An update on the role of prokineticins in human reproduction-potential therapeutic implications

Objective: Prokineticin-1 (PROK1) is a recently described protein with a wide range of functions including tissue specific angiogenesis, modulation of inflammatory responses and regulation of haematopoiesis. PROK1 has been found in the steroidogenic organs like ovary, testis, adrenal and specially placenta and they have been found to have a role in development of the olfactory system and GnRH system. The aim was to update the role of PROK1 and PROK2 inhuman reproduction since the review was provided by Maldono-Perez (2007) on the potentials of prokineticins in reproduction. Design: A review of international scientific literature by a search of Pubmed and the authors files was done for citation of articles relevant to prokineticins in reproduction, be it its role in ovary, testis, uterus with special emphasis on implantation, normal pregnancy, in labour, pathophysiological states like tubal pregnancy, pcos, various genital tumours, and cases of isolated hypogonadotropic hypogonadism with mutations with PROK2/ PROKR2 and studies detailing functional mechanisms. Results: In the normal cycle, PROK1 has been found to have important roles in implantation, regulating several genes like COX-2, IL-8, IL-11, CTGF related to implantation. Initially murine studies revealed a critical role of PROK2 pathway on olfactory bulb morphogenesis and GnRH secretion which was accidentally discovered and since then several studies on mutations in PROK2/PROKR2 showed that they underlie some case of KS in humans. Although in mouse heterozygote state is not associated with clinical phenotype, most of human mutations are heterozygous. Conclusions: Role of PROK-1 in the process of implantation, with a deeper understanding of the process success rates in IVF and ART can be improved, besides understanding the pathophysiology of tubal pregnancy. Paradoxical Low Flow Low Gradient Severe Aortic Stenosis—An Under-Recognized Entity

Some patients with severe aortic stenosis (AS), due to restrictive cardiac physiology, paradoxically have relatively low flow and low gradients across stenotic aortic valves despite preserved left ventricular (LV) systolic function. It results in symptoms and reduced quality of life and carries a high mortality. Whilst this form of severe AS, termed paradoxical low flow low gradient (pLFLG), is well reported, patients with this diagnosis experience inappropriate barriers to aortic valve replacement (AVR), the only efficacious treatment. We present the case of an 88-year-old female with 12 months of exertional dyspnoea on a background of hypothyroidism and hypercholesterolemia. Transthoracic echocardiogram (TTE) revealed LV hypertrophy, with a small LV cavity size and reduced stroke volume, yet normal systolic function. A heavily calcified aortic valve was identified with severe aortic stenosis, based on valve area, yet with incongruous mean transvalvular gradient of 25 mmHg (severe ≥ 50 mmHg). Following exclusion of other differential diagnoses, her symptoms were attributed to paradoxical LFLG severe AS. She was however declined definitive transcatheter aortic valve implantation (TAVI) due to her paradoxically low mean aortic gradient. Following further deterioration in her symptoms and supportive quantification of poor exercise performance, she was ultimately re-referred, accepted, and underwent TAVI. Following her AVR, the patient experiences significant improvement in both symptoms and quality of life after only one month. Paradoxical LFLG severe AS remains a well-documented yet under recognized disease. It carries high morbidity and mortality if untreated, yet is significantly less likely to be referred and accepted for intervention. With its prevalence expected to rise with an ageing population, this case serves as a timely reminder for clinicians to address the under recognition of important pathology.

Factors associated with change in frailty scores and long-term outcomes in older adults with coronary artery disease

Reconstruction of chronic acromioclavicular joint disruption with artificial ligament prosthesis

Objective： Management of Rockwood type 3 acromioclavicular disruptions is a matter of debate. Should we adopt conservative or operative measures at first presentation？ It is not clear but most of the evidences are in favour of conservative management. We present our expe- rience in managing these patients surgically. Methods： We present a prospective series of eight cases of chronic Rockwood type 3 acromioclavicular joint disruptions treated surgically. Anatomical reconstruction of the coracoclavicular ligament was done by artificial braided polyester ligament prosthesis： Results： All the patients were able to perform daily activities from an average of the 14th postoperative day. All patients felt an improvement in pain, with decrease in ave- rage visual analogue scale from preoperative 6.5 points （range 3-9 points） to 2.0 points （range 0-5 points）, Constant score from 59% to 91% and American Shoulder and Elbow Surgeons shoulder score from 65 to 93 points postoperatively. These results improved or at least remained stationary on midterm follow-up, and no deterioration was recorded at an average follow-up of 46 months. Conclusion： This midterm outcome analysis of the artificial ligament prosthesis is the first such follow-up study with prosthesis. Our results are encouraging and justify the further use and evaluation of this relatively new and easily reproducible technique.

