Therapeutic Education of Heart Failure: Prospective Study in the Cardiology Department of the Dalal Jamm National Hospital Center in Dakar

Context: Heart failure (HF) is a common pathology requiring many types of treatments, including therapeutic education, which has demonstrated a major benefit in terms of reducing re-hospitalization and mortality from HF or all other causes. Objective: The aim of this study was to evaluate the contribution of therapeutic education (TPE) in patients with heart failure. Methodology: This was a single-centre, interventional, non-randomized, before-and-after type study. It focused on hospitalized heart failure patients aged 18 and over. We evaluated the effect of TPE on therapeutic compliance, knowledge, practical attitudes, patient perceptions and quality of life. Results: Our study included 41 patients prior to therapeutic education: 17 men (41%) and 24 women (59%). The mean age was 54.7 ± 16.2 years. After the TPE sessions we evaluated 32 patients: The patients with good compliance increased from 75% to 90.63%, and patients with a low level of compliance (90.6%) decreased to 34.4%. An increase in the number of patients who had an attitude adapted to not exceeding the recommended amount of salt (6 before TPE and 22 after TPE), as well as in the number of patients who had an attitude adapted to physical activity (12 before TPE and 16 after TPE). There was also an increase in the number of patients who were confident about the outcome of their disease (12 before TPE and 16 after TPE), and a decrease in those who were hopeless (8 before TPE and 5 after TPE), an improvement in the quality of life score (45.03 before TPE and 15.78 after TPE). Conclusion: This study shows the beneficial effect of therapeutic education in patients hospitalized for heart failure in improving their level of knowledge, compliance with treatment, lifestyle habits and quality of life.

Vaccination against COVID-19 is the most recognised means of containing the pandemic. Vaccines are not without side effects, particularly vascular thrombosis. But before blaming the vaccines, a thorough assessment of thrombotic risk factors is necessary. We report a case of arterial and venous thrombosis after vaccination with AstraZeneca revealing an exaggeration of factor VIII in a 37-year-old female patient. The angioscanner showed a venous thrombosis of the right subclavian, a pulmonary embolism and the presence of a thrombus in the aorta. The biology was in favour of a high level of factor VIII. The patient was treated with an antivitamin K, and the clinical evolution was favourable.

为了满足移动电子设备和电动车对动力能源的需要,急需使用其他的材料来优化锂离子技术。硅,及其硅化合物,和过渡金属用于负极材料在过去几年吸引了大量研究人员的关注。由于这些材料的电化学活性、催化活性和结构的多样性,研究者仍集中于研究他们的制备方法和新材料合成。在第2、3、4章分别介绍了三种新型锂离子电池负极材料复合型负极纳米材料SiO2/C/alginate和Fe2O3/C/PAN/G球磨方法制备。并将所有的样品进行了元素分析,扫描电子显微镜(SEM),X射线衍射(XRD)表征等。在第二章中介绍了硅化合物的制备和电化学性能。通过水溶剂制备的纳米硅复合结构在0.1C下能达到达2491mAh/g的高放电容量。通过NMP做溶剂制备的电极也拥有高达2075mAh/g的容量和在600mAh/g循环50圈容量保持率,并且用PVDF和海藻酸钠做粘结剂制备电极的容量也达1041mAh/g。X射线衍射表明复合物具有层状分布结构。但是,充放电过程中体积的变化依然是影响复合材料的电化学性能的重要因素。在第三章中,Fe2O3/C/PAN/G复合物通过球磨在室温下使用聚丙烯腈和石墨作为碳源进行热处理。包覆后的材料在充放电过程中有利于提高电导率。活性物质利用SEM,TEM和XRD进行详细表征。测试中发现,在1000mA/g的恒电流下有稳定的可逆容量630mAh/g,首圈库伦效率为75%。另一个重要的现象,在多次循环后比容量增加到1025mAh/g。优良的电化学性能归功于活性材料的Fe203纳米粒子效应和Sucrose-PAN-G包覆层。这能有效的缓解充放电过程中体积的急剧变化,稳定主体结构,防止电极材料的团聚。使其在嵌锂和脱锂过程中保持良好的电导。循环多次容量下降的原因是电解液和内部Fe203粒子间没有有效的接触。在第四章中,通过球磨方法制备纳米级SiO2/C/alginate复合负极材料。形貌和电化学性能的研究通过调节不同比例的海藻酸钠和二氧化硅。电池在充放电过程中有很大的极化。

