Advanced Anderson-Fabry disease presenting with left ventricular apical aneurysm and ventricular tachycardia

A 54-year-old female with Anderson-Fabry disease(AFD)-R342 Q missense mutation on exon 7 in alphagalactosidase A(GLA) gene- presented with sustained ventricular tachycardia. Imaging confirmed the presence of a new left ventricular apical aneurysm(LVAA) and a significantly reduced intra-cavitary gradient compared to two years prior. AFDcv is an X-linked lysosomal storage disorder caused by GLA enzyme deficiency. The phenotypic expression of AFD in the heart is not well described. Cardiac involvement can include left ventricular hypertrophy(LVH), which is typically symmetric, but can also mimic hypertrophic cardiomyopathy(HCM). Left ventricular apical aneurysm is a rare finding in HCM. We suggest a shared mechanism of LVAA formation in AFD and HCM, independent of the underlying cardiomyopathy. Mechanisms of LVAA formation in HCM include genetic predisposition and long-standing left ventricular wall stress from elevated intra-cavitary systolic pressures due to mid-cavitary obstruction. Both mechanisms are supported in this patient(a brother with AFD also developed a small LVAA). Screening for AFD should be considered in cases of unexplained LVH, particularly in patients with the aneurysmal variant of HCM.

Sugarcane bagasse was pyrolyzed using a laboratory fixed bed reactor to produce char and its by-product(pyrolysis liquid).The pyrolysis experiments were carried out using different temperatures(400℃ and 500℃),heating rate(1℃/min and 10℃/min),and holding time(30 min and 60 min).Char was characterized according to its thermal properties,while the pyrolysis liquid was tested for its anti-fungal and anti-termite *** temperature and heating rate had a significant influence on the char properties and the yield of char and pyrolysis liquid,where a high-quality char and high yield of pyrolysis liquid can be obtained at a temperature of 500℃ and a heating rate of 10℃/*** yield of char and pyrolysis liquid was 28.97%and 55.46%,*** principal compounds of pyrolysis liquid were water,acetic acid,glycolaldehyde,1-hydroxy-2-propanone,methanol,formic acid,levoglucosan,furfural,followed by some phenol compounds and guaiacol *** liquid at a concentration of 0.20%and 0.25%(v/v)caused a 100%inhibition of Coniophora puteana and Trametes versicolor,respectively,when performing inhibition growth tests in Petri *** paper treated with 10%of pyrolysis liquid caused 100%of termite mortality,while only 5.65%–7.03%of the treated filter papers consumed by termites at such *** liquid is potentially effective to be used in the formulation of wood protection against fungi and termites.

We give a new justification of the linear membrane and flexural shell models. We prove that the sequence of scaled energy functionals associated with the scaled problem r-converges to the energy functional associated with a two-dimensional model. This two-dimensional model is a membrane or flexural one, depending on the geometric and kinematic conditions. Then, a classical argument allows to give a new proof of the convergence theorems recently obtained by P.G. Ciarlet, V. Lods and B. Miara.

Objective To evaluate the potential value of intravascular ultrasound(IVUS)imaging in the diagnosis of aortic intramural hematoma(AIH).Methods From September 2002 to May 2005,a consecutive series of 15 patients with suspected aortic dissection(AD)underwent both IVUS imaging and spiral computed tomography(CT).Six patients diagnosed as acute type B AIH by CT or IVUS composed the present study *** The study group consisted of five males and one female with mean age of 66 years *** of them had chest or back *** one patient,CT omitted a localized AIH and an associated penetrating atherosclerotic ulcer(PAU),which were detected by *** another patient,CT mistaken a partly thrombosed false lumen as an AIH,whereas IVUS detected a subtle intimal tear and slow moving blood in the false *** the four rest patients,both CT and IVUS made the diagnosis of AIH,however,IVUS detected three PAUs in three of them,only one of them was also detected by CT,and two of them escaped initial CT and were confirmed by follow up CT or magnetic resonance *** IVUS imaging is a safe examination and has high accuracy in the diagnosis of AIH,particularly for diagnosing localized AIH,distinguishing AIH with thrombosed classic AD and detecting accompanied small PAUs.

