Breast reconstruction is rapidly evolving,thanks to the growing acceptance of synthetic meshes as innovative biomaterials.276 patients undergoing mastectomy(total of 328 mastectomies)were analyzed in a retrospective observational study to evaluate the pre-pectoral immediate breast reconstruction(IBR)using an implant wrapped with Titanium-Coated Polypropylene Mesh(TCPM)*** treated with tissue expander(TE),equally placed pre-pectorally(and wrapped with the same TCPM in 74.3%of the control group’breasts).163 patients,of the study group(SG),underwent mastectomy and pre-pectoral IBR with implant wrapped with TCPM,in a one-step surgery,called direct-to-implant technique(DTI),while 113 patients control group(CG)underwent mastectomy and *** technique has been performed in 192 breasts of the SG while TE procedure in 136 breasts of the *** BREAST-Q questionnaire has been provided before the treatment and 2 years *** scale has been used to evaluate capsular ***,surgical,and aesthetic outcomes along with BREAST-Q scores were ***,a histologic evaluation was conducted in 11 capsules’samples randomly chosen(6 derived from SG patients and 5 derived from CG).Complications were recorded in 43 cases(29SG-14CG):8 skin-nipple necrosis(5SG-3CG),8 wound dehiscence(6SG-2CG),3 hematomas(1SG-2CG),and 24 infections(8SG-16CG).Grade IV capsular contracture was detected in 9 breasts(1SG-8CG),whereas 254 breasts were grade I(110SG-144CG),33(10SG-23CG)grade II,and 32(4SG-28CG)grade *** wrinkling was detected in 18 cases(10SG-8CG)after 30 *** local tumor recurrence rate was 5.8%.Three recurrences were on the nipple-areola complex(1.9%).SG patients showed significantly higher rates in the BREAST-Q overall Satisfaction with Outcome(74.1),overall Satisfaction with Breasts(69.1),Psychosocial Well-being(81.9),and Sexual Well-being(63.1),versus CG’s patients(p<0.05).Histological analysis showed a process of normal tissue repair with

To evaluate the efficacy and feasibility of external beam radiotherapy(EBRT) for duodenal adenocarcinoma in an 84-year-old female who underwent EBRT(2.2 Gy/d for a total dose of 46.2 Gy) using helical tomotherapy(HT). Toxicity was evaluated on the National Cancer Institute's common toxicity criteria(CTCAE 3.0). The patient completed the treatment without G3-G4 toxicity. After 22-mo follow-up, she is alive and well, in complete remission with no late side effects. HT seems to be feasible and effective for duodenal adenocarcinoma in old to very old patients.

This paper considers the torque control problem for robots with flexible joints driven by electrical actuators. It is shown that the achievable closed-loop tracking bandwidth using PI torque controllers may be limited due to transmission zeros introduced by the load dynamics. This limitation is overcome by using positive feedback from the load motion in unison with PI torque controllers. The positive feedback is given in terms of load velocity, acceleration and jerk. Stability conditions for designing decentralized PI torque controllers are derived in terms of Routh-Hurwitz criteria. Disturbance rejection properties of the closed system are characterized and an analysis is carried out investigating the use of approximate positive feedback by omitting acceleration and/or jerk signals. The results of this paper are illustrated for a two DoF （degrees of freedom） system. Experimental results for a one DoF system are also included.

We read with pleasure papers by Cai et al. and Jia et al. published in HepatoBiliary Surgery and Nutrition, 2018, Volume 7, Issue 2 and 4, respectively (1,2). Both papers introduced comprehensive up-to-date literature reviews on indications and patient selection, surgical instruments, techniques, and outcomes of laparoscopic liver resections (LLRs) (1,2). Papers mentioned final statements by 2008 Louisville Statement and 2014 Morioka Consensus Conference (1,2). They stressed LLR non-inferiority to open procedures, in both minor and major liver resections as far as oncological and intraoperative/perioperative outcomes are concerned: reduced blood loss, lower postoperative morbidity and shorter length of hospitalization (1,2). Such results were corroborated by recent meta-analyses (3,4). Furthermore, some previous limitations for LLR look like they have been recently overcome (1-5). For example, tumor size, patient's old age and high BMI are no longer limitations for LLR, as Southampton Consensus guidelines suggested (5). Likewise, cirrhosis has been suggested as a further indication of LLR in highly selected patients with hepatocellular carcinoma and where it was performed by experienced surgeons (1-5).

This work presents the feasibility study regarding the realization of hail sensing probes for remote sensing and study of hail. The probes are designed as artificial hailstones in order to study both the physical properties of the portion of atmosphere where the formation of hail occurs and the modification of atmospheric conditions while the hailstones are falling to the ground. The basic idea is to realize sensors with a similar fluid-dynamic behavior with respect to hailstones;the density, the weight and the size of the probes are determined. Consequently, the specification of the electronic boards, sensors, and material to realize the probes are studied and presented. The hail sensing probes can be dropped by a plane, or potentially a UAV (Unmanned Aircraft Vehicle), which fly above and through the clouds where the hail formation occurs. During their falling to the ground, the sensors installed on the probe measure directly different parameters such as humidity, temperature, pressure, acceleration. All data are sent to a receiver located on the ground. The study of the sampling frequency of each sensor is presented, together with the analysis of the propagation channel, in order to assure a robust communication link between probes and the ground receiver. The energy balance is also computed. The work demonstrates that a set of this kind of disposable sensors can be realized. They can be used for efficient monitoring operations and studies of hail formation dynamics and conditions, thus increasing the set of instruments for the monitoring and remote sensing of hail.

Management of neuroendocrine carcinomas of the pancreas (WHO G3): A tailored approach between proliferation and morphology

Neuroendocrine carcinomas(NEC) of the pancreas are defined by a mitotic count > 20 mitoses/10 high power fields and/or Ki67 index > 20%, and included all the tumors previously classified as poorly differentiated endocrine carcinomas. These latter are aggressive malignancies with a high propensity for distant metastases and poor prognosis, and they can be further divided into small- and large-cell subtypes. However in the NEC category are included also neuroendocrine tumors with a well differentiated morphology but ki67 index > 20%. This category is associated with better prognosis and does not significantly respond to cisplatin-based chemotherapy, which represents the gold standard therapeutic approach for poorly differentiated NEC. In this review, the differences between well differentiated and poorly differentiated NEC are discussed considering both pathology, imaging features, treatment and prognostic implications. Diagnostic and therapeutic flowcharts are proposed. The need for a revision of current classification system is stressed being well differentiated NEC a more indolent disease compared to poorly differentiated tumors.

Extreme Rainfall Event Analysis Using Rain Gauges in a Variety of Geographical Situations

About 30 years of measurements made by the rain gauges located in Piedmont (Italy) have been analyzed. Rain gauges have been divided into 4 datasets considering the complex orography near Turin, namely the flatlands, mountains, hills and urban areas. For each group of gauges, the Generalized Extreme Values (GEV) distributions are estimated considering both the entire dataset of available data and different sets of 3 years of data in running mode. It is shown that the GEV estimated parameters temporal series for the 3 years dataset do not present any specific trend over the entire period. The study presented here is preliminary to a future extreme rainfall event analysis using high temporal and spatial resolution X-band weather radar with a limited temporal availability of radar maps covering the same area.
