BACKGROUND Emergency pancreaticoduodenectomy(EPD)is a rare event for complex periam-pullary *** intraoperative blood loss is correlated with poor post-operative *** SUMMARY Two patients underwent EPD using a no-touch isolation technique,in which all arteries supplying the pancreatic head region were ligated and divided before manipulation of the pancreatic head and *** operative times were 220 and 239 min,and the blood loss was 70 and 270 g,*** patients were discharged on the 14^(th) and 10^(th) postoperative day,***-two patients underwent EPD for the treatment of neoplastic *** mean operative time was 361.6 min,and the mean blood loss was 747.3 *** comp-lication rate was 37.5%.The in-hospital mortality rate was 9.38%.CONCLUSION The no-touch isolation technique is feasible,safe,and effective for reducing intraoperative blood loss in EPD.

In order to screen the antioxidant capacity of commonly consumed indigenous foods of tropical region, vegetables, fruits, tea, oil seeds, legumes, spices and cereal were analyzed. Among the vegetables, green chili showed the highest total oxygen radical absorbance capacity （ORACFL） and lipophilic ORAC （L-ORACFL）; whereas, bean showed highest hydrophilic ORAC （H-ORACFL） content. The H-ORACFL of vegetable samples varied between 2.26 to 23.08 ~tmol of Trolox equivalent （TE）/g of fresh weight （FW） and L-ORACFL content widely varied from 0.20 （sweet pumpkin） to 16.95 （green chili）. The range of H-ORACFL values of fruits showed large variation （0.45-178 Ixmol of TE/g FW）. All four different types of tea showed high H-ORACFL （1,295.84-2,709.11 lamol of TE/g）, in contrast to negligible amount of L-ORACFL. In legumes, H-ORACFL content ranged from 16.22 to 140.54; whereas, L-ORACFL ranged from 4.43 to 25.78. Porso millets showed higher L-ORACFL value （19.4） as compared to H-ORACFL （0.8）. In case of spices, L-ORACFL content varied from 29.75 ± 0.00 to 1,575.24 ± 39.63, which is considerably higher as compared to H-ORACFL （61.68 ± 1.71 to 272.82± 1.05） content. Comparatively, spices showed much higher H-ORACFL values as compared to fruits analyzed. In case of selective vegetables, fruits and tea samples, the total phenol content was highly correlated with the H-ORACFL content.

An efficient fibrous catalyst for the biodiesel fuel production has been synthesized by radiation-induced graft polymerization of 4-chloromethylstyrene onto a nonwoven polyethylene (NWPE) fabric followed by amination with trimethylamine (TMA) and further treatment with NaOH. The degree of grafting of NWPE fabric and TMA group density of fibrous catalyst could easily and reproducibly be controlled within a range of up to 340% and 3.6 mmol-TMA/g-catalyst, respectively. In the transesterification of triglycerides and ethanol using the synthesized fibrous catalyst, the conversion ratio of triglycerides reached 95% after 4 h reaction at 50°C.

AIM： To elucidate the sequential gene expression profile in AGS cells co-cultured with wild-type Helicobacter pylori （H pylon） as a model of Hpylori-infected gastric epithelium, and to further examine the contribution of cag-pathogenicity islands （cagPAI）-coding type IV secretion system and the two pathways, nuclear factor kappa B （NF-κB） and extracellular signal-regulated kinases （ERK） on wild-type Hpylori-induced gene expression. METHODS： Gene expression profiles induced by Hpylori were evaluated in AGS gastric epithelial cells using cDNA microarray, which were present in the 4600 independent clones picked up from the human gastric tissue. We also analyzed the contribution of NF-κB and ERK signaling on H pylori-induced gene expression by using inhibitors of specific signal pathways. The isogenic mutant with disrupted cagE （△cagE） was used to elucidate the role of cagPAI-encoding type IV secretion system in the gene expression profile. RESULTS： According to the expression profile, the genes were classified into four clusters. Among them, the clusters characterized by continuous upregulation were most conspicuous, and it contained many signal transducer activity-associated genes. The role of cagPAI on cultured cells was also investigated using isogenic mutant cagE, which carries non-functional cagPAI. Then the upregulation of more than 80% of the induced genes （476/566） was found to depend on cagPAI. Signal transducer pathway through NF-κB or ERK are the major pathways which are known to be activated by cagPAI-positive H pylori. The role of these pathways in the whole signal activation by cagPAI-positive H pyloriwas analyzed. The specific inhibitors against NF-κB or ERK pathway blocked the activation of gene expression in 65% （367/566） or 76% （429/566） of the genes whose activation appealed to depend on cagPAI. We previously studied the mechanism underlying the adsorption of oral bacteria on the surfaces of dental prosthetic materials such as ceramics and resins in vitro. The aim of the present study was to examine bovine serum albumin (BSA) adsorption on crown composite resin surfaces by means of zeta potential. We measured the zeta potentials of resins alone, BSA alone, and resins after BSA adsorption. Eight resins were pulverized into powders (300 - 1000 nm). All experiments were conducted in 10 mM sodium chloride solution (pH 6.5). BSA was dissolved in 10 mM NaCl with a concentration of 2.0 × 10-5 mol/l. An adsorption assay was performed for one hour at 37°C under continuous rotation (6 rpm). The zeta potentials of both resins and BSA were negative, with BSA itself less negative than the resins themselves as an absolute value (p < 0.0001). The zeta potentials of seven resin surfaces after BSA adsorption were significantly less negative than were those of the resins without BSA adsorption (p < 0.0001). Eight resins were divided into two classes based on the size of the surface potential difference between each resin and the BSA. The difference in surface potential between the resins and the BSA were small, leading to the theory that particles with identical charges repulse each other, and the amounts of adsorbed BSA on these resins might be less. On the other, when the differences between the other resins and BSA are large, so that the repulsive force between two nonidentical particles becomes zero and an attractive force might be generated, then more BSA might be adsorbed on those resins. Therefore, the zeta potentials were affected by BSA adsorption and became less negative. These results suggested that electrostatic interactions play an important role in the adsorption of BSA on resin surfaces.

