Outcome of Patients with Placenta Accreta at El Shatby Maternity University Hospital

Objective: The aim of this study was to determine the incidence, risk factors, and outcomes of management of patients with placenta accreta. Background Placenta accreta occurs when the placental implantation is abnormal. The marked increase in incidence has been attributed to the increasing prevalence of cesarean delivery in recent years. The most common theory is defective decidualization. The most important risk factor for placenta accreta is placenta previa after a prior cesarean delivery. The first clinical manifestation of placenta accreta is usually profuse, life-threatening hemorrhage. The recommended management of suspected placenta accreta is planned preterm cesarean hysterectomy with the placenta left in situ. Patients and methods: It’s a study of all cases of placenta accreta at El-Shatby Maternity University Hospital starting from 1/4/2016 till 1/10/2016. Selection of the cases will only be dependent upon their pregnancy gestational age above 28 weeks of gestation. Results: The incidence of placenta accreta was 1/75 cesarean deliveries. The ultrasonography and doppler had a false negative rate of 54.6% and a sensitivity of 45.2% in diagnosis of placenta accreta. The rate of blood transfusion was 79.6%. Uterine preserving procedures performed in 66%. Cesarean hysterectomy performed in 34%. Intensive care unit admission occurred in 27.3%. The mean gestational age at delivery was 33.8 ± 4.6 weeks’ gestation. 31.8% admitted to the neonatal intensive care unit. Conclusion: The incidence of placenta accreta increased due to the increasing rate of cesarean deliveries, prenatal diagnosis of placenta accreta is paramount, as most women are asymptomatic. Prenatal diagnosis allows time for a multidisciplinary team to make delivery plans, which will help decrease surgical complications.

New Pneumatic System for Tidal Energy Conversion

In this work, a new device for tidal energy conversion is presented. The main purpose of this research is to investigate the energy conversion of tidal energy into electrical one by building a small scale prototype using a pneumatic system representing the energy conversion device. The tidal energy conversion device consists of a concrete in cylindrical shape, a moving base that moves inside the concrete cylindrical body and two single acting pistons connected to a power turbine. The system specifications that mainly affect the amount of energy are the spring stiffness and hose diameter. It was found that there is a possibility to convert the tidal energy into electrical energy using the designed prototype. The maximum amount of electricity generated using the proposed prototype was about 5 Volts.

In this paper, lab scale production carried out of calcium carbonate in 400 mL open cylindrical beaker reactor following fuzzy logic approach is reported. 10 grams of Calcium hydroxide is mixed in 250 mL deionized water. Continuous jet supply of carbon dioxide is maintained at controlled flow rate. Reaction histories are noted for different reaction temperatures. Continuous constant magnetic stirring is applied to maintain homogeneity. The data obtained is fuzzified by constructing universe of discourse of temperature, reaction time, and amounts of reactants with reaction conversion. Rule based model is tabulated and results show that fuzzy logic approach is promising to set on data to plan and scale up the process. It is also found that a jump can not be made at this time with few studies of fuzzy logic applications to physiochemical processes unless otherwise amassing and storing up plentiful deduced explorations.

Influence of Boron Additions on Mechanical Properties of Carbon Steel

This work aims at the development of carbon steel AISI 1536 through the microalloying addition of boron. Three grades of this steel with different content of boron up to 0.0055% were melted in 100 kg induction furnace. The pro- duced steels were hardened at 960°C for 30 min., followed by tempering at different temperatures and durations. All hardened steels have martensite phase as illustrated with microstructures and X-ray diffraction. Hardness of all tem- pered steel samples was measured to calculate the activation energies of carbon migration through martensite phase. The results indicated that the activation energies of carbon migration through martensite phase decreases with the in- crease of boron content due to its positive effect on the crystallinity of martensite phase. Also, the results showed that the addition of boron up to 0.0023% can improve the steel properties at the lowest temperature and tempered time.

