague-Dawley rats with spontaneous mammary gland tumors were examined by light and electron microscope. To detect of mast cells and leukotriene receptor, toluidine blue stain and immunohistochemical stain were performed. The rats were orally administered one of leukotriene receptor antagonists. We found that the presence of numerous mast cells and expression of leukotriene receptors in various tumor (human tumors and rat spontaneous tumors). And the therapeutic effects, which suppressed not only tumor progress but also angiogenesis and nerve formation, for rat tumors by administration of leukotriene receptor antagonist could obtain. Our data suggest the possibility that allergic reactions, in which leukotriene and leukotriene receptors in particular appear to play an important role, are involved in the development and progression of tumors and that it may be possible to control tumor progression by regulating such reactions. Our results might be useful for clinical applications of oncotherapy.

A 42-year-old Japanese man with liver cirrhosis by hepatitis C virus (HCV) had successful interferon therapy in May 1991. Since then, serum HCV-RNA and liver function tests had been negative. He had continued to drink more than 100 g/d of alcohol as before. In June 2003, a 5-cm tumor was found in the posterior segment of the liver. The tumor was curatively resected and the surgical specimen showed a well-differentiated hepatocellular carcinoma (HCC). Non-cancerous lesions of the liver revealed fibrosis at stage F3 with minimal to mild inflammation of grade A1. Heavy drinking may retard the dissolution of fibrosis and accelerate HCC development in patients with sustained virological response.

At present time, there has been a demand for management systems that can survey and monitor a PC （personal computer） practice room, movement of people in an office, situation of utilization of facilities and so on in real time without causing psychological stress. For example, PC administrators in Japanese national educational institutions must report operation status of PC practice rooms once a year. But, there is currently no system for automatically recording PC operating situations. Therefore, the burden on the PC administrators is big. In this study, we aimed at systems for accurately managing the sitting and work time without psychologically stressing PC users. This time, we propose uniform management systems of sitting and work time using smart tap node and mat sensor node by IoT （Internet of Things） technology. The smart tap was connected to PCs to acquire the operating status of the PCs. In parallel with this smart tap, the mat sensor was used to acquire human presence state. By calculating binary data representing the PC operating status and human presence state from the two sensing data （smart tap and mat sensor） by the proposed technique, we can exactly calculate wasteful power consumption etc. The use of IoT technology makes it unnecessary to use large installation services when introducing our system. Therefore, this our proposal system can be easily installed even by unskilled workers.

A New Therapy for Human Endometriosis: The Therapeutic Value of Leukotriene Receptor Antagonist for Endometriosis

Some drugs that regulate estrogen are currently used as therapy for endometriosis. However, these medical treatments have significant side effects and patients who hope to become pregnant cannot overcome infertility. To compare morphological alterations and clinical symptoms, revised American Fertility Society (r-AFS) scores were compared between patients with and without leukotriene receptor antagonist (LTR-A) treatment. LTR-A-treated cases showed significantly decreased r-AFS score. Furthermore, a significant correlation was seen between r-AFS score and LTR-A treatment period. In treated cases, clinical symptoms decreased and some patients achieved pregnancy. Morphologically, lesions in LTR-A-treated cases showed apoptotic fibroblasts and degeneration of collagen fibers. Our findings revealed that LTR-As had significant therapeutic value for the treatment of human endometriosis. Anti-LT therapy appears efficacious not only in the treatment of clinical symptoms and lesions, but also in improving fertility.

In recent years, maritime transportation has played an important role in global economy development. As a result, ship traffic has become more congested. Moreover, ship navigation is susceptible to weather and environmental conditions, and in some cases, it may become dangerous. Therefore, vessels are subjected to high-risk navigation conditions. To understand the latent risk of ship navigation, this study focused on the actual ship behavior. Thus, an analysis of ship behavior was carded out using historical ship navigation based on automatic identification system data. Consequently, a dynamic analysis of the speed and encounter situation was performed. One of the main results of this work was the understanding of the latent risk involved in ships navigating the Seto Inland Sea, which is one of the most congested routes in Japan. Moreover, the risk areas were obtained, and visualized using a geographical information system. The obtained results can be applied to ensure safe navigation and the development of a safe and efficient navigation model.
