Burden of chronic illness and associated disabilities in Bangladesh:Evidence from the Household Income and Expenditure Survey

OBJECTIVE:The purpose of this study was to investigate the distribution of chronic illness and associated disability, out-of-pocket payment (OOPP), and other related factors using survey data from Bangladesh.;METHODS:This study analyzed Bangladesh Household Income and Expenditure Survey data that include socio-economic and demographic data, such as consumption, expenditures, and assets, along with information regarding chronic illness and disability. Multiple linear regression models were used to identify factors significantly associated with OOPP. Furthermore, a binary Logistic regression model was employed to assess the association of the explanatory variables with disability status.;RESULTS:A higher prevalence of chronic illness was found for those with chronic gastritis (18.70%), and 41.92% of the population had at least one side disability. The average OOPP healthcare expenditure for chronic illness was estimated to be US$7.59. Higher OOPP was found among the upper 2 wealth quintiles. Overall OOPP health expenditure was significantly higher among individuals with an associated disability ( < 0.001). The likelihood of having an associated disability was higher among those individuals with a lower education level ( = 2.36, 95% : 1.95-4.06), those who not earning an income ( = 2.85, 95% : 2.53-3.21), those who did not seek care ( = 1.73, 95% : 1.57-1.90), those who sought care from a pharmacy ( = 8.91, 95% : 7.38-10.74), and those in the lowest wealth quintile ( = 7.21, 95% : 6.41-8.12).;CONCLUSIONS:The high OOPP illustrates the necessity of financial risk protection for the population at low socio-economic status. Therefore, we recommend that the government strengthen the healthcare system with appropriate support directed to the rural and elderly populations.

Introduction of Zika virus in Bangladesh:An impending public health threat

Objective: To explore the presence of Zika virus(ZIKV) in Bangladesh and to understand the associated risk ***: A retrospective sero-surveillance was performed on stored serum samples of dengue surveillance conducted from 2013 to 2016. Real time RT-PCR was performed on randomly selected acute serum samples to detect the Zika virus nucleic ***: Of 200 samples screened, one was found positive for ZIKV by real time RTPCR and further confirmed by genome sequencing. The case was a 65 years old male from a metropolitan city of Bangladesh who had no history of travel outside *** analysis of partial E gene sequences from Bangladeshi isolates demonstrated a close relationship with ZIKV from Brazil and current South American strains clustering within a monophyletic clade distinct from African ***: Presence of ZIKV raises serious public health concerns in Bangladesh owing to its association with congenital anomalies/neurological-manifestations. We,therefore, recommend every suspected viral fever patient, particularly pregnant women be screened for ZIKV infection to rule out yet another emerging infection in Bangladesh.

The association between drinking water salinity and hypertension in coastal Bangladesh

Aims:This study aims to explore the association between drinking water salinity and hypertension in three coastal sub-districts of ***:The study uses complete data on 6,296 individuals extracted from the latest Bangladesh Poverty and Groundwater Salinity Survey and a mixed-effects logistic regression model as the analytical ***:Mixed-effects logistic regression analysis shows a significant association of medium or higherlevel salinity with hypertension(adjusted odds ratio[AOR]1.650,95%confidence interval[CI]:1.101—2.473).Other variables significantly associated with hypertension are age,sex,education status,water source,and geographical location.A sizable proportion of the total individual-level variance in the probability of being hypertensive was at household-level(20%)and cluster-level(8%).Conclusion:The findings from this study suggest that greater salinity in potable water common in coastal areas in Bangladesh is associated with increased risk of *** study refrains from asserting causality but seeks to stimulate public health and policy interventions to address the increased risk.

Integrated physiological and molecular approaches to improvement of abiotic stress tolerance in two pulse crops of the semi-arid tropics

Chickpea(Cicer arietinum L.) and pigeonpea [Cajanus cajan L.(Millsp.)] play an important role in mitigating protein malnutrition for millions of poor vegetarians living in regions of the semi-arid tropics. Abiotic stresses such as excess and limited soil moisture(water-logging and drought), heat and chilling(high and low temperature stresses), soil salinity, and acidity are major yield constraints, as these two crops are grown mostly under rainfed conditions in risk-prone marginal and degraded lands with few or no inputs. Losses due to such stresses vary from 30% to 100% depending on their severity. The literature abounds in basic information concerning screening techniques, physiological mechanisms, and genetics of traits associated with resistance/tolerance to abiotic stresses in these two crops. However, the final outcome in terms of resistant/tolerant varieties has been far from satisfactory. This situation calls for improving selection efficiency through precise phenotyping and genotyping under high-throughput controlled conditions using modern tools of genomics. In this review, we suggest that an integrated approach combining advances from genetics, physiology, and biotechnology needs to be used for higher precision and efficiency of breeding programs aimed at improving abiotic stress tolerance in both chickpea and pigeonpea.

Mathematical Analysis of the Transmission Dynamics of Tuberculosis

We develop a dynamical model to understand the underlying dynamics of TUBERCULOSIS infection at population level. The model, which integrates the treatment of individuals, the infections of latent and recovery individuals, is rigorously analyzed to acquire insight into its dynamical features. The phenomenon resulted due to the exogenous infection of TUBERCULOSIS disease. The mathematical analysis reveals that the model exhibits a backward bifurcation when TB treatment remains of infected class. It is shown that, in the absence of treatment, the model has a disease-free equilibrium (DEF) which is globally asymptotically stable (GAS) and the associated reproduction threshold is less than unity. Further, the model has a unique endemic equilibrium (EEP), for a special case, whenever the associated reproduction threshold quantity exceeds unity. For a special case, the EEP is GAS using the central manifold theorem of Castillo-Chavez.

Fast Track Surgery and Its Outcome in Colorectal Surgery in a Tertiary Care Hospital

Background: Fast track surgery is an evidence-based multidisciplinary approach. The underlying principle is to enable patients to recover from surgery and leave the hospital sooner by minimizing the stress responses on the body during surgery. Our aim was to compare the outcome of fast-track protocol and conventional methods in colorectal surgery. Method: It was an observational cross-sectional study carried out at the Department of Surgery in different tertiary level hospitals, Dhaka Bangladesh during the period January 2014 to December 2017. Among this population, 50 patients were placed in the fast-track program (Group A) from January 2014 to December 2015 and 50 patients were in the conventional method (Group B), from January 2016 to December 2017. The fast-track patients were selected after receiving ethical approval from the Bangladesh College of Physician & Surgeons. Data analysis was done using the statistical package for social science (SPSS) for windows version 20. Results: The average age of the patients was 45.24 ± 16.65 years (range: 11 - 70 years) in the fast track group (Group A) and 43.24 ± 17.76 years in the conventional method (Group B). The majority were female between two groups, with 56% in group A and 52% in group B. General and surgical complications occurred in 9 (18%) patients and 11 (22%) patients respectively in group A. On the other hand in group B general and surgical complications occurred in 10 (20%) patients and 13 (26%) patients respectively. The average hospital stay was 9.24 ± 5.99 days in group A and the average hospital stay was 10.10 ± 6.04 days in group B. Conclusion: Making the decision to adopt fast-track surgery will challenge current traditional practice for all members of the multidisciplinary team across the whole local health community.

A cross sectional study on antibiotic resistance pattern of Salmonella typhi clinical isolates from Bangladesh
