Dear Editor,Monkeypox is an infectious disease that is endemic in a dozen of African *** imported cases have been also reported outside of Africa in the past[1].Since early May 2022,monkeypox infections including human-to-human transmission,were reported in a multi-country outbreak in non-endemic countries and declared Public Health Emergency of International Concern(PHEIC)by the World Health Organization(WHO)in July 2022[2].As of 20 September 2022,a total of at least 62,798 human cases of monkeypox with 20 deaths have been confirmed in 115 countries in five WHO regions[3].

Background: Resistance to anti-thyroid drugs (ATDs) is a rare entity recently described. We report two African observations in the treatment of Graves’ disease. Case 1: A 19-year-old Senegalese woman presented on admission with thyrotoxicosis syndrome associated with diffuse goitre and Grave’s orbitopathy. TSH levels were low (0.005 mIU/ml;N = 0.27 - 4.20) and fT4 elevated (60 pmol/L;N = 12 - 22]. Combination therapy with propranolol (40 mg/day) and carbimazole (starting dose of 45 mg/day and increased to 60 mg/day) was initiated. In view of the persistence of symptoms despite good therapeutic compliance, carbimazole was replaced by methimazole with an initial starting dose of 40 mg/day, followed by 60 mg/day. Despite the change in therapy, clinical symptoms of thyrotoxicosis persisted, and fT4 levels remained elevated. The patient was diagnosed with resistance to ATDs in Graves’ disease. Total thyroidectomy following 10 days of preoperative preparation with 1% Lugol’s solution was performed successfully. Introduction: Cystic lymphangiomas are rare benign malformative tumors of the lymphatic system of obscure etiopathogenesis. The cervico-facial location remains the most common (75%). Although benign, these tumors remain potentially fatal, due to possible compression of the upper aero-digestive tract. The aim of this work is to study the epidemiological, diagnostic and therapeutic characteristics of cervico-mandibular congenital cystic lymphangiomas in the pediatric surgery department of the Donka National Hospital (HND) Conakry. Patients and methods: This is a retrospective and descriptive study of 13 files lasting 7 years from January 2015 to December 31, 2021. The files of children whose age is less than or equal to 15 years operated on cervical tumor with histological evidence of cystic lymphangioma were retained. The data were analyzed using SPSS statistical software 21 and anonymously. Results: The incidence of this study was 1.86 cases per year and a sex ratio of 0.62 in favor of girls. The average age was 8 months 19 days. In the antecedents, we only find poorly monitored pregnancies. The average size of the tumors was 11.85 cm. Cervical ultrasound and standard x-ray of the cervical mass were the only examinations performed. Total surgical excision of the cervical tumor was performed in all patients. The mass was polycystic on exploration. The histological examination of the surgical specimens was in favor of a cystic lymphangioma. The surgical consequences were simple in 11 patients (84.62%) and complicated by parietal suppuration in 2 cases (15.38%). There were no cases of recurrence after one year of follow-up. Conclusion: Cervico-mandibular cystic lymphangiomas are the most frequent locations of congenital lymphangiomas in children. Their severity is linked to the risk of compression of the aero-digestive tracts. Their diagnosis must be confirmed by the histology of the surgical specimen. Climatological Analysis of Aerosols Optical Properties by Airborne Sensors and in Situ Measurements in West Africa: Case of the Sahelian Zone

This paper deals with the climatology of aerosols in West Africa based on satellite and in situ measurements between 2001 and 2016 and covers four sites in the Sahelian zone. There are indeed Banizoumbou (13.541°N, 02.665°E), Cinzana (13.278°N, 05.934°W), Dakar (14.394°N, 16.959°W) and Ouagadougou (12.20°N, 1.40°W) located respectively in Niger, Mali, Senegal and Burkina Faso. Thus, an intercomparison between the satellite observations and the in situ measurements shows a good correlation between MODIS and AERONET with a correlation coefficient R = 0.86 at Cinzana, R = 0.85 at Banizounbou, R = 0.84 at Ouagadougou and a low correlation coefficient R = 0.66 calculated on the Dakar site. Like MODIS, SeaWiFS shows a very good correspondence with measurements of the ground photometer especially for Banizoumbou (R = 0.89), Cinzana (R = 0.88) and Dakar (R = 0.75) followed by a low correlation coefficient calculated on the Ouagadougou site (R = 0.64). The performance of these airborne sensors is also corroborated by the calculation of root mean square error (RMSE) and the mean absolute error (MAE). Following this validation, a climatological analysis based on aerosol optical depth (AOD) shows the seasonality of aerosols in West Africa strongly influenced by the climate dynamics illustrated by the MERRA model reanalysis. This seasonal spatial distribution of aerosols justifies the temporal variability of the particles observed at the different sites in the Sahel. In addition, a combined analysis of AOD and Angstrom coefficient indicates the aerosol period in the Sahel in spring (March-April-May) and summer (June-July-August). However, these aerosols are strongly dominated by desert dust whose main sources are located north in the Sahara and Sahel.

