AIM To demonstrate the non-inferiority(15% non-inferiority limit) of monotherapy with tenofovir disoproxil fumarate(TDF) vs the combination of lamivudine(LAM) plus adefovir dipivoxil(ADV) in the maintenance of virologic response in patients with chronic hepatitis B(CHB) and prior failure with *** This study was a Phase IV prospective, randomized, open, controlled study with 2 parallel groups(TDF and LAM+ADV) of adult patients with hepatitis B e antigen(HBe Ag)-negative CHB, prior failure with LAM, on treatment with LAM+ADV for at least 6 mo, without prior resistance to ADV and with an undetectable viral load at the start of the study, in 14 Spanish hospitals. The follow-up time for each patient was 48 wk after randomization, with quarterly visits in which the viral load, biochemical and serological parameters, adverse effects, adherence to treatment and consumption of hospital resources were *** Forty-six patients were evaluated [median age: 55.4 years(30.2-75.2); 84.8% male], including 22 patients with TDF and 24 with LAM+ADV. During study development, hepatitis B virus DNA(HBV-DNA) remained undetectable, all patients remained HBe Ag negative, and hepatitis B surface antigen(HBs Ag) positive. Alanine aminotransferase(ALT) values at the end of the study were similar in the 2 groups(25.1± 7.65, TDF vs 24.22 ± 8.38, LAM+ADV, P = 0.646). No significant changes were observed in creatinine or serum phosphorus values in either group. No significant differences between the 2 groups were noted in the identification of adverse effects(AEs)(53.8%, TDF vs 37.5%, LAM+ADV, P = 0.170), and none of the AEs which occurred were serious. Treatment adherence was 95.5% and 83.3% in the TDF and the LAM+ADV groups, respectively(P = 0.488). The costs associated with hospital resource consumption were significantly lower with the TDF treatment than the LAM+ADV treatment(€4943 ± 1059 vs €5811 ± 1538, respectively, P < 0.001).CONCLUSION TDF monotherapy proved to be safe

To the Editor:We sincerely applaud the work done by Gupta et al.,in which they analyzed a single-center experience regarding gallbladder per-foration(GBP)management[1].Currently,there is a lack of cohort studies to adequately describe the approach strategies and man-agement for this pathology,with international guidelines making ambiguous recommendations[2,3].Their study stratified GBP types and provided details regarding clinical presentation,comorbidities,preoperative diagnosis,etiology,imaging techniques,and surgical *** study reported a high conversion rate when a la-paroscopic approach was attempted for cholecystectomy.

A “false positive” octreoscan in ileal Crohn’s disease

We present a case report of a patient with a suspicious ileal carcinoid tumour. Clinical examination as well as computer tomograghy (CT) scan suggested a tumour. Octeotride scan showed uptake in the same bowel loop reported as pathological in CT. The patient underwent surgery and biopsy which reported Crohn’s disease (CD). The interest in the case is due to the fact that this is, to the best of our knowledge, the second report of Crohn’sdisease as a cause of false positive octeotride scan. Unfortunately, no somatostatin recptors could be found in the sample, so further studies should be performed.

Thermal analysis of olive tree pruning and the by-products obtained by its gasification and pyrolysis: The effect of some heavy metals on their devolatilization behavior

In this work, the effect of the presence of nickel and lead in thermal decomposition of olive tree pruning (OTP), OTP-char and OTP-ashes was studied by thermogravimetry. Experiments were conducted in two kinds of atmosphere (nitrogen atmosphere and oxidizing atmosphere with 20% of O2) at a heating rate of 10 ℃/min. This investigation describes the chemical, physical and fuel properties of the OTP, which shows a similar composition to other lignocellulosic materials such as hazelnut husk and wood sawdust. In addition, SEM analysis indicated that OTP-char surface is higher than OTP surface with plenty of holes and channels. It makes the char an ideal support for metal retention specially for Pb metal (OTP retained 8.55 mg/g whereas OTP char retained 11.57 mg/g). On the other hand, metal retention occurred by adsorption or ion exchange, according to the IR spectrum of the samples. The results of thermogravimetric tests proved that the presence of lead did not have a strong effect on the decomposition of the samples, since TG and DTG curves were very similar. However, nickel increased the mass loss rate, accelerating the decomposition process, showing higher peaks in DTG curves. Additionally, for temperatures higher than 360 ℃, the volatilization of the samples was improved in the nickel-polluted sample, achieving a higher mass loss, getting more energy from the biomass and reducing the quantity of residues left after the process.

Salt/Sodium Intake Estimation in Children and Adolescents of Costa Rica

The objectives of this study were to estimate sodium intake and to identify the main dietary sources of this nutrient in young people. Cross-sectional study performed in 2685 students from 64 schools and high-schools of Costa Rica. A food frequency survey was applied to study the food and beverage intake habits of the participants. The average daily sodium intake for children and adolescents 7 to 18 years of age was 3214 mg. Around 97% of the students had sodium intakes > 2300 mg per day with significant differences by age subgroup, sex and nutritional status due to excess body weight (p marketing campaigns that include educational processes with sense and meaning for children, adolescents and families, in such a way that these groups of the population become motivated to modify gradually their eating habits, such as reducing the use of sauces, seasonings and salt in food preparation and at the table. If the habit of adding salt to the served food is gradually reduced or avoided, the children and adolescents in Costa Rica could decrease the intake of salt per day from 1 to 2 grams. This decrease could be even more effective if youngsters additionally would remove the intake of sauces, contributing in this way to achieve the maximum salt intake recommendation (5 g per day).
