Evaluation of Prognostic Factor for Perforation Closure Rate in Patient with Chronic Otitis Media

Objective: To evaluate factors affecting the perforation closure rate for perforated otitis media. Methods: Between 2008 and 2012, a total of 118 patients with perforated chronic otitis media underwent tympanoplasty at Hyogo College of Medicine Hospital by a single surgeon. Prognostic factors were analyzed using multivariate analysis with logistic regression. Results: The success rate for tympanic perforation was 93.2%. Factors showing significance on multivariate analysis were dryness of perforation and patient age at the time of surgery. Conclusion: The results suggest that pre-surgical treatment is a key factor influencing the closure rate.

Background: With the recent aging of society, the need for medical treatment of elderly patients with head and neck cancer seems to have been increasing. Method: The present study analyzed all 103 patients with head and neck cancer ≥80 years, and we compared results with those of the previous generation (Group P;range: 75 - 79 years) comprising 104 patients treated in the same period. Results: We provided treatment just as wanted and could not choose it often. The reasons were oncological factors such as unresectable tumor or distant metastasis, refusal of treatment, and physical factors such as poor PS or number of comorbidities. Conclusion: Treatment choices should be based on the wishes and motivations of the patient and the medical assessment of physical function. When a patient ≥80 years old is treated, the high incidence of complications and severity of the disease should be considered.

目的:使用前庭诱发肌源性电位对骨导声反应(BC-VEMP)阐明耳硬化症患者平衡问题的起源。方法:受试者包括25例非进展性的耳硬化症患者(9例男性,16例女性),依据平衡问题的类型被分为两组。对两个组的耳蜗前庭功能的结果包括纯音测听、温度试验和BC-VEMP测试进行对比。结果:10例患者曾抱怨头晕和(或)眩晕(失调组),其他15例患者没有这种症状(非失调组)。失调组中9例患者在BC-VEMP测试中结果异常,而非失调组中1例患者结果异常(P<0.01)。结论:耳硬化症患者平衡问题的一个主要原因是球囊功能障碍。BC-VEMP的测试对于这些患者的诊断是有用的。

Objective: To evaluate the incidence of cervical angiopathy caused by radiation therapy for head and neck cancer. Methods: Segments of 57 cervical arteries were obtained during surgery for head and neck malignant tumors and divided into two groups (irradiated group and non-irradiated group) based on the treatment prior to vascular resection. In order to evaluate vascular injury after radiation therapy, we examined the degree of medial atrophy, medial fibrosis, smooth muscle cell (SMC) differentiation in the media and intima, intimal hyperplasia and endothelial cell (EC) injury. Sections of arterial segments were stained with hematoxylin-eosin, Elastica van Gieson and Masson’s trichrome, and immunohistochemistry for α-smooth muscle actin (α-SMA), smoothelin, S100A4 and CD31 in the resected vessels was conducted. Results: The median interval between the completion of radiation therapy and vascular resection was nine months. No significant differences were observed between the two groups in terms of medial atrophy, medial fibrosis and intimal hyperplasia. The ratio of the smoothelin-positive area per α-SMA-positive area in the media and the S100A4-positive proportion in the intima, indicating the degree of differentiation of the medial SMC and dedifferentiation of the intimal SMC, respectively, showed no significant differences, despite the tendency toward a lower smoothelin-positive area per α-SMA-positive area in the media of the irradiated arteries. The EC coverage revealed on CD31 immunohistochemistry was significantly decreased, with mural thrombus adhesion, in the irradiated group. Conclusions: The ECs of small arteries are damaged by irradiation. Although we did not confirm the statistical significance of medial SMC dedifferentiation, a decreased expression of smoothelin tended to be observed in the media of the irradiated arteries. Corporations need to improve business processes in order to enhance velocity and service levels while reducing their processing costs and differentiating themselves in the face of competition. The levitation of importance beyond support roles has raised IT investment decisions to high priority in chief executive officers＇ agendas. Corporate planning groups as well as lines of business are increasingly applying techniques of IT applications portfolio management in a more systematic fashion to improve decision-making and resource-allocation processes. Recent advances in software engineering and IT service delivery methodologies have achieved the logical separation of business functions from implementation. This separation has made a new breed of innovative IT project possible with a new project risk structure; the adjustment of portfolio management techniques is appropriate. We present an integrated portfolio management model so that the corporation can focus on organic growth through sources at both the department and top management levels. The research gives clear advice as to how top management can seek economic growth by selecting an entrepreneurial strategic posture, implying a strong risk-taking propensity. By integrating a risk-return model and risk-tolerance paradigm to cope with today＇s risk structure, overall capabilities can improve the decision process and the corporation＇s performance as well. The application of the integrated technique to a Japanese manufacturing firm is described.

The purpose of this paper is to do a report on applied present status and future assignment of recycled material and building structure in Japan.

Penetration analysis of elements and bioleaching treatment of spent refractory for recycling

Acidithiobacillusferrooxidans ATCC23270 and Acidithiobacillus thiooxidans TM-32 were used for bioleaching of spent refractories of aluminium and copper melting furnaces for their recycling. Firstly, penetration of elements into ahiminium melting furnace refractory was investigated and it was found that up to 7 cm from surface was contaminated. Comparison on leaching efficiency by the strains ATCC23270 and TM-32 found that the strain ATCC23270 could treat larger amount of the refractories than the strain TM-32 could do. In the experiment of bioleaching of spent refractory alumiulum melting furnace by the strain ATCC23270, high leaching efficiency were obtained on A1, Si, and Ca, and extremely low leaching performance was, however, shown on the rest of elements i.e., Na, Mn, and Zn. Under the strain TM-32 use, relatively high leaching performance was recognized on A1, Si, Ca, Na, Mn, and Zn. In the experiment of bioleaching for spent refractory copper melting furnace, almost the same leaching trends were shown on Cu, Zn, A1, and Si under the strains ACTT23270 and TM-32 uses.

A 62-year-old man with progressive thickening of the gallbladder wall visited our outpatient clinic. The biliary amylase level in the common bile duct was 19900 IU/L and that of the gallbladder was 127000 IU/L, although endoscopic retrograde cholangiopancreatography revealed no pancreaticobiliary maljunction. Histology demonstrated a moderately differentiated adenocarcinoma of the gallbladder. Pancreatobiliary reflux and associated gallbladder carcinoma were confirmed in the present case, in the absence of a pancreaticobiliary maljunction. Earlier detection of the pancreatobiliary reflux and progressive thickening of the gallbladder wall might have led to an earlier resection of the gallbladder and improved this patient＇s poor prognosis.

A new Dicroglossid species is described from Mawphlang,Meghalaya,northeastern India. The new species differs from all the congeners occurring in this region in body size and call characteristics. It is larger than Fejervarya syhadrensis and F. nepalensis,but smaller than F. teraiensis. It overlaps F. pierrei in snout-vent length,but clearly differs from it in advertisement call,with much shorter note duration,much longer inter note interval,and much lower dominant frequency. Paucity of genetic information and underestimation of species diversity in this region are discussed.
