Hearing Disability in Colombia Self-Perception and Associated Factors 2002-2008

Introduction: Hearing disability is a condition that affects normal ear function, as much in adulthood as in the first years of life. According to the 2005 Census, 6.3% of the Colombian population has some type of disability, of which 17.4% have hearing limitations, including those with hearing devices. Elucidating the conditions of this population and identifying the factors related to hearing disability will permit the management of strategies from different sectors to mitigate the consequences associated with this limitation. Objective: To estimate the self-perception of hearing disability in Colombia and to determine the factors associated with this limitation. Materials and Methods: The present study is cross-sectional, based on the analysis of secondary information obtained from the Registry for Localization and Characterization of Persons with Disability (RLCPD) during the 2002-2008 period. Socio-demographic and healthcare variables were analyzed. Results: 13.6% (102,648/750,377) of the population reported hearing limitations even with the use of special hearing devices. 43.52% (44,041) of people over 3 years of age could neither read nor write. 29.39% (30,145) of people who reported this limitation are not affiliated to any sort of health insurance system. Factors found to be associated with hearing limitations were: socioeconomic stratus (OR: 1.33;CI 95% 1.25;1.42), illiteracy (OR: 1.44;CI 95% 1.42;1.46) and lack of affiliation to a health insurance system (OR: 1.03;CI 95% 1.01;1.04). Conclusion: People registered with hearing disability live under vulnerable conditions;among them, most/the majority pertained to a segment of the population with low economic resources and had difficulty obtaining/accessing work, education and healthcare services.

Links between institutional sectors and economic activities with National Accounting System of Mexico are studied used accounting multipliers. Key sectors had changed to strategic or leading as oil and gas extraction and dairy product manufacturing, i.e., Mexico went from a producer of goods to a service provider country, losing value added in their production chains. The mixed income is leading/independent economic activity for woman whose income is between 1 and 5 minimum wages. This is really important in the domestic economy through its impact on solidarity activities： providing care and support and providing food. Moreover, non-financial corporations and households of 10 or more minimum wages for income investing in strategic sectors such as retail trade and wholesale and manufacturing products derived from oil and coal. This is evidence of two Mexicos： the traditional and industrialized.

De novo intraocular amyloid deposition after hepatic transplantation in familial amyloidotic polyneuropathy

The familiar amyloid polyneuropathy(FAP) is a rare autosomal-dominant systemic amyloidosis. Amyloid deposition occurs more frequently and extensively in the vitq. The increase in intraocular pressure(IOP) is a result of deposition of transthyretin(TTR) in trabecular meshwork. Rarely, the amyloid deposition in anterior segment can be more exuberant than in posterior segment. A 42 years old man, with FAP(Val30Met mutation), liver transplantation in 1997. He was asymptomatic, without any significant ocular abnormality until 2011. In 2011 he had an episode of pain in right eye(RE). Scalloped pupils, pupillary amyloid deposits and subtle vitreous opacities were detected. The IOP was 40 mmHg in RE and 28 mmHg in left eye(LE) with open angle. Optical coherence tomography detected a temporal superior retinal nerve fiber layer defect in LE and perimetry was normal. Topical timolol was initiated, and brimonidine was subsequently added to improve IOP control, which was achieved with topical medication until last evaluation. No progression occurred since 2011. Actually, with longer life expectancies, there is an increased risk of ocular involvement in FAP, even after liver transplantation. Although rare, a more exuberant amyloid deposition in anterior segment vs posterior segment can occur, and supports an important role of amyloid production in ciliary pigment epithelium in these patients. Medical control of IOP and a stable course are unusual in this secondary glaucoma. Ophthalmologists have an important task in the follow-up of patients and early diagnosis of risk factors for secondary glaucoma, such as scalloped pupils with amyloid deposits.

The objective of this study is to determine the auditory gain, quality of life, audiological benefits, in bone-anchored hearing device users (BAHA). It is a retrospective and concurrent evaluation of thirty patients fitted unilaterally and seven fitted bilaterally for at least six months. Patients were assessed with audiometric testing and application of Glasgow Benefit Inventory (GBI) and Abbreviated Profile of Hearing Aid Benefit (APHAB). Regarding sound-field pure audiometry results, we found a statistically significant gain in all frequencies using the bone-anchored device. APHAB scores showed statistically significant subjective audiological gains in all subscales except for the aversiveness subscale. GBI mean scores for all items in both groups were all above 3, suggesting quality of life improvement in conductive and mixed hearing loss patients. BP100 users showed a greater clinical gain in the APHAB global score and subscales compared with Divino users. In conclusion the BAHA provides significant auditory gain, subjective audiological benefits and improves quality of life in all BAHA users. This study shows a significant clinical and statistical benefit of BAHA measured by audiometric testing and by the APHAB and GBI questionnaires.

