Isolated colonic neurofibroma in the setting of Lynch syndrome:A case report and review of literature

bACKGROUND Gastrointestinal neurofibromas are commonly found in patients diagnosed with neurofibromatosis type ***,isolated gastrointestinal neurofibromas are a rare entity and only fourteen cases of isolated colorectal neurofibromas have been documented in *** gastrointestinal neurofibromas have not been associated with Lynch syndrome(LS).Patients with LS are at an increased risk of colorectal cancer,and are recommended to undergo screening *** SUMMARY A 33-year-old healthy female with a family history of LS was found to have unresectable polyp in the ascending colon on screening colonoscopy suspicious for *** patient was asymptomatic and had no stigmata of neurofibromatosis.A staging workup for colorectal cancer revealed no evidence of metastatic disease.A discussion with the patient resulted in the decision to undergo a segmental resection with ongoing *** patient underwent a laparoscopic right *** was consistent with a gastrointestinal ***-operatively,the patient recovered *** will not require further treatment with regards to her colonic neurofibroma,but will continue to follow-up for ongoing surveillance of her *** We present the first case of an isolated colonic neurofibroma in a patient with *** case explores considerations for the management of isolated gastrointestinal neurofibromas given the lack of guidelines in literature.

Adaptive Classification Methods for Predicting Transitions in the Nursing Workforce

Earlier analyses of transitions from licensed practical nurse (LPN) to registered nurse (RN) in the North Carolina (NC) nursing workforce in terms of 11 categorical predictors were limited by not considering parsimonious classifications based on these predictors and by substantial amounts of missing data. To address these issues, we formulated adaptive classification methods. Secondary analyses of data collected by the NC State board of Nursing were also conducted to demonstrate adaptive classification methods by modeling the occurrence of LPN-to-RN transitions in the NC nursing workforce from 2001-2013. These methods combine levels (values) for one or more categorical predictors into parsimonious classifications. Missing values for a predictor are treated as one level for that predictor so that the complete data can be used in the analyses;the missing level is imputed by combining it with other levels of a predictor. An adaptive nested classification generated the best model for predicting an LPN-to-RN transition based on three predictors in order of importance: year of first LPN licensure, work setting at transition, and age at first LPN licensure. These results demonstrate that adaptive classification can identify effective and parsimonious classifications for predicting dichotomous outcomes such as the occurrence of an LPN-to-RN transition.

Functional MRI was used to map the brains of subjects on-line during the process of media training for the acquisition and improvement of self-regulation mechanisms. The temporal and spatial dynamics of the new neural network formation were studied in real and simulated (false) biofeedback game, and their qualitative characteristics were discussed. It has been shown that immersion into a virtual competitive game, controlled by physiological responses, causes a wide involvement of the cortices, characterized by a high volume of activation in the mid-temporal, occipital and frontal areas, the cuneus and the precuneus. In both forms of media training, high values of activation volume were identified in the cerebellar structures.

Over-expression of the cytochrome P450 CYP6CM1 gene has been associated with imidacloprid resistance in a number of Q and b biotype bemisia tabaci laboratory strains from distinct geographical origins worldwide. We recently demonstrated that the Q biotype version of the CYP6CM 1 protein （CYP6CMlvQ） is capable of metabolizing imida- cloprid. Here, we show that the levels of btCYP6CMlvQ were also elevated in laboratory- resistant strains and field-derived populations, with variable imidacloprid resistance levels, collected in Crete. High levels of CYP6CMlvQ transcripts were also determined in survivors of a heterogeneous field population, after exposure to discriminating imidacloprid dosage. Using peptide antibody-based detection assays, we demonstrated that in line with transcriptional data, the CYP6CMlvQ protein levels were higher in imidacloprid-resistant insects, which further implicates the gene as the causal factor of resistance. Finally, assess- ment of the cross-metabolism potential of CYP6CMlvQ against additional neonicotinoid molecules used for b. tabaci control revealed that clothianidin and thiacloprid, but not acetamiprid or thiamethoxam, are metabolized by the recombinant enzyme in vitro.

