AIM: To compare the clinical efficacy of the secondgeneration H2 RA lafutidine with that of lansoprazole in Japanese patients with mild gastroesophageal reflux disease(GERD). METHODS: Patients with symptoms of GERD and a diagnosis of grade A reflux esophagitis(according to the Los Angeles classification) were randomized to receive lafutidine(10 mg, twice daily) or lansoprazole(30 mg, once daily) for an initial 8 wk, followed by maintenance treatment comprising half-doses of the assigned drug for 24 wk. The primary endpoint was the frequency and severity of heartburn during initial and maintenance treatment. The secondary endpoints were the sum score of questions 2 and 3 in the Gastrointestinal Symptom Rating Scale(GSRS), and the satisfaction ***: Between April 2012 and March 2013, a total of 53 patients were enrolled, of whom 24 and 29 received lafutidine and lansoprazole, respectively. After 8 wk, the frequency and severity of heartburn was significantly reduced in both groups. However, lafutidine was significantly inferior to lansoprazole with regard to the severity of heartburn during initial and maintenance treatment(P = 0.016). The sum score of questions 2 and 3 in the GSRS, and satisfaction scores were also significantly worse in the lafutidine group than the lansoprazole group(P = 0.0068 and P = 0.0048, respectively).CONCLUSION: The clinical efficacy of lafutidine was inferior to that of lansoprazole, even in Japanese patients with mild GERD.

Comparison of Measured Trough Values after Deriving the Initial Dose Setting of Vancomycin with a Conventional Computer Software and a Nomogram Based on the Revised Japanese 2016 Therapeutic Drug Monitoring Guidelines for Antimicrobial Agents

Background: Due to the relatively high renal toxicity of vancomycin injection (VCM), setting an initial dose that achieves a trough that ranges between 10 and 20 μg/mL on day 3 is important to ensure safety and minimize side-effects, especially for patients with low renal function. To address these issues, the revised 2016 Therapeutic Drug Monitoring (TDM) Guidelines for Antimicrobial Agents (GL2016) proposed the use of a renal function-based, estimate glomerular filtration rate (eGFR) nomogram for setting the dose of VCM in Japan. Methods: Our hospital introduced the use of the GL2016 in September 2016 for the patients administered VCM. After setting the initial VCM dose using 1) a conventional VCM analysis software and 2) the GL2016 eGFR nomogram, the measured trough values on day 3 were compared and evaluated in this study. Results: With the VCM analysis software, the mean measured trough value in the a-total group (n = 53) was 12.8 ± 4.7 μg/mL. With the eGFR nomogram, the mean measured trough value in the b-total group (n = 13) was 9.6 ± 4.6 μg/mL. However, when the different severities of renal function were compared, the mean measured trough value was more significantly lower in the b-1 group than in the a-1 group among subjects with G2 and above (eGFR ≥ 60 mL/min/1.73 m2), but it was similar between the a-2 group and the b-2 group among subjects with G3 and below (eGFR 60 mL/min/1.73 m2). The proportion of subjects reaching the various trough ranges shows similar tendency. Conclusions: These data suggested that the measured trough value on day 3 was generally lower when the initial dose was established using the eGFR nomogram based on the GL2016, and this was especially prominent among patients with normal renal function. As for subjects with low renal function, the trough values were relatively high while ensuring safety.

Wind Turbine Power Output Assessment in Built Environment

In future planning of the city, it is very important to consider the proper intelligent integration of renewable energy sources into the built environment for developing smart cities. Analysis of the wind velocity profile in the built environment is very important for finding out the energy content in the wind and also to analyze the performance of wind turbines in the built environment. In this study, building topologies of smart city are investigated for finding out the wind velocity profile and the wind turbine power output in the built environment. The wind velocity distribution across buildings is numerically simulated by using commercial CFD (Computational Fluid Dynamics) software CFD-ACE+. Wind turbine power output is estimated by using the power curve of real commercial wind turbine and wind velocity distribution simulated by CFD software. It has been observed that the wind is accelerated in the intervening space between the buildings irrespective of distance between the walls of adjacent buildings under the condition, which are investigated in this study. The wind is accelerated across buildings, and is reduced rapidly after blowing through buildings, and recovered gradually. Since the wind is accelerated in the intervening space between buildings and reduced in the area at the back of buildings, a wind turbine should be installed at the area at the back of the buildings and located on center between the buildings. In this work, it is observed that size dimensions and layout of the building are effective in realizing a smart city for utilizing renewable energy such as wind turbine in the built environment.

