Australian tertiary care outcomes of entecavir monotherapy in treatment naive patients with chronic hepatitis B

AIM:To evaluate the long-term treatment outcomes of entecavir monotherapy in treatment naive patients in an Australian tertiary care setting. METHODS:A retrospective analysis of treatment naive patients receiving entecavir monotherapy through westmead Hospital was *** were excluded if they had received previous treatment with another nucleoside or nucleotide analogue,were pregnant or less than 18 years old. RESULTS:Out of 336 patients,163 patients fulfilled the selection *** of follow up was 3-46 mo (mean 26 mo).134 patients(82.2%)had pre-treatment biopsies,with 26 patients(16.0%)demonstrating F3-4 *** total,153 patients(93.9%)achieved at least Partial Virological Suppression(PVS),with 134 patients (82.2%)achieving complete virological suppression. The cumulative CVS and PVS rates at 36 mo were 82.1%and 96.4%,respectively.3 patients(1.8%)failed to achieve PVS,while 5 patients(3.0%)developed virological rebound.128 patients(78.5%)maintained CVS throughout follow *** of CVS included lower baseline DNA level(P=0.001),hepatitis B virus e antigen negative status(P=0.001)and increasing age at treatment(log rank 0.001).No significant adverse effects were reported necessitating cessation of entecavir. CONCLUSION:Entecavir monotherapy is efficacious and safe in an Australian tertiary care *** and rebound rates are very *** is similar to data from controlled and uncontrolled trials around the world.

The Prevalence, Patterns, and Trend of Antimicrobial Resistance among Patients at Kumi Orthopaedic Center: A Retrospective Study

Background: Antimicrobial Resistance (AMR) has been recognized as a global public health problem of utmost importance that needs to be tackled. Low-income countries such as Uganda have the greatest burden of severe and life-threatening infections and are most likely to suffer more from the spread of untreatable resistant bacteria. Despite the generally increasing trends in AMR, there is hardly any documented data on AMR in orthopedic care. Methods: we analyzed data from the pathogenic isolates from pus and tissue from the laboratory from 2019 to 2023. we calculated the proportions of isolates resistant to commonly used antimicrobial classes. we used the chi-square test for trends to evaluate changes in AMR across the study period. Results: Out of the 286 isolates, 104 (43.5%) were from pus, 112 (46.9%) were from tissue samples and the remaining 9.6% were from different fluid aspirates like synovial fluid. Most of the isolates were from males (64.2%) and aged between 19 and 45 years (46.2%). The distribution of whether the bacteria were gram-positive or negative was relatively equal with 114 (45.2%) being gram-negative and 120 (47.6%) gram-positive. Generally, there was increased AMR across all antibiotics from 2019 to 2021 e.g. for Imipinem, it went from 0% in 2019 to 70.8% in 2021 and for Amoxyclav, it went from 10% in 2019 to 93.2% in 2021. There was a general decline in AMR noted from 2021 to 2023. However, currently, the highest resistance is noted in ceftriaxone (80.0%) and the lowest in Imipinem (11.76%). Conclusion: There is a general decreasing trend in AMR, most probably due to increasing policies governing the use of antibiotics. However, there is still high resistance to commonly used and affordable antibiotics. Continuous monitoring of AMR is still recommended to reduce the AMR problem in Uganda at large through public health policy and planning.

