Background and Objectives: The indication for treatment in HBsAg-positive patients with low viral load and normal transaminases requires an assessment of fibrosis. In resource-limited settings, free hepatic fibrosis evaluation tests can aid in therapeutic decision-making. Our study aims to demonstrate the utility of assessing hepatic fibrosis using non-invasive markers (APRI and FIB-4) in patients with chronic B viral hepatitis without cytolytic activity and low viral replication in our context. Patients and Methods: This is a retrospective cross-sectional study conducted between January 2018 and December 2021 at the University Hospital Center of Bouaké. Included were all patients aged ≥18 with normal transaminases (Results: Our study included 241 patients, with a mean age of 36.19 years (±10.52 years) and a male predominance of 52%. The mean FibroScan® value was 6.44 ± 2.3 kPa, and 68 patients (28.22%) had fibrosis >7 kPa. To exclude significant fibrosis (FS Conclusion: A significant proportion of HBV-infected patients with normal ALT and low viral load have active liver disease. Both FIB-4 and APRI biological scores are useful in identifying individuals without significant fibrosis with a good negative predictive value (>50%).

Introduction: Pericarditis is an inflammation of the pericardium with or without pericardial fluid effusion. Its prevalence is difficult to determine given the many forms that are not symptomatic. In Africa, its prevalence was 6.3% in Gabon in 2020 and 7.2% in Mali in 2022. In Europe, an Italian study estimates the incidence of acute pericarditis at 27.7 cases per 100,000 people per year. In another study conducted in Finland over a period of 9 years, the incidence of pericarditis requiring hospitalisation was 3.32 cases per 100,000 people per year. The aim of our study was to describe the clinical and paraclinical characteristics of pericarditis observed in the cardiology department of the regional hospital in Mali. Methodology: This was a single centre cross-sectional study from 30 January 2018 to 30 June 2020 in the cardiology department of the Ségou regional hospital. All consenting patients, regardless of age or sex hospitalised in the department for pericarditis confirmed on cardiac ultrasound were included. Data were collected using an individual patient follow-up form recording sociodemographic, clinical, biological, electrocardiographic and echocardiographic data, as well as the course of the disease. Results: Out of 879 patients hospitalized, the hospital frequency was 7.28%. Females predominated, with a sex ratio of 0.42. More than half the patients were aged 45 or younger (59.4%). The mean age of patients was 41.8 ± 18.1 years. Cardiovascular risk factors were dominated by hypertension and smoking (46.9% and 12.5% respectively). The reasons for consultation were dyspnoea (84.3%), chest pain (54.7%), cough (71.9%) and fever (34.4%). Physical signs included muffled heart sounds (76.6%), tachycardia (70.3%), pericardial friction (17.2%) and signs of peripheral stasis in 53.1% of cases. Introduction: High blood pressure is defined as blood pressure greater than or equal to 140 mm Hg for systolic and or 90 mm Hg for diastolic. It constitutes a major public health problem, the leading chronic disease in the world. The objective was to determine the prevalence, treatment rate and control of hypertension. Methods: This was a cross-sectional and descriptive study which took place over a period of 6 months in a hospital environment and in the general population. Results: Of the 1000 participants, 637 had hypertension, giving a prevalence of 63.70% with a female predominance. Thirty-three percent (33%) were unaware of their high blood pressure. The age group 60 and more was the most represented (44%). A proportion of 33 and 23.20 were overweight and obese participants, respectively. Male subjects were more overweight than female, unlike obesity which was more common among female subjects. Sixty-two percent (62%) of hypertensives were treated, of whom 44% were non-compliant. The excessively high cost and consumption of medications as needed were the main factors in therapeutic non-compliance. Twenty-two percent (22%) of all hypertensive patients and 35% of treated hypertensive patients were controlled. Women were more treated but less observant and less controlled than men. Therapeutic coverage and combination therapy rates were lower in rural areas. Hypertensives who had a high level of education were better treated and controlled than those who had no level. Conclusion: High blood pressure remains a real public health problem in Mali. It is more common in people aged 60 and over and in females. One in three hypertensives were unaware of their hypertension. The majority received antihypertensive treatments, but only a minority of them had their hypertension controlled.

