Contribution of CT Scanning in the Investigation of Cranioencephalic Lesions in Bangui

Introduction: Cranioencephalic exploration has always played a major role in CT scans. In the Central Africa Republic (CAR), the lack of cross-sectional imaging before the year 2020 meant that no study had focused on cranioencephalic lesions. The aim of this study was to contribute to improving the management of cranioencephalic pathologies in CAR. Patients and Method: The study took place at the Bangui National Medical Imaging Centre (CNIMB). It was a retrospective study over a two-year period (March 1, 2021 to February 30, 2023). All patients referred for cranioencephalic CT scans were included, regardless of age or sex. Results: 1745 CT scans were performed, 575 of which were cranioencephalic CT scans. The majority of patients were male (53%). Most lived in the capital Bangui (90.9%). Patients aged 61 and over were the most representative. The distribution of patients by requesting department showed that the reception and emergency department was one of the least requesting departments. The main abnormalities observed were strokes, 82.1% of which were ischaemic strokes and 17.9% haemorrhagic strokes. Strokes were followed by degenerative lesions. Post-traumatic injuries included haemorrhagic contusions (38.3%), subdural haematomas in 20.5% of cases, and extradural haematomas (9.3%). Craniofacial lesions (fractures) were observed in 45.8% of cases. Conclusion: Cranioencephalic scans accounted for 1/3 of CT examinations performed during the study period. It revealed pathologies that could not be detected by conventional means. All in all, CT scans contributed to the diagnosis of cerebral pathologies.

Individual and geospatial factors associated with receipt of colorectal cancer screening:a state-wide mixed-level analysis

Introduction :Congenital adrenal hyperplasia (CAh) is the most common cause of primary adrenal insufficiency. It is a rare monogenic recessive disorder. In African setting in absence of neonatal screening, the diagnosis is still late, based on a clinical approach. During this clinical enquiry, information from past history or pedigree of the patient is of a huge importance and may revealed surprises. Patients and Methods: In this observational study, we retrospectively included all patients with a diagnosis of CAh. The diagnosis of CAh was retained based on a high 17 hydroxyprogesterone level in addition to clinical and morphological findings. From patients’ files, we extracted data on family history of disease, pedigree, clinical findings and genetics when available of 39 patients from two endocrinopeadiatric centers. Results: In 13 (30%) families, we found 20 reported deaths of infant less than 12 months. In these 13 families, half of the patients followed had 21 hydroxylase deficiencies and had 11 hydroxylase deficiencies. Unsurprisingly, we suspected adrenal insufficiency in these patients at verbal autopsy even in families with a patient with 11 hydroxylase deficiency. Other non DSD malformations or genetic disorders with apparently no link with CAh were reported in 3 families. The father of a patient reported to have hypospadias. Conclusion: each diagnosis of CAh made in our context is visible part of an iceberg. Behind a diagnosis of CAh made in our setting, is a long course of care, a dramatic past history revealing access to appropriate care disparity. Neonatal screening should thus be considered as an emergency.

Contribution of CT in the exploration of Non-Traumatic Pathologies of the Lumbar Spine in Adults in Bangui

Background: In Central African Republic, a study carried out in 2019 highlighted the limits of conventional radiography in the diagnosis of non-traumatic lower back pain in adults. Objective: The objective of this study is to show the value of CT scanning in the exploration of non-traumatic pathologies of the lumbar spine in adults. Patients and Method: Retrospective and descriptive cross-sectional study covering the files of patients aged at least 18 years old, sent for lumbar scanning from March 1 to December 31, 2021 at the medical imaging center. Results: In total, 593 scan examinations were performed including 159 (26.8%) examinations of the lumbar spine. The average age of the patients was 49.84 years. The majority were male (66%). 127 (79.8%) were referred mainly to the neurology and rheumatology departments. Common low back pain (n = 97, 61%) was the main reason for requesting a CT scan. The lumbar scan was performed without contrast product in 88.7% of cases. In total, 148 (93.1%) examinations were pathological and dominated by overall disc overhang (62.2%) and somatic and interapophyseal osteoarthritis (16.9%). The other lesions were spondylodiscitis (7.4%), tumor-like lesions (3.4%) and narrow lumbar canal (1.4%).

