Objectives:Patients with obstructive sleep apnea(OSA)are at increased risk of perioperative and postoperative *** use of continuous positive airway pressure(CPAP)in the perioperative period may be of potential ***,among patients who have undergone endonasal skull base surgery,many surgeons avoid prompt re-initiation of CPAP therapy due to the theoretical increased risk of epistaxis,excessive dryness,pneumocephalus,repair migration,intracranial introduction of bacteria,and cerebrospinal fluid(CSF)*** objective of this article is to review the most up-to-date literature regarding when it is safe to resume CPAP usage in the patient undergoing endonasal skull base *** Sources and Methods:This review combines the most recent literature as queried through PubMed regarding the safety of CPAP resumption following endonasal skull base ***:Recent surveys of skull base surgeons demonstrate little consensus regarding the post-operative management of *** cadaveric studies suggest that approximately 85%of delivered CPAP pressures are transmitted to the sphenoid ***,at frequently prescribed CPAP pressure settings,common sellar reconstruction techniques maintain their integrity while preventing very little transmission of pressure into the *** small retrospective case series,patients with OSA who received CPAP immediately following transsphenoidal pituitary surgery had similar rates of surgical complications as OSA patients who did not receive CPAP in the immediate post-operative *** of reinitiating CPAP too early,such as the development of pneumocephalus,rarely ***:There remains a paucity of objective data regarding when it is safe to resume CPAP following endonasal skull base *** cadaveric studies and small retrospective case series suggest that it may be safe to resume CPAP earlier than is often practiced following endonasal skull base surgery.

Monkey b virus(Macacine herpesvirus 1; bV) is noted for its extreme neurovirulence in humans. Since the vhs protein encoded by the UL41 gene has been shown to be a neurovirulence factor in the related human herpes simplex viruses, the role of the UL41 gene in bV neurovirulence was investigated. bV mutants were constructed that lacked the entire UL41 ORF(Δ41) or had the RNase active site mutated(Δ41A). Neither mutant shut off host protein synthesis, degraded β-actin mRNA, or prevented an IFN-β response, indicating that the vhs protein and its RNase activity are both necessary for these activities. Replication of both mutants in primary mouse cells was impaired and they exhibited a prolonged disease course in mice. Whereas Δ41 infected mice were euthanized for symptoms related to central nervous system(CNS) infection, Δ41A infected mice were euthanized primarily for symptoms of autonomic nervous system dysfunction. While neuroinvasiveness was not affected, lesions in the CNS were more limited in size, anatomical distribution, and severity than for wild-type virus. These results indicate that the vhs protein affects the general replicative efficiency of bV in vivo rather than being a specific neurovirulence factor critical for invasion of or preferential replication in the CNS.

Fatigue Performance of Fabrics of Stent-Grafts Supported with Z-Stents vs. Ringed Stents

Stent-grafts were commercialized rapidly and gained a broad clinical acceptance over the past two decades. However,relatively more recent recognition of particular stent-graft design shortcomings have been identified which need to be addressed. It appears that various stent-graft designs may be more or less resistant to metal fatigue and /or fabric abrasions which can lead to type III and type IV endoleaks over the long term. Therefore,it is necessary to investigate the fatigue performance of the most common stentgraft designs: Z-stents and ringed stents,in a long-term in vitro fatigue simulation environment. This paper aimed to analyze nondestructively( gross observations) and destructively( fabric characteristics,mechanical and chemical properties) in order to put forward suggestions to improve the fabric and stent characteristics that may prevent type III and IV endoleaks. The fabric supported with ringed stent-grafts remained nearly completely intact after 168h. However, the fabric supported with Z-stents demonstrated significant damage. Fabric characteristics and tensile strength of the fibers did not present a significant difference between the control and fatigue simulated specimens. The crystallinity declined for both specimens. The fatigue performance of fabrics supported with ringed stents appears to be superior to that supported with Z-stents. The potential for a dynamic and destructive interaction between the apices of Z-stents which can lead to fraying and /or tearing of the graft fabric must be addressed in future designs.