Introduction: Ocular inflammation is a frequently encountered entity in ophthalmology. Depending on the inflamed part of the eye, different forms of ocular inflammation are distinguished. The challenge for the clinician will be to differentiate a benign pathology from a serious pathology that threatens visual function and requires urgent management. The aim is to make an epidemiological analysis of inflammatory eye diseases at the CHU-BSS of Kati. Materials and Methods: The study was carried out at the CHU-BSS of Kati between January 1 and December 31, 2023 and involved 1159 patients with various inflammatory eye diseases. Results: During the study period, 1159 cases of ocular inflammation were diagnosed out of 2111 new consultations, i.e. an incidence of 54.90%. Patients aged between 0 and 20 years were more affected with 41%. Women were more affected with 63.7%. The predominant symptom was pruritus with 57.8%. The main pathologies diagnosed were dominated by allergic conjunctivitis, bacterial conjunctivitis, with respectively 57.9% and 23.4%. Discussion: The results obtained reflect the findings made in the current practice of ophthalmology. Indeed, inflammatory diseases of the ocular surface, generally benign, are more frequent than intraocular inflammations which can jeopardize the functional or even anatomical prognosis of the affected eye. Conclusion: Endoocular inflammations are less frequent. Despite the possibility of functional complications, a rigorous diagnosis and a well-conducted medical treatment overcome most inflammatory ocular diseases.

Objective: Evaluate the immunological status to hepatitis B virus of Senegalese pregnant women by screening HBs antigen. Material and methods: The selection criteria of women were presence at the laboratory for biological exams of pregnancy follow-up. All volunteers for the study were screened for HBs antigen (HBs Ag). Investigation of chronic hepatitis B markers (HBe Ag, anti-HBe, viral quantification) was performed in HBs Ag positive participants. The concentration of anti-HBs antibodies was assessed in HBs Ag negative women. Results: One hundred and fifteen (115) pregnant women were included in the study from July to October 2014. The mean age was 29 ± 6 years, ranging from 16 to 47. The seroprevalence of HBs Ag was 12% and the majority of women (90%) were not vaccinated. Any of the 14 HBs Ag-positive subjects did not express serum HBe Ag, (marker of active viral replication) and all were positive for anti-HBe antibodies. Their viral load (HBV DNA) was undetectable and serum transaminases were normal. The anti-HBs antibodies titrated in HBs-Ag negative women serum revealed that only 46 had protective levels against HBV whilst 55 of them were unprotected.

Introduction: We assess the severity of the rheumatoid arthritis in a Senegalese African black population. Patients and methods: It is a retrospective study achieved in the service of Internal Medicine of Aristide Le Dantec teaching hospital of Dakar between January 2005 and December 2016 in patients suffering from rheumatoid arthritis. We specified for every patient the predictive data of severity of the rheumatoid arthritis. Results: Four hundred and three patients have been gathered (39 men and 364 women), with the mean age of 45.8 years. An active tobacco addiction was noticed in 10 patients. The diagnostic delay was on average of 72 months. Characteristic articular deformations were noticed in 215 patients (53.3%). They were correlated to male (p = 0.038), to age (p = 0.001) and to the activity of the rheumatoid arthritis (p = 0.0445). Systemic manifestations have been observed in 213 cases (52.9%), particularly anemia (50.8%). They were correlated to the anti-CCP antibodies (p = 0.047). The ESR was increased at the first hour in 84.4% of cases (median: 43 mm;extreme: 1and 160). CRP was elevated in 63.71% of cases (median of 12 mg/l;extreme: 1 and 384). The rheumatoid factor was positive in 57.6% of the cases. The anti-CCP antibodies were present in 89.2% of the cases. Articular erosions were objectified in all cases. A DAS 28 superior to 5.1 was noticed in 71% of cases. Conclusion: The rheumatoid arthritis was severe in our study.
