Gastroesophageal reflux disease has an increasing incidence and prevalence worldwide.A significant proportion of patients have a suboptimal response to proton pump inhibitors or are unwilling to take lifelong medication due to concerns about long-term adverse *** anti-reflux therapies offer a minimally invasive option for patients unwilling to undergo surgical treatment or take lifelong *** best candidates are those with a good response to proton pump inhibitors and without a significant sliding hiatal *** incisionless fundoplication and nonablative radiofrequency are the techniques with the largest body of evidence and that have been tested in several randomized clinical ***-assisted ligation techniques,anti-reflux mucosectomy,antireflux mucosal ablation,and new plication devices have yielded promising results in recent noncontrolled ***,the role of endoscopic procedures remains controversial due to limited long-term and comparative data,and no consensus exists in current clinical *** review provides an updated summary focused on the patient selection,technical details,clinical success,and safety of current and future endoscopic anti-reflux techniques.

Recent introduction of an allodapine bee into Fiji： A new model system for understanding biological invasions by pollinators

Morphology-based studies have suggested a very depauperate bee fauna for islands in the South West Pacific, and recent genetic studies since have indicated an even smaller endemic fauna with many bee species in this region resulting from human-aided dispersal. These introduced species have the potential to both disrupt native pollinator suites as well as augment crop pollination, but for most species the timings of introduction are unknown. We examined the distribution and nesting biology of the long-tongued bee Braunsapis puangensis that was first recorded from Fiji in 2007. This bee has now become widespread in Fiji and both its local abundance and geographical range are likely to increase dramatically. The impacts of this invasion are potentially enormous for agriculture and native ecosystems, but they also provide opportunities for understanding how social insect species adapt to new environments. We outline the major issues associated with this recent invasion and argue that a long-term monitoring study is needed.

We have recently proposed an optical method for assessing heart structure that uses polarized light measurement of birefringence as an indicator of tissue *** highly aligned nature of healthy cardiac muscle tissue has a detectable effect on the polarization of light,resulting in a measurable phase shift(“retardance”).When this organized tissue structure is perturbed,for example after cardiac infarction(heart attack),scar tissue containing disorganized collagen is formed,causing a decrease in the measured retardance ***,these are dependent not only on tissue anisotropy,but also on the angle between the tissue’s optical anisotropy direction and the beam interrogating the *** remove this experimental ambiguity,we present a method that interrogates the sample at two different incident beam angles,thus yielding enough information to uniquely determine the true magnitude and orientation of the tissue optical *** use an infarcted porcine heart model to compare these polarimetryderived anisotropy metrics with those obtained with diffusion tensor magnetic resonance imaging(DT-MRI).The latter yields the anisotropy and the direction of tissue water diffusivity,providing an independent measure of tissue *** optical and MR results are thus directly compared in a common ex vivo biological model of interest,yielding reasonable agreement but also highlighting some technique-specific differences.

Correction of hypovitaminosis D improved global longitudinal strain earlier than left ventricular ejection fraction in cardiovascular older adults after orthopaedic surgery

Background Cardiovascular diseases and insufficient levels of vitamin D are risk factors for adverse surgical outcomes, and they are both commonly present among older adults undergoing orthopaedic surgery. Giving the cardiovascular effects of vitamin D, pre-operative diagnosis of hypovitaminosis D would be a valuable step for the implementation of supplementation protocols. We investigated if the nor- malization of serum 25 [OH] D could ameliorate cardiac performance of older adults suffering from cardiovascular diseases. Methods We enrolled 47 older adults scheduled for major orthopaedic surgery and suffering from hypovitaminosis D. Patients underwent 6-months cal- cifediol supplementation with a starting dose at first post-operative day of 50 ~tg/die in liquid preparation. Down-titration to 20 Ixg/die at 3-months assessment was planned. Cardiac performance was evaluated by measuring left ventricular ejection fraction （LVEF） and global longitudinal strain （GLS） during pre-operative assessments and at 1-month, 3-months, 6-months follow-ups. Results Six months of cal- cifediol supplementation were associated with a significant improvement of both LVEF （＋ 3.94%; 95% CI： -4.0789 to -0.8232; P 〈 0.01） and GLS （＋ 18.56%; Z = -5.895; P 〈 0.0001）. Conclusions Calcifediol supplementation normalized serum 25 [OH] D concentration after 1-month treatment. GLS offered better insights into myocardial contractile amelioration than LVEF, thus being useful for detecting earlier subclinical changes that may anticipate hemodynamic modifications.

Capital Structure of Start-ups： Evidence on Non-accounting Characteristics

Research is scarce on non-financial characteristics, including owner characteristics and their influences on capital structure in the establishment phase. The objective of this study is to examine the factors affecting firms＇ availability of external finance, utilizing a sample of Finnish start-up micro firms. The overall purpose is not to produce a comprehensive model for capital structure, but to illustrate the importance of non-accounting characteristics in explaining the attractiveness of the new venture from the lender＇s point of view. The results show that a founder who has prior work experience from the same sector and has little or no personal financial property seems to have higher leverage. After splitting the data into survived and failed firms, lenders viewed work experience positively only in the sample of failed firms. In addition, limited liability companies, firms located in bigger cities, and firms with good demand evaluated by the lender have a lower level of leverage.

