Efficacy and safety of endoscopic submucosal dissection for gastric tube cancer:A multicenter retrospective study

BACKGROUND Recent improvements in the prognosis of patients with esophageal cancer have led to the increased occurrence of gastric tube cancer(GTC)in the reconstructed gastric ***,there are few reports on the treatment results of endoscopic submucosal dissection(ESD)for *** To evaluate the efficacy and safety of ESD for GTC after esophagectomy in a multicenter *** We retrospectively investigated 48 GTC lesions in 38 consecutive patients with GTC in the reconstructed gastric tube after esophagectomy who had undergone ESD between January 2005 and December 2019 at 8 institutions participating in the Okayama Gut Study *** clinical indications of ESD for early gastric cancer were similarly applied for GTC after *** specimens were evaluated in 2-mm slices according to the Japanese Classification of Gastric Carcinoma with curability assessments divided into curative and non-curative resection based on the Gastric Cancer Treatment *** characteristics,treatment results,clinical course,and treatment outcomes were *** The median age of patients was 71.5 years(range,57-84years),and there were 34 men and 4 *** median observation period after ESD was 884 d(range,8-4040 d).The median procedure time was 81 min(range,29-334 min),the en bloc resection rate was 91.7%(44/48),and the curative resection rate was 79%(38/48).Complications during ESD were seen in 4%(2/48)of case,and those after ESD were seen in 10%(5/48)of *** survival rate at 5 years was 59.5%.During the observation period after ESD,10 patients died of other *** there were differences in the procedure time between institutions,a multivariate analysis showed that tumor size was the only factor associated with prolonged procedure *** ESD for GTC after esophagectomy was shown to be safe and effective.

Distribution characteristics and source identification of polychlorinated dibenzo-p-dioxin and dibenzofurans,and dioxin-like polychlorinated biphenyls in the waters from River Kanzaki,running through Osaka urban area,Japan

This is the first to elucidate the distribution and sources of polychlorinated dibenzo-p-dioxins and dibenzofurans （PCDD/PCDFs）,and dioxin-like polychlorinated biphenyls in the waters from Kanzaki River,which is one of the most heavily polluted rivers in *** World Health Organization （WHO）-toxic equivalent quantities （TEQs） in waters from the mainstream exceeded the Japanese environmental standard （1.0 pg-TEQ/L）.The PCDD/PCDFs were dominated by highly chlorinated DFs,which predominantly contributed to the WHO-TEQs,suggesting that the main causes would be the incineration-related *** find the sources,the dioxin congener concentrations in water and sediment samples from its tributary small waterways were *** high WHO-TEQs were detected in a water （50 pg-TEQ/L） and a sediment sample （41,000 ng-TEQ/kg dry weight） near the industrial solid waste incinerators （ISWIs）.The PCDD/PCDF characteristics agreed well with those of the incinerator-related wastes as seen in the *** facts indicate that the dioxin pollution in the mainstream could be largely related to the industrial wastes from the ***,a TEQ apportionment method was used to understand the contribution of the pyrogenic sources to the *** average contribution ratios of the pyrogenic sources to WHO-TEQs were more than 80% for river waters from the mainstream,indicating that the elevated WHO-TEQs in the mainstream had been largely caused by the ISWIs.

Efficient Fixation of Carbon Dioxide by Electrolysis——Facile Synthesis of Useful Carboxylic Acids

Electrochemical fixation of atmospheric pressure of carbon dioxide to organic compounds is a useful and attractive method for synthesizing of various carboxylic acids. Electrochemical fixation of carbon dioxide, electrochemical carboxylation, organic halides, organic triflates, alkenes, aromatic compounds, and carbonyl compounds can readily occur in the presence of an atmospheric pressure of carbon dioxide to form the corresponding carboxylic acids with high yields, when a sacrificial anode such as magnesium or aluminum is used in the electrolysis. The electrochemical carboxylation of vinyl bromides was successfully applied for the synthesis of the precursor of nonsteroidal anti-inflammatory agents such as ibuprofen and naproxen. On the other hand, supercritical carbon dioxide （scCO2） has significant potential as an environmentally benign solvent in organic synthesis and it could be used both as a solvent and as a reagent in these electrochemical carboxylations by using a small amount of cosolvent.

Creep-fatigue interaction properties of nickel-based superalloy 617

High-temperature fatigue properties were investigated for a nickel-based superalloy 617 as a typical candidate material for high-temperature components using an advanced ultra super critical （A-USC） power plant. Creep-fatigue data obtained by strain-controlled tests at 700 ？ C was analyzed for the superalloy 617 focused on position of hold at peak strain in comparison with continuously cycled tests. The fatigue life was the shortest with the hold tensile strain wave, which showed mainly intergranular fracture appearance. The life was the longest with symmetrical triangular wave which fractured from the specimen surface, while the wave with the hold compressive strain showed partially intergranular fracture appearance and the life was middle in the both waves. In this study, it is discussed with the creep-fatigue interaction for the superalloy 617.

