Predictive findings forHelicobacter pylori-uninfected, -infected and -eradicated gastric mucosa: Validation study

AIM:To validate the usefulness of screening endoscopy findings for predictingHelicobacter pylori (H. pylori) infection status. METHODS:H. pylori infection status was determined by histology, serology, and the urea breast test in 77 consecutive patients who underwent upper endoscopy. Based on the findings, patients were categorized as H. pylori -uninfected, -infected, or -eradicated cases. Using six photos of certain sites in the stomach per case, we determined the presence or absence of the following endoscopic findings:regular arrangement of collecting venules (RAC), linear erythema, hemorrhage, fundic gland polyp (FGP), atrophic change, rugal hyperplasia, edema, spotty erythema, exudate, xanthoma, and mottled patchy erythema (MPE). The diagnostic odds ratio (DOR) and inter-observer agreement (Kappa value) for these 11 endoscopic findings used in the determination of H. pylori infection status were calculated. RESULTS:Of the 77 patients [32 men and 45 women; mean age (SD), 39.7 (13.4) years] assessed, 28 were H. pylori uninfected, 28 were infected, and 21 were eradicated. DOR values were significantly high (< 0.05) for the following H. pylori cases:uninfected cases with RAC (11.5), linear erythema (24.5), hemorrhage (4.1), and FGP (34.5); for infected cases with atrophic change (8.67), rugal hyperplasia (15.8), edema (14.2), spotty erythema (11.5), and exudate (3.52); and for eradicated cases with atrophic change (32.4) and MPE (103.0). Kappa values were excellent for FGP (0.93), good for RAC (0.63), hemorrhage (0.79), atrophic change (0.74), and MPE (0.75), moderate for linear erythema (0.51), rugal hyperplasia (0.49), edema (0.58), spotty erythema (0.47), and exudate (0.46), and poor for xanthoma (0.19). CONCLUSION:The endoscopic findings of RAC, hemorrhage, FGP, atrophic change, and MPE will be useful for predicting H. pylori infection status.

In this paper,we address the problem that standard stochastic Landau-Lifshitz-Gilbert(sLLG)simulations typically produce results that show unphysical mesh-size *** root cause of this problem is that the effects of spin-wave fluctuations are ignored in *** propose to represent the effect of these fluctuations by a full-spin-wave-scaled stochastic LLG,or FUSSS LLG *** FUSSS LLG,the intrinsic parameters of the sLLG simulations are first scaled by scaling factors that integrate out the spin-wave fluctuations up to the mesh size,and the sLLG simulation is then performed with these scaled *** developed FUSSS LLG by studying the Ferromagnetic Resonance(FMR)in Nd_(2)Fe_(14)B *** nominal scaling greatly reduced the mesh size dependence relative to *** then performed three tests and validations of our FUSSS LLG with this modified scaling.(1)We studied the same FMR but with magnetostatic fields included.(2)We simulated the total magnetization of the Nd_(2)Fe_(14)B cube.(3)We studied the effective,temperature-and sweeping rate-dependent coercive field of the *** all three cases,we found that FUSSS LLG delivered essentially mesh-size-independent results,which tracked the theoretical expectations better than unscaled *** by these successful validations,we propose that FUSSS LLG provides marked,qualitative progress towards accurate,high precision modeling of micromagnetics in hard,permanent magnets.

Imaging of community-acquired pneumonia: Roles of imaging examinations, imaging diagnosis of specific pathogens and discrimination from noninfectious diseases

This article reviews roles of imaging examinations in the management of community-acquired pneumonia(CAP), imaging diagnosis of specific CAP and discrimination between CAP and noninfectious diseases. Chest radiography is usually enough to confirm the diagnosis of CAP, whereas computed tomography is required to suggest specific pathogens and to discriminate from noninfectious diseases. Mycoplasma pneumoniae pneumonia, tuberculosis, Pneumocystis jirovecii pneumonia and some cases of viral pneumonia sometimes show specific imaging findings. Peribronchial nodules, especially tree-in-bud appearance, are fairly specific for infection. Evidences of organization, such as concavity of the opacities, traction bronchiectasis, visualization of air bronchograms over the entire length of the bronchi, or mild parenchymal distortion are suggestive of organizing pneumonia. We will introduce tips to effectively make use of imaging examinations in the management of CAP.

