Background and Aims: Lifestyle (exercise and dietary) modification is the mainstay of treatment for non-alcoholic fatty liver disease (NAFLD). However, there is paucity of data on effect of intensity of exercise in management of NAFLD, and we aimed to study the effect of variable intensities of exercise on NAFLD. Methods: The study was performed in the Department of Gastroenterology of the SCB Medical Col-lege, Cuttack and the Biju Patnaik State Police Academy, Bhu-baneswar. The subjects were police trainees [18 in a moderate intensity exercise group (MIG) and 19 in a low in-tensity exercise group (LIG)] recruited for a 6-month physical training course (261.8 Kcalorie, 3.6 metabolic equivalent in MIG and 153.6 Kcalorie, 2.1 metabolic equivalent in LIG). NAFLD was diagnosed by ultrasonography, with exclusion of all secondary causes of steatosis. All participants were eval-uated by anthropometry (weight, height, body mass index (BMI), waist circumference), assessed for blood pressure and biochemical parameters (blood glucose, liver function test, lipid profile, serum insulin), and subjected to transabdo-minal ultrasonography before and after 6 months of physical training, and the results were compared. Results: Both the groups had similar BMI, fasting plasma glucose, AST, gamma-glutamyl transpeptidase, insulin, and homeostatic model as-sessment-insulin resistance (known as HOMA-IR) (p>0.05). However, subjects in the LIG were older and had lower alanine transaminase, higher triglycerides and lower high-density lip-oproteins than MIG subjects. There was a significant reduc-tion in BMI (27.0±2.1 to 26.8±2.0;p=0.001), fasting blood glucose (106.7±21.6 to 85.8±19.0;p<0.001), serum trigly-cerides (167.5±56.7 to 124.6±63.5;p=0.017), total choles-terol (216.8±29.2 to 196.7±26.6;p=0.037), low-density lipoprotein cholesterol (134.6±21.4 to 130.5±21.9;p=0.010), serum aspartate transaminase (39.3±32.2 to 30.9±11.4;p<0.001), serum alanine transaminase (56.6 ±28.7 to 33.0±11.3;p<0.

Objective: To study the outcome of normothermic beating heart pulmonary thromboendarterectomy (PTE) performed in patients with chronic pulmonary thromboembolism. Methodology: A retrospective analysis of 15 patients who underwent PTE under normothermic beating heart cardio-pulmonary bypass from July 2009 to September 2018 was done. The patients were followed up with clinical assessment, transthoracic echocardiography and computerized tomographicangiography (CTA) for 1 month to 82 months. Results: Data were analysed for 15 consecutive patients. Mean age of the patients was 35.28 years and 10 patients were male. Pre-operatively 6 (40%) patients presented with New York Heart Association (NYHA) class II dyspnoea on exertion symptomatology, 7 (47%) were in NHYA class III, and 2 (13%) were in NYHA class IV. The preoperative and postoperative mean pulmonary artery pressures (mPAP) (mmHg) were 36.50 ± 11.3 and 20.21 ± 7.19, the systolic PAPs (mmHg) were 73.35 ± 14.12 and 35.21 ± 7.89 and the diastolic PAPs (mmHg) were 19.28 ± 8.60 and 12.85 ± 7.26 respectively. 2 (13%) patients had Jamieson’s type I and 12 (87%) had Jamieson type II disease. One patient (7%) expired on 9th postoperative day. All the patients had improved pulmonary gas exchange and did not require oxygen supplementation from 5th postoperative day;symptoms improved to NYHA class I & II in 12 (80%) & 3 (20%) of patients respectively. There was no reperfusion pulmonary edema or any neurologic complications. Postoperative echocardiogram showed improved right ventricular function and Computerised Tomographic Angiogram showed completeness of the procedure. Conclusion: Pulmonary thromboendarterectomy under normothermic beating heart cardiopulmonary bypass has good immediate postoperative results with significant progressive improvement in hemodynamics and quality of life during the course of follow-up. Background: Junctional ectopic tachycardia (JET) is one troublesome supraventricular arrhythmia in postoperative pediatric cardiac surgical patients. Unless treated timely and effectively it may lead to morbidity and even mortality. Aim: To understand the role of Ivabradine in the treatment of JET in postoperative pediatric cardiac surgical patients. Case: We present a postoperative case of complete repair of Tetralogy of Fallot who was in normal sinus rhythm in the preoperative period and developed JET 4 hours later in postoperative period which was persistent despite measures to optimize the internal milieu of the body and conventional antiarrhythmics, that was successfully treated with Ivabradine at a dose of 0.05 mg/kg/12 hourly. Conclusion: Ivabradine can be used to successfully treat postoperative JET in cases ofrefractory to other antiarrhythmic drugs like Digoxin and Amiodarone.

