Immune signature of small bowel adenocarcinoma and the role of tumor microenvironment

In this editorial we comment on the article published“Clinical significance of programmed cell death-ligand expression in small bowel adenocarcinoma is determined by the tumor microenvironment”.Small bowel adenocarcinoma(SBA)is a rare gastrointestinal neoplasm and despite the small intestine's significant surface area,SBA accounts for less than 3%of such *** detection is challenging and the reason arises from its asymptomatic nature,often leading to late-stage discovery and poor *** involves platinum-based chemotherapy with a 5-fluorouracil combination,but the lack of effective chemotherapy contributes to a generally poor *** are linked to genetic disorders and risk factors,including chronic inflammatory *** unique characteristics of the small bowel,such as rapid cell renewal and an active immune system,contributes to the rarity of these tumors as well as the high intratumoral infiltration of immune cells is associated with a favorable *** cell death-ligand 1(PD-L1)expression varies across different cancers,with potential discrepancies in its prognostic *** instability(MSI)in SBA is associated with a high tumor mutational burden,affecting the prognosis and response to *** presence of PD-L1 and programmed cell death 1,along with tumor-infiltrating lymphocytes,plays a crucial role in the complex microenvironment of SBA and contributes to a more favorable prognosis,especially in the context of high MSI *** tumor-infiltrating lymphocytes are identified as independent prognostic indicators and the association between MSI status and a favorable prognosis,emphasizes the importance of evaluating the immune status of tumors for treatment decisions.

Revolutionizing gastric cancer treatment:The potential of immunotherapy

Oxidative damage,pro-inflammatory cytokines,TGF-αand c-myc in chronic HCV-related hepatitis and cirrhosis

AIM： To assess whether a correlation exists between oxidative DNA damage occurring in chronic HCV-related hepatitis and expression levels of pro-inflammatory cytokines, TGF-α and c-myc. METHODS： The series included 37 patients with chronic active HCV-related hepatitis and 11 with HCV-related compensated cirrhosis. Eight-hydroxydeoxyguanosine in liver biopsies was quantified using an electrochemical detector. The mRNA expression of TNF-α, IL-1β, TGF-α and c-myc in liver specimens was detected by semiquantitative comparaUve RT-PCR. RESULTS： TNF-α levels were significantly higher in hepatitis patients than in cirrhosis patients （P=0.05）. IL-1β was higher in cirrhosis patients （P=0.05）. A sig- nificant correlation was found between TNF-α and staging （P= 0.05） and between IL-1β levels and grading （P=0.04）. c-myc showed a significantly higher expression in cirrhosis patients （P=0.001）. Eight-hydroxydeoxyguanosine levels were significantly higher in cirrhosis patients （P=0.05） and in HCV genotype 1 （P=0.03）. Considering all patients, 8-hydroxydeoxyguanosine levels were found to be correlated with genotype （P=0.04） and grading （P=0.007）. Also multiple logistic regression analysis demonstrated a significant correlation among the number of DNA adducts, TNF-α expression and HCV genotype （P= 0.02）. CONCLUSION： In chronic HCV-related liver damage, oxidative DNA damage correlates with HCV genotype, grading and TNF-α levels. As HCV-related liver damage progresses, TNF-α levels drop while IL-1β and c-myc levels increase, which may be relevant to liver carcinogenesis.

