Diagnostic yield of EUS-FNA of small(≤15 mm) solid pancreatic lesions using a 25-gauge needle

Background: Early detection of small solid pancreatic lesions is increasingly common. To date, few and contradictory data have been published about the relationship between lesion size and endoscopic ultrasound-guided fine needle aspiration(EUS-FNA) diagnostic yield. The aim of this study was to assess the relation between the size of solid pancreatic lesions and the diagnostic yield of EUS-FNA using a 25-gauge needle in a center without available rapid on-site ***: In the retrospective cohort study, we selected patients who underwent EUS-FNA for solid pancreatic lesions with a 25-gauge needle from October 2014 to October 2015. Patients were divided into three groups(≤15 mm, 16–25 mm and >25 mm), and the outcomes were ***: We analyzed 163 patients. Overall adequacy, sensitivity, specificity and accuracy were 85.2%,81.8%, 93.7%, and 80.4%, respectively. When stratified by size, the sensitivity and accuracy correlated with size(P = 0.016 and P = 0.042, respectively). Multivariate analysis showed that lesion size was the only independent factor(P = 0.019, OR = 4.76) affecting accuracy. The role of size as an independent factor affecting accuracy was confirmed in a separate multivariate analysis, where size was included in the model as a covariate(P = 0.018, OR = 1.08).Conclusion: Our study demonstrates that, in the absence of rapid on-site evaluation, mass size affects the accuracy of EUS-FNA of solid pancreatic lesions.

Proton pump inhibitors therapy and risk of Clostridium difficile infection: Systematic review and meta-analysis

AIM To perform a systematic review and meta-analysis on proton pump inhibitors(PPIs) therapy and the risk of Clostridium difficile infection(CDI). METHODS We conducted a systematic search of MEDLINE/Pub Med and seven other databases through January 1990 to March 2017 for published studies that evaluated the association between PPIs and CDI. Adult case-control and cohort studies providing information on the association between PPI therapy and the development of CDI were included. Pooled odds ratios(ORs) estimates with 95% confidence intervals(CIs) were calculated using the random effect. Heterogeneity was assessed by I^2 test and Cochran's Q *** publication bias was evaluated via funnel plot, and quality of studies by the Newcastle-Otawa Quality Assessment Scale(NOS). RESULTS Fifty-six studies(40 case-control and 16 cohort) involving 356683 patients met the inclusion criteria and were analyzed. Both the overall pooled estimates and subgroup analyses showed increased risk for CDI despite substantial statistical heterogeneity among studies. Meta-analysis of all studies combined showed a significant association between PPI users and the risk of CDI(pooled OR = 1.99, CI: 1.73-2.30, P < 0.001) as compared with non-users. The association remained significant in subgroup analyses: by design-case-control(OR = 2.00, CI: 1.68-2.38, P < 0.0001), and cohort(OR = 1.98, CI: 1.51-2.59, P < 0.0001); adjusted(OR = 1.95, CI: 1.67-2.27, P < 0.0001) and unadjusted(OR = 2.02, CI: 1.41-2.91, P < 0.0001); unicenter(OR = 2.18, CI: 1.72-2.75, P < 0.0001) and multicenter(OR = 1.82, CI: 1.51-2.19, P < 0.0001); age ≥ 65 years(OR = 1.93, CI: 1.40-2.68, P < 0.0001) and < 65 years(OR = 2.06, CI: 1.11-3.81, P < 0.01). A Preliminary Study of Three Finishing Materials for Traditional Chinese Furniture

The traditional Chinese furniture evolved as an important part of the Chinese culture and civilization. Alongside design, structure, ornaments, the original finishes applied define the particularities and value of traditional Chinese furniture. This is because wood finishing not only enhances wood natural beauty by colour, gloss and highlighting of wooden texture, but also can be a means of exquisite decoration (e.g. engraved, gilded or painted lacquer works). Chinese lacquer, Tung oil and Chinese wax represent some of the most important finishing materials employed and were, therefore, selected for this study. The paper presents a brief macroscopic, microscopic and FTIR characterization of these materials. These results will be used as starting data in a further study on the ageing properties of these traditional finishing materials, useful for scientific conservation purposes.

Biodiversity Assessment of Sugar Beet Species and Its Wild Relatives:Linking Ecological Data with New Genetic Approaches

The value of crop wild relatives has long been acknowledged and this wild resource has been used to improve crop performance with clear economic benefits. Sugar beet (Beta vulgaris subsp. vulgaris) is the most economically valuable crop species in the order Caryophyllales, B. vulgaris subsp. maritima being the ancestor of the cultivated beets. The wild species of the genus Beta s.l. are commonly found in coastal areas of Europe and Mediterranean Region, where a rich genetic heritage still exists. Broadening the genetic base of sugar beet by introgression with wild relatives is a growing need regarding the maintenance of ecologically important traits. Since wild relatives have adapted to specific habitats, they constitute an important source of novel traits for the beet breeding pool. So, we conducted a broader research project aiming to delimit taxa and identify priority locations to establish genetic reserves of the wild Beta species occurring in Portugal (Western Iberian Peninsula). The aim of this study was: 1) to identify and characterize the main habitats of these wild Beta species;and 2) to present a review of some genetic tools available for future application in sugar beet breeding. In this review, we have focused on EcoTILLING as a molecular tool to assess DNA polymerphisms in wild populations of Beta and identify candidate genes related to drought and salt tolerance, as well as addressed some issues related to next-generation sequencing (NGS) technologies as a new molecular tool to assess adaptive genetic variation on the wild relatives of sugar beet.

H pylori infection and systemic antibodies to CagA and heat shock protein 60 in patients with coronary heart disease

AIM: To determine the overall prevalence of H pylori and CagA positive H pylori infection and the prevalence of other bacterial and viral causes of chronic infection in patients with coronary heart disease (CHD), and the potential role of anti-heat-shock protein 60 (Hsp60) anti- body response to these proteins in increasing the risk of CHD development. METHODS: Eighty patients with CHD and 160 controls were employed. We also compared the levels of anti- heat-shock protein 60 (Hsp60) antibodies in the two groups. The H pylori infection and the CagA status were determined serologically, using commercially available enzyme-linked immunosorbent assays (ELISA), and a Western blotting method developed in our laboratory. Systemic antibodies to Hsp60 were determined by a sandwich ELISA, using a polyclonal antibody to Hsp60 to sensitise polystyrene plates and a commercially available human Hsp60 as an antigen. RESULTS: The overall prevalence of H pylori infec- tion was 78.7% (n = 63) in patients and 76.2% (n = 122) in controls (P = 0.07). Patients infected by CagA- positive (CagA+) H pylori strains were 71.4% (n = 45) vs 52.4% of infected controls (P = 0.030, OR = 2.27). Sys-temic levels of IgG to Hsp60 were increased in H pylori- negative patients compared with uninfected controls (P < 0.001) and CagA-positive infected patients compared with CagA-positive infected controls (P = 0.007). CONCLUSION: CagA positive H pylori infection may concur to the development of CHD; high levels of anti- Hsp60 antibodies may constitute a marker and/or a con- comitant pathogenic factor of the disease.

According to World Health Organization(WHO)estimates,in 2020,there were 1.8 million human immunodeficiency virus(HIV)-infected children below 15 years old living with HIV and 150,000 children who acquired the infection[1].Every year,110,000 children die as a consequence of acquired immune deficiency syndrome-related causes[2].As in adults,optimal management of infected children depends on early diagnosis and treatment with antiretroviral drugs based on their viral genotype.