Purpose–Forecasting of stock indices is a challenging issue because stock data are dynamic,non-linear and uncertain in *** of an accurate forecasting model is very much essential to predict the next-day closing prices of the stock *** purpose of this paper is to develop an efficient and accurate forecasting model to predict the next-day closing prices of seven stock ***/methodology/approach–A novel strategy called quasi-oppositional symbiotic organisms search-based extreme learning machine(QSOS-ELM)is proposed to forecast the next-day closing prices *** in the prediction of closing price depends on output weights which are dependent on input weights and *** paper mainly deals with the optimal design of input weights and biases of the ELM prediction model using QSOS and SOS optimization ***–Simulation is carried out on seven stock indices,and performance analysis of QSOS-ELM and SOS-ELM prediction models is done by taking various statistical measures such as mean square error,mean absolute percentage error,accuracy and paired sample *** performance analysis reveals that the QSOS-ELM model outperforms the SOS-ELM model in predicting the next-day closing prices more accurately for all the seven stock indices under ***/value–The QSOS-ELM prediction model and SOS-ELM are developed for the first time to predict the next-day closing prices of various stock *** paired t-test is also carried out for the first time in literature to hypothetically prove that there is a zero mean difference between the predicted and actual closing prices.

The evolution in communication techniques has created wide threats for crucial information transfer through a communication channel. Covert communication with steganography is a skill of concealing secret information within cover object and hence shields the data theft over rapidly growing ***, diverse steganography techniques using edge identification have been proposed in *** methods however utilize certain pixels in the cover image for inserting edge information,resulting in significant deformation. The conventional edge detection method limits the deployment of edge detection in steganography as concealing the information would introduce some variations to the cover image. Hence inserting data in pixel areas recognized by existing conventional edge detection techniques like canny cannot ensure the recognition of the exact edge locations for the cover and stego images. In this paper, an Adaptive steganography method based on novel fuzzy edge identification is proposed. The method proposed is proficient of estimating the precise edge areas of a cover image and also ensures the exact edge location after embedding the secret message. Experimental results reveal that the technique has attained good imperceptibility compared to the Hayat AI-Dmour and Ahmed AIAni Edge XOR method in spatial domain.

Crowd behaviors analysis is the‘state of art’research topic in the field of computer vision which provides applications in video surveillance to crowd safety,event detection,security,*** presents some of the works related to crowd behavior detection and *** crowd behavior detection,varying density of crowds and motion patterns appears to be complex occlusions for the *** work presents a novel crowd behavior detection system to improve these *** proposed crowd behavior detection system is developed using hybrid tracking model and integrated features enabled neural *** object movement and activity in the proposed crowded behavior detection system is assessed using proposed GSLM-based neural *** based neural network is developed by integrating the gravitational search algorithm with LM algorithm of the neural network to increase the learning process of the *** performance of the proposed crowd behavior detection system is validated over five different videos and analyzed using *** experimentation results in the crowd behavior detection with a maximum accuracy of 93%which proves the efficacy of the proposed system in video surveillance with security concerns.

A numerical algorithm is developed for the approximation of the solution to certain boundary value problems involving the third-order ordinary differential equation associated with draining and coating flows. The authors show that the approximate so- lutions obtained by the numerical algorithm developed by using uonpolynomial quintic spline functions ave better than those produced by other spline and domain decomposition methods. The algorithm is tested on two problems associated with draining and coating flows to demonstrate the practical usefulness of the approach.

This paper presents the trajectory tracking control of an autonomous underwater vehicle(AUV). To cope with parametric uncertainties owing to the hydrodynamic effect, an adaptive control law is developed for the AUV to track the desired trajectory. This desired state-dependent regressor matrix-based controller provides consistent results under hydrodynamic parametric *** of the developed controller is verified using the Lyapunov s direct method. Numerical simulations are carried out to study the efficacy of the proposed adaptive controller.