Effect of zinc supplementation in patients with type C liver cirrhosis

Zinc is often deficient in patients with liver cirrhosis, and treatment with zinc provides short-term improvement in protein metabolism. However, the long-term effects of zinc have not been fully clarified. The present study aimed to analyze the effect of zinc on the long-term clinical course, especially hepatocarcinogenesis, in type C liver cirrhosis. Am- ong patients with type C liver cirrhosis visiting our hospital between June 1998 and January 2009, th- ose with a serum albumin level ≤3.5 g/dL and a serum zinc level ≤70 μg (1.07 μmol)/dL were selected. Thirty-seven patients were randomly divided into 2 groups: group B (12 g/day branched-chain amino acid granules) and group BZ (same as group B plus 100 mg/day - 600 mg/day zinc sulfate or 150 mg/day - 225 mg/day polaprezinc). Multivariate analysis revealed that the administration of zinc was not a significant determinant, but pretreatment serum zinc levels (hazard ratio [HR], 0.921;95% confidence interval [CI], 0.853–0.994) and serum zinc levels less than 80 μg (1.22 μmol)/dL 12 months after beginning this study (HR, 6.866;95% CI, 1.399 - 33.707) were significant determinants of carcinogenesis and death. Serum albumin levels in patients whose serum zinc levels had not increased up to 80 μg/dL by the third year of this study were significantly lower (p = 0.023) than those of patients that had increased up to 80 μg/dL. Conclusions: In type C liver cirrhosis with zinc deficiency, administration of zinc does not improve cancer-free survival. However, serum zinc levels can predict outcomes in patients with type C liver cirrhosis. However, although zinc may play a role in hepatocarcinogenesis, the precise implications remain to be clarified.

The film quality improvement of a-C layers for C/a-C superlattice solar cells has been investigated. A-C films were formed by C-cage breaking, and hydrogen radicals were irradiated during the a-C deposition. The hydrogen radical irradiation from the rf plasma source effectively reduce the spin defect density of a-C films in two orders of magnitude, from 2.7×10 cm to 2.0×l0 cm. Furthermore, it was suggested that hydrogen atoms affected with the reduction of structural disorder from the Raman scattering measurements. Therefore, it is thought that hydrogen atoms reduce the defect density by etching the weak carbon bond structures and/or passivating the dangling bonds.

＜正＞This paper describes the summary of "Green-Hydrogen （G-H）" project being developed by Waseda University together with several other universities and companies, which intends to establish "G-H" based society at Honjo-Waseda area in Saitama Prefecture. The project involves three tasks of technological development of;（1） process and facility for "G-H" production, （2） facility and system for "G-H" refining, transportation and storage, and （3） utilization system of the "G-H". In the project, "G-H" will be produced from waste aluminum, silicon, and biomass which are generated in the Honjo-Waseda area, then, be refined, transported, and stored by using high-performance metal hydride, which is synthesized in hydriding combustion process, bound with silicone, and will be utilized for economically feasible fuel cell systems such as mobility, traffic, cooling, and so on.

BACKGROUND A clutch cutter is a scissor-type knife used in endoscopic submucosal dissection(ESD)for gastrointestinal tract *** assistant during the ESD using a clutch cutter(ESD-C)needs to rotate the device and grasp the target tissue appropriately;therefore,the assistant’s skill may affect the technical outcomes of *** To determine how assistant skill level affected the technical outcomes of gastric ESD-C using an ex vivo porcine training *** In this pilot study,mock lesions of 15-30 mm in diameter were created in the middle or lower third of the porcine stomach.A total of 32 ESD-C procedures were performed by 16 *** trainee operator performed two ESD-C procedures;one ESD-C was assisted by an expert(ESD-C-E),and the other was assisted by a non-expert(ESD-C-NE).The total procedure time of the ESD was set as the primary outcome,and en bloc resection rate,complete procedure rate,perforation rate,and each procedure time/speed for mucosal incision or submucosal dissection were set as the secondary *** addition,we investigated factors associated with the difficulty of ESD including incompletion of ESD procedure,a long procedure time(≥20 min)or intraoperative *** The median total procedure time of the ESD-C-E was significantly shorter than that of the ESD-C-NE(12.9 min vs 21.9 min,P=0.001).The en bloc resection rate was 100%in both *** resection rates of the ESD-C-E and ESD-C-NE groups were 100%and 93.8%,*** intraoperative perforation was observed in both *** the multivariate analysis,assistant skill was significantly associated with the difficulty of ESD,with the highest odds ratio of *** Assistance by an expert is an important factor when trainees perform ESD-C procedures.

Spinal Epidural Abscess in Patients with Sepsis Who Experienced Significant Improvements after Complete Paralysis: Two Case Reports

Spinal epidural abscess (SEA) is a rare condition, and a delay in its diagnosis causes paralysis. In this study, we report two rare cases of delayed diagnosis of SEA whose conditions improved after a state of complete paralysis. The first case was a 71-year-old diabetic man who received a corticosteroid injection for shoulder pain that caused intensified pain. Thereafter, the patient developed paralysis of both legs in stage IV according to Heusner staging. Subsequently, he was diagnosed with multiple abscesses and sepsis. He was in a poor state of health. Therefore, we treated his epidural abscess conservatively. After a month, his muscle strength had improved to Heusner stage III-A, and he was transferred to another hospital. The second case was a 64-year-old diabetic man who received an epidural corticosteroid injection for lower back pain. However, the pain intensified and was admitted to the hospital for pyelonephritis. He developed paralysis in both of his arms and legs presenting as a Heusner stage IV, caused by a cervical epidural abscess. A laminoplasty was performed and paralysis was improved. However, the patient subsequently developed a left subcortical hemorrhage. He underwent surgery. However, his right hemiplegia persisted, and on Day 21, he was transferred back to the previous hospital with a Heusner III-A. We could improve the patients’ paralysis by cooperating closely with infectious disease specialists and spine surgeons, taking intensive care, applying antibacterial agents appropriately, and operating quickly.
