Background and Purpose:Carotid and Vertebral Artery Transluminal Angioplasty Study (CAVATAS) patients with carotid stenosis were randomized between endovascu lar treatment and endarterectomy. The rates of residual severe stenosis and rest enosis and their contribution to recurrent symptoms was ***:Endovas cular patients:were treated by balloon angioplasty alone (88%) or stenting (22 %). Patches were used in 63%of endarterectomy patients. Carotid stenosis was c ategorizedas mild (0%to 49%), moderate (50%to 69%), severe (70%to 99%), or occluded, using standardized Doppler ultrasound criteria at the examination clo sest to 1 month (n=283) and 1 year (n=347) after treatment. Recurrent cerebrovas cular symptoms during follow-up were analyzed. Results:More patients had ≥70 %stenosis of the ipsilateral carotid artery 1 year after endovascular treatment than after endarterectomy (18.5%versus 5.2%, P=0.0001). Residual severe steno sis was present in 6.5%of patients at 1 month after endovascular *** een 1 month and 1 year, restenosis to ≥70%stenosis occurred in 10.5%of the en dovascular group. After endarterectomy,1.7%had residual severe stenosis at 1 mo nth, and 2.5%developed severe restenosis. The results were significantly better after stenting compared with angioplasty alone at 1 month(P< 0.001) but not at 1 year. Recurrent ipsilateral symptoms were more common in endovascular patients with severe stenosis(5/32 [15.6%]) compared with lesser degrees of stenosis at 1 year (11/141 [7.8%], P=0.02), but most were transient ischemic attacks and n one were disabling or fatal strokes. There were no recurrent symptoms in the 9 e ndarterectomy patients with≥70%stenosis at 1 year. Conclusions:Carotid stenos is 1 year after endovascular treatment is partly explained by poor initial anato mical results and partly by restenosis. The majority of patients were treated by angioplasty without stenting. Further randomized studies are required to determ ine whether new

Background: Angiotensin-converting enzyme is involved in the pathophysiology of heart failure. We hypothesized that clinical characteristics as well as survival rate in patients with heart failure of different etiologies may be modulated by functional variants DD, ID and II of the angiotensin-converting enzyme gene. Methods: We studied 333 patients with heart failure, aged 43.3±10.5 years, 262(78.7%) men and 71(21.3%) women. Heart failure was ascribed to idiopathic dilated cardiomyopathy in 125 patients. Heart failure was caused by ischemic heart disease in 63 patients, Chagas’disease in 58, hypertensive heart disease in 41, alcoholic cardiomyopathy in 24, and was due to other etiologies in 22 patients. Statistical analysis was performed with the χ2 test, Student’s t-test, analysis of variance, Kaplan-Meier and Cox proportional hazards methods. Results: The DD genotype was associated with increased systolic left ventricular diameter(p=0.031). Earlier onset of symptoms was observed in patients with alcoholic cardiomyopathy and DD genotype(p=0.033, codominant D) and in patients with hypertensive cardiomyopathy and DD genotype(p=0.048, codominant D; p=0.024, recessive D). Mortality was higher in patients older than 50 years with DD genotype(p=0.007, codominant D; p=0.002, recessive D). Variables independently associated with higher mortality in patients older than 50 years were age, diabetes mellitus, Chagas’disease etiology and DD genotype. Conclusions: These results add evidence for an association of the DD genotype of the angiotensin-converting enzyme gene with earlier onset of symptoms and decreased survival rate of selected patients with heart failure.

At present,natural orifice specimen extraction surgery(NOSES)has attracted more and more attention worldwide,because of its great advantages including minimal cutaneous trauma and post-operative pain,fast post-operative recovery,short hospital stay,and positive psychological ***,NOSES for the treatment of gastric cancer(GC)is still in its infancy,and there is great potential to improve its theoretical system and clinical ***,several key points including oncological outcomes,bacteriological concerns,indication selection,and standardized surgical procedures are raised with this innovative ***,it is necessary to achieve an international consensus to regulate the implementation of GC-NOSES,which is of great significance for healthy and orderly development of NOSES worldwide.

