Impact of obesity on the surgical outcome following repeat hepatic resection in Japanese patients with recurrent hepatocellular carcinoma

AIM: To evaluate the impact of obesity on the posto- perative outcome after hepatic resection in patients with hepatocellular carcinoma (HCC). METHODS: Data from 328 consecutive patients with primary HCC and 60 patients with recurrent HCC were studied. We compared the surgical outcomes between the non-obese group (body mass index: BMI < 25 kg/m2) and the obese group (BMI ≥ 25 kg/m2). RESULTS: Following curative hepatectomy in patients with primary HCC, the incidence of postoperative complications and the long-term prognosis in the non- obese group (n = 240) were comparable to those in the obese group (n = 88). Among patients with recurrent HCC, the incidence of postoperative complications after repeat hepatectomy was not significantly different between the non-obese group (n = 44) and the obese group (n = 16). However, patients in the obese group showed a significantly poorer long-term prognosis than those in the non-obese group (P < 0.05, five-yearsurvival rate; 51.9% and 92.0%, respectively). CONCLUSION: Obesity alone may not have an adverse effect on the surgical outcomes of patients with primary HCC. However, greater caution seems to be required when planning a repeat hepatectomy for obese patients with recurrent HCC.

Influence of the water jet system vs cavitron ultrasonic surgical aspirator for liver resection on the remnant liver

BACKGROUND Several methods,such as finger fracture,Pean crush,cavitron ultrasonic surgical aspirator(CUSA),and water jet(WJ),are used for hepatic parenchymal dissection in liver *** is the conventional method in *** is a relatively novel method for parenchymal *** it has several advantages,such as lower volume of blood loss and shorter operative time,the effect of the WJ system for hepatic dissection on the remnant liver has not yet been *** To investigate and compare the effect of the WJ method vs CUSA on the remnant liver cut *** This observational study compared the two types of parenchymal transection methods(WJ vs CUSA)in liver *** total,24 and 40 patients who underwent hepatectomy using the WJ method and CUSA,respectively,were included in the ***,the clinicopathological characteristics and clinical outcomes of 24 and 40 patients were ***,postoperative contrast-enhanced computed tomography(CT)scan was performed to assess the cut surface length of the remnant liver and the degenerative thickness of the areas with a reduced contrast effect in the dissected ***,the two groups were *** On CT scan,the median areas of denaturation in the liver dissection planes were 522(range:109.5-1242)mm^(2) in the CUSA group and 324(range:93.6-1529)mm^(2) in the WJ *** area did not significantly differ between the two groups;however,the denaturation thickness of the WJ group was significantly lower than that of the CUSA group[5.8(range:0.7-11.1)mm vs 3.3(range:1.7-10.4)mm,P<0.001].CONCLUSION The WJ group had significantly thinner contrast-enhanced areas in the post hepatectomy detached section than the CUSA group.

Aim:To clarify the significance of surgical resection for pulmonary recurrence after curative esophagectomy for esophageal ***:Clinical details,such as the recurrence site,timing,and contents of therapies for recurrence,and the prognosis,were examined in 14 patients who underwent surgical resection for pulmonary recurrence that developed after curative ***:The median disease-free interval after esophagectomy was 17.2 *** patients underwent pulmonary resection two times,and in one patient,three *** pulmonary resections were performed when other extra-pulmonary recurrences had been controlled,and R0 resection was *** and/or radiotherapy were additionally performed for pulmonary metastasis in 13 *** median survival time after initial pulmonary resection was 45.5 months,and the 1-,3-,and 5-year overall survival rates were 93%,68%,and 43%,*** 5-year overall survival rate after initial pulmonary resection was 13%in patients with Stage III or IV esophageal cancer and 100%in those with Stage I or II disease(P=0.010).The rate was 56%in patients with tumors<20 mm in size,while all 5 patients with lesions≥20 mm in size died within 3 years(P=0.005).Conclusion:Surgical resection along with systemic therapy is a promising treatment strategy for pulmonary recurrence after curative esophagectomy when it is solitary and *** factors,such as the esophageal cancer stage and the size of the pulmonary metastasis,are useful for deciding on the surgical indication.

