BACKGROUND Left bundle branch pacing(LBBP)is a novel pacing modality of cardiac resynchronization therapy(CRT)that achieves more physiologic native ventricular activation than biventricular pacing(BiVP).AIM To explore the validity of electromechanical resynchronization,clinical and echocardiographic response of *** Systematic review and Meta-analysis were conducted in accordance with the standard guidelines as mentioned in detail in the methodology *** In our analysis,the success rate of LBBP-CRT was determined to be 91.1%.LBBP CRT significantly shortened QRS duration,with significant improvement in echocardiographic parameters,including left ventricular ejection fraction,left ventricular end-diastolic diameter and left ventricular end-systolic diameter in comparison with *** A significant reduction in New York Heart Association class and B-type natriuretic peptide levels was also observed in the LBBP-CRT group vs BiVP-CRT ***,the LBBP-CRT cohort had a reduced pacing threshold at follow-up as compared to BiVP-CRT.

Hepatocellular carcinoma(HCC) is considered the fifth most prevalent cancer among all types of cancers and has the third most morbidity value. It has the most frequent duplication time and a high recurrence rate. Recently, the most unique technique used is liquid biopsies, which carry many markers;the most prominent is circulating tumor DNA(ctDNA). Varied methods are used to investigate ctDNA, including various forms of polymerase chain reaction(PCR) [emulsion PCR(ePCR), digital PCR(dPCR), and bead, emulsion, amplification, magnetic(BEAMing) PCR]. Hence ctDNA is being recognized as a potential biomarker that permits early cancer detection,treatment monitoring, and predictive data on tumor burden are subjective to therapy or surgery. Numerous ctDNA biomarkers have been investigated based on their alterations such as 1) single nucleotide variations(either insertion or deletion of a nucleotide) markers including TP53, KRAS, and CCND1;2) copy number variations which include markers such as CDK6, EFGR, MYC and BRAF;3) DNA methylation(RASSF1A, SEPT9, KMT2C and CCNA2);4) homozygous mutation includes ctDNA markers as CDKN2A, AXIN1;and 5) gain or loss of function of the genes, particularly for HCC. Various researchers have conducted many studies and gotten fruitful ***, there are some drawbacks to ctDNA namely low quantity, fragment heterogeneity, less stability, limited mutant copies and standards, and differential sensitivity. However, plenty of investigations demonstrate ctDNA's significance as a polyvalent biomarker for cancer and can be viewed as a future diagnostic, prognostic and therapeutic agent. This article overviews many conditions in genetic changes linked to the onset and development of HCC, such as dysregulated signaling pathways, somatic mutations, single-nucleotide polymorphisms, and genomic instability. Worldwide,a majority of routine endoscopic procedures are performed under some form of sedation to maximize patient ***,benzodiazepines and opioids continue to be widely ***,in recent years,Remimazolam is gaining immense popularity for procedural sedation in gastrointestinal(GI)*** is an ultra-short-acting benzodiazepine sedative which was approved by the Food and Drug Administration in July 2020 for use in procedural *** has shown a favorable pharmacokinetic and pharmacodynamic profile in terms of its non-specific metabolism by tissue esterase,volume of distribution,total body clearance,and negligible drug-drug *** also has satisfactory efficacy and has achieved high rates of successful sedation in GI ***,studies have demonstrated that the efficacy of Remimazolam is non-inferior to Propofol,which is currently a gold standard for procedural sedation in most parts of the ***,the use of Propofol is associated with hemodynamic instability and respiratory *** contrast,Remimazolam has lower incidence of these adverse effects intra-procedurally and hence,may provide a safer alternative to Propofol in procedural *** this comprehensive narrative review,highlight the pharmacologic characteristics,efficacy,and safety of Remimazolam for procedural *** also discuss the potential of Remimazolam as a suitable alternative and how it can shape the future of procedural sedation in gastroenterology.

