The COVID-19 pandemic highlights the implications of chronic underinvestment in health workforce development,particularly in resource-constrained health *** health workforce diversity,insufficient training and remuneration,and limited support and protection reduce health system capacity to equitably maintain health service delivery while meeting urgent health emergency *** the Health Worker Life Cycle Approach provides a useful conceptual framework that adapts a health labour market approach to outline key areas and recommendations for health workforce investment-building,managing and optimising-to systematically meet the needs of health workers and the systems they *** also emphasises the importance of protecting the workforce as a cross-cutting investment,which is especially important in a health crisis like *** the global pandemic has spurred intermittent health workforce investments required to immediately respond to COVID-19,applying this‘lifecycle approach’to guide policy implementation and financing interventions is critical to centering health workers as stewards of health systems,thus strengthening resilience to public health threats,sustainably responding to community needs and providing more equitable,patient-centred care.

The Free-Flowing Voice and its Connection With the Mother Tongue

Does the native tongue confer greater authenticity and connection？ And how does this connect with languages acquired later in life？ From thirty years of directing, training, and auditioning actors from a range of ethnicities, I have believed that the mother-tongue has a particular and organic connection for an actor, one difficult to achieve in any other language. This belief was confounded in a laboratory conducted with Romanian actors, March 2013. The work was performed in both English and Romanian and it was with a sense of shock that I observed that the work was more vital, compelling, and physically and vocally engaged when they spoke in English. What were the factors at play here and what are the implications for future work？ Patsy Rodenburghas written of the giddy delight children find in language. Under what conditions does the native tongue evoke that ＂giddy delight＂ and where and when does it become an obstacle to such pleasure？

Hypoglycemia in the ICU: Discrepancy between capillary and serum glucose measurements in patients with renal disease

Recently, a focus on tight glycemic control in intensive care units (ICU) has resulted in implementation of strict insulin protocols requiring frequent glucose monitoring. The use of point-of-care (POC) capillary glucose testing is widespread, but its validity in the ICU has been questioned. Our objective is to better understand the use of POC glucose at the extremes of glycemic control through a case review at our institution. We describe the case of a 75-year-old non-diabetic female with end stage renal disease (EsRD) on hemodialysis who was admitted with apparent hypoglycemia. After extensive workup was done for a seemingly refractive hypoglycemia, a discrepancy between POC capillary glucose and central serum glucose levels was discovered, revealing actual euglycemia and false low POC glucose values. Cases of hypoglycemia can be challenging, especially in non-diabetic patients with EsRD. While glucometers assessing capillary glucose are used both in the outpatient and inpatient environment, their validity in the critically ill patient has known limitations. Cases such as this have led to the development of systemic checks and balances, as well as further investigations regarding the use of POC glucose meters in the ICU. This case serves as a reminder to evaluate for all causes for abnormal laboratory values, including technological limitations.

The Patatin-Containing Phospholipase A pPLAIIα Modulates Oxylipin Formation and Water Loss in Arabidopsis thaliana

The patatin-related phospholipase A （pPLA） hydrolyzes membrane glycerolipids to produce monoacyl compounds and free fatty acids. Phospholipids are cleaved by pPLAIIα at the sn-1 and sn-2 positions, and galactolipids, including those containing oxophytodienoic acids, can also serve as substrates. Ablation of pPLAIIα decreased lysophosphatidylcholine and lysophosphatidylethanolamine levels, but increased free linolenic acid. pPLAIIα-deficient plants displayed a higher level of jasmonic acid and methyl jasmonate, as well as the oxylipin-biosynthetic intermediates 13-hydroperoxylinolenic acid and 12-oxophytodienoic acid than wild-type （WT） plants. The expression of genes involved in oxylipin production was also higher in the pPLAIlec-deficient mutant than in WT plants. The mutant plants lost water more quickly than WT plants. The stomata of WT and mutant plants responded similarly to abscisic acid. In response to desiccation, the mutant and WT leaves produced abscisic acid at the same rate, but, after 4 h of desiccation, the jasmonic acid level was much higher in mutant than WT leaves. These results indicate that pPLAIIα negatively regulates oxylipin production and suggest a role in the removal of oxidatively modified fatty acids from membranes.

