Flavonoid chemical composition and antidiabetic potential of Brachychiton acerifolius leaves extract

Objective:To evaluate Brachychiton acerifolius leaf extracts as antidiabetic potential agent and to identify the main active constituents using bioactivity guided ***:In vitro antioxidant activity was evaluated for *** different extracts using DPPH assay and vitamin C as *** activity was then determined using STZ-induced rats treated daily with ethyl acetate and 70% ethanol leaf extracts for4 weeks at a dose of 200 g/kg body weight against gliclazide reference *** glucose,a-amylase,lipid profile,liver function enzymes and oxidative stress markers were assessed along with histopathological study for liver and pancreatic *** and structural elucidation of active compounds were made using Diaion and Sephadex followed by spectral ***:The results indicated that ethyl acetate and ethanol leaf extracts exhibited the strongest antioxidant activity compared to that of vitamin C(IC500.05,0.03 and 12 mg/m L,respectively).Both extracts showed potent anti-hyperglycemic activity evidenced by a significant decrease in serum glucose levels by 82.5% and 80.9% and a-amylase by45.2% and 53.6%,as compared with gliclazide 68% and 59.4%,*** of ethanol extract resulted in the isolation of 9 flavonoids including apigenin-7-O-arhamnosyl(1/2)-b-D-glucuronide,apigenin-7-O-b-D-glucuronide,apigenin-7-O-b-Dglucoside and ***:This study highlights the potential use of *** leaf extract enriched in flavones for the treatment of diabetes that would warrant further clinical trials investigation.

Antifouling evaluation of extracts from Red Sea soft corals against primary biofilm and biofouling

Objectives: To evaluate antifouling property of extracts from Red Sea soft corals against primary biofilm and ***: Seven species of soft corals Sarcophyton glaucum(a), Sinularia compressa,Sinularia cruciata(a), Heteroxenia fuscescens(a), Sarcophyton glaucum(b), Heteroxenia fuscescens(b) and Sinularia cruciata(b) were chosen to test their extracts as antibacterial and antifouling agents in Eastern Harbour of Alexandria, Mediterranean *** compounds of soft corals were extracted by using methanol and concentrated under vacuum. The residues of extracts were mixed in formulation of inert paint which consisted of rosin, chlorinated rubber and ferrous oxide against micro and macro fouling organisms. The formulated paints were then applied on PVC panels twice by brush,hanged in a steel frame and immersed in Eastern Harbour of Alexandria Mediterranean Sea followed by visual inspection and photographic ***: After 185 days of immersion in seawater, the antifouling results agreed with the antibacterial results where extracts of Sinularia compressa and Heteroxenia fuscescens(b) gave the best activity against marine fouling tubeworms and barnacles. The inhibition activity was correlated with the major functional groups(hydroxyl, amino, carbonyl,aliphatic(fatty acids), C]C of alkene or aromatic rings and CeC l of aryl halides) of the ***: The strong antifouling activity makes them promising candidates for new antifouling additives. After the screening and application of natural organic compounds from soft corals, marine organisms show activity against micro and macro fouling organisms.

Dendritic cell co-stimulatory and co-inhibitory markers in chronic HCV: An Egyptian study

Melatonin and/or rowatinex attenuate streptozotocin-induced diabetic renal injury in rats

The study aimed to explore the prophylactic effect of melatonin, rowatinex; a naturally occurring renal drug, and its combination on diabetic nephropathy in type 2 diabetic rats. Diabetes was induced by intraperitoneal injection of a single dose of streptozotocin(50 mg/g body weight). Three days before diabetes induction, rats were daily treated with melatonin, rowatinex and their combination continuously for 8 weeks. Evaluation was done through measuring blood urea nitrogen(BUN), serum uric acid, serum creatinine, urine creatinine, creatinine clearance, nitric oxide(NO), malondialdehyde(MDA), superoxide dismutase(SOD), glutathione(GSH), total antioxidant capacity(TAC),kidney injury molecule-1(KIM-1), heat shock protein-70(HSP-70), caspase-3, transforming growth factor β1(TGFβ1), DNA degradation by the comet assay and total protein contents. Histopathologic study was also done for the kidney and the pancreas. Drastic changes in all measured parameters of the diabetic rats were observed. Treatment with melatonin and rowatinex showed amelioration to variable degrees. In conclusion, melatonin showed the most potent effect on protecting rats from deleterious action of diabetic nephropathy followed by its combination with rowatinex.