This work promotes the room temperature energy storage properties of the *** this approach,impacts of PrFeO_(3)doping on PT-based solid solutions(Pb1−xPrxTi1−xFexO_(3),x=0.21,0.22,0.23,0.24,0.25 and 0.26)have been explored.X-ray diffraction(XRD)patterns were used to estimate the crystallographic parameters,confirming the single phase tetragonal *** ferroelectric Curie temperature(Tc FE)is observed to drop from 410 K to below room temperature as the Pr concentration *** ferroelectric P-E loops were used to determine the energy storage values at room *** sample x=0.24 achieved the maximum value of energy storage density of 362.25 mJ/cm^(3)with the efficiency of 40.5%.The ferroelectric P-E loops were used to determine the energy storage values at room *** validity of magnetoelectric coupling in all samples was confirmed by magneto-dielectric studies and found that the sample x=0.24 shows the maximum response with the coupling coefficient(γ)=15.54 g^(2)/emu^(2).

This work harmonizes the experimental and theoretical study of electrocaloric effect(ECE)in(Pb_(0.8)Bi_(0.2))(Zr_(0.52)Ti_(0.48))O_(3)solid solution by optimizing sintering ***^(3+)-doped PbZr_(0.52)Ti_(0.48)O_(3)solid solutions were synthesized by the conventional solid-state reaction *** samples were prepared by varying the sintering temperature.X-ray diffraction study con-firms the crystalline nature of all the *** immense value of polarization has been acquired in the optimized *** maximum adiabatic temperature change of order 2.53 K with electrocaloric strength of 1.26 K mm kV^(−1)has been achieved *** a comparatively close value of ECE has been acquired from the theoretical calculations using a phenomeno-logical ***,a large value(218 mJ cm^(−3))of thermal energy conversion has been obtained using the Olsen cycle.

Asymptomatic Screening of Clients on Opioid Substitution Therapy for Tuberculosis: An Experience from India

Clients on Opioid Substitution Therapy (OST) may be at risk of TB. We screened for tuberculosis in asymptomatic OST clients using Chest-X-ray (CXR) and sputum from those with CXR suggestive of TB were tested using Xpert MTB/Rif. Among 472 who underwent CXR, 0.1% had CXR suggestive of TB. The number needed to screen was 118 clients for one CXR suggestive of TB. All four clients with CXR suggestive of TB underwent MTB/Rif assay and one was diagnosed with TB. The TB patient was asymptomatic and could have been missed through the passive approach. We recommend further studies to explore ACF among OST clients.

Intraoperative blood loss in orthotopic liver transplantation:The predictive factors

Liver transplantation has been associated with massive blood loss and considerable transfusion requirements. Bleeding in orthotopic liver transplantation is multifactorial. Technical difficulties inherent to this complex surgical procedure and pre operative derangements of the primary and secondary coagulation system are thought to be the principal causes of perioperative hemorrhage. Intraoperative practices such as massive fluid resuscitation and resulting hypothermia and hypocalcemia secondary to citrate toxicity further aggravate the preexisting coagulopathy and worsen the perioperative bleeding. Excessive blood loss and transfusion during orthotopic liver transplant are correlated with diminished graft survival and increased septic episodes and prolonged ICU stay. With improvements in surgical skills, anesthetic technique, graft preservation, use of intraoperative cell savers and overall perioperative management, orthotopic liver transplant is now associated with decreased intra operative blood losses. The purpose of this review is to discuss the risk factors predictive of increased intra operative bleeding in patients undergoing orthotopic liver transplant.
