A Comparative Retrospective Analysis of Mortality, Renal Dysfunction, and Incidence of Bleeding in Patients Receiving Hydroxyethyl Starch 130/0.4 (HES 130/0.4) or Albumin

Background: In June 2013, the FDA issued a Black Box Warning on synthetic colloids resulting from data indicating an increased risk of mortality, renal injury, and excess bleeding incidence in septic patients admitted to the Intensive Care Unit (ICU), and patients undergoing coronary artery bypass graft (CABG) surgery. The purpose of this study was to compare the incidence of mortality, incidence/severity of renal injury and bleeding in septic ICU patients, and patients undergoing CABG surgery who have received hydroxyethyl starch 130/0.4 or albumin. Methods: This was a retrospective, single center, cohort study conducted at a 433 bed tertiary care hospital in Lexington, Kentucky. Computer generated lists provided patients who received hydroxyethyl starch 130/0.4 or albumin during inpatient stay as either a sepsis patient in the ICU or post coronary artery bypass graft patient. All cause mortality was the primary endpoint and secondary endpoints included overall renal dysfunction, incidence of bleeding, and overall length of stay. Results: A total of 89 patients were evaluated that received albumin or hydroxyethyl starch 130/0.4 alone with either septic ICU or CABG diagnosis codes. There was no significant difference in all cause mortality (p = 0.64). Overall renal dysfunction showed no statistically significance between the two groups (p = 0.66). There was no statistically significant difference between the albumin or HES 130/0.4 arms with respect to bleeding (TIMI Minimal p = 0.57, TIMI Minor p = 0.69, TIMI Major p = 0.35). Patient who received albumin had a statistically significant longer hospital overall length of stay (p = 0.003). Conclusion: There was no difference between HES 130/0.4 and albumin in mortality, renal dysfunction, and bleeding in septic ICU and CABG patients. Implications of the VerifyNow P2Y12 Assay on Patient Outcomes

Background: The platelet inhibitory response of clopidogrel is substantially variable among patients, and numerous studies have shown that post-percutaneous intervention, patients with high on-treatment platelet reactivity have an increase in risk of major adverse cardiovascular events. No published studies to date have utilized platelet function monitoring assays prior to coronary artery bypass graft (CABG) surgery, but determination of patients’ antiplatelet effects prior to surgery may decrease time to surgery and length of hospital stay. The purpose of the study was to evaluate the clinical outcomes of non-elective CABG patients analyzed by the VerifyNow P2Y12 platelet-function monitoring assay prior to surgery compared to a similar set of patients not analyzed by the VerifyNow P2Y12 assay. Methods: This was a retrospective, single center, cohort study. The primary endpoints of this study were time to surgery and length of hospital stay. Results: From March 2013 to July 2013, 60 patient charts were reviewed and included in this study. 49 patients were analyzed by the VerfiyNow P2Y12 assay, and 16 of these patients underwent non-elective CABG surgery. Eleven patients underwent non-elective CABG surgery and were not analyzed by the VerifyNow P2Y12 assay. There was no difference between groups regarding time to surgery (p = 0.75) or length of stay (p = 0.42). Based on the assay’s P2Y12 reaction unit results, 69% of VerifyNow P2Y12 patients went to surgery sooner than the institution’s recommendations which generated more bleeding events, half of which were considered major bleeds. Conclusions: Utilization of the VerifyNow P2Y12 assay prior to non-elective CABG surgery does not shorten time to surgery or overall length of hospital stay. However, insufficient P2Y12 reaction units prior to surgery may lead to more bleeding events, thus the application of platelet function monitoring assays prior to procedures may be beneficial as a bleeding risk-assessment tool.

Single versus Dual Antiplatelet Therapy after CABG Surgery

Background: The optimal antiplatelet therapy regimen in the early post-operative CABG period is not well researched. Several studies have proven that use of clopidogrel and aspirin prior to surgery increases bleed risks, but very few studies have examined this therapy post-operatively. Due to the limited amount of data surrounding this topic, the goal of this study was to determine if single or dual antiplatelet therapy had better outcomes in those status-post CABG surgery. Methods: This was a retrospective, single center, cohort study performed at Saint Joseph Hospital in Lexington, Kentucky. Data was reviewed through the Society of Thoracic Surgeons (STS) database, pharmacy medication dispensing records, as well as manual chart review. The primary composite endpoint of the study consisted of in-hospital mortality, ischemic or thrombotic events, bleeding events, restenosis rates, and 30-day readmission rates. Results: The number of events with regard to the primary composite endpoint was 32 events with combination therapy and 39 events with aspirin monotherapy (p = 0.39). A greater decrease in hemoglobin and hematocrit was seen in the aspirin monotherapy group (p = 0.02 and p = 0.047). Patients with prior CVA or TIA were more commonly placed on combination therapy after surgery (p = 0.018). There were no differences in outcomes when type of antiplatelet therapy and type of CABG were analyzed. Conclusions: There was no difference seen between single versus dual antiplatelet therapy regarding the primary composite endpoint. There was an increase in bleeding events with aspirin monotherapy as defined by TIMI criteria as well as a statistically significant decrease in Hgb and HCT with aspirin monotherapy. Patients with previous CVA/TIA were more likely to receive combination therapy. The average number of vessels grafted per surgery was lower in the off-pump surgery cohort.

Impact of Brunswick River Mouth Training Walls on Adjacent Beaches,Brunswick Heads,New South Wales,Australia

River training walls have been built at scores of locations along the NSW coast and their impacts on shoreline change are still not fully understood. In this study, the Brunswick River entrance and adjacent beaches are selected for examination of the impact of the construction of major training walls. Thirteen sets of aerial photographs taken between 1947 and 1994 are used in a GIS approach to accurately determine the shoreline position, beach contours and sand volumes, and their changes in both time and space, and then to assess the contribution of both the structures and natural hydrodynamic conditions to large scale (years-decades and kilometres) beach changes. The impact of the training walls can be divided into four stages: natural conditions prior to their construction (pre 1959), major downdrift erosion and updrift accretion during and following the construction of the walls in 1959～1962 and 1966, diminishing impact of the walls between 1966 and 1987, and finally no apparent impact between 1987～1994. The impact extends horizontally about 8 km updrift and 17 km downdrift, and temporally up to 25 years.

Where physics meets chemistry： Thin film deposition from reactive plasmas

Functionalising surfaces using polymeric thin films is an industrially important field. One technique for achieving nanoscale, controlled surface functionalization is plasma deposition. Plasma deposition has advantages over other surface engineering processes, including that it is solvent free, substrate and geometry independent, and the surface properties of the film can be designed by judicious choice of precursor and plasma conditions. Despite the utility of this method, the mechanisms of plasma polymer growth are generally unknown, and are usually described by chemical （i.e., radical） pathways. In this review, we aim to show that plasma physics drives the chemistry of the plasma phase, and surface-plasma interactions. For example, we show that ionic species can react in the plasma to form larger ions, and also arrive at surfaces with energies greater than 1000 kJ·mol^-1 （〉10 eV） and thus facilitate surface reactions that have not been taken into account previously. Thus, improving thin film deposition processes requires an understanding of both physical and chemical processes in plasma.