Assessment of disease progression in patients with transfusion-associated chronic hepatitis C using transient elastography

METHODS： Between December 2006 and June 2008, a total of 524 transfusion-associated HCV-RNA positive patients with or without HCC were enrolled, Liver stiffness was obtained noninvasively by using Fibroscan （Echosens, Paris, France）, The date of blood transfusion was obtained by interview, Duration of infection was derived from the interval between the date of bloodtransfusion and the date of liver stiffness measurement （LSM）. Patients were stratified into four groups based on the duration of infection （17-29 years; 30-39 years; 40-49 years; and 50-70 years）. The difference in liver stiffness between patients with and without HCC was assessed in each group. Multiple linear regression analysis was used to determine the factors associated with liver stiffness.RESULTS： A total of 524 patients underwent LSM. Eight patients were excluded because of unsuccessful measurements. Thus 516 patients were included in the current analysis （225 with HCC and 291 without）. The patients were 244 men and 272 women, with a mean age of 67.8 ±9.5 years. The median liver stiffness was 14.3 kPa （25.8 in HCC group and 7.6 in non HCC group）. The patients who developed HCC in short duration of infection were male dominant, having lower platelet count, with a history of heavier alcohol consumption, showing higher liver stiffness, and receiving blood transfusion at an old age. Liver stiffness was positively correlated with duration of infection in patients without HCC （r = 0.132, P = 0.024） but not in patients with HCC （r = -0.103, P = 0.123）. Liver stiffness was significantly higher in patients with HCC than in those without in each duration group （P 〈 0.0001）. The factors significantly associated with high liver stiffness in multiple regression were age at blood transfusion （P 〈 0.0001）, duration of infection （P = 0.0015）, and heavy alcohol consumption （P = 0.043）CONCLUSION： Although liver stiffness gradually increases over time, HCC develops in patients with high stiffness value regardle

Purpose: The internal anal sphincter provides most of the resting anal tone and is the main muscle responseble for continence. This study was designed to estimate the prevalence of, and identify risk factors associated with, internal anal sphincter dysfunction in Japanese adults. Methods: Anorectal manometry was performed in 1193 women and 1124 men aged 20 years or older. The maximal resting pressure, measured by a rapid pull-through technique, was defined as the highest resting pressure recorded. Internal anal sphincter dysfunction was defined as a maximal resting pressure less than 30 mmHg. Potential risk factors were assessed through self-reports, interviews, physical examinations, and medical record reviews. Multivariate logistic regression analysis was used to identify independent risk factors for internal anal sphincter dysfunction. Results: Significant differences in maximal resting pressure were seen between women (58.1 ± 24.9 mmHg) and men (68.8 ± 23.5 mmHg, P < 0.001). Maximal resting pressure decreased significantly with increasing age in both sexes. The prevalence of internal anal sphincter dysfunction was 10.4% (15.5% in women, 5.1% in men). In a multivariate logistic regression model, age, mental disease, pelvic organ prolapse repair, and fecal incontinence were independently associated with a greater risk of internal anal sphincter dysfunction in women and men. Conclusions: Internal anal sphincter dysfunction is a common problem for women and men. Several of the identified risk factors are preventable or modifiable, and may direct future research in fecal incontinence therapy.