Serum Resistin Level and Polymorphonuclear Leukocytes Dysfunctions in Children on Regular Hemodialysis

Resistin is a secretory adipocytoine, which is expressed mainly in humans by inflammatory cells especially macrophages. Resistin serum levels are elevated in end-stage renal diseases of people having an increased risk of infections as a result of impaired polymorphonuclear leukocytes (PMNLs) functions. Objectives: To evaluate neutrophil functions (phagocytosis and oxidative burst) in children with end-stage renal disease (ESRD) on regular hemodialysis and to shed light on the contribution of resistin on neutrophil functions. Patients and Methods: The study included 40 children with ESRD on regular hemodialysis. Their ages ranged from 6 to 12 years, and they were selected from children attending the pediatric hemodialysis unit of AL-Zahraa Hospital, Al-Azher University during the period from October 2012 to December 2013. Another group of 40 apparently healthy children with matched age and sex with the patient group served as a control. Serum resistin, phagocytic index and nitro blue tetrazolium test (NBT%) were assessed in both groups. Results: There was a statistically more significant increase in resistin serum levels in cases than in controls;it was (3.25 ± 0.86 ng/ml) and (0.25 ± 0.16 ng/ml) respectively

High-sensitivity C-reactive protein as a marker of cardiovascular risk in obese children and adolescents

Background and aim of the work: High-sensiti- vity C-reactive protein (hsCRP) is a marker of low grade inflammatory state, which characterises an atherosclerotic process. The metabolic syndrome is associated with insulin resistance and a systemic low-grade inflammatory state. These disorders may arise at a very early age in obese children. We aimed to assess the utility of (hsCRP) as a marker of cardiovascular risk in obese children and adolescents. Patients and methods: This study was conducted on 100 obese child and adolescents (6-16 years). 50 apparently healthy children of matched age and sex served as control. All patients and controls were subjected to: 1-complete history taking. 2-anthropometric measurements and clinical examination including body height, weight, waist circumference, body mass index and blood pressure. 3-laboratory investigations in- cluding fasting glucose, lipid profile, apolipoprotiens and (hsCRP) were assessed. Metabolic syndrome patients had to meet three out of five criteria: concentration of triglycerides (TG) ≥ 110 mg/dL, high density lipoprotein cholesterol (HDL- C) ≤ 40 mg/dL, waist circumference ≥ 90th percentile, glucose concentration ≥ 110 mg/dL, and systolic or diastolic blood pressure ≥ 90th percentile. Results, height, weight BMI and blood pressure were significantly higher in the obese than the control. Obese group had significantly higher (hsCRP) levels than control group, (p 3.0 mg/L, a concentration considered to place adults at high risk for cardiovascular disease. In multiple logistic regression analysis only abdominal obesity was significantly associated with (hsCRP). Conclusion: me- tabolic syndrome and abdominal obesity among our patients predispose to cardiovascular disease later in life through early low grade inflammation. (hsCRP) is one of the inflammatory markers that can be easily estimated in these patients.

Assessment of hepatic fibrosis in pediatric cases with hepatitis C virus in Egypt

AIM: To assess hepatic fibrosis and factors associated with its progression in children with HCV infection. METHODS: At the Hepatology Unit,Cairo University Children's Hospital,a single liver biopsy was performed to 43 children with HCV infection after an informed consent between 1998-2004. Their mean age at liver biopsy was 8.67 ± 4.3 years. RESULTS: Among the 43 patients' biopsies,12 (27.9%) were having no fibrosis,20 (46.5%) mild fibrosis and 11 (25.6%) moderate to severe fibrosis. The median time for development of fibrosis was estimated to be 5.5 years. Developing fibrosis was significantly associated with shorter duration from first detected ALT elevation to biopsy (12 mo vs 1.2 mo,P = 0.015) and having higher levels of direct serum bilirubin (0.3 mg/dL vs 0.5 mg/dL,P = 0.048). No association was found between fibrosis stage and the presence of co-morbid conditions (P = 0.33). CONCLUSION: Hepatic fibrosis was present in 72.1% of children with HCV infection. The development of fibrosis was associated with higher levels of direct serum bilirubin. There was no significant association between fibrosis and age,duration of infection,risk factors,co-morbid conditions and most biochemical parameters.

