BACKGROUND The influence of opioid use disorder on implant related complications, infection and readmission rates, and total global episode-of-care costs following primary total knee arthroplasty(TKA) is *** To examine whether opioid abuse in patients undergoing primary *** A retrospective analysis of the Medicare dataset, using the PearlDiver database,from 2005-2014 comparing outcomes in patients with opioid abusers(OUD) to non-opioid abusers(NOU) undergoing primary TKA was performed. Patient outcomes were analyzed including implant complications, readmission rates, and day-of-surgery and 90-d cost. Statistical analysis was performed with R(University of Auckland, New Zealand) calculating odds-ratio(OR) along with their respective 95% confidence interval(95%CI) and *** The OUD group was at greater odds of having implant related complications overall(20.84% vs 11.25%; OR: 2.07; 95%CI: 1.93-2.23; P were also significantly increased. AIM To research the influence of season of the year on periprosthetic joint *** we conducted a retrospective review of the entire Medicare files from 2005 to 2014. Seasons were classified as spring, summer, fall or winter. Regional variations were accounted for by dividing patients into four geographic regions as per the United States Census Bureau(Northeast, Midwest, west and South). Acute postoperative infection and deep periprosthetic infections within 90 d after surgery were tracked. RESULTS In all regions, winter had the highest incidence of periprosthetic infections(mean 0.98%, SD 0.1%) and was significantly higher than other seasons in the Midwest, South and west(P were more frequent in the summer and were significantly affected by season of the year in the *** Season of the year is a risk factor for periprosthetic joint infection following total hip arthroplasty(THA). Understanding the influence of season on outcomes following THA is essential when risk-stratifying patients to optimize outcomes and reduce episode of care costs.

In burn treatments,microorganisms on pressure garments during pressure therapy can prevent rehabilitation by causing functional,hygienic,and aesthetic difficulties. As bacterium is one of the most trouble-causing organisms,they can threaten patients causing infection during the long period of use of these *** burn pressure garments having durable antimicrobial property were developed using polyhexamethylene biguanide( PHMB)antimicrobial agent procedure on highly elastic nylon 66 /spandex fabrics in powernet,flat warp and weft knitted structures using paddry-cure method. Commercial wireless pressure sensors were used to control pressures at an acceptable medical range. Antimicrobial activity,wash durability,Fourier transform infrared spectroscopy(FTIR) and Scanning electron microscopy(SEM) analyses were conducted for the treated samples. Antimicrobial test results following AATCC 100 Test Method showed 99% reduction of bacteria for the fabric samples treated with PHMB. A small but significant decrease in antimicrobial activity was observed even after50 launderings. These treatments also yield good results to prevent odor,decrease infection by preventing and /or blocking microbial growth according to the antimicrobial mechanism and support reducing of scarring by providing a hygienic environment around the scar.

Recently,McDonald et al.(2020)proposed that complement causes a metabolic switch to lipid consumption in amyotrophic lateral sclerosis(ALS),which raises the possibility of complement as a global immunometabolic *** ALS,neuromuscular degeneration occurs concurrently with complement-mediated inflammation,glucose intolerance,and elevated lipid ***,complement is being increasingly recognized as a metabolic regulator that can induce insulin resistance and alter fuel source ***,the authors propose that chronic complement activation may be driving metabolic reprogramming in ALS,which has broad implications for the field of immunometabolism and would be the first mechanistic explanation of the“lipid switch”in ***,it suggests that complement can drive a global shift in resources during inflammation,in which glucose is redirected from peripheral tissues to immune cells to support their inflammatory actions.

Human exposures to pesticides can occur in the workplace,in the household and through the ambient *** several articles have reviewed the impact of pesticide exposures on human respiratory health in occupational settings,to the best of our knowledge,this article is the first one to review published studies on the association between pesticide exposures and human respiratory health in the general *** this article,we critically reviewed evidences up to date studying the associations between non-occupational pesticide exposures and respiratory health in general *** article also highlighted questions arising from these studies,including our recent analyses using the data from the Canadian Health Measures Survey（CHMS）,for future *** found few studies have addressed the impact of environmental pesticide exposures on respiratory health,especially on lung function,in general *** the studies using the data from CHMS Cycle 1,exposures to OP insecticides,pyrethroid insecticides,and the organochlorine pesticide DDT were associated with impaired lung function in the Canadian general population,but no significant associations were observed for the herbicide 2,*** research should focus on the potential age-specific and pesticide-specific effect on respiratory health in the general population,and repeated longitudinal study design is critical for assessing the temporal variations in pesticide *** findings from current studies of non-occupational pesticide exposures and their health impact in general population will help to improve the role of regulatory policies in mitigating pesticide-related public health problems,and thereafter providing greater benefit to the general population.

Asymptomatic pancreatic lesions: New insights and clinical implications

Despite great efforts in experimental and clinical research, the prognosis of pancreatic cancer (PC) has not changed significantly for decades. Detection of pre-invasive lesions or early-stage PC with small resectable cancers in asymptomatic individuals remains one of the most promising approaches to substantially improve the overall outcome of PC. Therefore, screening programs have been proposed to identify curable lesions especially in individuals with a familial or genetic predisposition for PC. In this regard, Canto et al recently contributed an important article comparing computed tomography, magnetic resonance imaging, and endoscopic ultrasound for the screening of 216 asymptomatic high-risk individuals (HRI). Pancreatic lesions were detected in 92 of 216 asymptomatic HRI (42.6%). The high diagnostic yield in this study raises several questions that need to be answered of which two will be discussed in detail in this commentary: First: which imaging test should be performed? Second and most importantly: what are we doing with incidentally detected pancreatic lesions? which ones can be observed and which ones need to be resected?