Condition monitoring of the insulating system within power transformers has a massive importance according to the electrical *** gas analysis(DGA)is frequently used for this ***,DGA lacks the necessary level of accuracy to identify all equipment faults,particularly in their initial stages of ***,it does not have the capability for real-time monitoring and relies on manual sampling and laboratory testing,causing potential delays in fault ***,the interpretation of DGA data necessitates specialised expertise,which may pose difficulties for smaller entities that have limited access to ***,the contribution of this research is to use infrared spectroscopy measurements as a new effective technique substituting the DGA method for fault diagnosis in insulating *** inception faults that were considered in this study were the electrical fault(discharges of high energy)and the thermal fault(300°Cemperatureegarding that,two test cells were crafted especially for serving the simulation processes inside the laboratory for both types of inception ***,six samples of pure paraffinic mineral oil were taken to be degraded in the *** that,all of them besides another sample that were not subjected to any kind of faults were taken to be examined by Fourier transform infrared(FTIR)spectroscopy to obtain an overview of the oil's behaviour in each fault *** that,the FTIR analysis was initially verified utilising the DGA ***,for further affirmation,the dielectric dissipation factor(DDF)for all samples was *** the final analysis,the verification tests provide experimental evidence about the outperformance of this new optical technique in detecting the transformer's inception faults in addition to proving its potential for being a superior alternative to the well-known traditional diagnostic techniques.

Artificial intelligence(AI)as a multi-purpose technology is gaining increased attention and is now widely used across all sectors of the *** growing complexity of planning and operating power systems makes AI extremely valuable for the power *** now,there has been a lack of clarity regarding the specific points along the power system supply chain where AI applications demonstrate significant value,as well as which AI domains are best suited for such *** study employs an AI taxonomy and automated web search to qualitatively and quantitatively unveil the biggest potentials of AI in the power *** analysis,based on a review of 258’919 publications between 1982 and 2022,reveals where AI applications are particularly *** consider six AI domains(reasoning,planning,learning,communication,perception,integration&interaction)and 19 use cases from the power supply chain(i.e.,generation,transmission networks,distribution networks,isolated grids/microgrids,market operations and retail).Our findings indicate that,as of now,the focus is predominantly on AI applications in power retail(55%),transmission(14%)and generation(13%).Most analyzed works describe applications built on algorithms of the AI domains“learning”(45%)and“planning”(14%).Results also suggest that the current definition of AI domains is ambiguous,and they highlight missing information on the actual use and successful implementation of AI in power system use cases.

A new framework for reverse cholesterol transport: Non-biliary contributions to reverse cholesterol transport

Reduction of low-density lipoprotein-cholesterol through statin therapy has only modestly decreased coronary heart disease (ChD)-associated mortality in developed countries, which has prompted the search for alternative therapeutic strategies for ChD. Major efforts are now focused on therapies that augment high-density lipoprotein (hDL)-mediated reverse cholesterol transport (RCT), and ultimately increase the fecal disposal of cholesterol. The process of RCT has long been thought to simply involve hDL-mediated delivery of peripheral cholesterol to the liver for biliary excretion out of the body. however, recent studies have revealed a novel pathway for RCT that does not rely on biliary secretion. This nonbiliary pathway rather involves the direct excretion of cholesterol by the proximal small intestine. Compared to RCT therapies that augment biliary sterol loss, modulation of non-biliary fecal sterol loss through the intestine is a much more attractive therapeutic strategy, given that excessive biliary cholesterol secretion can promote gallstone formation. however, we are at an early stage in understanding the molecular mechanisms regulating the non-biliary pathway for RCT, and much additional work is required in order to effectively target this pathway for ChD prevention. The purpose of this review is to discuss our current understanding of biliary and nonbiliary contributions to RCT with particular emphasis on the possibility of targeting the intestine as an inducible cholesterol secretory organ.

Behavioral effects of social challenges and genomic mechanisms of social priming： What＇s testosterone got to do with it？

Social challenges from rival conspecifics are common in the lives of animals, and changes in an animal＇s social environment can influence physiology and behavior in ways that appear to be adaptive in the face of continued social instability （i.e. social priming）. Recently, it has become clear that testosterone, long thought to be the primary mediator of these effects, may not always change in response to social challenges, an observation that highlights gaps in our understanding of the proximate me- chanisms by which animals respond to their social environment. here, our goal is to address the degree to which testosterone me- diates organismal responses to social cues. To this end, we review the behavioral and physiological consequences of social chal- lenges, as well as their underlying hormonal and gene regulatory mechanisms. We also present a new case study from a wild songbird, the dark-eyed junco Junco hyemalis, in which we find largely divergent genome-wide transcriptional changes induced by social challenges and testosterone, respectively, in muscle and liver tissue. Our review underscores the diversity of mechani- sms that link the dynamic social environment with an organisms＇ genomic, hormonal, and behavioral state. This diversity among species, and even among tissues within an organism, reveals new insights into the pattern and process by which evolution may alter proximate mechanisms of social priming.