3-Bromopyruvate(3BP) is a new, promising anticancer alkylating agent with several notable functions. In addition to inhibiting key glycolysis enzymes including hexokinase II and lactate dehydrogenase(LDH), 3BP also selectively inhibits mitochondrial oxidative phosphorylation, angiogenesis, and energy production in cancer cells. Moreover, 3BP induces hydrogen peroxide generation in cancer cells(oxidative stress effect) and competes with the LDH substrates pyruvate and lactate. There is only one published human clinical study showing that 3BP was effective in treating fibrolamellar hepatocellular carcinoma. LDH is a good measure for tumor evaluation and predicts the outcome of treatment better than the presence of a residual tumor mass. According to the Warburg effect, LDH is responsible for lactate synthesis, which facilitates cancer cell survival, progression, aggressiveness, metastasis, and angiogenesis. Lactate produced through LDH activity fuels aerobic cell populations inside tumors via metabolic symbiosis. In melanoma, the most deadly skin cancer, 3BP induced necrotic cell death in sensitive cells, whereas high glutathione(GSH) content made other melanoma cells resistant to 3BP. Concurrent use of a GSH depletor with 3BP killed resistant melanoma cells. Survival of melanoma patients was inversely associated with high serum LDH levels, which was reported to be highly predictive of melanoma treatment in randomized clinical trials. Here, we report a 28-year-old man presented with stage IV metastatic melanoma affecting the back, left pleura, and lung. The disease caused total destruction of the left lung and a high serum LDH level(4,283 U/L). After ethics committee approval and written patient consent, the patient received 3BP intravenous infusions(1-2.2 mg/kg), but the anticancer effect was minimal as indicated by a high serum LDH level. Timed Intercourse versus Intrauterine Insemination with Mild Ovarian Stimulation for Unexplained Infertility

Background: Infertility is unexplained in about 22% - 28% of infertile couples. Spontaneous pregnancy may occur in them. If did not happen, Expectant management will be via clomiphene citrate (CC) administration, intrauterine insemination (IUI), and IVF & ICSI. Aim: to assess the effectiveness of intrauterine insemination with mild controlled ovarian stimulation compared with expectant management in couples with unexplained infertility more than one year. Methods: 160 couples with unexplained infertility were selected, The couples were randomly divided into two groups: Group (A) “80 couples”: Intrauterine insemination (IUI) with mild controlled ovarian stimulation (combination of CC + hMG) using prepared semen and was performed 36 hours after hCG injection, Group (B) “80 couples”: Couples had no ovulation induction. They encouraged for timed intercourse in the most fertile days of female cycle guided by folliculometry for 6 months. Data were collected quantitatively, coded and analyzed using SPSS. The power of study is 80% and 95% confidence interval. Result: The pregnancy rate was calculated in both groups: In group (A) (IUI/COS): OPR (Ongoing pregnancy rates) was 27.5% and PR (pregnancy rates)/cycle was 8.6%. In group (B) (Expectant management): OPR was 25% and PR/cycle was 5.3%. Conclusion: Mild controlled ovarian stimulation CC + hMG with IUI offers no statistical significance in terms of pregnancy outcomes over expectant management in this study however significance in the response to stimulation.

Doppler Study of Uterine Artery and Ultrasonography of Endometrial Thickness in Perimenopausal and Postmenopausal Bleeding

Background: Transvaginal color Doppler sonography of the uterine artery has been reported useful for evaluation of abnormal uterine bleeding in peri-/post-menopausal women. It may differentiate physiological from malignant endometrial changes. Objective: Evaluating the endometrial thickness and uterine artery Doppler as an initial diagnostic tool to identify patients with abnormal endometrial pathology. Methods: This is a prospective case control study consisted of total 50 women with peri-(n = 35) and post-(n = 15) menopausal bleeding, with 20 women consisting control group (no bleeding) (peri-(n = 10) and post-menopausal (n = 10)). Double layer endometrial thickness and uterine-artery-Doppler waveforms were measured, with the latter being S/D ratio, RI and PI. These were related to histological findings (endometrial biopsy). Result: Patients with abnormal uterine bleeding (both peri- and postmenopausal) had a significantly higher mean endometrial thickness than the control women. Study group showed a significantly lower value of all Doppler indices (S/D ratio, RI and PI). In study group, patients with malignant endometrial pathology showed significantly thicker endometrium and significantly lower Doppler indices than those with benign pathology. Conclusion: The combination of endometrial thickness and uterine artery color Doppler pattern might predict uterine endometrial malignancy, although whether it can discriminate (screen) patients requiring endometrial biopsy is not evident. Further study is needed.
