β-Globin Gene Cluster Haplotypes and Clinical Severity in Sickle Cell Anemia Patients in Southern Brazil

Hematopoietic stem cell transplantation(HSCT)has emerged as a curative strategy for sickle cell anemia(SCA);it is necessary to find markers of SCA clinical severity to spare those SCA patients whose clinical course is mild from the morbidity and mortality associated with HSCT. Haplotypes have been correlated with the severity of clinical manifestations in SCA patients, and fetal hemoglobin(HbF)and socioeconomic status(SeS)have also been described as negative factors. We studied these factors and their impact on clinical manifestations in a population of Southern Brazilian patients attending the Center for Sickle Cell Anemia at Hospital de Clínicas de Porto Alegre/RS, Brazil. Clinical severity was defined as two or more veno-occlusive episodes per year. The βS haplotypes were determined by PCR in 75 SCA patients. Among the 150 βS chromosomes analyzed, 99(66%)were identified as Bantu(Ban), 41(27%)asBenin(Ben), and 10(7%)as other haplotypes. Most patients in our sample(62.7%)belonged to lower SeS groups, precluding meaningful statistical analysis of SeS impact on clinical severity. There was no correlation between haplotypes or HbF level and SCA clinical severity. Gene polymorphisms and environmental issues have to be taken into consideration.

The concern of the ethics and the meaning of life in young adults became universal and are present in all contexts of human life. Such universality is due to the development of modern rationality, to establish a relationship between the intrinsic theoretical dimensions (scientific) and practical dimensions (ethical), originated that are present in the own array of knowledge. This descriptive cross-sectional study, aims to identify the values of the ethical dimension and meaning of life defended by the college students of the Universities of Madeira, Portugal and Fortaleza, Brazil. The sample consists of 605 University students (Madeira = 225 and Fortaleza = 380), with an average age of 21 years and a standard deviation of 2. We used the questionnaire on values (adapted from the European Values Survey). Most of the young people referred to have a religion (60% wood and 78.4% stronghold), being the most Catholic, however, present in your frequency disinterest and, yet, in Brazil there is a greater diversity of religions. Supporting the intervention of religious institutions in situations of everyday life is accepted mostly by Brazilians, with the exception of politics (35.5%). On the other hand, the university students of the University of Madeira understand that Religious Institutions should only give their opinion on racial discrimination (76.6%). Believe in God (90.8% and 55.1% Stronghold) and Soul (80.3% and 66.4% Stronghold) are beliefs more referred to by college students. These results are intended to contribute in the field of research on ethical values and meaning of life in young people. This study contributes to future comparative research among university students and to developing strategic programs in education and health, increasing the success and excellence in the formation of a full citizenship, preventing the fanaticisms and the social discrimination that today devastate the collective globally.

AIM To evaluate hepatitis B virus(HBV) vaccine response and correlation with human leukocyte antigens(HLA) and/or gluten intake in celiac patients at *** Fifty-one patients affected by celiac disease, diagnosed at the Department of Pediatrics of the University of Catania(Italy), were recruited. All patients were tested at admission for immunization against HBV, according to findings from analysis of quantitative HBV surface antibody(anti-HBs). The anti-HBs titer was measured by enzyme-linked immunosorbent assay. Following the international standards, subjects with antibody titer 0.05). With regard to the HLA haplotype, comparison of the distribution of vaccination response showed no statistically significant difference between the different genotypes(homozygosity for the HLADQ2 haplotype compared with HLADQ2/DQ8 heterozygosity or other haplotypes; P > 0.05). Moreover, distribution of the responders according to clinical features of celiac disease showed no statistically significant differences(P > 0.05).CONCLUSION This prospective study confirmed the lower percentage of response to HBV vaccine in celiac subjects. However, the underlying mechanism remains unclear and further studies are needed.

Obesity Effect on Children Hip and Knee Range of Motion

The aim of this study was to characterize the obesity effect on children knee and hip joint range of motion. A randomized controlled trial was performed with goniometry evaluation for hip flexion, extension, abduction, adduction, external and internal rotation, and knee flexion in fifty male children (20 obese and 30 non obese), with age ranging from 6 to 12 years old. The obesity effect was found to be non-uniform across the joint range of motion. In the obesity group, a statistically significant reduction in the range of motion of hip flexion and adduction, and knee flexion was observed;and also an increase in external rotation in the hip was observed. Conclusion: Obese and overweight children present alterations in hip and knee joints mobility when compared to eutrophic children. The obesity/overweight associated with factors previously described in the literature, such as joint overload, postural deviation and sedentariness may influence the musculoskeletal development and, consequently, the motor function.