We present a combination of machine learning and high throughput calculations to predict the points defects behavior in binary intermetallic(A–b)compounds,using as an example systems with the cubic b2 crystal structure(with equiatomic Ab stoichiometry).To the best of our knowledge,this work is the first application of machine learning-models for point defect *** throughput first principles density functional calculations have been employed to compute intrinsic point defect energies in 100 b2 intermetallic *** systems are classified into two groups:(i)those for which the intrinsic defects are antisites for both A and b rich compositions,and(ii)those for which vacancies are the dominant defect for either or both composition *** data was analyzed by machine learning-techniques using decision tree,and full and reduced multiple additive regression tree(MART)*** these three schemes,a reduced MART(r-MART)model using six descriptors(formation energy,minimum and difference of electron densities at the Wigner–Seitz cell boundary,atomic radius difference,maximal atomic number and maximal electronegativity)presents the highest fit(98%)and predictive(75%)*** model is used to predict the defect behavior of other b2 compounds,and it is found that 45%of the compounds considered feature vacancies as dominant defects for either A or b rich compositions(or both).The ability to predict dominant defect types is important for the modeling of thermodynamic and kinetic properties of intermetallic compounds,and the present results illustrate how this information can be derived using modern tools combining high throughput calculations and data analytics.

Testicular expression of survivin and human telomerase reverse transcriptase(hTERT)associated with spermatogenic function in infertile patients

Aim： To characterize the coexpression of survivin, an inhibitor of apoptosis （IAF）, and human telomerase reverse transcriptase （hTERT） in human testes with varying spermatogenic function. Methods： Transcript levels of survivin mRNA and hTERT mRNA were determined in normal testes （n = 11） and testes with defective spermatogenesis （n = 28） using real-time reverse-transcription polymerase chain reaction （RT-PCR）. The histological work-up was performed according to a modified Johnsen score. Results： Expressions of both survivin and hTERT were highest at median levels of 96.8 and 709 in normal spermatogenesis and dropped to 53.3 and 534 in testes with postmeiotic spermatogenic arrest （n = 10）. In severe spermatogenic failure （n = 18）, survivin expression was lacking in most specimens （n = 16）, whereas at least low levels of testicular hTERT expression were largely detectable with a normalized expression of 73 in premeiotic spermatogenic arrest （n = 7） and 45 in patients with Sertoli cell-only syndrome （SCOS）（n = 3）. both survivin and hTERT expressions increased with a progressing Johnsen score （P for trend = 0.001）. Conclusion： Although both survivin and hTERT are correlated with spermatogenic function, they show different expression patterns in testes of infertile patients. HAX1 deletion impairs bCR internalization and leads to delayed bCR-mediated apoptosis

Deletion of HAX1 in mice causes a severe reduction in the numbers of lymphocytes in the bone marrow and in the spleen. Additionally, b220＋ b progenitor cells in the bone marrow are reduced, suggesting an important function of HAX1 in b cell development. HAX1 is thought to play a protective role in apoptotic processes; therefore, we investigated the role of HAX1 in bone marrow b progenitor cells and splenic b cells. We did not observe an effect on the survival of Hax1-/- bone marrow cells but detected enhanced survival of splenic Hax1-/- b cells upon in vitro starvation/ growth-factor withdrawal. To explain this apparent inconsistency with previous reports of HAX1 function, we also studied the b cell receptor （bCR）-induced apoptosis of IgM-stimulated splenic naive b cells and found that apoptosis decreased in these cells. We further found impaired internalization of the bCR from Hax1-/- splenic b cells after IgM crosslinking; this impaired internalization may result in decreased bCR signaling and, consequently, decreased bCR-mediated apoptosis. We measured HAX1 binding to the cytoplasmic domains of different Ig subtypes and identified KVKWI（V）F as the putative binding motif for HAX1 within the cytoplasmic domains. because this motif can be found in almost all Ig subtypes, it is likely that HAX1 plays a general role in bCR-mediated internalization events and bCR-mediated apoptosis.
