Outcomes of Pregnancy with Group B Streptococcal Infections in Najran, Saudi Arabia

Background: The gram-positive, beta-hemolytic, and chain-forming Group B Streptococcus (GBS), commonly known as Streptococcus agalactiae, may asymptomatically invade the human gastrointestinal and vaginal tracts. However, GBS may become very invasive and pathogenic to the mother and baby during pregnancy, having negative effects. Study Aim: This study aims to investigate the pregnancy outcomes of women who tested positive for genitourinary GBS infection during pregnancy in Najran, Saudi Arabia. Methods: Data was collected retrospectively from patient files in Armed Forces Hospital, Najran, Saudi Arabia. Data collected were entered to a Microsoft Excel sheet, then imported and analysed using the Statistical Package for Social Sciences. Results: The study included 272 women of whom 66.5% were 31 to 45 years old. Gestational diabetes was diagnosed in 8.5% of the sample, 71.7% had a normal spontaneous vaginal delivery (NSVD), 1.8% had previous abortions, and 27.2% of new-borns were admitted to the NICU after delivery. There was a significant association between NICU admissions and women employment status (p = 0.001), gravidity (p = 0.001), parity (p = 0.001), history of abortions (p = 0.001), medical conditions (p = 0.049), and mode of delivery (p = 0.049). Conclusion: According to the findings of our study, GBS infection during pregnancy is associated to more NICU admissions. NICU admissions were significantly correlated with gestational diabetes, hypertension, and hypothyroidism in mothers but not with intrapartum antibiotic use.

The novel Coronavirus disease 2019(COVID-19)pandemic has begun in China and is still affecting thousands of patient livesworldwide *** X-ray and Computed Tomography are the gold standardmedical imaging modalities for diagnosing potentially infected COVID-19 cases,applying Ultrasound(US)imaging technique to accomplish this crucial diagnosing task has attracted many physicians *** this article,we propose two modified deep learning classifiers to identify COVID-19 and pneumonia diseases in US images,based on generative adversarial neural networks(GANs).The proposed image classifiers are a semi-supervised GAN and a modifiedGANwith auxiliary *** one includes a modified discriminator to identify the class of the US image using semi-supervised learning technique,keeping its main function of defining the“realness”of tested *** tests have been successfully conducted on public dataset of US images acquired with a convex US *** study demonstrated the feasibility of using chest US images with two GAN classifiers as a new radiological tool for clinical check of COVID-19 *** results of our proposed GAN models showed that high accuracy values above 91.0%were obtained under different sizes of limited training data,outperforming other deep learning-based methods,such as transfer learning models in the recent ***,the clinical implementation of our computer-aided diagnosis of US-COVID-19 is the future work of this study.

Validation of a serum microRNA panel as biomarkers for early diagnosis of hepatocellular carcinoma post-hepatitis C infection in Egyptian patients

AIM To investigate the prospective importance of serum micro(mi)RNAs(mi R-125 b, mi R-138 b, mi R-1269, mi R-214-5p, mi R-494, mi R375 and mi R-145) as early biomarkers for the diagnosis of hepatitis C virus(HCV)-related hepatocellular carcinoma(HCC).METHODS Two-hundred and fifty HCV4 a patients, 224 HCV4 aHCC patients, and 84 healthy controls were enrolled in the study. Expression levels of mi R214-5p, mi R-125 b, mi R-1269 and mi R-375 were quantified using quantitative real-time *** Expression of the selected mi RNAs in serum wassignificantly lower in HCC patients than in the healthy controls, except for mi R-1269 and mi R-494. There was a significant difference between HCC and HCV patients, in particular for HCC and late stage fibrosis, rather than HCV patients and early fibrosis. It is obvious that mi R-1269 was significantly upregulated in HCC cases compared to hepatic fibrosis cases. Each mi RNA can show HCC progression. Multivariate logistic regression analysis indicated that the tested panel of mi RNAs(mi R214-5p, mi R-125 b, mi R-1269 and mi R-375) represent accurate and specific indictors of HCC *** This study presents a panel of mi RNAs with strong power as putative diagnostic and prognostic biomarkers for HCV-induced HCC. Moreover, mi R-214-5p and mi R-1269 could be considered as early biomarkers for tracking the progress of liver fibrosis to HCC.