Celiac disease in patients with type 1 diabetes:a condition with distinct changes in intestinal immunity?

Two common chronic childhood diseases-celiac disease(CD)and type 1 diabetes(T1D)-result from complex pathological mechanisms where genetic susceptibility,environmental exposure,alterations in intestinal permeability and immune responses play central *** this study,we investigated whether these characteristics were universal for CD independently of T1D *** this purpose,we studied 36 children with normal small-bowel mucosa and 26 children with active CD,including 12 patients with *** samples from the small-bowel mucosa,we detected the lowest expression of tight junction protein 1(TJP1)mRNA in CD patients with T1D,indicating an increase in intestinal ***,these samples displayed the highest expression of forkhead box P3(FoxP3)mRNA,a marker for regulatory T cells,as compared with other patient *** the same time,serum levels of IgA antibodies specific for the CD-related antigens deamidated gliadin and tissue transglutaminase(tTG)were the highest in CD patients with *** contrast,no significant differences were found in IgA or IgG antibodies specific for bovine beta-lactoglobulin or Bifidobacterium adolescentis DSM 20083-derived *** were also no differences in the transamidating activity of serum autoantibodies between patients and control *** results show that patients with T1D and newly detected CD exhibit severely altered intestinal permeability,strong local immune activation and increased immunoregulatory mechanisms in the small *** study is required to determine whether these extreme changes in this CD subgroup are due to some specific environmental factors(virus infections),unknown genetic effects or autoimmune reactions to antigenic targets in intracellular tight junctions.

Analyzing Land Cover Change—The Impact of the Motorway Construction and Their Operation on Landscape Structure

The aim of our study was to assess the influence of motorways on landscape structure changes. The research was situated on segments of the D1 and D3 motorways in the Czech Republic. The method used for determining the impacts of the construction and operation of the motorway on the landscape structure was based on comparing the development of the land cover types in an area where the motorway was present for almost the entire monitored period (D1 motorway segment) with the development of the land cover types in an area where the motorway was constructed towards the end of the monitored period (D3 motorway segment). Monitored period?was?between years 1949, 1988 (1984 segment of motorway D3) and 2007. Source materials were processed and analyzed in the ArcGIS software environment. Our comparison indicated that the main differences between segments were in the level of development of commercial, industrial and agricultural built-up area in the vicinity of motorways. The existence of the motorway also contributed to the development of residential areas. It should be noted that in both cases there was also an increase in the extension?of shrubs and scattered vegetation, and in small quantities also category of forest.

Oxidative damage in the progression of chronic liver disease to hepatocellular carcinoma:An intricate pathway

The histo-pathologic and molecular mechanisms leading to initiation and progression of hepatocellular carcinoma (HCC) are still ill-defined; however, there is increasing evidence that the gradual accumulation of mutations, genetic and epigenetic changes which occur in preneoplastic hepatocytes results in the development of dysplastic foci, nodules, and finally, overt HCC. As well as many other neoplasias, liver cancer is considered an “inflammatory cancer”, arising from a context of inflammation, and characterized by inflammation-related mechanisms that favor tumor cell survival, proliferation, and invasion. Molecular mechanisms that link inflammation and neoplasia have been widely investigated, and it has been well established that inflammatory cells recruited at these sites with ongoing inflammatory activity release chemokines that enhance the production of reactive oxygen species. The latter, in turn, probably have a major pathogenic role in the continuum starting from hepatitis followed by chronic inflammation, and ultimately leading to cancer. The relationship amongst chronic liver injury, free radical production, and development of HCC is explored in the present review, particularly in the light of the complex network that involves oxidative DNA damage, cytokine synthesis, telomere dysfunction, and microRNA regulation.

Energy Planning in Small Municipalities Based on Monitoring Results and Demand Side Management

Recent estimates state that the European Union is on course to achieve only half of the 20% energy consumption reduction target by 2020. As the first governmental stakeholders involved in the implementation of energy saving initiatives, municipalities play a strategic role in the energy planning process. This paper focuses on establishment of an energy planning methodology for small municipalities with numbers of inhabitants in range of 1,000-10,000 which often face common problems associated with low efficient district heat supply systems and decreasing energy consumption in buildings. Particular attention is paid to DSM （demand side management） activities. DSM scheme includes legislative and financial flows with small investments from municipality side. Based on increased information and motivation it promotes reduction of energy consumption in all kinds of buildings. Practical experience has shown that application of DSM measures allows achieving 20% energy savings in municipal buildings during the first year.

Environmental pollution increases from year to year and concentrations of pollutants reach a colossal point, which ultimately affects the health of the living organisms. Industrial pollution and old mining methods are one of the sources of harmful agents in the environment. It is important to study and monitor environmental pollution in dynamics. During several years we have been researching heavy metals distribution in soils in the South East part of Georgia. “RMG Gold and Copper” mine in Kazreti is one of the biggest enterprises in Georgia. As a result of open-cast mining aerosols containing heavy metals are ejected which causes harmful effects of the environment. Within this research 21 villages in Bolnisi and Dmanisi municipalities were investigated;analysis of agricultural soils showed that the concentration of cadmium in the vicinity of the enterprise was significantly increased and exceeded maximum allowed concentrations.
