Hypertension (HTN) Knowledge and with Its Associated Factors: About 456 Outpatients Seen in Cardiology Department in University Hospital (UH) Gabriel Touré—Bamako (Mali)

Background: High Blood Pressure (HBP) is high prevalent among adult population in Bamako, but little is known about factors associated with knowledge. Methods: It was a cross-sectional study involving patients aged 15 years and more with a first classification in normal blood pressure (HTN-) and high blood pressure (HTN+), and Second classification inpatients without knowledge (who answered No) (K-) and patients with knowledge (who answered Yes) (K+). A logistic regression was performed to look up predictors among different variables. Results: The sample involved 456 patients with a mean age of 51.39 years and 65.1% of female. The age group 45 - 59 years old made 32.5% and unschooled patients 60.3%. Patients with HBP accounted for 69.7% and those reporting to know about it 67.3%. HTN- and HTN+ differed significantly except for HR, height, sex and level of schooling. HBP prevalence increased with age up to 74 years. Regarding knowledge, sex, age group and number of FDRs did not differ significantly. High education level and duration of HBP was predictive of knowledge with an OR of 1.186 [CI 0.058 - 0.796] and 1.192 [CI 0.332 - 4.275] respectively. Conclusions: Our study provided data on HBP knowledge among outpatients with high educational level and HBP duration associated with better knowledge on HBP.

Epidemiological, Clinical and Therapeutic Aspects of Acute Respiratory Distress in Children in Medical Emergencies at the Bangui Pediatric University Hospital

Introduction: Respiratory distress is a clinical condition accompanied by an increase in work of breathing, with the respiratory accessory muscles brought into play to ensure normal arterial oxygenation. It is a major cause of morbidity and mortality in pediatrics. The aim of our study was to investigate the epidemiological, clinical and therapeutic aspects of respiratory distress in children aged between 1 month and 15 years seen in the emergency department of the Bangui paediatric university hospital. Methodology: This was a 3-month descriptive cross-sectional study, from January 1 to March 31, 2023. All children aged 1 month to less than 15 years presenting with respiratory distress were included. Results: A total of 3021 children were admitted to the emergency medical services of Bangui’s pediatric university hospital. Of these, 164 were included in the study. The predominance was male, with a sex ratio of 1.09. The 0 - 2 age group was the most represented, with 67 patients (42.85%). The majority of patients came from Bangui, accounting for 146 (89.02%) of cases. Respiratory difficulty (59.15%), characterized by dyspnea and cough, associated with fever, vomiting, physical asthenia and diarrhea, were the main reasons for consultation. The main pathologies noted were respiratory 92 (56.10%), followed by cardiac pathologies 21 (12.8%). Antibiotic administration (76.82%) was the most common therapeutic procedure used in the management of respiratory distress. Conclusion: Respiratory distress remains an important cause of infant mortality in our context, with major management problems.

Geophysical Significance of the Senegalo-Mali Discontinuity: Evidence from Secondary Structures, Kédougou-Kéniéba Inlier, Western Mali

The present study focuses on the analysis and description of lineaments interpreted as secondary structures to describe the nature of Senegalo Malian Discontinuity. These lineaments cross-cut the large north-south oriented transcurrent lithospheric structure known as the Senegalo Malian Discontinuity (SMD). Two lineaments were selected oriented NNE (N15˚ to N25˚), one at Dialafara and one at Sadiola. Four profiles on each lineament of these 2 zones, so that there were 2 on each side of the SMD. The ground data collected were processed using proper parameter and software. Some filters were applied to enhance the signal level. These ground data were later compared to the existing airborne magnetic data for consistency and accuracy using the upward continuation filter. The results show that the quality of ground data is good. In addition, the ground magnetic data show the presence of certain local anomalies that are not visible in the regional data. The analytical signal was also used to determine domain boundaries or possible contact zones. The contact zone can be highlighted on certain profiles such as L300 and L600. The study showed that the west and east sides of the SMD are not the same. Secondary structures become wide when approaching the SMD on both sides. They are also duplicated to the east of the SMD when we move progressively away. In the Dialafara area, the ground magnetic data intersect an interpreted fold. The results of this work confirm the presence of the secondary structures and their evolution in relation to the SMD. The relationships between the secondary structures in the Dailafara and Sadiola zones and their relations with the SMD are highlighted. The technique used in this study, is an important approach to better description and interpreting of regional structures using the secondary structures and proposing a structural model.

Total Electron Content Diurnal and Seasonal Variations and Response to Solar Events at Koudougou Station in Burkina Faso

In this paper, we studied the seasonal behavior of the total electron content (TEC) during a part of solar cycle 24 ascending, maximum and decreasing phases at Koudougou station (Latitude: 12°15'09"N Longitude: 2°21'45"W). Response of TEC to solar recurrent events is presented. The highest values of the TEC in 2014, 2015 and 2016 were recorded on March and October, while in 2013 they were recorded on April and November, corresponding to equinox months. This observation shows that TEC values at the equinoxes are higher than those of solstices. Moreover, the monthly TEC varies in phase with the sunspots number showing a linear dependence of the TEC on solar activity. The ionospheric electron contents are generally very low both before noon and during the night, but quite high at noon and after noon. This pattern of TEC variation is due to the fluctuation of incident solar radiation on the Earth’s equatorial ionosphere. During quiet periods, the number of free electrons generated is lower than that generated during recurrent periods, which shows a positive contribution of recurrent activity to the level of the TEC. Investigations have also highlighted a winter anomaly and equinoctial asymmetry in TEC behavior at Koudougou station.