Investigation of climate change impacts on food security has become a global hot *** so,efforts to mitigate these issues in arid regions have been ***,in this paper,further research is discussed based on data obtained from various crop and climate *** DSSATcrop models(CMs)(CERESWheat and N-Wheat)were calibrated with two wheat cultivars(Gemiza9 and Misr1).A baseline simulation(1981-2010)was compared with different scenarios of simulations using three Global Climate Models(GCMs)for the 2030s,2050s and *** impacts of climate change were assessed using the GCMs and CMs under the high emission Representative Concentration Pathway(RCP8.5).Results predicted decreased wheat grain yields by a mean of 8.7%,11.4%and 13.2%in the 2030s,2050s and 2080s,respectively,relative to the baseline *** impacts of climatic change are probable,despite some uncertainties within the GCMs(i.e.,2.1%,5.0%and 8.0%)and CMs(i.e.,2.2%,6.0%and 9.2%).Changing the planting date with a scenario of plus or minus 5 or 10 days from the common practice was assessed as a potentially effective adaptation option,which may partially offset the negative impacts of climate *** the sowing date by 10 days(from 20 November to 30 November)proved the optimum scenario and decreased further reduction in wheat yields resulting from climate change to 5.2%,6.8%and 8.5%in the 2030s,2050s and 2080s,respectively,compared with the 20 November *** planting 5-days earlier scenario showed a decreased impact on climate change ***,the 10-days early planting scenario increased yield reduction under projected climate *** cultivar Misr1 was more resistant to rising temperature than *** the negative impacts of projected climate change on wheat production,water use efficiency would slightly *** ensemble of multi-model estimated impacts and adaptation uncertainties of climate change can assist decision-makers in planning clima

BACKGROUND Nonalcoholic steatohepatitis-related cirrhosis is one of the liver complications in type 2 diabetes mellitus(T2DM)and reported to be a risk factor for developing hepatocellular carcinoma(HCC).A reliable screening biomarker of liver cirrhosis(LC)and HCC among T2DM patients is important to reduce the morbidity and mortality of this ***(miRNA)is considered a key player in HCC and T2DM,and it might be a hidden culprit in diabetes-associated HCC,making it a promising reliable prognostic *** To investigate the signature of serum miRNAs as early biomarkers for the screening of HCC among diabetic *** Expression profiles of miRNAs in serum samples of diabetic LC and diabetic HCC patients were assessed using Illumina sequencing;then,RT-qPCR was used to validate significantly altered miRNAs between the two *** miRNAs were tested in serum samples of 200 T2DM patients,270 LC patients,200 HCC patients,and 225 healthy control ***,receiver operating characteristic(ROC)analysis,with area under the curve(AUC),was performed to assess the diagnostic performance of the screened miRNAs for discriminating HCC from LC and nonmalignant patients(LC+T2DM).RESULTS Expression of the sequenced miRNAs in serum was different in HCC vs LCpositive T2DM *** miRNAs(miR-34a,miR-221)were significantly upregulated and five miRNAs(miR-16,miR-23-3p,miR-122-5p,miR-198,miR-199a-3p)were significantly down-regulated in HCC compared to LC *** of ROC curve demonstrated that the combination of these seven miRNAs can be used as a reliable biomarker for detection of HCC in diabetic patients,as it could identify HCC with high diagnostic accuracy in diabetic LC patients(AUC=0.993)and in diabetic nonmalignant patients(AUC=0.961).CONCLUSION This study validates a panel of serum miRNAs that can be used as a reliable noninvasive screening biomarker of HCC among T2DM cirrhotic and noncirrhotic *** study recommends furt

Evaluation of GALAD Score in Diagnosis and Follow-up of Hepatocellular Carcinoma after Local Ablative Therapy

