AIM: To systematically characterize specific pain patterns in the most frequent pancreatic ***: Pain in patients with chronic pancreatitis(n = 314), pancreatic cancer(n = 469), and other pancreatic tumors(n = 249) including mucinous(n = 20) and serous cystadenoma(n = 31), invasive(n = 37) and non-invasive intraductal papillary mucinous neoplasia(IPMN; n = 48), low stage(n = 18) and high stage neuroendocrine neoplasia(n = 44), and ampullary cancer(n = 51) was registered and correlated with clinicopathological data. Survival times were estimated by the Kaplan-Meier method. Patients alive at the follow-up time were censored. Survival curves were compared statistically using the log-rank ***: Forty-nine point one percent of pancreatic cancer patients revealed no pain, whereas in chronic pancreatitis only 18.3% were pain free. In contrary, moderate/severe pain was registered in 15.1% in pancreatic cancer patients that was increased in chronic pancreatitis with up to 34.2%. Serous cystadenoma was asymptomatic in most cases(58.1%), whereas 78.9% of all mucinous cystadenoma patients suffered pain. In neuroendocrine neoplasia pain was not a key clinical symptom since 64% of low stage neuroendocrine neoplasia and 59% of high stage neuroendocrine neoplasia patients were pain free. Cancer localization in the pancreatic body and patients with malignant pancreatic neoplasms were associated with more severe pain. Tumor grading and stage did not show any impact on pain. Only in pancreatic cancer, pain was directly associated with impaired ***: Pancreatic pain depicts different patterns of abdominal pain sensation according to the respective pancreatic disorder and does not allow a unification of the term pancreatic pain.

目的探讨溶酶体α－甘露糖苷贮积症基因治疗的可行性。方法以逆转录病毒为载体，将编码人溶酶体α－甘露糖苷酶ｃＤＮＡ导入病猫皮肤成纤维细胞。结果人源ｃＤＮＡ在病猫细胞内表达高活性α－甘露糖苷酶，重组酶的ｐＨ活性范围与正常酶相同，定位于细胞的溶酶体，分泌到细胞外后能通过甘露糖－６－磷酸受体介导的胞饮作用被其他细胞吸收。在体内，由重组病毒感染细胞组成的类器官表达有活性酶可达６周。结论此表达系统是研究溶酶体α－甘露糖苷贮积症基因治疗的有用模型。

The way that humans think about the earth certainly affects the way that they relate and behave towards the earth and all its life forms. The various ideologies or worldviews that humanity has experienced such as dualism, capitalism, holism, animism, ubuntu, ukama, vital force, colonialism, and so forth are all different ways of thinking and relating to the earth. The paper argues that dualistic ways of thinking or viewpoints that radically separate the human as distinct from the earth are hostile to both human and earth flourishing. It further argues that the ecological crisis which has witnessed the destruction and pollution of the ecosystems and other devastations cannot be solved when humans continue to see the earth as an object for conquest and exploitation. Using a critical hermeneutics method, the paper affirms that the African principle of relationality can help in mitigating the environmental crisis. It finds and concludes that if this principle is radically practiced, a better environment will be created.

Background: Significant resource constraints and critical care training gaps are responsible for the limited development of intensive care units (ICUs) in resource limited settings. We describe the implementation of an ICU in Haiti and report the successes and difficulties encountered throughout the process. We present a consecutive case series investigating an anesthesiologist, emergency, and critical care physician implemented endotracheal intubation and mechanical ventilation protocol in an austere environment with the assistance of telemedicine. Methods: A consecutive case series of fifteen patients admitted to an ICU at St. Luc Hospital located in Portau-Prince, Haiti, between the months of February 2012 to April 2014 is reported. Causes of respiratory failure and the clinical course are presented. Patients were followed to either death or discharge. Results: Fifteen patients (eight women and seven men) were included in the study with an average age of 37.7 years. The mean duration of ventilation was three days. Of the fifteen patients intubated, five patients (33.3%) survived and were discharged from the ICU. Of the five surviving patients, two were intubated for status epilepticus, one for status asthmaticus and one for hyperosmolar coma associated with intracerebral hemorrhage. Of the patients dying on the ventilator, four patients died from pneumonia, two from renal failure, and one from tetanus. The remaining three died from strokes and cardiac arrests. Conclusions: Mortality of mechanically ventilated patients in a resource-limited country is significant. Focused training in core critical care skills aimed at increasing the endotracheal intubation and ventilatory management capacity of local medical staff should be a priority in order to continue to develop ICUs in these austere environments. The BHAP (Bruce Highway Action Plan) Program evaluation was a momentous task conducted in most part by the TMR (Transport and Main Roads) CBA (cost benefit analysis) team. The evaluation included 189 overtaking lanes, 404 km of road widening and shoulder sealing in various places between Brisbane and Cairns, 56 capacity focused projects and 16 flood immunity focused projects. The total projected capital costs of all projects proposed as part of the BHAP amounted to over 16 billion dollars. The program evaluation conducted, due to the short timeframes, lack of available data and strategic nature of the plan, has been "coined" a strategic evaluation. This paper focuses on the methodology applied to the projects proposed in the BHAP. A TMR designed project/program evaluation model (CARP (Concise Analysis of Road Programs) V1.0) was used to evaluate the majority of the proposed BHAP projects. The model produces streams of discounted benefits and costs of the projects and program using limited and incomplete data. The large scale of work and the close proximity of projects allowed for an integrated approach to the analysis, which considered the impact projects have on each other. The result of the program, if all evaluated projects are included, is a BCR (benefit cost ratio) of approximately 0.71 at a discount rate of 7%. If the less viable projects are removed from the program, the program can obtain a benefit cost ratio of greater than 1 with a sufficiently large number of projects remaining.