Introduction of Multiscaled Longitudinal Vortices by Fractal-Patterned Surface Roughness

To determine the type of surface roughness pattern that is suitable for adaptive suppression of the drag of an obstacle, we observed flow structures introduced by such obstacles. Several roughness patterns were tested: geometric patterns, fractal patterns, reptile-skin patterns, and patterns of circular cylinders arranged in a lattice and in a zigzag manner. A suitable pattern for adaptive control of flow is one that generates longitudinal vortices with nonconstant distances. The preferred instability mode of a laminar boundary layer is expected to be selected automatically from fluctuations involving many frequencies and caused by fractal patterns. Snake- and reptile-skin patterns may have a similar ability as fractal patterns because they consist of multiscale patterns. The longitudinal vortices generated from peculiar positions and concave corners in patterns were observed. The distance between these vortices is not constant because the onset of vortices is at concave corners in fractal patterns. These vortices have differing strengths and easily cause nonlinear interactions, so they can disturb a laminar boundary layer with several higher-harmonic frequencies. The velocity profiles of the laminar boundary-layer flow over the fractal patterns were measured by using hydrogen bubbles. The results show the down-wash flow between the longitudinal vortices, which means that the vortices may effectively suppress the boundary layer separation in an adverse pressure gradient.

Influence of NUDT15 variants on hematological pictures of patients with inflammatory bowel disease treated with thiopurines

AIM The single nucleotide polymorphism(SNP) c.415C>T in exon 3 of NUDT15 affects thiopurine-induced leukopenia in Asian patients with Crohn's disease. Meanwhile, three additional genetic variants of NUDT15 were reported in patients with acute lymphoblastic leukemia. We evaluated the effects of these additional genetic variants of NUDT15 in patients with inflammatory bowel disease(IBD) treated with *** Ninety-six Japanese patients with IBD were enrolled. Genotyping for the NUDT15 and TPMT genes was performed using Custom Taq Man SNP genotyping assays or Sanger sequencing. The changes in white blood cell(WBC) count, mean corpuscular volume(MCV), platelet count, hemoglobin, CRP, amylase, albumin, AST, ALT, and ESR were *** Genetic variants of exon 1 and exon 3 of NUDT15 were identified in 24 of 96 patients(25.0%). C.52G > A and c.36_37 insG GAGTC in exon 1 were found in three patients each. All three patients with c.36_37 insG GAGTC in exon 1 were heterozygotes of ***139 Cys in exon 3. Eighteen patients had ***139 Cys in exon 3 alone. The WBC count gradually decreased after initiation of thiopurine treatment in the mutated cases(n = 24), and was significantly lower at 6, 8, 10, and 16 wk(P = 0.0271, 0.0037, 0.0051, and 0.0185, respectively). The WBC counts were also evaluated in patients with and without prednisolone treatment. In the patients with prednisolone treatment, the WBC count tended to show a greater decrease in the mutated cases, with significant differences at 8 and 10 wk(P = 0.012 and 0.029, respectively). In the patients without prednisolone treatment, the WBC count was significantly lower at 2, 4, 8, and 14 wk in mutated cases(P = 0.0196, 0.0182, 0.0237 and 0.0241, respectively). MCV increased after starting thiopurine treatment in the mutated cases, and was significantly higher at 10 wk(P = 0.0085). Sufficiently Elevated Core Body Temperature May Be Necessary to Maintain Cerebral Blood Flow Response throughout the Morning