Anselm and Heidegger: A Disquieting Analogy

This article deals with the human design of salvation and the question of man under the common stimulus of two grand names of philosophical tradition, though parted by more than nine centuries: Saint Anselm and Martin Heidegger. Both defend the need for a God who saves man: according to Anselm, due to the consequences of evil, which man consented and cannot revert;according to Heidegger, due to the consequences of technicity, which man created and can no longer control. In any case, the design of salvation posits the question of man: Why man? This question was considered ever since the remote 12th century, in a time of renovation for European culture, and is once again posited with special acuteness in our days, in a time of accelerated technological progress of human civilization. Which is why Anselm’s reflection around salvation and the medieval question of man may open doors towards re-equating the value of man in its actual state and in the future horizon of humanity?

Portal hypertensive colopathy is associated with portal hypertension severity in cirrhotic patients

AIM： To assess the prevalence of portal hypertension （PH） related colorectal lesions in liver transplant candidates, and to evaluate its association with the severity of PH. METHODS： Between October 2004 and December 2005, colonoscopy was performed in 92 cirrhotic liver transplant candidates. We described the lesions resulting from colorectal PH and their association with the grade of PH in 77 patients who underwent measurement of hepatic venous pressure gradient （HVPG）. RESULTS： Mean age was 55 years and 80.7% of patients were men. The main etiology of cirrhosis wasalcoholism （45.5%）. Portal hypertensive colopathy （PHC） was found in 23.9%, colonic varices in 7.6% and polyps in 38% of patients （adenomatous type 65.2%）. One asymptomatic patient had a well-differentiated adenocarcinoma. The manifestations of colorectal PH were not associated with the etiology of liver disease or with the Child-Pugh grade. Ninety percent of patients with colopathy presented with gastroesophageal varices （GEV）, and 27.5% of patients with GEV presented with colopathy （P = 0.12）. A relationship between higher values of HVPG and presence of colopathy was observed （19.9：1：6.2 mmHg vs 16.8 ± 5.4 mmHg, P = 0.045）, but not with the grade of colopathy （P = 0.13）. Preneoplastic polyps and neoplasm （P = 0.02） and spontaneous bacterial peritonitis （P = 0.006） were more prevalent in patients with colopathy. We did not observe any association between previous β-blocker therapy and the presence of colorectal portal hypertensive vasculopathy. CONCLUSION： PHC is common in cirrhotic liver transplant candidates and is associated with higher portal pressure.

Background::The burden of human immunodeficiency virus(HIV)infection in people who use drugs(PWUD)is *** aimed to screen HIV infection among PWUD and describe their retention in HIV ***,we also screen for hepatitis C virus(HCV)infection among HIV-seropositive PWUD and describe their linkage to ***::We conducted a prospective study in 529 PWUD who visited the"Ca?ada Real Galiana"(Madrid,Spain).The study period was from June 1,2017,to May 31,*** diagnosis was performed with a rapid antibody screening test at the point-of-care(POC)and HCV diagnosis with immunoassay and PCR tests on dried blood spot(DBS)in a central *** PWUD were referred to the *** used the Chi-square or Fisher’s exact tests,as appropriate,to compare rates between ***::Thirty-five(6.6%)participants were positive HIV antibodies,but 34 reported previous HIV diagnoses,and 27(76%)had prior antiretroviral *** patients with a positive HIV antibody test,we also found a higher prevalence of homeless(P<0.001)and injection drug use(PWID)(P<0.001),and more decades of drug use(P=0.002).All participants received HIV test results at the *** the 35 HIV positives,28(80%)were retained in HIV medical care at the end of the HIV screening study(2018),and only 22(62.9%)at the end of ***,12/35(34.3%)were positive for the HCV RNA *** the latter,10/12(83.3%)were contacted to deliver the HCV results test(delivery time of 19 days),5/12(41.7%)had an appointment and were attended at the hospital and started HCV therapy,and only 4/12(33.3%)cleared ***::We found almost no new HIV-infected PWUD,but their cascade of HIV care was low and remains a challenge in this population at *** high frequency of active hepatitis C in HIV-infected PWUD reflects the need for HCV screening and reinforcing the link to care.