Severe injuries are the major cause of death in those aged under 40,mainly due to road traffic ***,metabolic and immune pathways respond to limit the tissue damage sustained and initiate wound healing,repair and regeneration ***,depending on age and sex,the response to injury and patient prognosis differ *** are catabolic and immunosuppressive and are produced as part of the stress response to injury leading to an intra-adrenal shift in steroid biosynthesis at the expense of the anabolic and immune enhancing steroid hormone dehydroepiandrosterone(DHEA)and its sulphated metabolite dehydroepiandrosterone sulphate(DHEAS).The balance of these steroids after injury appears to influence outcomes in injured humans,with high cortisol:DHEAS ratio associated with increased morbidity and *** models of trauma,sepsis,wound healing,neuroprotection and burns have all shown a reduction in proinflammatory cytokines,improved survival and increased resistance to pathological challenges with DHEA *** supplementation studies,which have focused on post-menopausal females,older adults,or adrenal insufficiency have shown that restoring the cortisol:DHEAS ratio improves wound healing,mood,bone remodelling and psychological ***,there are no DHEA or DHEAS supplementation studies in trauma patients,but we review here the evidence for this potential therapeutic agent in the treatment and rehabilitation of the severely injured patient.

Predictive health monitoring of micro gas turbines can significantly increase the availability and reduce the operating and maintenance *** for predictive health monitoring are typically developed for large-scale gas turbines and have often focused on single *** an effort to enable fleet-level health monitoring of micro gas turbines,this work presents a novel data-driven approach for predicting system degradation over *** approach utilises operational data from real installations and is not dependent on data from a reference *** problem was solved in two steps by:1)estimating the degradation from time-dependent variables and 2)forecasting into the future using only running *** regression technique is employed both for the estimation and forecasting of *** method was evaluated on five different systems and it is shown that the result is consistent(r>0.8)with an existing method that computes corrected values based on data from a reference system,and the forecasting had a similar performance as the estimation model using only running hours as an input.

Dielectric elastomers are widely investigated as soft electromechanically active polymers(EAPs)for actuators,stretch/force sensors,and mechanical energy harvesters to generate *** the performance of such devices is limited by the dielectric strength of the constitutive material,the electrical breakdown of soft elastomers for electromechanical transduction is still scarcely ***,we describe a custom-made setup to measure electrical breakdown of soft EAPs,and we present data for a widely studied acrylic elastomer(VHb 4905 from 3M).The elastomer was electrically stimulated via a planar and a hemispherical metal *** breakdown was characterized under different conditions to investigate the effects of the radius of curvature and applied force of the hemispherical *** a given radius of curvature,the breakdown field increased by about 50% for a nearly 10-fold increase of the applied mechanical stress,while with a given mechanical stress the breakdown field increased by about 20% for an approximately two fold increase of the radius of *** results indicate that the breakdown field is highly dependent on the boundary conditions,suggesting the need for reporting breakdown data always in close association with the measurement *** findings might help future investigations in elucidating the ultimate breakdown mechanism/s of soft elastomers.

big Data Knowledge Pricing Schemes for Knowledge Recipient Firms

big data knowledge,such as customer demands and consumer preferences,is among the crucial external knowledge that firms need for new product development in the big data *** research has focused on the profit of big data knowledge providers rather than the profit and pricing schemes of knowledge *** research addresses this theoretical gap and uses theoretical and numerical analysis to compare the profitability of two pricing schemes commonly used by knowledge recipients:subscription pricing and pay-per-use *** find that:(1)the subscription price of big data knowledge has no effect on the optimal time of knowledge transaction in the same pricing scheme,but the usage ratio of the big data knowledge affects the optimal time of knowledge transaction,and the smaller the usage ratio of big data knowledge the earlier the big data knowledge transaction conducts;(2)big data knowledge with a higher update rate can bring greater profits to the firm both in subscription pricing scheme and pay-per-use pricing scheme;(3)a knowledge recipient will choose the knowledge that can bring a higher market share growth rate regardless of what price scheme it adopts,and firms can choose more efficient knowledge in the pay-per-use pricing scheme by adjusting the usage ratio of knowledge usage according to their economic *** model and findings in this paper can help knowledge recipient firms select optimal pricing method and enhance future new product development performance.