Traversable wormhole solutions in the f(R)theories of gravity under the Karmarkar condition

This paper examines traversable wormhole models in the f(R) theories of gravity by applying the Karmarkar condition. For this purpose, we consider spherically symmetric space-time to examine the structure of wormholes. First, we investigate wormholes and their geometry using the redshift function under various conditions. Subsequently, we discuss the embedding diagram of the upper and lower universe using radial coordinates in two and three-dimensional Euclidean affine space. Three exclusive models are considered for the f(R) theories of gravity,and the radial and tangential pressures are observed. Furthermore, by taking a definite shape function, we observe the behavior of energy conditions. We determine that energy conditions are violated, and their violation is generic and represents the presence of exotic matter. According to Einstein’s field theory, the existence of wormholes is predicated on the occurrence of rare material. Hence, we conclude that our study is more realistic and stable.

A consensus meeting of national experts from all major national hepatobiliary centres in the country was held on May 26,2023,at the Pakistan Kidney and Liver Institute&Research Centre(PKLI&RC)after initial consultations with the *** Pakistan Society for the Study of Liver Diseases(PSSLD)and PKLI&RC jointly organised this *** effort was based on a comprehensive literature review to establish national practice guidelines for hilar cholangiocarcinoma(hCCA).The consensus was that hCCA is a complex disease and requires a multidisciplinary team approach to best manage these *** coordinated effort can minimise delays and give patients a chance for curative treatment and effective *** diagnostic and staging workup includes high-quality computed tomography,magnetic resonance imaging,and magnetic resonance *** cytology or biopsy utilizing endoscopic retrograde cholangiopancreatography is a mainstay for ***,histopathologic confirmation is not always required before *** ultrasound with fine needle aspiration of regional lymph nodes and positron emission tomography scan are valuable adjuncts for *** only curative treatment is the surgical resection of the biliary tree based on the Bismuth-Corlette *** patients with unresectable hCCA can be considered for liver *** chemotherapy should be offered to patients with a high risk of *** use of preoperative biliary drainage and the need for portal vein embolisation should be based on local multidisciplinary *** with acute cholangitis can be drained with endoscopic or percutaneous biliary *** chemotherapy with cisplatin and gemcitabine has shown improved survival in patients with irresectable and recurrent hCCA.

The charge state distribution of B, C, Si, Ni, Cu and Au ions on 5 MV pelletron accelerator

Stripper gas and terminal potential play a key role for the charge state distribution in a tandem pelletron accelerator. The knowledge of this distribution is important for experiments performed on tandem accelerators. The charge state distribution of B, C, Si, Ni, Cu and Au beams is measured by using Ar as stripper gas, and terminal potential is varied from 0.3 to 3.0 MV on 5UDH-2 tandem pelletron accelerator installed at the National Centre for Physics, Islamabad. The individual charge state is measured after the switching magnet at 15° in high-energy portion. It is observed that the higher charge states are stable in the range of lower and middle atomic masses of periodic table, whereas higher atomic mass(Au) shows beam current instability in higher charge states. For carbon,the charge distribution at 1.7 MV terminal potential by varying stripper gas pressure is also studied, which resulted in decreased overall transmission with good current value for higher charge states.

Deep Learning Based Classification of Wrist Cracks from X-ray Imaging

Wrist cracks are the most common sort of cracks with an excessive occurrence *** the routine detection of wrist cracks,conventional radiography(X-ray medical imaging)is used but periodically issues are presented by crack *** cracks often appear in the human arbitrary bone due to accidental injuries such as ***,many hospitals lack experienced clinicians to diagnose wrist ***,an automated system is required to reduce the burden on clinicians and identify *** this study,we have designed a novel residual network-based convolutional neural network(CNN)for the crack detection of the *** the classification of wrist cracks medical imaging,the diagnostics accuracy of the RN-21CNN model is compared with four well-known transfer learning(TL)models such as Inception V3,Vgg16,ResNet-50,and Vgg19,to assist the medical imaging technologist in identifying the cracks that occur due to wrist *** RN-21CNN model achieved an accuracy of 0.97 which is much better than its competitor`s *** results reveal that implementing a correct generalization that a computer-aided recognition system precisely designed for the assistance of clinician would limit the number of incorrect diagnoses and also saves a lot of time.
