Efficacy and safety of vedolizumab in elderly patients with inflammatory bowel disease:a matched case-control study

Background:Vedolizumab was demonstrated to be safe and effective in adults with moderately to severely active inflammatory bowel disease(IBD)in clinical ***,there are limited data regarding its efficacy and safety in elderly ***:This was a case-control study comparing the efficacy(measured by rates of mucosal healing and need for IBD surgery)and safety of vedolizumab in IBD among patients65 years of age(the elderly group)vs thosep).The two groups were matched individually on a 1:4 ratio based on gender and type of *** logistic regression was used for stratified analysis to calculate odds ratios and confidence ***:We included 25 IBD patients in the elderly group and 100 matched patients in the comparison *** patients had Crohn’s disease and 45 had ulcerative *** baseline,the groups were comparable with regard to duration of IBD,prior anti-TNF therapy,and prior IBD *** rate of mucosal healing on follow-up endoscopy was comparable between the elderly and control groups(50%vs 53%,p=0.507).Although more patients in the elderly group required IBD-related surgery while on vedolizumab,the difference did not reach statistical significance(40%vs 19%,p=0.282).Rates of vedolizumab-related adverse effects—rash,arthralgia,infections,infusion reactions,and dyspnea—were comparable between the two groups(all p>0.05).Conclusions:In a real-world setting,vedolizumab was demonstrated to have an efficacy and safety profile among elderly IBD patients that were comparable to younger controls.

Aortoesophageal fistula(AEF)is a rare cause of massive upper gastrointestinal *** aortic aneurysm,esophageal foreign body,esophageal cancer and post-surgical complications are common causes of AEF;however,AEF induced by radiation therapy is a rare phenomenon and seldom described in the *** is a catastrophic condition which requires rapid implementation of resuscitative measures,broad-spectrum antibiotics and surgical or endovascular *** endovascular aortic repair(TEVAR)is a newer and less invasive technique,which helps to achieve rapid hemostasis in patients with severe hemodynamic instability and offers advantages over conventional repair of the aorta in emergency *** initial TEVAR should be followed up with a more definitive surgical repair of the aorta and the esophagus,to lower the mortality rate and achieve better *** describe here a case of a seventyyear-old male who presented with massive upper gastrointestinal bleeding due to AEF induced by radiation therapy,and his subsequent successful initial management with TEVAR.

Experimental investigation and prediction of wear behavior of cotton fiber polyester composites

The cotton fiber reinforced polyester composites were fabricated with varying amount of graphite fillers (0,3,5 wt.％) with a hand lay-up *** tests were planned by using a response surface (Box Behnken method) design of experiments and conducted on a pin-on-disc machine (pOD) test *** effect of the weight percentage of graphite content on the dry sliding wear behavior of cotton fiber polyester composite (CFpC) was examined by considering the effect of operating parameters like load,speed,and sliding *** wear test results showed the inclusion of 5 wt.％ of graphite as fillers in CFpC increase wear resistance compared to 3 wt.％ of graphite *** graphite fillers were recommended for CFpC to increase the wear resistance of the material.A scanning electron microscope (SEM) was used to study the wear *** predict the wear behavior of the composite material,comparisons were made between the general regression technique and an artificial neural network (ANN).The conformation test results revealed the predicted wear with the ANN was acceptable when compared with the actual experimental results and the regression mathematical models.

Indigenous plant medicines for health care:treatment of Diabetes mellitus and hyperlipidemia

Medicinal plants have played an important role in treating and preventing a variety of diseases throughout the world. Metabolic syndrome had become a global epidemic, defined as a cluster of three of five criteria： insulin resistance and glucose intolerance, abdominal obesity, hypertension, low high-density cholesterol, and hypertriglyeeridemia. The current review fo- cuses on Indian medicinal plant drugs and plants used in the treatment of diabetes and hyperlipidemia. Though there are various approaches to reduce the ill-effects of diabetes and hyperlipidemia and its secondary complications, plant-based drugs are preferred due to lesser side effects and low cost. The current review focuses on twenty-three medicinal plants used in the treatment of Diabe- tes mellitus and nine medicinal plants used in the treatment of hyperlipidemia. The wealth of knowledge on medicinal plants points to a great potential for research and the discovery of new drugs to fight diseases, including diabetes and hyperlipidemia.

Quantification of phenolic compounds and antioxidant capacity of an underutilized Indian fruit:Rayan[Manilkara hexandra(Roxb.)Dubard]

The fruit of Manilkara hexandra(Roxb.)Dubard is one of the most underutilized fruits of India and in Gujarat *** is popularly known as‘Rayan’.The fruit and seed of Rayan were analysed for their total phenolic and flavonoid content,phenolic compounds and total antioxidant capacity with six different assay *** results indicated that the methanolic extract of Rayan fruit being a good source of phenolic(811.3 mg GAE/100 g fw)and flavonoid(485.56 mg RE/100 g fw)***,eleven known phenolic compounds were tentatively identified for the first time from the fruit and seed of *** LC–MS/MS analysis of fruit revealed the presence of major phenolic compounds such as gallic acid,quercetin and kaempferol,while quercetin,gallic acid and vanillic acid in *** presence of quercetin suggests health *** fruit of Rayan was also proved to be a better source of antioxidants as measured by FRAp,RpA,DppHRSA,ABTSRSA and HRSA except NORSA in comparison with that of *** current study explains that *** is a relatively good source of antioxidants such as phenols and flavonoids for diet.