自1978年改革开放以来,中国经济的迅速发展已成为国际体系中显著的特征之一,改变了21世纪的世界经济发展和世界格局演变。中国很可能在不久的将来成为世界上最大的经济体,这引起了相对实力正在下降的美国的不安,新冠疫情加剧了中美两大经济体之间的冲突。自2018年以来,特朗普政府将“中国威胁”作为美国国际政策的基础,试图重建新“冷战”和两极世界的叙事,同时复兴国际共产主义威胁论,而拜登政府一直在将这一政策激进化。中国特色的社会主义制度的优越性使中国成功地避免了资本主义正在经历的周期性危机,与美国政府的宣传相反,中国在抗击新冠疫情中所付出的努力堪称典范,其在控制疾病的同时也致力于国内经济社会的发展,并已取得一定成效。

通过两组试验,探索了不同青贮饲料和日粮中粗蛋白质(CP)水平对内洛尔公牛的影响。日粮分为柱花草青贮饲料(STS)和玉米青贮饲料(CS)两种,均包括两种水平的CP:110 g·kg^(-1)和130 g·kg^(-1)干物质基础(DM),精粗比为5050。在试验1中,选取4头平均初始体重(BW)为450±37.9 kg的公牛,并在瘤胃和皱胃进行手术插管。随机分配到一个4×4拉丁方阵中,包含2×2因素设计,共4个阶段,每阶段16 d。在试验阶段,收集瘤胃、皱胃食糜、粪便、尿、血。此外,对DM、CP的动力学参数,中性洗涤纤维(NDF)含量进行检测。在试验2中,选取40头BW为374±16.5 kg的公牛,随机分配到2×2方阵中,每组10个重复,为期99 d,包括15 d适应期和84 d数据采集期。对试验2中公牛营养素的摄入量和消化率及动物的生产性能进行检测。STS组的DM、CP的可降解部分和NDF均极显著低于CS组(P0.050),青贮饲料添加玉米及不同的CP含量对生产性能无显著影响,但STS有降低饲料利用率的趋势(P=0.085)。因此,选取CP含量130 g·kg-1的柱花草青贮饲料可饲喂育肥肉牛。

Context: Weight loss elicits physiological adaptations relating to energy inta ke and expenditure that antagonize ongoing weight loss. Objective: To test wheth er dietary composition affects the physiological adaptations to weight loss, as assessed by resting energy expenditure. Design, Study, and Participants: A rando mized paralleldesign study of 39 over-weight or obese young adults aged 18 to 4 0 years who received an energy-restricted diet, either low-glycemic load or lo w-fat. Participants were studied in the General Clinical Research Centers of th e Brigham and Women’s Hospital and the Children’s Hospital, Boston, Mass, befo re and after 10%weightless. The study was conducted from January 4,2001, to May 6, 2003. Main Outcome Measures: Resting energy expenditure measured in the fast ing state by indirect calorimetry, body composition by dual-energy X-ray absor ptiometry, cardiovascular disease risk factors, and self-reported hunger. Resul ts: Resting energy expenditure decreased less with the low-glycemic load diet t han with the low-fat diet, expressed in absolute terms(mean[SE], 96 [24] vs 176 [27] kcal/d; P=.04) or as a proportion(5.9%[1.5%] vs 10.6%[1.7%]; P=.05). Pa rticipants receiving the low-glycemic load diet reported less hunger than those receiving the low-fat diet(P=.04). Insulin resistance(P=.01), serum triglyceri des(P=.01), C-reac-tive protein(P=.03), and blood pressure(P=.07 for both syst olic and diastolic) improved more with the low-gly-cemic load diet. Changes in body composition (fat and lean mass) in both groups were very similar(P=.85 and P=.45, respectively). Conclusions: Changes in dietary composition within prevai ling norms can affect physiological adaptations that defend body weight. Reducti on in glycemic load may aid in the prevention or treatment of obesity, cardiovas cular disease, and diabetes mellitus.
