AIM:To describe epidemiological aspects,clinical findings,and visual results in eye trauma patients requiring urgent surgery at a tertiary ***:Retrospective review of adult patients requiring urgent surgery for trauma to the eyeball from January 2010 and April *** was collected relative to age,gender,mechanism of the injury,type of wound,initial and final visual acuity,number and type of surgeries carried *** were classed according to the Birmingham Eye Trauma Terminology(BETT)and the Ocular Trauma Score(OTS)***:The survey included 92 *** age was 54.76±22.18 *** most frequent cause was domestic accidents(44.6%),followed by aggression(22.80%)and occupational accidents(17.4%).Trauma from aggression was more frequent in men(P=0.006)and accidents in the home more common in women(P=0.011).Patients over 65 y presented an odds ratio(OR)of 10.71 for suffering a domestic *** between 15-45 and 46-65 y were at higher risk of trauma from aggression(OR=17.52 and OR=10.94,respectively).As for the type of injury,63.04%were open-globe lesions,Zone II being the most frequently affected(27.2%).In Logistic regression analysis,old age(P=0.05)and retinal involvement(P=0.001)were found to be associated with higher rate of unfavourable visual ***:The epidemiological aspects of eye trauma are highly dependent on the area of population *** accidents are more relevant than workplace accidents in older urban areas with high socioeconomic status.A better knowledge of the epidemiological characteristics is useful for implementing specific prevention measures and appropriate treatment strategies.

BACKGROUND Irritable bowel syndrome(IBS) is a highly prevalent condition. It is diagnosed on the basis of chronic symptoms after the clinical and/or investigative exclusion of organic diseases that can cause similar symptoms. There is no reproducible noninvasive test for the diagnosis of IBS, and this raises diagnostic uncertainty among physicians and hinders acceptance of the diagnosis by *** gastrointestinal(GI) syndromes often present with overlapping upper and lower GI tract symptoms, now believed to be generated by visceral hypersensitivity. This study examines the possibility that, in IBS, a nutrient drink test(NDT) provokes GI symptoms that allow a positive differentiation of these patients from healthy *** To evaluate the NDT for the diagnosis of *** This prospective case-control study compared the effect of two different nutrient drinks on GI symptoms in 10 IBS patients(patients) and 10 healthy controls(controls). The 500 kcal high nutrient drink and the low nutrient 250 kcal drink were given in randomized order on separate days. Symptoms were assessed just before and at several time points after drink ingestion. Global dyspepsia and abdominal scores were derived from individual symptom data recorded by two questionnaires designed by our group, the upper and the general GI symptomquestionnaires, respectively. Psycho-social morbidity and quality of life were also formally assessed. AIM: To investigate the potential therapeutic effect of exopolysaccharide (EPS)-producing Streptococcus thermophilus (S. thermophilus) CRL 1190 fermented milk on chronic gastritis in Balb/c mice. METHODS: Balb/c mice were fed with the fermented milk for 7 d after inducing gastritis with acetyl-salicylic acid (ASA, 400 mg/kg body weight per day for 10 d). Omeprazole was included in this study as a positive therapeutic control. The gastric in? ammatory activity was evaluated from gastric histology and in? ammation score, number of interleukin-10 (IL-10), interferon-γ (INFγ) and tumor necrosis factor-α (TNF-α) cytokine-producing cells in the gastric mucosa, and thickness of the mucus layer. RESULTS: Animals receiving treatment with the EPS-producing S. thermophilus CRL 1190 fermented milk showed a conserved gastric mucosa structure similar to that of healthy animals. In? ammation scores of the fermented milk-treated mice were lower than those of mice in the gastritis group (0.2 ± 0.03 vs 2.0 ± 0.6, P < 0.05). A marked decrease in INFγ+ (15 ± 1.0 vs 28 ± 1.2, P < 0.05) and TNF-α+ (16 ± 3.0 vs 33 ± 3.0, P < 0.05) cells and an increase in IL-10+ (28 ± 1.5 vs 14 ± 1.3, P < 0.05) cells compared to the gastritis group, was observed. Also, an increase in the thickness of the mucus gel layer (2.2 ± 0.6 vs 1.0 ± 0.3; 5.1 ± 0.8 vs 1.5 ± 0.4 in the corpus and antrum mucosa, respectively, P < 0.05) compared with the gastritis group was noted. A milk suspension of the purif ied EPS from S. thermophilus CRL1190 was also effective as therapy for gastritis.