snack (Re)formulation in the Improvement of Health Effects on Glycaemia and satiety Responses: Preliminary Results

snacking impacts the overall quality of dietary patterns, nutrition and health outcomes. Highly processed snack foods are normally unhealthy due to higher saturated fat content, refined grains (carbohydrates), and artificial ingredients. snack product (re)formulation using plant-based wholesome ingredients has the potential to improve health effects including glycaemia, satiety responses, and lipid metabolism. Oat (span>span>span>span>Avena sativa L.span>span>span>span>) has gained increased popularity as a food item in recent decades due to its perceived or real health benefits, principally related to the bran content of oats. We have developed a healthier snack prototype using rolled oats as main ingredients. Other ingredients include oat bran, dried fruits, and nuts. For initial trials, these ingredients were combined in various combinations and composition to produce a formula having a good nutrient profile score (≤4) that meets the criteria of FSANZ for making health-related claims. The proportions of the ingredients in the formula were adjusted to enable higher fiber and protein contents. From the initial 10 combinations prepared, one prototype was selected based on four quality attributes (visual appearance, texture, taste, and water activity). The selected prototype was tested for glycemic index (n = 10) and consumer liking (n = 67). Our preliminary results demonstrated that the prototype has a low glycemic index,span>span> span><

Jams and marmalades are some of the most popular food products because of their low cost, all year long availability and appealing sensory properties. These products are described as gels with pectin as the gelling agent used in its formulation. Gels are a form of matter intermediate between a solid and a liquid. They consist of polymeric molecules cross-linked to form tangles, and interconnected molecular network immersed in a liquid medium. However, the jams and marmalades require stabilization by hydrocolloids. The most common hydrocolloid stabilizer is pectin which is available as a low methoxy pectin or high methoxy pectin. Unfortunately, under mechanical stress, pectin gels may be damaged leading to the release of colloidal water. The release of the colloidal water is termed as syneresis. This problem may be solved by integrating pectin with other stabilizers having thickening properties. Jams were prepared using plums and pineapples while marmalades were prepared from oranges and lemons. Gum Arabic from Acacia senegal var. kerensis was added in the range of 1% - 5% which fell within the additives category. The prepared jams and marmalades underwent sensory evaluation using semi-trained panelists. The prepared jams and marmalades showed no evidence of syneresis. The jams and marmalades were subjected to a sensory panel who scored the different attributes against a 7-point hedonic scale. Gum Arabic at different levels was found to have a significant contribution to the consumer acceptance of the fruit spreads prepared. This is the first time that syneresis reduced jams and marmalades containing gum Arabic from Acacia senegal var. kerensis are being reported. sensory evaluation was carried out on different fruit spreads used in the study to assess the impact of gum Arabic from Acacia senegal var. A 2D GIs Approach for Mapping Aquiferous Zones Using Remotely sensed Data within Obubra, southeast-Nigeria

Groundwater is vital to the sustenance and well-being of man-kind, although it is constantly under immense pressure. For this reason, there is need to develop an effective, reliable, scientific and sustainable means of delineating zones of groundwater occurrence and distribution with high precision in other to effectively explore for this resource. In this study, remote sensing (Rs) and geographical information system (GIs) have been combined to develop thematic maps of the zones of groundwater occurrence and distribution based on variable factors such as;elevation, drainage, lineament, slope, lithology and soil. The analytical hierarchy procedure (AHP) was employed to classify and subsequently assign weight to each variable factor through weighted overlay analysis. Integration of these factors with their relative classes defined was used to produce a 2D-model for predicting surface aquifers mapped within Obubra. The study delineated three (3) surface aquifer zones representing groundwater potential zones. Zones representing high groundwater potential cover an area of approximately 331.94 Km2, accounting for 29.58% of the study area. The region that represents moderate to good groundwater occurrence and distribution covers an estimated area of approximately 648.42 Km2, occupying 57.78% of the study area. Zones with groundwater of low potential account for a total surface area of approximately 141.81 Km2 which is about 12.64% of the entire study area. Regions that show good to moderate and high groundwater potentials, have a wider distribution all across the study area except at the southernmost part. The study therefore shows that it is effective in delineating surface potential zones and hitherto a 2D surface aquifer model for groundwater exploration campaigns within Obubra and its environs.