Clinical predictors of thiopurine-related adverse events in Crohn's disease

AIM: To determine the incidence and predictors of thiopurine-related adverse events. METHODS: Subjects with Crohn's disease who were followed in the Alberta Inflammatory Bowel Disease Consortium patient database registry were identified. Retrospective chart review was conducted between August 5th, 2010 and June 1st, 2012. We collected data on: age at diagnosis; sex; disease location and behaviour at time of prescribing thiopurine; perianal fistulising disease at or prior to thiopurine prescription; smoking status at time of thiopurine prescription, use of corticosteroid within 6 mo of diagnosis; dosage, age at onset, and cessation of 5-aminosalicyclic acid(5-ASA); anti-tumour necrosis factor medication exposure and intestinal resection before thiopurine prescription. The primary outcome of interest was the first adverse event that led to discontinuation of the first thiopurine medication used. Logistic regression models were used to associate clinical characteristics with outcomes after adjusting for potential confounders. Risk estimates were presented as odds ratios(OR) with 95% CI. Effect modification by age and sex were ***: Our cohort had a median follow-up duration of 5.8 years [interquartile range(IQR 25th-75th) 2.7-9.1]. Thiopurine therapy was discontinued in 31.3% of patients because of: hypersensitivity reactions(7.1%), acute pancreatitis(6.2%), gastrointestinal intolerance(5.4%), leucopenia(3.7%), hepatotoxicity(3.4%), infection(1.1%) and other reasons(4.3%). A higher incidence of thiopurine withdrawal was observed in patients over the age of 40(39.4%, P = 0.007). A sexby-age interaction(P = 0.04) was observed. Females older than 40 years of age had an increased risk of thiopurine discontinuation due to an adverse event(age above 40 vs age below 40, adjusted OR = 2.8; 95%CI: 1.4-5.6). In contrast, age did not influence thiopurine withdrawal in males(age above 40 vs below 40, adjusted OR = 0.9; 95%CI: 0.4-2.1). Other clinical variables(disease

In this paper, we analyse the main strategy for the development of Beijing as it was presented by the last Master Plan: “Beijing should be, by the middle of the 21st century an international modern metropolis of the first world rank”. At first, we summarize the current Chinese studies and urban changes that are being carried out in Beijing, both in spatial and function aspects. Further, we underline the necessity to Beijing to become a powerful economically modernized metropolis. We emphasize with three main reasons in order to explain this necessity: the new “socialist economic market” in China, the inter urban competition among Chinese cities, and the comparison with the current metropolises of the first world rank. Finally, after a summary about the internalization process of London, Paris, New York and Tokyo, we present a simplified analysis of the urban internalization process. According to this analysis, we conclude that Beijing must combine two status: a powerful economically modernized metropolis and the Asian Cultural Capital status, so that could project Beijing into the next century as one of the elite metropolis in the world.

This article introduces and evaluates a Soil Trafficability Model (STRAM) designed to estimate and forecast potential rutting depth on forest soils due to heavy machine traffic. This approach was developed within the wood-forwarding context of four harvest blocks in Northern and Central New Brunswick. Field measurements used for model calibration involved determining soil rut depths, volumetric moisture content, bulk density, soil resistance to cone penetration (referred to as cone index, or CI), and the dimensionless nominal soil cone index (NCI) defined by the ratio of CI over wheel foot print pressure. With STRAM, rut depth is inferred from: 1) machine dimensions pertaining to estimating foot print area and pressure;2) pore-filled soil moisture content and related CI projections guided by year-round daily weather records using the Forest Hydrology Model (ForHyM);3) accounting for within-block soil property variations using multiple and Random Forest regression techniques. Subsequent evaluations of projected soil moisture, CI and rut-depth values accounted for about 40 (multiple regression) and 80 (Random Forest) percent of the corresponding field measured values.
