Objective: Prokineticin-1 (PROK1) is a recently described protein with a wide range of functions including tissue specific angiogenesis, modulation of inflammatory responses and regulation of haematopoiesis. PROK1 has been found in the steroidogenic organs like ovary, testis, adrenal and specially placenta and they have been found to have a role in development of the olfactory system and GnRH system. The aim was to update the role of PROK1 and PROK2 inhuman reproduction since the review was provided by Maldono-Perez (2007) on the potentials of prokineticins in reproduction. Design: A review of international scientific literature by a search of Pubmed and the authors files was done for citation of articles relevant to prokineticins in reproduction, be it its role in ovary, testis, uterus with special emphasis on implantation, normal pregnancy, in labour, pathophysiological states like tubal pregnancy, pcos, various genital tumours, and cases of isolated hypogonadotropic hypogonadism with mutations with PROK2/ PROKR2 and studies detailing functional mechanisms. Results: In the normal cycle, PROK1 has been found to have important roles in implantation, regulating several genes like COX-2, IL-8, IL-11, CTGF related to implantation. Initially murine studies revealed a critical role of PROK2 pathway on olfactory bulb morphogenesis and GnRH secretion which was accidentally discovered and since then several studies on mutations in PROK2/PROKR2 showed that they underlie some case of KS in humans. Although in mouse heterozygote state is not associated with clinical phenotype, most of human mutations are heterozygous. Conclusions: Role of PROK-1 in the process of implantation, with a deeper understanding of the process success rates in IVF and ART can be improved, besides understanding the pathophysiology of tubal pregnancy. OBJECTIVE Older adults with coronary artery disease(CAD)are at risk for ***,little is known regarding transition in frailty measures over time or its impact on *** sought to determine the association of temporal change in frailty with long-term outcome in older adults with *** We re-assessed for phenotypic frailty using the Fried index(0=not frail;1−2=pre-frail;≥3 frail)in a cohort of CAD patients≥65 years old at 2 time points 5 years *** associated with frailty worsening were assessed with scatterplots and outcomes estimated using the Kaplan-Meier *** models were used to assess the risk of worsening frailty on *** There were 45 subjects that completed both baseline and 5-year Fried frailty *** age was 74.6±5.9 and 30(67%)were *** incidence increased over time:baseline(3%frail,37%pre-frail);5 years(10%frail,40%pre-frail).Baseline factors were not predictors of worsening frailty score,while both slower walk time(r=0.46;P=0.004)and diminishing grip strength(r=−0.39;P=0.01)were associated with worsening frailty *** follow-up(median 5.2 years),long-term major adverse cardiac event(MACE)free survival(P=0.12)or hospitalization(P=0.98)was not different for those with worsening frailty score(referent:improved/unchanged frailty).Frailty worsening had a trend towards increased risk of MACE(HR=1.86;95%CI:0.65−5.27,P=0.25).CONCLUSIONS Frailty transitions,specifically,declines in walk time and grip strength,were strongly associated with worsening frailty score in a cohort of older adults with CAD than were baseline indices,though frailty change status was not independently associated with MACE outcomes.

Objective:To do the systematic comparison of prevalence of anaplasmosis by PCR and Giemsa stained thin blood smear(GSTBS) based parasitological assays in dairy cattle of Punjab,which has not been reported *** analyse the haematobiochemical alterations in infected animals to arrive at the conclusion regarding the pathogenicity induced by Anaplasma marginale (***) in latent and patent ***:Study was conducted on 320 animals (236 cows,62 calves and 22 buffaloes) of Punjab,*** on genome of *** was performed by targeting msp1βgene using specific primers BAP-2/AL34S,amplifies products of size 407 *** based data on the characteristics of the infected animals and management strategies of the farm were collected and ***:Higher prevalence and more significant association was observed in the PCR based molecular diagnosis(P=0.00012) as compared to that in GSTBS(P=0.028 8) based diagnosis with various regions under *** respect to the regions,highest prevalence was recorded in Ferozepur by PCR based diagnosis, while that in Jalandhar by GSTBS *** marked significant association of the PCR based diagnosis with the age of the animals under study(P=0.00013) was observed elucidating no inverse age resistance to *** in cow *** profile of infected animals revealed marked anemia,liver dysfunction and increase globulin concentrate indicating rise in immunoglobulin level to counteract ***:PCR is far more sensitive in detecting the disease even in latent infection which may act as nidus for spread of anaplasmosis to susceptible animals in endemic *** of anaemia and liver dysfunction were comparable both in patent as well as latent infection indicating pathogenicity of both.

Clients on Opioid Substitution Therapy (OST) may be at risk of TB. We screened for tuberculosis in asymptomatic OST clients using Chest-X-ray (CXR) and sputum from those with CXR suggestive of TB were tested using Xpert MTB/Rif. Among 472 who underwent CXR, 0.1% had CXR suggestive of TB. The number needed to screen was 118 clients for one CXR suggestive of TB. All four clients with CXR suggestive of TB underwent MTB/Rif assay and one was diagnosed with TB. The TB patient was asymptomatic and could have been missed through the passive approach. We recommend further studies to explore ACF among OST clients.

Intraoperative blood loss in orthotopic liver transplantation:The predictive factors

Liver transplantation has been associated with massive blood loss and considerable transfusion requirements. Bleeding in orthotopic liver transplantation is multifactorial. Technical difficulties inherent to this complex surgical procedure and pre operative derangements of the primary and secondary coagulation system are thought to be the principal causes of perioperative hemorrhage. Intraoperative practices such as massive fluid resuscitation and resulting hypothermia and hypocalcemia secondary to citrate toxicity further aggravate the preexisting coagulopathy and worsen the perioperative bleeding. Excessive blood loss and transfusion during orthotopic liver transplant are correlated with diminished graft survival and increased septic episodes and prolonged ICU stay. With improvements in surgical skills, anesthetic technique, graft preservation, use of intraoperative cell savers and overall perioperative management, orthotopic liver transplant is now associated with decreased intra operative blood losses. The purpose of this review is to discuss the risk factors predictive of increased intra operative bleeding in patients undergoing orthotopic liver transplant.

Thin-film electronics played an important role in flexible healthcare sensor *** common status of their constituent blocks are solid film and network ***,the solid film could only sustain bend in a narrow range due to cracks,and the network structure decreased the sensitivity of flexion sensors due to the strong interactions between *** materials and technologies are urgently required for flexible sensing electronics,to produce the reliable data for assessment of the human ***,we report on a novel three-dimensional(3D)carbon nanorods array(CNA)that is characterized as vertically aligned nanorods and self-cross-linked *** also demonstrate the CNA-based flexible healthcare sensors in monitoring the Parkinsonian *** with two-dimensional(2D)carbon nanotube networks and solid thin films,such self-cross-linked geometries are highly resistant to crack and fragmentation under *** the meantime,it shows high sensitivity and good stability(~10,000 times)to detect the *** CNA-based flexible devices are capable of recording low-frequency vibrations(<6 Hz)and make it excellent to monitor the rest tremor of the human body,which is an initial symptom of Parkinson’s *** 3D self-cross-linked CNA film shows great potential in the fabrication of cost-effective and durable flexible sensors for early diagnosis of disease by monitoring the health-related rest tremors.
