Outpatient insulin use in type 2 diabetes mellitus and acute respiratory distress syndrome outcomes:A retrospective cohort study

Connective tissue diseases (CTDs) are Autoimmune diseases (AIDs) characterized by the appearance of autoantibodies, which are diagnostic markers. Investigations of these autoantibodies play a major role in the management of several autoimmune diseases. The objective of this study was to describe the profile of anti-ENA antibodies according to the clinical symptoms of mixed CTDs in Conakry teaching Hospital. We performed a cross-sectional study during six months. A total of 20 patients was recruited and we measured antibodies using the ELISA technique. The mean age of our patients was 36.5 years, with a predominance of females. Cutaneous and rheumatological signs were the main clinical manifestations. SLP was the most frequent CTDs;the threshold of ENA antibodies positivity was higher in scleroderma with and SLP. Anti-ENA identification reveals the frequency of anti-SSA (83.33%), anti-U1RNP (66.66%) and anti-histone (50%) antibodies. Antinuclear antibodies (ANA) react with various components of the cell nucleus. Their detection is of major interest in the diagnosis of CTDs. Our results highlight the importance of determining the specificity of these antibodies to guide differential diagnosis.

Wavelet Neural Networks for Adaptive Equalization by Using the Orthogonal Least Square Algorithm

Equalizers are widely used in digital communication systems for corrupted or time varying channels. To overcome performance decline for noisy and nonlinear channels, many kinds of neural network models have been used in nonlinear equalization. In this paper, we propose a new nonlinear channel equalization, which is structured by wavelet neural networks. The orthogonal least square algorithm is applied to update the weighting matrix of wavelet networks to form a more compact wavelet basis unit, thus obtaining good equalization performance. The experimental results show that performance of the proposed equalizer based on wavelet networks can significantly improve the neural modeling accuracy and outperform conventional neural network equalization in signal to noise ratio and channel non-linearity.

We read with interest the study from Jolissaint et *** issued in the Annals of Surgery(1).In this single center retrospective study,the authors compared oncological outcomes in patients with pancreatic ductal adenocarcinoma(PDAC)and vascular involvement who underwent pancreatectomy(n=105)*** dose ablative radiation(A-RT,n=104).All patients received induction chemotherapy first,over a recent study period[2014-2018].While the rationale for submitting patients to pancreatectomy or A-RT was not clearly stated in the Methods section,comparison between the two groups showed that patients allocated to A-RT were frailer with higher Charlson Comorbidity indexes and Eastern Cooperative Oncology Group(ECOG)scores and harbored significantly larger tumors,more often left-sided,with more arterial involvement as compared to patients in the pancreatectomy group.

This contribution presents experimental and numerical investigations of the concept jet propulsion augmentation using bubble injection. A half-3D （D-shaped cylindrical configuration to enable optimal visualizations） divergent-convergent nozzle was designed, built, and used for extensive experiments under different air injection conditions and thrust measurement schemes. The design, optimization, and analysis were conducted using numerical simulations. The more advanced model was based on a two-way coupling between an Eulerian description of the flow field and a Lagrangian tracking of the injected bubbles using our Surface Averaged Pressure （SAP） model. The numerical results compare very favorably with nozzle experiments and both experiments and simulations validation the thrust augmentation concept. For a properly designed nozzle and air injection system, air injection produces net thrust augmentation, which increases with the rate of bubble injection. Doubling of thrust was measured for a 50% air injection rate. This beneficial effect remains at 50% after account for liquid pump additional work to overcome increased pressure by air injection.

Testicular sperm extraction is widely used in the treatment of male infertility in cases of non-obstructive azoospermia. Identifying spermatogenetic foci within the testes is critical for testicular sperm extraction. Two-photon laser scanning microscopy （TPLSM） is an autofluorescence-based microscopy technique that allows observation at a cellular level in the depth of fresh living tissues and does not require any histological processing （fixation or staining）. The wavelengths previously used have shown no phototoxicity on sperm. We used TPLSM to detect spermatogenetic foci in fresh mouse testicular parenchyma without disrupting the tunica albuginea. Fresh surgically retrieved testes were observed using TPLSM within 1 h after extraction. Contralateral testes for each animal were observed using standard histology. Using TPLSM we were able to observe and measure the diameter of seminiferous tubules through the tunica albuginea, similar to the histological control. Structures within epithelial tubules were also observed, although their nature has yet to be identified. TPLSM is a real-time microscopy technique that could detect spermatogenetic foci.

YIKI is an antimalarial phytomedicine used by a traditional healer to treat malaria in Bobo-Dioulasso. However, there is no scientific evidence to support its use by local populations. The aim of this study was to identify the medicinal practices of the healer holder of YIKI and assess the clinical evidence of its phytomedicine in the uncomplicated malaria treatment. Ethnomedical survey based on a semi-structured and open questionnaire was conducted from October to December 2019 with the healer. Malaria knowledge and diagnosis methods, patient treatment and monitoring, and recipe formulation steps were surveyed. Moreover, thick and thin blood smears were taken, haemoglobin levels and temperature of consenting patients were measured before treatment, mid-treatment and at the end of treatment. The survey revealed that the healer has a good knowledge of malaria symptoms and his diagnosis is based on observation and physical examination of patients. The healer’s malaria diagnosis was rudimentary and had accuracy problems, with only 62.79% of malaria cases confirmed by microscopy. The formulation of YIKI and its use to treat malaria follow a standard process for plant harvesting, powder quantities and posology, but do not use any reproducible parameters for dose adjustment. Forty-three patients diagnosed and treated by the healer participated in the study. Laboratory results revealed 27 Plasmodium falciparum infection cases, including 2 with parasitaemia ≥ 200,000 p/µl blood. 25 patients were selected for therapeutic evidence assessment. There was a 48% elimination of parasites, a 28% parasitaemia decrease without complete cure, with gametocytogenesis in some patients, and a 24% parasitaemia increase. Haemoglobin and temperature results suggested that YIKI was not cytotoxic and reduced fever. Encouraging preliminary results have been obtained, but in view of the low number of patients, further YIKI efficacy and toxicity studies will be necessary for patient safety.

The aims of this study were(1)to define speed and acceleration thresholds from youth match activity of soccer players and(2)to analyze the activity performed by young soccer players during a match using GPS devices and according to these *** U11 soccer players from a professional club participated in a 7-sided match being equipped with global positioning system(GPS)devices(sampling frequency 5 Hz)to measure the speed of each *** Kernel Density Esti-mate(KDE)was used to observe the occurrences of *** range of velocities was described by a distribution curve,from which the speed and acceleration thresholds for each category of movement were defined and time–motion analysis of the match was *** model with 4 Gaussian laws was the best when using the Akaike Information Criterion(AIC).In this study,the thresholds defined for each category of movement were:standing(<0.1 km/h),walking(0.1–5.6 km/h),slow running(5.7–7.7 km/h),fast running(7.8–12.7 km/h)and sprint(≥12.8 km/h).Acceleration thresholds were calculated to define movement considered as a sprint for each category of movement:standing(0.3 m/s^(2)),walking(0.66 m/s^(2)),slow running(1.01 m/s^(2))or fast running(0.97 m/s^(2)).All the acceleration thresholds were significantly different from each other(P<0.001)with large effect sizes,excepting the comparison between slow and fast running(P=0.41)with a small effect *** U-11 soccer players,time–motion analysis may be performed according to five categories of *** study has also defined sprint and acceleration thresholds for this category of age.