In comparison to a carbohydrate-rich breakfast, a nutritionally balanced breakfast reportedly leads to a higher core body temperature because of diet-induced thermogenesis (DIT) and also results in higher task performance. This study aimed to examine the relationships among the core body temperature, blood glucose level, cerebral blood flow, and cognitive performance when the core body temperature is raised to a similar extent as in DIT in the morning. This crossover study included 18 male participants who performed four sets of cognitive tests in the morning with four different foot baths and glucose intake conditions. In elevated body temperature (EBT) conditions, the core body temperature was increased by a foot bath at 42˚C or 39˚C, while in low body temperature (LBT) conditions, it was maintained at 35˚C by a foot bath;the participants received no glucose or two intakes of 20-g glucose for each thermal condition. In addition to the core body temperature measurement, the cerebral blood flow in the dorsolateral prefrontal cortex (DLPFC) was measured using near-infrared spectroscopy. Three blood collections were performed to measure the changes in blood glucose levels. The results revealed that in the EBT conditions, the core body temperature remained 0.3˚C - 0.5˚C higher than that at wake-up time, while the glucose intake conditions increased blood glucose levels which remained higher than those during fasting. No significant between-treatment difference was observed in the results of cognitive tests. However, the blood flow in the DLPFC increased during the second test period in the EBT/glucose and LBT/glucose conditions, whereas during the fourth test period, it increased solely in the EBT/glucose condition. Thus, in addition to the blood glucose level, an elevated core body temperature within the physiological range may be needed for long-term maintenance of the cerebral blood flow response.

The photovoltaic performance of perovskite solar cells(PSCs)can be improved by utilizing efficient front ***,it has always been a significant challenge for fabricating high-quality,scalable,controllable,and cost-effective front *** study proposes a realistic multi-layer front contact design to realize efficient single-junction PSCs and perovskite/perovskite tandem solar cells(TSCs).As a critical part of the front contact,we prepared a highly compact titanium oxide(TiO2)film by industrially viable Spray Pyrolysis Deposition(SPD),which acts as a potential electron transport layer(ETL)for the fabrication of *** and reproducibility of the TiO2 ETL were discreetly investigated while fabricating a set of planar *** the front contact has a significant influence on the optoelectronic properties of PSCs,hence,we investigated the optics and electrical effects of PSCs by three-dimensional(3D)finite-difference time-domain(FDTD)and finite element method(FEM)rigorous *** investigation allows us to compare experimental results with the outcome from ***,an optimized single-junction PSC is designed to enhance the energy conversion efficiency(ECE)by>30% compared to the planar reference ***,the study has been progressed to the realization of all-perovskite TSC that can reach the ECE,exceeding 30%.Detailed guidance for the completion of high-performance PSCs is provided.

Analysis of aristolochic acids, aristololactams and their analogues using liquid chromatography tandem mass spectrometry

More than 80 aristolochic acids(AAs) and aristololactams(ALs) have been found in plants of the Aristolochiaceae family, but relatively few have been fully studied. The present study aimed at developing and validating a liquid chromatography tandem mass spectrometry(LC/MS^n) for the analysis of these compounds. We characterized the fragmentation behaviors of 31 AAs, ALs, and their analogues via high performance liquid chromatography coupled with electrospray ionization mass spectrometry. We summarized their fragmentation rules and used these rules to identify the constituents contained in Aristolochia contorta, Ar. debilis, Ar. manshurensis, Ar. fangchi, Ar. cinnabarina, and Ar. mollissima. The AAs and ALs showed very different MS behaviors. In MS1 of AAs, the characteristic pseudomolecular ions were [M + NH_4]^+, [M + H]^+, and [M + H - H_2O]^+. However, only [M + H]^+ was found in the MS1 of ALs, which was simpler than that of AAs. Distinct MSn fragmentation patterns were found for AAs and ALs, showing the same skeleton among the different substituent groups. The distribution of the 31 constituents in the 6 species of Aristolochia genus was reported for the first time. 25 Analogues of AAs and ALs were detected in this genus. A hierarchical schemes and a calculating formula of the molecular formula of these nitrophenanthrene carboxylic acids and their lactams were proposed. In conclusion, this method could be applied to identification of similar unknown constituents in other plants.