Background and Aims:Strategies for detection of early hepatocellular carcinoma(HCC)are still *** GALAD score is a serum biomarker-based model designed to predict the probability of having *** aimed to assess the ability of GALAD score to diagnose early HCC and its validity to follow patients after local ablation ***:This multicenter prospective study included 108 patients in two groups,58 HCC patients(67 focal lesions)with local ablative therapy(study group),and a control group of 50 patients with liver *** GALAD scores of the study and control groups,and of the HCC patients before and after ablative therapy were ***:Most patients were men(74.1%in study group and 76%in controls)with hepatitis C virus infection(98.30%in the study group,and 94%in controls).GALAD scores were significantly higher in HCC patients than in those with benign cirrhosis(2.65 vs.−0.37,p=0.001).Ablative therapy was successful in 94.4%of focal lesions<2 cm,and in 86.10%of 2–5 cm *** GALAD score was also significantly lower at 1 month after ablation in patients with well-ablated tumors(2.19 vs.0.98,p=0.001).The best cutoff values of GALAD score for diagnosis of early HCC,and for prediction of well ablation of HCC were 0.74 and≤3.31(areas under the curve of 0.92 and 0.75,sensitivities of 84.48%and 76.19%,specificities of 89.13%and 83.33%,positive predictive values of 90.74%and 94.1%,and negative predictive values of 82%and 35.7%respectively).Conclusion:The GALAD score was effective for the diagnosis of early HCC and for followup after ablative therapy.

Lightweight deep convolutional neural networks(CNNs)present a good solution to achieve fast and accurate image-guided diagnostic procedures of COVID-19 ***,advantages of portable Ultrasound(US)imaging such as simplicity and safe procedures have attracted many radiologists for scanning suspected COVID-19 *** this paper,a new framework of lightweight deep learning classifiers,namely COVID-LWNet is proposed to identify COVID-19 and pneumonia abnormalities in US *** to traditional deep learning models,lightweight CNNs showed significant performance of real-time vision applications by using mobile devices with limited hardware *** main lightweight deep learning models,namely MobileNets,ShuffleNets,MENet and MnasNet have been proposed to identify the health status of lungs using US *** image dataset(POCUS)was used to validate our proposed COVID-LWNet framework *** classes of infectious COVID-19,bacterial pneumonia,and the healthy lung were investigated in this *** results showed that the performance of our proposed MnasNet classifier achieved the best accuracy score and shortest training time of 99.0%and 647.0 s,*** paper demonstrates the feasibility of using our proposed COVID-LWNet framework as a new mobilebased radiological tool for clinical diagnosis of COVID-19 and other lung diseases.

Surgical outcomes of complicated cataract with pediatric trematode granulomatous uveitis

AIM:To describe the clinical characteristics and surgical outcomes of complicated cataract with pediatric trematodal granulomatous uveitis(TGU).METHODS:Patients of cataract with TGU in the membranous(inactive) stage underwent cataract surgery with intraocular lens(IOL) *** history and ophthalmic examination were conducted for all cases,whereas Schimphlug imaging and corneal topography were done for some *** follow up was done on the 1st,2nd,and 5th postoperative days after ***,it was done at least at one,three,and six months *** and postoperative complications and the methods of their management were ***:Twelve eyes of 12 male children were included in this study ranging from 8 to *** mean best corrected Snellen visual acuity(BCVA) was significantly improved from 0.09±0.06 preoperatively to 0.37±0.11 at the final visit 6mo postoperatively(P<0.001).Schimphlug imaging and corneal topography showed flattening of the anterior surface of the inferior *** difficulties and complications included the poor dilatability of the drown down pupil,strong posterior synechia between the anterior lens capsule and the iris at the site of the inferior retrocorneal vascularized membrane and *** cases underwent primary hydrophobic IOL ***:Surgery for this type of cataract is relatively safe and *** is associated with some specific difficulties and complications that should be considered during surgery and follow up.