Dear Editor,Short-term fasting(up to 48 hours)activates strong physiological and molecular responses[1].We and others showed a p53-independent transcriptional activation of the cell cycle inhibitor p21 upon fasting,most strongly in the liver,muscles,and many other tissues[2,3].Studies with mice[4,5]and human patients[6]have shown the beneficial effects of short-term fasting during anti-cancer chemotherapy treatments.

BACKGROUND Hepatocellular carcinoma(HCC) is the most common type of primary liver cancer. Current guidelines for HCC management recommend surveillance of high-risk patients every 6 mo using ultrasonography. Serum biomarkers, like alpha-fetoprotein(AFP), protein induced by vitamin K absence/antagonist-Ⅱ(PIVKA-Ⅱ) and lectin-reactive AFP, show suboptimal performance for detection of HCC, which is crucial for successful resection or treatment. Thus, there is a significant need for new biomarkers to aid early diagnosis of HCC. Studies have shown that the expression level of human microRNAs(miRNAs), a small, non-coding RNA species released into the blood, can serve as an early marker for various diseases, including *** To evaluate the diagnostic role of miRNAs in HCC as single markers, signatures or in combination with known protein *** Our prospective, multicenter, case-control study recruited 660 participants(354 controls with chronic liver disease and 306 participants with HCC) and employed a strategy of initial screening by two independent methods, real-time quantitative PCR(n = 60) and next-generation sequencing(n = 100), to assess a large number of miRNAs. The results from the next-generation sequencing and real-time quantitative PCR screening approaches were then combined to select 26 miRNAs(including two putative novel miRNAs). Those miRNAs were analyzed for their diagnostic potential as single markers or in combination with other miRNAs or established protein biomarkers AFP and PIVKA-Ⅱ via real-time quantitative PCR in training(n = 200) and validation cohorts(n = 300).RESULTS We identified 26 miRNAs that differentiated chronic liver disease controls from(early) HCC via two independent discovery approaches. An association of organic-walled microfossils consisting of filamentous cyanobacteria, algal coenobia and acanthomorphic acritarch have been documented from non-calcareous claystones and mudstones of the Pepper Mountains Shale Formation（PMSF）, located in its stratotype area in the Pepper Mountains, which are part of the Holy Cross Mountains in Poland. These sediments represent the oldest strata of the ？ysogóry Unit, deposited on the edge of the East European Craton（Baltica）. Non-branched, ribbon-like and thread-like cyanobacteria trichomes exhibit morphological similarities to families Nostocaceae and Oscillatoriaceae. Cells assembled in rounded to irregular clusters of monospecific agglomerations represent multicellular algal coenobia, attributed to the family Scenedesmaceae. The co-occurrence of acritarchs belonging to species as Eliasum llaniscum, Cristallinium ovillense and Estiastra minima indicates that the studied material corresponds to the lower Middle Cambrian. Deposition of the PMSF took place in shallow marine environment, influenced by periodical freshwater inputs. The varying degree of coloration of organic-walled microfossils is interpreted in this study as factor indication of possible different source of their derivation. Dark brown walls of cells assembled in algal coenobia might have sustained previous humification in humid, terrestrial environments, which preceded their river transport into the sea together with nutrients, causing occasional blooms of cyanobacteria in the coastal environment and the final deposition of both groups of organisms in marine deposits.