True, cardiac myxoma is a benign tumor. On the other hand, it is serious by its complications, in particular embolic. The diagnosis of acute coronary syndrome in connection with a myxoma of the left atrium is retained before the association of an emboligenic intracardiac myxoma and an acute coronary syndrome in the absence of atherosclerotic lesions and the absence of intracardiac thrombus. We report a case of left atrium myxoma complicated by acute coronary syndrome in a 50-year-old patient operated on in our department and we will review the literature.

Trauma to the external genitalia by tearing is rare. We report the case of a 30-year-old patient, admitted to the emergency room for trauma of the external genitalia by traction following a blow and intentional injury. The authors highlight the lesions that can occur during this type of trauma and insist on the systematic search for urethral and cavernous lesions. Conclusion: Trauma to the external genitalia by intentional assault and battery is rare in our context. They occur most often in a young person and are frequently associated with damage to the corpora cavernosa and urethra.

Objective: The objective is to determine the prevalence of Helicobacter pylori (Hp) infection and highlight the determinants of the infection as well as the gastric histopathological lesions associated with this infection. Methods: This is a retrospective study carried out from August 01, 2015 to December 31, 2020 in Bouaké. It included all patients with gastric and/or duodenal lesion on upper gastrointestinal endoscopy in whom gastric biopsies and histopathology results are available. Results: The study involved 510 patients (301 men and 209 women). The prevalence of Hp was 66.47% (339/510 patients). The presence of Hp was not significantly related to age and gender. Epigastralgia was the most common indication with no significant difference between the positive and negative Hp groups (65.37% vs 34.63%, p = 0.35). A significant difference was only observed for duodenal ulcers (84.31% Hp+ vs 15.69% Hp-, p = 0.004). Regarding histological lesions: Chronic and active gastritis was strongly related to the presence of Hp (p < 0.05). Intestinal metaplasia and gastric atrophy were not significantly associated with the presence of Hp. These precursor lesions of gastric cancer (metaplasia and atrophy) were, however, significantly related to chronic gastric disease with p = 0.02 and p Conclusion: The prevalence of Hp is high in Bouaké. Our study confirms the link between Hp infection and chronic and active gastritis.

Introduction: Bladder cancer is a new tissue formation most often of urothelial origin with potential for local, locoregional and distant invasion. Among its risk factors, urinary bilharzia is endemic in our study area. The aim of our study was to present the epidemiological, clinical and histopathological aspects of bladder cancer in our department. Patients and method: This was a descriptive cross-sectional study over a 12-month period from January 1 to December 31, 2019. The study took place in the urology department of the university hospital Point “G”. It included all patients hospitalized for bladder cancer. The epidemiological, clinical and histopathological characteristics have been sought and described. Results: A total of 74 patients were included in the study. Bladder cancer was the leading cause of cancer, accounting for 57.9% of all urological cancers. The mean age of the patients was 52.8 ± 16.25 years. A slight male predominance was observed with a sex-ratio of 1.2. Housewives were the most represented with 43.2% followed by farmers with 24.3%. Hematuria was the reason for consultation in 87.8% cases. The main risk factors found were urinary schistosomiasis (48.6%) and smoking (31.1%). These two factors were associated in 23.0% of cases. At diagnosis, 85.7% of patients were classified as T3 or T4 stage. Squamous cell carcinoma with 58.2% was the most common histological type followed by urothelial carcinoma in 26.0% of cases. Conclusion: Bladder cancer is very common of cancer in hospitalized patients in our department. Diagnosis is more often made at an advanced stage. The most common histological type is squamous cell carcinoma.

Arteriovenous fistulas have a substantial impact on systemic hemodynamics, however their effect on extracorporeal circulation is not well understood. We report our clinical observation on the management under extracorporeal circulation of a patient with renal insufficiency with a high-flow arteriovenous fistula. This is a 59-year-old man who was referred to us for surgical treatment of ischemic coronary artery disease in a context of anuric chronic renal failure. Hypothermia at 32°C is started from the start in CEC due to hyperflow at the level of the arteriovenous fistula. We performed two coronary artery bypasses of the marginal and IVA via the two internal thoracic arteries. The patient is hemofiltered in order to avoid hyperkalaemia and possibly avoid fluid overload related to filling per CEC. The clamping time was 71 minutes and the SCC lasted 141 minutes. There was no homologous transfusion in the operating room. It turns out that the input/output balance is zero at the end of the CEC. The postoperative course was simple.