Stroke Epidemiological Study at the Centre Hospitalier Universitaire (CHU) de Cocody in Abidjan, Côte D’Ivoire

Background: Recent literature reported a recurrent increase in hemorrhagic stroke (HS) rates in low- and middle-income countries. However, the causes and mechanisms of the rising HS rates are unknown. To address these issues, the present epidemiological study was carried out in the neurovascular unit of the neurology department at the Centre Hospitalier Universitaire (CHU) de Cocody in Abidjan (Africa). Methods: The study included 60 patients hospitalized from January 1 to December 31, 2016, who underwent brain CT (computerized tomography) scans. We examined the medical records and survey forms in these patients with acute stroke (within 7 days) admitted to the Neurovascular Unit receiving only stroke patients. In these patients, we explored stroke distribution according to socio-demographic variables: age, gender and socio-professional categories. In addition, common risk factors were assessed. Results: In Côte d’Ivoire, our studies showed an ischemic stroke (IS) rate of 66.67% and a hemorrhagic stroke (HS) rate of 33.33%. Our studies indicated that stroke particularly affects the 40 - 60 age group. Then, stroke frequency has fallen in the 30 - 40 and 60 - 70 age groups, becoming rare in relatively young (70 years) subjects. In the sample analyzed, stroke occurred in women (56.67%) compared with men (43.33%). However, there was no gender-specific predilection for stroke. In addition, 90% of strokes occur in the working population and 10% in the non-working population. Arterial hypertension was the highest medical risk factor among patients (66.13%), compared with the other risk factors i.e. alcohol (17.42%), smoking (12.90%), previous stroke (9.67%), diabetes (6.65%), heart disease (6.65%) and dyslipidemia (3.23%). Specifically, hypertension was the main risk factor causing HS (45%) and IS (21.66%) in patients. Long-term survival following radiofrequency ablation of colorectal liver metastases: A retrospective study

AIM: To retrospectively evaluate the long-term survivalof patients that received radiofrequency ablation(RFA) therapies of colorectal liver metastases. METHODS: In 2005 to 2008, RFA of 105 colorectal liver metastases(CRLM) were performed on 49 patients in our institution. The liver metastases were evaluated, both before and after ablation therapies, with contrast enhanced computerised tomography and contrast enhanced ultrasonography. Histological evidence of malignant liver metastases was obtained in the few instances where contrast enhanced ultrasonography gave equivocal results. Accesses to the CRLM were guided ultrasonically in all patients. The data obtained from records of these ablations were retrospectively analysed and survival data were compared with existing studies in the ***: 1-, 2-, 3-, 4- and 5-year survival rates, when no stringent selection criteria were applied, were 92%, 65%, 51%, 41% and 29% respectively. To explore the impact of the number and size of CRLM on patients' survival, an exclusion of 13 patients(26.5%) with number of CRLM ≥ 5 and tumour size ≥ 40 mm resulted in 1-, 2-, 3-, 4- and 5-year survival rates improving to 94%, 69%, 53%, 42% and 31% respectively. It is of note that 9 of 49 patients developed extra-hepatic metastases, not visible or seen on pretreatment scans, just after RFA treatment. These patients had poorer survival. The development of extra-hepatic metastases in nearly 20% of the patients included in our study can partly account for modestly lower survival rates as compared with earlier studies in the ***: Our study underscores the fact that optimum patients' selection before embarking on RFA treatment is vitally important to achieving a superior outcome.

Imatinib-induced fatal acute liver failure

Imatinib mesylate is a drug that has been approved for treatment of chronic myeloid leukemia (CML) in blast crisis, accelerated or chronic phase, and also for advanced gastrointestinal stromal tumors. Severe hepatic toxicity and three deaths from hepatic failure have been reported. We report the case of a 51-year-old woman who was admitted to our institution with severe acute hepatitis. She was diagnosed with CML and began treatment with imatinib mesylate at a dose of 400 mg/d. Five months after beginning treatment, she developed severe hepatitis associated with coagulopathy, and was admitted to our institution. She had been consuming acetaminophen 500-1000 mg/d after the onset of symptoms. She had a progressive increase in bilirubin level and a marked decrease of clotting factor Ⅴ. Five days after admission, grade Ⅱ encephalopathy developed and she was referred for liver transplantation. Her clinical condition progressively deteriorated, and 48 h after being referred for transplantation she suffered a cardiac arrest and died. This report adds concern about the possibility of imatinib-mesylate-induced hepatotoxicity and liver failure, particularly in the case of concomitant use with acetaminophen. Liver function tests should be carefully monitored during treatment and, with the appearance of any elevation of liver function tests, treatment should be discontinued.