An uncommon source of upper gastrointestinal bleeding:epiphrenic esophageal diverticulum

Esophageal diverticula are rare findings that have an estimated incidence of 1 per 500000 people per year,even though acute upper gastrointestinal bleeding is a relatively common medical emergency with an incidence of up to 150 per 100000 people per year and a mortality rate of 7-14%.An 83-year-old man presented with hematemesis and *** upper endoscopy revealed an esophageal diverticulum,within which was an adherent *** of the clot identified a bleeding vessel within the diverticulum;this was successfully clipped and hemostasis was *** the best of our knowledge,this is the first case report of a bleeding epiphrenic esophageal diverticulum that was successfully managed endoscopically with hemostatic clips *** rare,our case serves as a reminder that bleeding epiphrenic esophageal diverticula can present as massive upper gastrointestinal bleeding and urgent endoscopic therapy can be life-saving.

Improving intermittent androgen deprivation therapy： lessons learned from basic and translational research

Intermittent androgen deprivation therapy （IADT） is an alternative to continuous androgen deprivation therapy （ADT） in prostate cancer patients with nonmetastatic disease. ADT is associated with numerous side effects such as hot flashes, sexual dysfunction, anemia, fatigue, loss of muscle mass, osteoporosis, metabolic syndrome and premature cardiovascular disease. IADT was developed with the intention of improving the quality of life and to delay progression of prostate cancer to castration resistance. The benefits of slightly improved quality of life by IADT compared to ADT were demonstrated in multiple clinical trials. IADT was noted to be noninferior to ADT in patients with biochemical recurrence of prostate cancer but in studies performed in patients with metastatic prostate cancer, the results were inconclusive. Our recent studies suggested that the administration of 5 alpha-reductase inhibitors during the off-cycle of IADT can significantly prolong the survival of mice bearing androgen-sensitive prostate tumors when off-cycle duration was short. This review discusses the survival benefit of 5 alpha-reductase inhibition in IADT in animal models and the potential translation of this finding into clinic.

Immunohistochemical analysis of the Wnt/β-catenin signaling pathway in pancreatic neuroendocrine neoplasms

AIM: To investigate the role of the Wnt/β-catenin pathway in pancreatic neuroendocrine neoplasms(panN ENs). METHODS: Tissue microarrays containing 88 panN ENs were immunohistochemically labeled with antibodies to β-catenin, E-cadherin, adenomatous polyposis coli(ApC), chromogranin and synaptophysin. One case had only metastatic tumors resected, whereas others(n = 87) received pancreatectomy with or without partial hepatectomy. pathology slides, demographic, clinicopathologic, and follow up data were reviewed. patients' demographics, clinicopathologic features, and immunohistochemical results from 87 primary tumors were compared between patients with low stage(stage Ⅰ/Ⅱ) and high stage(stage Ⅲ/Ⅳ) tumors. In addition, correlation of immunohistochemical results from primary tumors with disease-specific survival(DSS) was evaluated. RESULTS: Strong membranous β-catenin staining in the primary tumor was observed in all 13 stage Ⅲ/Ⅳ pan NENs as compared to 47%(35/74) of stage Ⅰ/Ⅱtumors(p pan NENs as compared to 0%(0/74) of stage Ⅰ/Ⅱ tumors(p = 0.02). The case with metastasectomy only also showed nuclear β-catenin staining. Two of the three cases with nuclear β-catenin staining were familial adenomatous polyposis(FAp) patients. Lack of ApC expression was seen in 70%(57/81) of the cases, including the 3 cases with nuclear β-catenin staining. Expression of E-cadherin and ApC in primary tumor was not correlated with tumor grade, tumor stage, or disease specific survival. CONCLUSION: The Wnt/β-catenin pathway was altered in some panN ENs, but did not Impact DSS. panN ENs in FAp patients demonstrated nuclear β-catenin accumulation and loss of ApC.

Intermittent gastric outlet obstruction caused by a prolapsing antral gastric polyp

Most gastric polyps have an asymptomatic presentation and are an incidental finding on upper endoscopy. Symptomatic presentations can range from an ulcerated polyp leading to anemia and occult bleed to complete gastric outlet obstruction. We report a case of an 89-year-old woman who presented with postprandial nausea and early satiety. Her upper endoscopy revealed a 2 cm pedunculated hyperplastic polyp arising from the antrum of the stomach which was seen prolapsing into the pylorus causing intermittent gastric outlet obstruction. In the present report, we statistically analyzed 39 prolapsing gastric polyps previously reported in the English literature and demonstrate the current utility of monopolar snare polypectomy in establishing a histological diagnosis while offering simultaneous treatment. Additionally, we review the literature for the management of all hyperplastic gastric polyps in relation to advancements in digestive endoscopy.