Effect of Titanium Addition on Behavior of Medium Carbon Steel

This work aims at investigating the influence of titanium addition on behavior of medium carbon steel. Three types of medium carbon steel with different titanium content and one reference steel titanium free were produced in 100 kg induction furnace. Titanium addition was increased up to 0.230%. The produced steels were forged at start temperature 1150°C. Forging process was finished at temperatures 900°C, 975°C, and 1050°C. Microstructure examination and hardness measurement were carried out for forged steels. Mechanical properties and impact measurements were carried out for quenched tempered steels. Ti addition was found to have significant influence on refinement of grains and increase of ferrite/pearlite ratio. It was also, observed that grain size decreases as finishing temperature of forging process decreases. Both Ti addition and lowering finishing forging temperature have positive effect on hardness. In addition, results indicated that addition of titanium has significant effect on the mechanical properties and toughness.

The linear seat suspension is considered due to the low cost consideration therefore, the optimal linear seat suspension design method can be used for this purpose. In this paper, the design of a passive vehicle seat suspension system was handled in the framework of linear optimization. The variance of the dynamic load resulting from the vibrating vehicle operating at a constant speed was used as the performance measure of a suspension system. Using 4-DOF human body model developed by Abbas et al., with linear seat suspension and coupled with half car model. A genetic algorithm is applied to solve the linear optimization problem. The optimal design parameters of the seat suspension systems obtained are kse = 3 012.5 N/m and cse = 1 210.4 N.s/m, respectively.

Synthesis, Molecular Spectroscopy, Computational, Thermal Analysis and Biological Activity of Some Orotic Acid Complexes

Binary orotic acid metal complexes of Fe(III), Co(II), Ni(II), Cu(II), Zn(II), Cd(II), Hg(II), and two mixed metals complexes of (Co(II), Ni(II)) and (Ni(II), Cu(II)) were synthesized and characterized by elemental analysis, IR, electronic spectra, magnetic susceptibility, and ESR spectra. The Analysis proved that the ligand has different coordination modes and the complexes were of octahedral, tetrahedral, and trigonal bipyramidal geometries. Molecular modeling techniques and quantum chemical methods have been performed for orotic acid to calculate charges, bond lengths, bond angles, dihedral angles, electronegativity (χ), chemical potential (μ), global hardness (η), softness (σ) and the electrophilicity index (ω). The thermal decomposition of the complexes was monitored by TGA, DTA, and DSC techniques under the N2 atmosphere. The thermal decomposition mechanisms of the complexes were suggested. The biological activity of orotic acid and some of the complexes are tested against antibacterial and antifungal organisms.

Objective: To evaluate the antidiabetic and antioxidant ef ects of aqueous ethanolic extract of Caesalpinia ferrea(C. ferrea) leaf in normal and streptozotocin(STZ) induced diabetic ***: Male Sprague-Dawley rats divided into 6 groups of 6 rats each were assigned into diabetic and non-diabetic groups. Diabetes was induced in rats by single intraperitoneal administration of STZ(65 mg/kg body weight). C. ferrea extract at the doses of 250 and 500 mg/kg body weight was orally administered to both diabetic and non-diabetic animals for a period of 30 days. After completion of experimental duration serum, liver and pancreas were used for evaluating biochemical and histopathological ***: Oral administration of C. ferrea leaf extract significantly reduced elevated serum glucose, α-amylase, liver function levels and signii cantly increased serum insulin, total protein and body weight as well as improved lipid proi le due to diabetes. Furthermore, the treatment resulted in a marked increase in glutathione peroxidase, superoxide dismutase, catalase and reduced glutathione, and diminished levels of lipid peroxidation in liver and pancreas of diabetic rats. Histopathological studies demonstrated the reduction in the pancreas and liver damage and coni rmed the biochemical i ***: From the present study, it can be concluded that the C. ferrea leaf extract ef ectively improved hyperglycaemia while inhibiting the progression of oxidative stress in STZ-induced diabetic rats. Hence, it can be used in the management of diabetes mellitus.