Background:Cholera is endemic in Guinea,having suffered consecutive outbreaks from 2004 to 2008 followed by a lull until the 2012 *** we describe the temporal-spatial and behavioural characteristics of cholera cases in Conakry during a three-year period,including the large-scale 2012 ***:We used the national and African Cholera Surveillance Network(Africhol)surveillance data collected from every cholera treatment centre in Conakry city from August 2011 to December *** prevalence of suspect and confirmed cholera cases,the case fatality ratio(CFR),and the factors associated with suspected cholera were described according to three periods:pre-epidemic(A),epidemic 2012(B)and post epidemic(C).Weekly attack rates and temporal-spatial clustering were calculated at municipality level for period *** was confirmed by culture at the cholera national reference ***:A total of 4559 suspect cases were reported:66,4437,and 66 suspect cases in periods A,B and C,*** the 204 suspect cases with culture results available,6%,60%,and 70%were confirmed in periods A,B,and C,*** 0.3%,the CFR was significantly lower in period B than in periods A(7.6%)and C(7.1%).The overall attack rate was 0.28%in period B,ranging from 0.17%to 0.31%across ***,a cluster of cases was identified in two districts in the northern part of *** 14%,rice water stools were less frequent in period A than in period B and C(78%and 84%).Dehydration(31%vs 94%and 89%)and coma(0.4%vs 3.1%and 2.9%)were lower during period B than in periods A and *** treatment of drinking water was less frequent in period A,while there were more reports of recent travel in period ***:The epidemic dynamic and the sociological description of suspect cases before,during,and after the large-scale epidemic revealed that the Vibrio cholerae was already present before the ***,it appeared that infected individuals react

Object: The aim of this study was to describe a familial dyslipidemia revealed by a corneal arch. Familial hypercholesterolemia is a rare and severe hereditary dyslipidemia with an exceptional homozygous form. He was a 43-year-old patient admitted with visual disturbance, xanthomatous papule and orange coloring at the level evolving for 5 years, no personal history of cardiovascular disease, having a family history of lipid disorders in siblings, as a cardiovascular risk factor: age over 50 and the male gender, normal cardio pulmonary examination, blood pressure at 120/80 mmhg, heart rate 78 bpm, in whom the ophthalmological examination shows a yellowish spot on the cornea (Figure 1). We report the case of familial dyslipidemia revealed by a corneal spot in Guinea.More recent studies increasingly suggest that it is much more widespread than previously thought: 1/200 to 1/400. Oral Lesions Observed in Tobacco Consumers: Socio-Demographical and Clinical Aspects in the Prefectures of Kankan and Siguiri Republic of Guinea

Introduction: Oral lesions observed in Chewing Tobacco (CT) consumers are histological alterations caused by direct contact with the tissues of the oral cavity. The objectives of this study were to determine the socio-demographic status of consumers and describe the clinical characteristics of the oral lesions observed. Material and Method: It was a descriptive cross-sectional study lasting 12 months (January 2017 to January 2018), in the prefectures of Kankan and Siguiri as a framework. Individuals who use chewing tobacco and whose oral examination revealed oral lesions were included in this study. Results: The frequency of oral lesions induced by the consumption of chewing tobacco was 90%. Dental pain was the most cited reason for using CT by respondents, i.e., 25.41%. The most concerned age group was that of 50 - 64 years or 23.81 with a male predominance of 84.13% (n = 53). A number of 61 respondents came from rural areas. Farmers were the predominant socio-professional group, i.e., 63.49% (n = 40). In 60.32% (n = 38) of respondents, the duration of CT consumption was greater than 10 years. Black villous tongues followed by neoplastic ulcerations were the most common oral lesions with 32 (50.79%) and 14 (22.22%) cases, respectively. The site of the lesion was the tongue in 47 consumers (74.61%) and in the lower vestibule in 15 users (23.80%). Among the oral diseases, mylolysis was the most common, i.e., 95.24% (n = 60). In 82.54% of cases (n = 52), Simplified Oral Hygiene Index (SOHI) was low and in 12.70% (n = 8) of respondents, SOHI was good. Conclusion: Chewing tobacco predisposes to a high risk of damage to the oral cavity and the development of potentially malignant lesions. In addition, it exerts some detrimental effects on the teeth and adjacent structures.

Introduction: High blood pressure is a public health problem in Guinea as well as everywhere in Africa. The objective of our study was to assess the epidemiological, clinical and therapeutic aspects of High blood pressure in the general medicine department of the Nzérékoré regional hospital. Methodology:This was a prospective study of a descriptive type, lasting 06 months, from July 1 to December 31, 2014. Results: We recorded 156 cases of High blood pressure among a total of 570 patients, for a hospital frequency of 27%. The 56 - 65 age group was the most affected, at 29.4%, with a predominance of men (sex ratio: 1.5). Alcoholism was the most common cardiovascular risk factor with a frequency of 51.2%, smoking and physical inactivity were found in 35.8% and 30.7% respectively;psychosocial stress was found in 24.3% of patients. The clinic was dominated by headache (100%) and dizziness (57.6%). All our patients were subjected to a healthy diet. Fixed dual therapy was the most widely used treatment regimen, i.e. 91 patients (58.3%) and included a diuretic in 37.1% of cases, a calcium channel blocker in 25% of cases and an ACE inhibitor in 24.3%. Conclusion: This study revealed a high frequency of hypertension in the general medicine department of the regional hospital of N’zérékoré.