This study investigated the use of a product based on yacon (PBY) in microbiological, physical-chemical and intestinal characteristics of Wistar rats artificially constipated with Loperamide®. Thirty-two rats were divided into four groups: Control (C), Constipated Control (CC), PBY (not constipated) and Constipated PBY (PBYC). The dosage of 0.14 g of FOS+ inulin/kg was tested. Microbiota, pH and faeces characteristics of faeces and caecal contents were evaluated. Caecal weight, morphometry of caecal villi and the concentration of short-chain fatty acids were determined. Higher caecal weight was identified in the PBYC animals as well as higher width, height and depth of cripts. The PBY group showed the highest (p < 0.05) concentration of butyrate (93.2 ± 65.5 mmol/L). The supplementation with PBY positively altered the intestine epithelial tissue in constipated animals, keeping the integrity of the caecum crypts.

Introduction: Women’s sexual function is a complex ensemble of variables that involve physical, emotional, and psychosocial states. Objective: This review aims to evaluate the evidence about the sexual behavior of women during pregnancy. Methods: Survey of PubMed for the period from 1996 to 2011 for prospective, retrospective and case-control studies, randomized clinical trials, meta-analyses and systematic reviews on the sexual function of women during pregnancy using the terms “sexual function”, “sexual dysfunction”, “sexuality”, “pregnancy”, and “pregnant woman”. Results: Changes occur in the sexual function of women during pregnancy;however, due to the heterogeneity of the studies and the incomparability of samples for lack of pairing for cultural, age, length of relationship and gestational age variables, and other methodological biases, it is not possible to characterize the sexual response of pregnant women. Conclusions: The changes observed are not sufficient to characterize sexual behavior during pregnancy as pathological. It is therefore necessary to standardize the study groups and the instruments for the assessment of sexual function, to determine the presence of distress concomitant with changes in sexual function, and to assess the sexual function of the partner and the impact of changes on the dyadic relationship before and after the gestational period.

Fibrinolytic enzymes have received attention regarding their medicinal potential for thrombolytic diseases, a leading cause of morbidity and mortality worldwide. Various natural enzymes purified from animal, plant and microbial sources have been extensively studied. The aim of this work was to produce fibrinolytic protease by solid state fermentation using agro industrial substrates. Rhizopus arrhizus var. arrhizus UCP 1295 and Mucor subtillissimus UCP 1262 filamentous fungi species isolated from soil of Caatinga-PE, Brasil, were used as producer microorganisms. Wheat bran was shown to be the best substrate for the production of the enzyme and by using a 23 full factorial design the main effects and interactions of the quantity of the substrate wheat bran, moisture and temperature on the fibrinolytic enzyme production and protease were evaluated. The best results for fibrinolytic and protease activities, 144.58 U/mL and 48.33 U/mL, respectively, were obtained with Mucor subtillissimus UCP 1262 using as culture medium 3 g wheat bran, 50% moisture at a temperature of 25°C for 72 hours. The optimum temperature for the produced enzyme was 45°C and most of its original activity was retained after being subjected to 80°C for 120 min. The protease activity was enhanced by K+, Ca+ and Mn+;but with Cu+ there was an inhibition. The specificity to chromogenic substrate and the inhibition by PMSF indicates that it is a chymotrypsin-like serine protease. Presented results suggest that this enzyme produced by solid-state fermentation is an interesting alternative as a candidate for thrombolytic therapy.

Transcranial Doppler ultrasonography: From methodology to major clinical applications

Non-invasive Doppler ultrasonographic study of cerebral arteries [transcranial Doppler(TCD)] has been extensively applied on both outpatient and inpatient settings. It is performed placing a low-frequency(≤ 2 MHz) transducer on the scalp of the patient over specific acoustic windows, in order to visualize the intracranial arterial vessels and to evaluate the cerebral blood flow velocity and its alteration in many different conditions. Nowadays the most widespread indication for TCD in outpatient setting is the research of right to left shunting, responsable of so called "paradoxical embolism", most often due to patency of foramen ovale which is responsable of the majority of cryptogenic strokes occuring in patients younger than 55 years old. TCD also allows to classify the grade of severity of such shunts using the so called "microembolic signal grading score". In addition TCD has found many useful applications in neurocritical care practice. It is useful on both adults and children for day-to-day bedside assessment of critical conditions including vasospasm in subarachnoidal haemorrhage(caused by aneurysm rupture or traumatic injury), traumatic brain injury, brain stem death. It is used also to evaluate cerebral hemodynamic changes after stroke. It also allows to investigate cerebral pressure autoregulation and for the clinical evaluation of cerebral autoregulatory reserve.