Capillary filling in closed-end nanotubes

Capillary filling in small length scale is an important process in nanotechnology and microfabrication. When one end of the tube or channel is sealed, it is important to consider the escape of the trapped gas. We develop a dynamic model on capillary filling in closed-end tubes, based on the diffusion-convection equation and Henry＇s law of gas dissolution. We systematically investigate the filling dynamics for various sets of parameters, and compare the results with a previous model which assumes a linear density profile of dissolved gas and neglect the convective term.

Design of an Interpolated Controller for Stabilization of a Plant with Variable Operating Condition

In this paper, the stabilization of a linear SISO plant with variable operating condition is considered. The plant is described by a linear interpolation of proper stable co-prime factorizations of the transfer functions at two representative operating points. An interpolation of the stabilizing controllers for the representative models is designed to stabilize the plant, and the necessary and sufficient condition for the plant to be stabilized by the proposed controller is presented using the Nevanlinna-Pick interpolation theory. It is shown that the class of stabilization plants via the proposed controller in the paper is larger than that by the controller in reference. An example is also given to illustrate this fact.

INTEGRATED CONTROL FOR VEHICLE YAW MOTION USING DOUBLE-COST-FUNCTION LQR

The front steering angle control and its integration with the direct yaw moment control are suggested to enhance the handling performance for heavy-duty vehicles based on a 3D vehicle model. The double-cost-function LQR methodology (DLQR), which extends the concept and the applicable field of LQR to express the practical requirements more plainly, is proposed to determine the control input value: compensation value of front steering angle in the optimal model. A simple auto-tuning proportion controller is employed in direct yaw moment control to regulate the moment timely. The simulation results show that front steering angle control using DLQR has good ability to achieve the desired steering output in the different running conditions. DLQR is more powerful to balance many factors effectively based on the explicit mathematics expression of the physics of the problem than LQR does.

A new index for non-invasive assessment of liver fibrosis

AIM:To construct and evaluate a new non-invasive fibrosis index for assessment of the stage of liver f ibrosis. METHODS:A new f ibrosis index (Fibro-Stiffness index) was developed in 165 of 285 patients with chronic hepatitis C, and was validated in the other 120 patients where liver biopsy was performed. Its usefulness was compared with liver stiffness (LS) measured by FibroScan, the aminotransferase-to-platelet ratio index, the Forns index and the FibroIndex. RESULTS: The Fibro-Stiffness index consists of LS,platelet count and prothrombin time. The values of the Fibro-Stiffness index differed signif icantly between neighboring f ibrosis stages except F0-F1. The area under the receiver operating characteristics curves of the Fibro-Stiffness index for prediction of F≥2 (0.90), F≥ 3 (0.90) and F= 4(0.92) in the estimation group and those for F≥ 3 (0.93) and F =4 (0.97) in the validation group were the highest among the 5 methods examined. The accuracy of the Fibro-Stiffness index had highest values for F≥2, F≥3 and F=4 in both the estimation and validation groups. The diagnostic performance for F= 4 was improved by a combination of the Fibro-Stiffness index with serum hyaluronic acid level. CONCLUSION: The Fibro-Stiffness index was constructed and validated. It showed superior diagnostic performance to other indices for F ≥ 2,3 and 4.

Prediction of Aerodynamic Noise in a Ring Fan Based on Wake Characteristics

A ring fan is a propeller fan that applies an axial-flow impeller with a ring-shaped shroud on the blade tip side. In this study, the entire flow field of the ring fan is simulated using computational fluid dynamics (CFD); the accuracy of the CFD is verified through a comparison with the aerodynamic characteristics of a propeller fan of current model. Moreover, the aerodynamic noise generated by the fan is predicted on the basis of the wake characteristics. The aerodynamic characteristic of the ring fan based on CFD can represent qualitatively the variation in the measured value. The main flow domain of the ring fan is formed at the tip side of the blade because blade tip vortex is not formed at that location. Therefore, the relative velocity of the ring fan is increased by the circumferential velocity. The sound pressure levels of the ring fan within the frequency band of less than 200 Hz are larger than that of the propeller fan. In the analysis of the wake characteristics, it revealed that Karman vortex shedding occurred in the main flow domain in the frequency domain lower than 200 Hz ; the aerodynamic noise of the ring fan in the vortex shedding frequency enlarges due to increase in the relative velocity and the velocity fluctuation.
