Annonaceous acetogenins enclose a large number of biological activities, among which the insecticidal stands out. Spodoptera frugiperda is a pest that affects corn crops among others and has a great capacity to develop resistance to traditional insecticides, which represents sufficient reasons for the search for new alternatives for their control. The objective of this study was to determine the appropriate concentration and screening new natural insecticides against second instar larvae of S. frugiperda, under glasshouse conditions on Zea mays L. Natural products such as annonaceous acetogenins and some acetylated and methoxy methylated ACG derivatives and the commercial product Lambda-cyhalotrin (LC) were evaluated. The percent mortalities of S. frugiperda larvae in glasshouse conditions were recorded after 24, 48 and 72 h of treatment application. The results showed that the acetogenins tested were significantly (P 5) (100 μg/mL) + squamocin (6) (100 μg/mL) + LC (50 μg/mL) that proved the most effective and gave (100%), while rolliniastatin-2 (5) at 100 μg/mL alone, gave lowest percent mortality (65%), followed by squamocin (6) at 100 μg/mL (55%) and LC at 50 μg/mL (30%). The acetylated and methoxy methylated ACGs derivatives caused very low mortality (25% - 35%). It is recommended the mixture as a management option of S. frugiperda as a component of integrated pest management. The results allow us to infer that the addition of natural ACGs synergizes the insecticidal activity of the commercial product. Finding a new ecological alternative for insect control.

AIM To identify short-term and oncologic outcomes of pelvic exenterations(PE) for locally advanced primary rectal cancer(LAPRC) in patients included in a national prospective *** Few studies report on PE in patients with LAPRC. For this study, we included PE for LAPRC performed between 2006 and 2017, as available, from the Rectal Cancer Registry of the Spanish Association of Surgeons [Asociación Espa?ola de Cirujanos(AEC)]. Primary endpoints included procedure-associated complications, 5-year local recurrence(LR), disease-free survival(DFS) and overall survival(OS). A propensitymatched comparison with patients who underwent non-exenterative surgery for low rectal cancers was performed as a secondary *** Eight-two patients were included. The mean age was 61.8 ± 11.5 years. More than half of the patients experienced at least one complication. Surgical site infections were the most common complication(abdominal wound 18.3%, perineal closure 19.4%). Thirty-three multivisceral resections were performed, including two hepatectomies and four metastasectomies. The long-term outcomes of the 64 patients operated on before 2013 were assessed. The five-year LR was 15.6%, the distant recurrence rate was 21.9%, and OS was 67.2%, with a mean survival of 43.8 mo. R+ve resection increased LR [hazard ratio(HR) = 5.58, 95%CI: 1.04-30.07, P = 0.04]. The quality of the mesorectum was associated with DFS. Perioperative complications were independent predictors of shorter survival(HR = 3.53, 95%CI: 1.12-10.94, P = 0.03). In the propensity-matched analysis, PE was associated with better quality of the specimen and tended to achieve lower LR with similar *** PE is an extensive procedure, justified if disease-free margins can be obtained. Further studies should define indications, accreditation policy, and quality of life in LAPRC.

Improving outcomes in pancreatic cancer: Key points in perioperative management

This review focused in the perioperative management of patients with pancreatic cancer in order to improve the outcome of the disease. We consider that the most controversial points in pancreatic cancer management are jaundice management, vascular resection and neo-adjuvant therapy. Preoperative biliary drainage is recommended only in patients with severe jaundice, as it can lead to infectious cholangitis, pancreatitis and delay in resection, which can lead to tumor progression. The development of a phase III clinical trial is mandatory to clarify the role of neo-adjuvant radiochemotherapy in pancreatic adenocarcinoma. Venous resection does not adversely affect postoperative mortality and morbidity, therefore, the need for venous resection should not be a contraindication to surgical resection in selected patients. The data on arterial resection alone, or combined with vascular resection at the time of pancreatectomy are more heterogeneous, thus, patient age and comorbidity should be evaluated before a decision on operability is made. In patients undergoing R0 resection, arterial resection can also be performed.

A Fatigue Analysis of a Hydraulic Francis Turbine Runner

In this work, the estimation of crack initiation life of a hydraulic Francis turbine runner is presented. The life prediction is based on the local strain approach to predict the initiation life. First, the analysis is carried out in air and in water condition and the runner’s natural frequencies were calculated using the finite element (FE) method. The analysis in air is compared with experimental analysis in order to have a representative model of real runner and subsequently the numerical analysis was perform in water. In the case of the runner immersed in water, the added mass effect due to the fluid structure interaction (FSI) is considered. Second, the static and dynamic stresses were calculated according to life estimation. For the calculation of static stresses, the pressure distribution of water and the centrifugal forces were applied to the runner. The dynamic stresses were estimated for interactions between the guide vane and the runner. Lastly, the estimation of the crack initiation life of the runner was obtained.

Pancreatic cancer(Pa C) shows a clear tendency to increase in the next years and therefore represents an important health and social challenge. Currently, there is an important need to find biomarkers for PaC early detection because the existing ones are not useful for that purpose. Recent studies have indicated that there is a large window of time for PaC early detection, which opens the possibility to find early biomarkers that could greatly improve the dismal prognosis of this tumor. The present manuscript reviews the state of the art of the existing PaC biomarkers. It focuses on the anomalous glycosylation process and its role in PaC. Glycan structures of glycoconjugates such as glycoproteins are modified in tumors and these modifications can be detected in biological fluids of the cancer patients. Several studies have found serum glycoproteins with altered glycan chains in PaC patients, but they have not shown enough specificity for PaC. To find more specific cancer glycoproteins we propose to analyze the glycan moieties of a battery of glycoproteins that have been reported to increase in PaC tissues and that can also be found in serum. The combination of these new candidate glycoproteins with their aberrant glycosylation together with the existing biomarkers could result in a panel, which would expect to give better results as a new tool for early diagnosis of PaC and to monitor the disease.

Pharmacodynamics for predicting therapeutic outcome and countering resistance spread:The cefditoren case

The relationship between pharmacokinetics and pharmacodynamics is a key instrument to improve antimicrobial stewardship and should be aimed to identification of the drug exposure measure that is closely associated not only with the ability to kill organisms but also to suppress the emergence of resistant subpopulations. This article reviews published studies for efficacy prediction with cefditoren and those aimed to explore its potential for countering resistance spread, focusing on the three most prevalent community-acquired isolates from respiratory infections: Streptococcus pneumoniae(S. pneumoniae), Haemophilus influenzae(H. influenzae) and Streptococcus pyogenes(S. pyogenes). Studies for efficacy prediction include in vitro pharmacodynamic simulations(using physiological concentrations of human albumin) and mice models(taking advantage of the same protein binding rate in mice and humans) to determine the value of the pharmacodynamic indices predicting efficacy, and Monte Carlo simulations to explore population pharmacodynamic coverage, as weapons for establishing breakpoints. Studies exploring the potential of cefditoren(free concentrations obtained with 400 mg cefditoren bid administration) for countering spread of resistance showed itscapability for countering(1) intra-strain spread of resistance linked to fts I gene mutations in H. influenzae;(2) the spread of H. influenzae resistant strains(with fts I gene mutations) in multi-strain H. influenzae niches or of S. pneumoniae strains with multiple resistance traits in multi-strain S. pneumoniae niches; and(3) for overcoming indirect pathogenicity linked to β-lactamase production by H. influenzae that protects S. pyogenes in multibacterial niches.