Steroid-sparing strategies in the management of ulcerative colitis:Efficacy of leukocytapheresis

Active ulcerative colitis （UC） is frequently associated with infiltration of a large number of leukocytes into the bowel mucosa. Leukocytapheresis is a novel nonphar- macologic approach for active UC, in which leukocytes are mechanically removed from the circulatory system. Current data indicate that leukocytapheresis is effica- cious in improving response and remission rates with excellent tolerability and safety in patients with UC. Corticosteroid therapy remains a mainstay in the treat- ment of active UC, however, long-term, high doses of corticosteroids usually produce predictable and po- tentially serious side effects. If leukocytapheresis can spare patients from exposure to corticosteroids, the risk of steroid-induced adverse events should be mini- mized. This may be of great benefit to patients because severe side effects of steroids seriously impair health- related quality of life. In this article, we reviewed cur- rent evidence on whether leukocytapheresis can avoid or reduce the use of corticosteroids in the manage- ment of patients with UC. Several studies have shown that leukocytapheresis was effective for steroid-nafve patients with active UC. Furthermore, both short-term and long-term studies have demonstrated the steroid- sparing effects of leukocytapheresis therapy in patients with UC. Although the evidence level is not striking, theavailable data suggest that leukocytapheresis can avoid or reduce the use of corticosteroids in the management of UC. Large, well-designed clinical trials are necessary to more accurately evaluate the steroid-sparing effects of leukocytapheresis in the management of UC.

Stent placement is effective on both postoperative hepatic arterial pseudoaneurysm and subsequent portal vein stricture:A case report

To treat postoperative bleeding after hepato-pancreato-biliary surgery, interventional radiology has become essential. We report a case of coincidental pseudoan-eurysm and jejunal varices that were both successfully treated by stent-grafts. After a pancreaticoduodenec-tomy, the patient developed a pseudoaneurysm in the hepatic artery and a stenosis in its periphery. After establishing hepatic arterial flow by placing stent-grafts over both the pseudoaneurysm and the stenosis, the pseudoaneurysm was embolized with microcoils. Nine months later, the patient developed jejunal varices caused by a severe stricture in the main trunk of the portal vein. Percutaneous transhepatic portography was performed and stent-grafts were placed over the stenotic segment. A venoplasty using stent-grafts nor-malized the portal blood flow and the jejunal varices vanished. Although stenosis occurred due to scarred tissues from leakage after pancreaticoduodenectomy, stent-grafts were useful for managing jejunal bleeding post-operatively.

The effect of the 18R-LPSO phase on the fatigue behavior of extruded Mg/LPSO two-phase alloy through a comparative experimental-numerical study

The fatigue behavior of four extruded Mg-Y-Zn alloys containing different volume fractions of long-period stacking ordered(LPSO)grains was investigated through a comparative study combining experiments and crystal plasticity finite element *** controlled low-cycle fatigue experiments were conducted at different strain amplitudes and revealed a limited cyclic hardening in Mg_(89)Zn_(4)Y_(7)alloy or softening in Mg_(99.2)Zn_(0.2)Y_(0.6)and Mg_(97)Zn_(1)Y_(2)alloys.A decrease in the fatigue life against the plastic strain with the increase in LPSO phase volume fraction was observed and was related the limited ductility of extruded LPSO ***-strain hysteresis curves were used to calibrate and validate a crystal plasticity model taking into account twinning and *** interaction of the different phases on the distribution of local micro-mechanical fields at the grain scale was then analyzed on synthetic microstructures under strain-controlled *** twinning activity was predicted in coarse unrecrystallized grains and tended to disappear with the increase in the LPSO phase volume ***-like facets observed in LPSO grains were related to high tensile stress,especially at the Mg/LPSO interface,due to the limited number of deformation mechanisms in LPSO crystal to accommodate out-of-basal plane *** increase of the fatigue limit with the increase in LPSO phase volume fraction was finally associated with the decreasing presence of coarse unrecrystallizedα-Mg grains due to a higher dynamic recrystallization activity during the extrusion process.