Deep Vein Thrombosis Revealing Prostate Cancer about a Case in the Cardiology Department of the Ignace Deen National Hospital in Conakry

Introduction: Deep vein thrombosis is a frequent disease, its origin is most often multifactorial. Venous thromboembolic disease (MVTE) and cancer are two frequently entangled pathologies. Here we report the diagnosis of deep vein thrombosis that discovered prostate cancer in an 88-year-old Guinean man. On clinical examination, there was a painful and hot swelling of the right leg, an absence of sloshing of the calf, a positive sign of Homans. The digital rectal examination revealed an enlarged prostate with an irregular surface. Cardiopulmonary auscultation was normal. The electrocardiogram showed a regular sinus rhythm at 65 cycles/min, with no sign of enlarged cavities or conduction disturbance. Venous Doppler ultrasound of the lower limbs showed the presence of an extensive acute deep venous thrombosis of the right sural vein extended to the popliteal and to the homolateral deep femoral. The reino-vesico-prostatic ultrasound concluded in a heterogeneous prostatic hypertrophy with projection of a median lobe associated with a bladder of fight with an important post voiding residue evaluated at 170 ml;the rate of specific antigen of the prostate PSA was at 84.87 ng/ml. The pathology analysis made after a prostate biopsy puncture concluded with an adenocarcinoma with a Gleason score of 3. Conclusion: The association of venous thromboembolic disease (MVTE) and neoplasia is frequent. The existence of active cancer in a patient is a known risk factor for MVTE and, conversely, the discovery of a first episode of deep vein thrombosis (DVT) or pulmonary embolism (PE) may be the mode of revelation of cancer.

Clinical and Therapeutic Aspects of Temporomandibular Ankylosis at the Conakry Teaching Hospital

Introduction: Temporomandibular joint ankylosis (TMJA) is a fusion of joint surfaces by fibrous or bone tissue and the resulting limitation of mouth opening. The objectives of this study were to determine the frequency of TMJA, describe the clinical aspects and assess management. Materials and Method: It was a retrospective study that was carried out in the Department of Odontostomatology and Maxillofacial Surgery of Donka National Hospital for a period of 5 years (January 2016 to December 2020). Included were all records of inpatients and managed cases of TMJA during the study period. Socio-demographic, clinical and therapeutic variables were analyzed. Results: During the study 13 TMJA cases were collected with the frequency of 0.47%. The age group 1 - 9 was the most affected (61.54%) with extremes of 4 and 28 years. Men were the most concerned (53.87%). Restriction of mouth opening was the main reason for consultation (69.23%). The etiologies of ankylosis were dominated by infection including cellulitis of dental origin (53.85%) followed by facial trauma (30.77%). Arthroplasty was the most commonly used technique (73%), two cases of recurrence were noted (15.38%). Conclusion: TMJA affects mainly children and the etiology is dominated by cellulite of dental origin hence the interest of sensitization of the population for early management of oral diseases.

Cardiac Amyloidosis: A Case Report of Seven Patients

Introduction: Cardiac amyloidosis is a rare and under-diagnosed disease. The objective of this study was to collect cases of cardiac amyloidosis in patients hospitalized in the cardiology department of the Centre Hospitalier Intercommunal Alençon-Mamers (CHICAM). Patients and Methods: This was a retrospective descriptive study of the records of patients diagnosed with cardiac amyloidosis. The study took place in the cardiology department of the CHICAM over 12 months (from January 1, 2020 to December 31, 2020). We included the files of patients in whom the diagnosis of cardiac amyloidosis was confirmed on the basis of clinical, echocardiographic, biological, scintigraphic and cardiac MRI evidence. Results: Seven patients were included. The mean age was 86.71 years. Six men for one woman. Most of the patients were in heart failure. There was one case of periorbital ecchymosis. Troponinemia was increased in three patients, NTproBNP was always high with a mean of 1000 ng/L. Cardiac echography showed a hyperbright septum in four cases, concentric hypertrophy of the left ventricle with a mean interventricular septum in diastole of 13.85 mm. Six cases were senile transthyretin amyloidosis (TTR), one case was mutated transthyretin amyloidosis (TTRm). Two cases of death were noted. Conclusion: Cardiac amyloidosis has a poor prognosis due to delayed diagnosis.
