How can we increase attraction and retention of nurses? A research with young nurses

The persistent challenges in attracting and retaining a diverse healthcare workforce, with a specific focus on nurses, have become increasingly pronounced in recent years. These hurdles have been exacerbated by a growing difficulty in retaining young nurses, thereby exacerbating labor shortages driven by demographic shifts and the retirement of experienced nursing professionals. While most research efforts have concentrated on the broader issue of nurse retention, our study is centered on a specific demographic—young nurses. Our research endeavors to shed light on the unique challenges faced by young nurses through a qualitative survey involving nursing students who are simultaneously employed. We seek to discern the multifaceted obstacles they encounter in both their academic environment and the healthcare organizations where they work. While certain challenges are linked to course organization, examinations, and the time required for studying, our respondents overwhelmingly emphasize the pivotal role of the work environment in facilitating the harmonization of work, family, and educational commitments. This reconciliation is achieved through measures such as flexible working arrangements and the efficient organization of nursing duties. The primary objective of our research is to provide insights into how these diverse challenges can be effectively addressed and how a range of measures can significantly contribute to the attraction and retention of nursing students, as well as the long-term retention of nurses within the healthcare system. Our recommendations are intended to be of practical use to a wide array of stakeholders, including academic institutions, particularly colleges and universities offering nursing programs, as well as hospitals, clinics, and other healthcare institutions that hire nurses. 小胶质细胞是调节大脑突触发育和可塑性的重要细胞类型,但其影响突触的正常功能的机制尚不清楚。在本研究中,我们通过PLX5622造成小胶质细胞耗竭,并观察其对成年野生型小鼠海马CA3-CA1突触的影响。在小胶质细胞耗竭后,与树突棘密度降低相关的自发和诱发谷氨酸能活动的减少,出现未成熟突触特征以及突触的可塑性提高。小胶质细胞耗竭的小鼠在新物体识别任务的获取方面表现出缺陷。海马星形胶质细胞出现增生,但并没有神经炎症反应。在Cx3cr1-/-小鼠中,PLX不能导致海马出现上述改变。这说明CX3CL1/CX3CR1轴在小胶质细胞对突触功能的控制中有重要作用。PLX5622停用后,小胶质细胞的重新增殖,海马突触恢复,小鼠的学习功能也出现恢复。综上所述,小胶质细胞对维持成人大脑的突触的正常功能用重要的作用,去除小胶质细胞会导致谷氨酸能突触组织和活动的可逆变化。

Zeros in Accelerometer Files A Comparison Study of Ten Published Nonwear Algorithms

Despite the increasing use of accelerometers, the handling of zero values in data files remains understudied. Using data from the QUALITY （QUebec Adipose and Lifestyle Investigation in Youth） cohort, output from 10 published nonwear algorithms were compared in 520 children aged 8-10 years who wore an Actigraph accelerometer for 7 days. These algorithms differed by： 1） the minimal duration of consecutive zeros required to be designated as nonwear （10-90 minutes）; 2） the presence and magnitude of an allowance period, and 3） the presence of a specific time frame. Significant differences were obtained between all but two algorithms, including： 1） wear time differences up to 117 minutes per day; 2） differences in counts per minutes as high as 80; and 3） differences in sedentary time up to two hours daily. Estimates of light and moderate-to-vigorous physical activity were less affected. This study suggests that the nonwear algorithm used can have a significant effect on estimates of wear duration, overall physical activity level and sedentary time in children. Until technological developments bring to zero nonwear time, specifying the nonwear algorithm used when publishing results is essential to ensure accurate comparison between studies.

An In Vivo Evaluation of the Mycobacterial Filtration Efficacy of Three Breathing Filters Used in Anesthesia

Background:The use of breathing filters (BFs) has been recommended to protect the anesthesia apparatus in proven or suspected cases of tuberculosis. Some investigators have also suggested the use of BF to alleviate the need to change anesthesia breathing circuits after each case. This study evaluated the filtration efficacy of three different BFs to prevent mycobacterial contamination of breathing circuits in a model that uses a test animal. Methods:Ten Pall BB25A (pleated hydrophobic) (Pall Canada Ltd., Mississauga, Ontario, Canada), six DAR Barrierbac S (felted electrostatic; Mallinckrodt DAR, Mirandola, Italy), and six Baxter Airlife (felted electrostatic; Baxter Canada, Mississauga, Ontario, Canada) BFs were studied. For each BF tested, 20 ml of a high concentration suspension of Mycobacterium chelonae (range, 2.0×10^7 to 9.0×10^7 colony-forming units/ml) was nebulized during 2h at the proximal end of the endotracheal tube of anesthetized pigs. At the end of the nebulization period, the BFs were sampled for culture. The titer reduction value (number of microorganisms challenging the BF divided by the number of microorganisms recovered downstream of the BF) and the removal efficiency (difference between the number of microorganisms challenging the BF and the number of microorganisms recovered downstream of the BF, divided by the number of microorganisms challenging the BF) were calculated. Results:The median titer reduction values were 5.6×10^5, 6.0×10^5, and 8.0×10^8 (P vely. Conclusions:Among the three BFs studied, only the Pall BB25A completely prevented the passage of M. chelonae, thus protecting the anesthesia breathing circuit from mycobacterial contamination.

小胶质细胞是大脑的常驻巨噬细胞,通过感知神经元活动并支持突触重塑和可塑性积极促进脑实质的稳态。虽然已有研究表明星形胶质细胞在睡眠中的不同作用,但小胶质细胞在调节睡眠/觉醒周期和白天不同阶段调节突触活动中的作用尚未深入研究。以光作为授时提示,我们研究了用集落刺激因子-1受体拮抗剂PLX5622消除小胶质细胞对雄性小鼠睡眠/觉醒周期和海马突触传递的影响。我们的数据表明,小胶质细胞几乎完全的耗竭会增加NREM睡眠的持续时间并减少海马兴奋性神经传递。由于cx3cr1GFP/GFP小鼠显现与PLX5622处理的小鼠中发现的相同情况,趋化因子fractalkine受体CX3CR1在这些效应中起相关作用。此外,在光照阶段小胶质细胞表达较低水平的cx3cr1;当培养的小胶质细胞受到ATP(一种在睡眠期间释放的嘌呤能分子)的刺激时其cx3cr1表达降低。我们的研究结果表明,小胶质细胞参与调节睡眠,调整其cx3cr1表达以响应光/暗阶段,并以时相依赖性方式调节突触活动。
