Complications of middle ear cholesteatoma are well described in the literature and are classified into two major categories: intratemporal and intracranial. They are due either to infection or to local extension of the disease and consequent invasion of surrounding tissues. We describe an unusual case of an extratemporal complication in a young woman who was treated at our service. Ten years previously she had undergone canal-wall-down tympanoplasty that did not achieve complete control of the disease. At this admission, the patient was found to have peculiar extension of the recurrent cholesteatoma into the temporozygomatic area. She underwent tympanoplasty of the canal wall using a targeted surgical technique. We resected the recurrent cholesteatoma en bloc without damaging the facial nerve despite extension of the mass to the ear and face. Follow-up was performed each year for 7 years without evidence of recurrence. Cholesteatomas are highly osteolytic, and extension of any residual cholesteatoma is unpredictable. Cholesteatoma of the middle ear requires a careful surgical approach to avoid complications, maintaining awareness of its possible extension into surrounding structures.

Maximising access to and the success of fertility treatments should be a priority for global reproductive health,as should overall patient *** demand for in vitro fertilization(IVF)and other assisted fertility treatments has increased over the past decade and is likely to further increase in years to ***,there is still considerable unmet demand for infertility support ***,the high emotional,physical and financial burden experienced by individuals undergoing IVF cycles can be a risk for their mental and physical health,which in turn can influence treatment continuation and the likelihood of IVF *** from various parts of the world show that most individuals undergoing IVF also use adjunct alternative medicines and procedures,the most common being traditional Chinese medicine(TCM).The complementary and synergistic role of TCM for individuals undergoing IVF is an area that merits further attention and research,both for its potential positive effects on IVF success rates and for its broader physical and mental health ***,much of the existing evidence is not sufficiently robust or consistent for findings to be adopted with *** commentary argues that much work must be done to understand the efficacy and clinical best practices for these integrated *** can be achieved in part by developing more robust and clinically relevant randomized controlled trial protocols,collecting and triangulating evidence through a variety of study designs and methods,and strengthening the collection and pooling of clinic-level data.

Purpose:To construct a symptoms-based prediction tool to assess the likelihood of superior canal dehiscence(SSCD)on high-resolution *** and methods:Mathematical modeling was employed to predict radiologic evidence of SSCD at a tertiary neurotology referral ***:A total of 168 patients were included,of which 118 had imaging-confirmed *** univariate analysis significant predictors of SSCD presence were:sound/pressure-induced vertigo(p?0.006),disequilibrium(p?0.008),hyperacusis(p?0.008),and autophony(p?0.034).Multivariate analysis enabled a 14-point symptom-weighted tool to be developed,wherein a score of6 raised the suspicion of SSCD(70%likelihood of being present),R2?***:The likelihood of SSCD on CT scan can be determined with a high degree of certainty based on symptoms recorded at *** the evidenced-based diagnostic tool validated herein,a score6 with any symptom combination justifies ordering a CT scan.

Recent literature based on peripheral immunity findings speculated that neuroinflammation,with its connection to microglial activation,is linked to bipolar *** endorsement of the neuroinflammatory hypotheses of bipolar disorder requires the demonstration of causality,which requires longitudinal *** aimed to review the evidence for neuroinflammation as a pathogenic mechanism of the bipolar *** carried out a hyper inclusive PubMed search using all appropriate neuroinflammation-related terms and crossed them with bipolar disorder-related *** search produced 310 articles and the number rose to 350 after adding articles from other search engines and reference *** papers were included that appropriately tackled the issue of the presence(but not of its pathophysiological role)of neuroinflammation in bipolar *** these,15 were postmortem and 5 were carried out in living *** articles were consistent with the presence of neuroinflammation in bipolar disorder,but factors such as treatment may mask *** studies were cross-sectional,preventing causality to be ***,no inference can be currently made about the role of neuroinflammation in bipolar disorder,but a link is *** issue remains little investigated,despite an excess of reviews on this topic.

L’esame delle affinità reciproche dei taxa noti ha portato oltre che alla descrizione di una nuovaspecie e di tre nuovi sottogeneri anche a stabilire la sinonimia Muelleriella bonzanoi casale, 1984 =Phaneropella (Epiroella) muelleriana Paoletti, 1975 el’attribuzione di Muelleriella epirota Giachino, 1989al genere Phaneropella (nov. comb.). Attualmente il genere risulta cosi articolato: subgen. ***, per P. lesinae Reitter, Epiroella n. subgen, per P. (E.) muelleriana Paoletti, P. (E.) epirota (Giachi-no), Hittitia n. subgen, per P. (H.) turcica Reitter e Uludagites n. subgen, per P. (U.) minuta n. ***, vengono discussi alcuni aspetti filetici e zoogeografici relativi alla 《linea filetica di Phaneropella》intesa in senso nuovo, formata cioè, per ora, da questo solo genere. This article considers the morphologic and systematic re-definition of the genus Phaneropella Jeannel, 1910, at the light of the new data and of the re-examinationof the species attributed to this genus.