Dynamic failures, or ‘‘bumps", remain an imperative safety concern in underground coal mining, despite significant advancements in engineering controls. The presence of spatially discrete, stiff roof units are one feature that has been linked to these events. However, an empirical stratigraphic review indicates that no significant difference exists in the relative commonality of discrete units between bumping and non-bumping deposits. Instead an apparent relationship exists between reportable bumping and the overall stiffness of the host rock. However, this initial study is too simplistic to be conclusive; to weight the relative impact of changes in a single variable, such as the thickness or location of sandstone members, it must be examined in isolation—i.e., in a setting where all other variables are held *** modelling provides this setting, and the effects of variability in a stiff discrete member in a hypothetical longwall mining scenario are investigated within the context of three stratigraphic ‘‘types",Compliant, Intermediate and Stiff. A modelling experiment examines changes in rupture potential in stiff roof units for each stratigraphic type as discrete unit thickness and location are manipulated through a range of values. Results suggest that the stiff-to-compliant ratio of the host rock has an impact on the relative stress-inducing effects of discrete stiff members. In other words, it is necessary to consider both the thickness and the distance to the seam, within the context of the host rock, to accurately anticipate areas of elevated rupture-induced hazard; acknowledging the presence of a discrete unit within the overburden in general terms is an insufficient indicator of risk. This finding helps to refine our understanding of the role of individual stiff, strong roof members in bumping phenomena, and suggests that a holistic view of overburden lithology and site-specific numerical modelling may be necessary to improve miner safety.

Obesity has a negative effect on male reproductive function. It is associated with low testosterone levels and alteration in gonadotropin secretion. Male obesity has been linked to reduced male fertility. Data regarding the relation of obesity to sperm parameters are conflicting in terms of the nature and magnitude of the effect. New areas of interest are emerging that can help explain the variation in study results, such as genetic polymorphism and sleep apnea. Sleep disorders have been linked to altered testosterone production and hypogonadism in men. It was also correlated to erectile dysfunction. The relation of sleep disorders to male fertility and sperm parameters remains to be investigated. Men with hypogonadism and infertility should be screened for sleep apnea. Treatment of obesity and sleep apnea improves testosterone levels and erectile function.

This contribution describes development and application of a user-friendly finite element program,UT3PC, to address three important problems in underground coal mine design:(1) safety of main entries,(2) barrier pillar size needed for entry protection, and(3) safety of bleeder entries during the advance of an adjacent longwall *** the finite element method is by far the most popular engineering design tool of the digital age, widespread use by the mining community has been impeded by the relatively high cost of and the need for lengthy specialized training in numerical *** of UT3PC overcomes these impediments in three easy ***, a material properties file is prepared for the considered ***, mesh generation is automatic through an interactive process.A third and last step is simply execution of the *** using data from several western coal mines illustrate the ease of using the application for analysis of main entries, barrier pillars, and bleeder entry safety.

During spermiogenesis, the protamine proteins play an integral role in spermatid chromatin compaction. Recent research has focused on many facets of protamine biology, including protamine gene and protein structure/ function relationships, mechanisms of protamine expression regulation and involvement of the protamines in male fertility. In this paper, we review our current understanding of the structure and function of the protamine-1 (P1) and protamine-2 (P2) proteins and genes, the expression and regulation of these genes and the relationship between the protamines and male fertility. In addition, we offer a brief outlook on future investigation into protamine proteins.

CLIL, which stands for Content and Language Integrated Learning, is an instructional approach that gives ample curricular and pedagogical attention to content and language outcomes in multilingual educational settings. Increasingly, it is heralded as a way to responsibly enact top-down English-Medium-of-Instruction (EMI) policies at the university level, where teachers and students are tasked with developing their English proficiency while remaining competitive in the international job market. However, teachers and teacher educators hoping to implement this approach in their science, technology, engineering and mathematics (STEM) content courses face significant challenges. This article serves as an introduction to a guest-edited special issue that reports on several aspects related to a project of international collaboration called Project SCILLA, an acronym for “STEM Content Integrated with Language-Learning Activities”. we first provide a brief overview of the project, which was developed and carried out in collaboration between Michigan State University and a consortium of 10 rural universities in Kazakhstan as a way to support STEM educators who wish to adapt their teaching practices to Kazakhstan’s Ministry of Education. we then offer an overview of the six articles that comprise the special issue, and call for deliberate and dialogic international collaboration as a way to support teachers responding to language policy demands.