AIM To show the existence of a structural formative role of magnetic fields(MFs) with respect to biological objects by using our proposed model of an *** We introduced a magnetised 10-100 μT metal rod(needle) into culture dishes with a negatively charged working surface and observed during 24 h how cells were arranged by MFs and by electrical fields(EFs) when attached. Rat and human bone marrow-derived stromal stem cells(r BMSCs and h BMSCs), human nonadherent mononuclear blood cells, NCTCs and A172 cells, and Escherichia coli(E. coli) were evaluated. The dish containing BMSCs was defined as the model of an acupoint. r BMSCs proliferative activity affected by the needle was investigated. For investigating electromagnetic field structures, we used the gas discharge visualisation(GDV) *** During 24 h of incubation in 50-mm culture dishes, BMSCs or the nonadherent cells accumulated into a central heap in each dish. BMSCs formed a torus(central ring) with an inner diameter of approximately10 mm only upon the introduction of the needle in the centre of the dish. The cells did not show these effects in 35- or 90-mm culture dishes or hydrophobic dishes or rectangular cuvettes. NCTCs and A172 cells showed unstable the effects and only up to two weeks after thawing. Moreover, we observed that the appearance of these effects depended on the season. In winter, BMSCs showed no the effects. GDV experiments revealed that the resonant annular illumination gradually formed from 10 to 18-20 s in polar solutions with and without cell suspension of BMSCs, NCTCs and E. coli when using circular 50-mm dishes, stimulation at 115 V and switching of the electrode poles at 1 kH z. All these data demonstrate the resonant nature of the central ring. Renal aspergillosis in a liver transplant patient: A case report and review of literature

BACKGROUND Aspergillosis is a frequent invasive fungal infection in liver recipients(affecting 1%-9.2% of all patients), second only to candidiasis. Significant risk factors for invasive aspergillosis in liver recipients include corticosteroid therapy, neutropenia, T-cell dysfunction, renal failure and requirement for renal replacement therapy. Aspergillus infection usually affects the lungs of liver recipients, with hematogenous dissemination occurring in 50%-60% of cases. Renal involvement is rare and is considered to occur in 0.4% of all cases of invasive *** SUMMARY This paper describes a case of a liver recipient presenting with a newly formed renal mass a year after liver transplantation. The patient underwent liver transplantation due to alcoholic liver cirrhosis, with preoperative corticosteroid therapy and postoperative immunosuppressants(tacrolimus and mycophenolate mofetil). His 1-year follow-up was uneventful, with a satisfying graft function and lack of any symptoms. During a routine follow-up abdominal ultrasound, he was diagnosed with a renal tumor. The renal ima-ging findings were inconclusive(with a differential diagnosis to renal cell carcinoma), while the computed tomography(CT) of the chest showed scar tissue in the lungs suggestive of previous inflammation. The patient underwent radical nephrectomy, with histopathological analysis showing renal aspergilloma, yielding postoperative treatment with voriconazole. His follow up was uneventful, and the chest CT did not show any change in pulmonary lesions. This case illustrates the possibility of aspergillosis affecting the lungs of liver recipients, subsequently affecting the kidney and forming an *** Clinicians should be aware of aspergilloma mimicking solid organ tumors in organ recipients.

Geographical Distribution and Morphological Variability of the Rapid Racerunner,Eremias velox (Pallas,1771)(Reptilia,Lacertidae) in the Eastern Periphery of Its Range

Phenotypic traits are usually correlated with the environment where organism occurs. In this study, the distribution of Eremias velox in the eastern periphery of its range was specified, and its morphological variation was analyzed. Linear dimensions, pholidosis, coloration and pattern features were compared among 135 specimens from nine populations inhabiting the Balkhash, Ili and Alakol basins, Junggar and Turpan depressions in the territory of Southeast Kazakhstan and Xinjiang, Northwest China. The populations from the Junggar Depression(Kuytun, Shihezi and Urumqi) were characterized by higher mean values of linear characters, the number of scales across the middle of the body and gular, and were similar in the dominant coloration patterns. Small size, dark coloration and almost complete dominance of the striped-type coloration pattern among the specimens from the Alakol Lake islands seem to have an adaptive significance associated with the isolation of the population and type of their habitats. The subspecies Eremias velox roborowskii(endemic to the Turpan Depression) is elevated to species level, as supported by morphological divergence congruent with molecular and geographical data, including its peculiar type of coloration pattern, significantly lower amount of femoral pores and a higher percentage of specimens with one enlarged preanal scale(72.7%). These results together confirm a high degree of variability in morphology for ***х in the eastern periphery of its range, reflecting a complex orography and the existence of multiple geographical barriers in this territory.
