AIM: An increase in bile ductular structures is observed in diverse human liver diseases, especially in primary biliary cirrhosis (PBC). These structures harbor the progenitor cell component of the liver. Caveolins are cholesterol-binding proteins involved in the regulation of several intracellular processes including cholesterol transport. This study aims to examine the role of caveolin in ***: Immunohistochemical and Western blottingstudies were performed on human liver specimens obtained from patients with PBC and normal liver samples. The expression of caveolin (CAV)-1 and -2 was determined using specific ***: In normal liver, scanty immunostaining for CAV1 and -2 was observed in bile ductules. In PBC liver samples, the expression levels of CAV-1 and -2 were increased on proliferating bile ductules especially in stage 3 cases, but was sparse on interlobular bile duct in stage 1 specimens. Especially, the regenerating bile ductules at the interface of portal tracts and necrotic areas were immunostained intensely for CAV-1 and -2. These phenomena were confirmed by Western ***: The present results demonstrate increased expression of caveolins in proliferating bile ductules in PBC, which may be related to the homeostasis of cholesterol transport in regenerating bile ductules in PBC liver.

Neuroendocrine carcinoma of the esophagus: Report of a case and review of the literature

＜正＞Objectives: We evaluated that feasibility of using Tc-99m tin colloid to identify sentinel nodes （SNs） gastrointestinal malignancies. Methods: The subjects were patients diagnosed with gastrointestinal malignancies considered to be surgically respectable with no clinically obvious nodal metastases. One hundred and sixty-seven patients with gastric cancer, 52 with esophageal cancer, and 61 with colorectal cancer were enrolled in this study. Tc-99m labeled tin colloid （Tc-99m: SnCl2=1:1） was endoscopically injected into the submucosal layer around the tumor. On the following day, patients underwent resection surgery, including resection of regional lymph nodes （LNs）. The radioactivity of resected LNs was measured using a well-type scintillation counter and /or a surgical probe. Any LN with a radioactivity higher than 1/10 of the highest one was defined as a SN. The pathological state of each of the resected LNs was examined using routine H-E staining. Results: The detectability of SNs as well as the sensitivity, false negative （FN） ratio, and diagnostic accuracy of the metastatic status of regional LNs as judged by the pathological results are shown below. Absolute subject numbers （positive cases/total cases examined） are shown in parentheses. Conclusion: Tc-99m tin colloid was able to detect over 90% of the SNs of gastrointestinal malignancies. We suggest that sentinel node concept could be applied to the gastrointestinal tumors as well as breast cancer and malignant melanoma.

Esophageal neuroendocrine carcinomas are rare, aggressive and have a poor prognosis. Combined therapy using chemotherapy, radiotherapy and/or surgery appear effective for them. Upper gastrointestinal endoscope of a 65-year-old male revealed a localized ulcerative lesion in the middle esophagus. Histology of biopsy specimens indicated a neuroendocrine carcinoma. A computed tomography showed an esophageal tumor with enlarged mediastinal lymph nodes. The patient was administered neoadjuvant chemo-therapy consisting of 5-fluorouracil and cisplatin, which led to partial response. Subtotal esophagectomy with three-field lymphadenectomy was performed. Pathologically, the tumor was 25 mm and infiltrated the proper muscle layer. The tumor cells were arranged in microtubular structures, with small and round cells containing scanty cytoplasm, and exhibited intense mitosis 51/10 HPF. They were positive for synaptophysin and chromogranin A, and Ki-67 labeling index was 70% - 80%. These findings led to the diagnosis of neuroendocrine carcinoma of small cell type. The patient was administered adjuvant chemotherapy using cisplatin and CPT-11, and he is now alive disease-free at the time of this writing.

AIM: To examine the expression of intercellular adhesion molecule-1 (ICAM-1) and lymphocyte function-associated antigen-1 (LFA-1) expression on canals of Hering (CoH)and bile ductules associated with the autoimmune process of bile duct destruction in primary biliary cirrhosis (PBC).METHODS: Ten wedged liver biopsies of PBC (five cases each of stages 2 and 3) were studied. The liver specimens were processed for transmission electron *** was performed using anti-ICAM-1 and anti-LFA-1 mouse mAbs. In situ hybridization was done to examine the messenger RNA expression of ICAM-1 in formalin-fixed, paraffin-embedded sections using peptide nucleic acid probes and the catalyzed signal amplification (CSA) technique. Immunogold-silver staining for electron microscopy was performed using anti-ICAM and anti-LFA-1 mouse mAbs. The immunogold particles on epithelial cells of bile ductules and cholangiocytes of CoH cells were counted and analyzed *** blotting was performed to confirm ICAM-1 protein ***: In liver tissues of PBC patients, immunohistochemistry showed aberrant ICAM-1 expression on the plasma membrane of epithelial cells lining bile ductules,and also on mature cholangiocytes but not on hepatocytes in CoH. LFA-1-positive lymphocytes were closely associated with epithelial cells in bile ductules. ICAM-1 expression at protein level was confirmed by Western blot. In situ hybridization demonstrated ICAM-1 mRNA expression in bile ductules and LFA-1 mRNA in lymphocytes infiltrating the bile ductules. By immunoelectron microscopy, ICAM-1 was demonstrated on the basal surface of epithelial cells in bile ductules and on the luminal surfaces of cholangiocytes in damaged CoH.