Multi-Physics CFD Simulations in Engineering

Nowadays Computational Fluid Dynamics （CFD） software is adopted as a design and analysis tool in a great number of engineering fields. We can say that single-physics CFD has been suffciently matured in the practical point of view. The main target of existing CFD software is single-phase flows such as water and air. However, many multi-physics problems exist in engineering. Most of them consist of flow and other physics, and the interactions between different physics are very important. Obviously, multi-physics phenomena are critical in devel- oping machines and processes. A multi-physics phenomenon seems to be very complex, and it is so difficult to be predicted by adding other physics to flow phenomenon. Therefore, multi-physics CFD techniques are still under research and development. This would be caused from the facts that processing speed of current computers is not fast enough for conducting a multi-physics simulation, and furthermore physical models except for flow physics have not been suitably established. Therefore, in near future, we have to develop various physical models and ef- ficient CFD techniques, in order to success multi-physics simulations in engineering. In the present paper, I will describe the present states of multi-physics CFD simulations, and then show some numerical results such as ice accretion and electro-chemical machining process of a three-dimensional compressor blade which were obtained in my laboratory. Multi-physics CFD simulations would be a key technology in near future.

Effect of End Plates on the Performence of a Wells Turbine for Wave Energy Conversion

In order to improve the performance of the Wells turbine for wave energy conversion, the effect of end plates on the turbine characteristics has been investigated experimentally by model testing under steady flow conditions. The end plate attached to the tip of the original rotor blade is slightly larger than the original blade profile. The characteristics of the Wells turbine with end plates have been compared with those of the original Wells turbine, i.e., the turbine without end plate. As a result, it has been concluded that the characteristics of the Wells turbine with end plates are superior to those of the original Wells turbine and the characteristics are dependent on the size and position of end plate. Furthermore, the effect of annular plate on the turbine performance, which encircles the turbine and is attached to the tip, was investigated as an additional experiment. However, its device was not effective in improving the turbine characteristics.

Static Stability Analysis of a Single Planar Object Grasped by *** Hand

In this study, the static stability of the grasp of a single planar object is analyzed using the potential energy method. In previous papers, we considered cases in which individual fingers were replaced by a multidimensional translational spring model, in which each finger is constructed with prismatic joints. Human hands and the most developed mechanical hands are constructed with revolute joints. In this paper, the effects of fingertip rotation and a revolute joint spring model are investigated. A grasp stiffness matrix is analytically derived by considering not only frictional rolling contact but also frictionless sliding contact. The difl＇erence between the frictional stiffness matrix and the frictionless one is analytically obtained. The effect of local curvature at contact points is analytically derived. The grasp displacement directions affected by the change in curvature and the contact condition are also obtained. The derived stiffness matrix of the revolute joint model is compared with that of the prismatic joint model, and then the stiffness relation is clarified. The gravity effect of the object is also considered. The effectiveness of our method is demonstrated through numerical examples. The stability is evaluated by the eigenvalues of the grasp stiffness matrix, and the grasp displacement direction is obtained by the corresponding eigenvectors. The effect of joint angle is also discussed.

This paper explores the possibilities of very simple analysis on derivation of spiral regions for a single segment of cubic function matching positional,tangential,and curvature end *** are curves of monotone curvature with constant sign and have the potential advantage that the minimum and maximum curvature exists at their end ***,spirals are free from singularities,inflection points,and local curvature *** properties make the study of spiral segments an interesting problem both in practical and aesthetic applications,like highway or railway designing or the path planning of non-holonomic mobile *** main contribution is to simplify the procedure of existence methods while keeping it stable and providing flexile constraints for easy applications of spiral segments.
