Unusual histopathological findings in appendectomy specimens:A retrospective analysis and literature review

AIM: To document unusual findings in appendectomy specimens. METHODS: The clinicopathological data of 5262 patients who underwent appendectomies for presumed acute appendicitis from January 2006 to October 2010 were reviewed retrospectively. Appendectomies performed as incidental procedures during some other operation were excluded. We focused on 54 patients who had unusual findings in their appendectomy specimens. We conducted a literature review via the PubMed and Google Scholar databases of English language studies published between 2000 and 2010 on unusual findings in appendectomy specimens. RESULTS: Unusual findings were determined in 54 (1%) cases by histopathology. Thirty were male and 24 were female with ages ranging from 15 to 84 years (median, 32.2 ± 15.1 years). Final pathology revealed 37 cases of enterobiasis, five cases of carcinoids, four mucinous cystadenomas, two eosinophilic infiltrations, two mucoceles, two tuberculosis, one goblet-cell carcinoid, and one neurogenic hyperplasia. While 52 patients underwent a standard appendectomy, two patients who were diagnosed with tuberculous appendicitis underwent a right hemicolectomy. All tumors were located at the distal part of the appendix with a mean diameter of 6.8 mm (range, 4-10 mm). All patients with tumors were alive and disease-free during a mean follow-up of 17.8 mo. A review of 1366 cases reported in the English literature is also discussed. CONCLUSION: Although unusual pathological findings are seldom seen during an appendectomy, all appendectomy specimens should be sent for routine histopathological examination.

评估Ahmed青光眼阀(Ahmed glaucoma valve,AGV)植入治疗难治性青光眼的长期疗效及并发症。方法:回顾性分析13例13眼接受Ahmed青光眼阀植入,最短随访18mo的难治性青光眼患者。所有患者均在术前及术后1,3,6mo,1a及1a以后行全面的眼科检查,眼压测量,记录并发症及抗青光眼的药物需求。结果:患者平均年龄27.3±16.0岁。所有患眼(100%)接受至少一次前置小切口手术。平均随访61.3±30.8mo。在少量药物的作用下,12mo时眼压由35.0±7.0mmHg降至18.2±7.9mmHg,96mo时降至17.0±4.1mmHg,76.9%患者需其他辅助治疗以达到预期的成功疗效。最常见的并发症包括包裹性囊肿8眼(61.5%),置管脱出4眼(30.8%)。结论:包裹性囊肿是最常见的并发症,阻碍了Ahmed青光眼阀植入治疗难治性青光眼。虽然囊肿被抗纤维化药物切除,但没有抗青光眼药物的治疗,眼压仍旧不能很好控制。

Major histocompatibility complex(MHC) genes,coding molecules which play an important role in immune response,are the most polymorphic genes known in *** the present study,MHC classⅡlocus DRB was investigated by cloning and sequence analysis in two local sheep breeds in Xinjiang,named Kalakuer sheep and Bashibai sheep.285bp length of exon 2 sequences was produced.58 haplotypes were found.35 haplotype were detected in Kalakuer sheep population,32 haplotype were detected in Bashibai sheep,sharing nine haplotypes. Sequences exhibited a high degree of nucleotide and amino acid *** frequency of haplotypes in Kalakuer sheep ranged from 0.033 to 0.3;the haplotype frequency of Bashibai sheep ranged from 0.029 to 0.206.A significantly higher rate of non-synonymous(d=0.079±0.015) than synonymous(d=0.033±0.010) substitutions indicated positive selection for diversity in the DRB *** on the sequencing,it was found that transitions and transversions rather than indel events commonly *** were more frequently than transitions although they both were *** selection pressure analysis,positive selection signal were detected in all sheep breeds studied.

We report here, a young male patients referred with ＂Obsessive Compulsive Disorder＂ symptoms which emerged after the successful treatment of pineal germinoma. OCD （obsessive-compulsive disorder） is a frequent, chronic, and clinically disorder which may presents in several neurologic disorders, especially occurs, in early adult life. Essential features of OCD are obsessional thoughts, compulsive acts as the ritualistic behavior, anxiety, and specific cognitive impairments. The cause of obsessive-compulsive disorder isn＇t fully understood. One of the many theories of the pathophysioiogy about to OCD is includes with hyperactivity in certain subcortical and cortical regions of brain also, dysfunction of the cortico-striatal circuits, particularly implicated in orbitofrontal cortices and basal ganglions. Additionally, pineal gland functioning is remarkable for the mental health disorders, particularly in OCD. On the basis of the investigations to present case report, we discussed the probable reasons of OCD symptoms, emphasizing the role of pathophysiology including the cortico subcortical pathways in genesis of the symptoms.

Influence of Soil Application of Micronized-Sulfur with Bentonite on Tomato Growth under Greenhouse Condition

In this study, the influence of four different sulfur products applied to the soil on the organically grown greenhouse tomato was investigated. The sulfur treatments were：（1） micronized-sulfur with bentonite （MSB） alone （90% sulfur ＋ 10% bentonite）; （2） MSB ＋ Fe （87% sulfur ＋ 10% bentonite ＋ 3% Fe）; （3） MSB ＋ Zn （87% sulfur ＋ 10% bentonite ＋ 3% Zn）; （4） MSB ＋ Fe ＋ Zn （86% sulfur ＋ 10% bentonite ＋ 2% Fe ＋ 2% Zn） and （5） control without sulfur treatment. The effects of sulfur products on plant growth, total crop yield, some fruit quality parameters, Fe and Zn content of the leaves and soil, pH and electrical conductivity （EC） of soil, were investigated. Results showed that the effects of treatments on plant growth parameters and fruit quality properties were not significant. However, total fruit yield increased by about 14%, 21%, 15% and 27% in MSB, MSB ＋ Fe, MSB ＋ Zn and MSB ＋ Fe ＋ Zn, respectively, as compared with control. Leaves Fe and Zn content were increased in some sampling times in related treatments. Soil Fe content was not changed by the treatments, however Zn content was significantly increased by the MSB ＋ Zn and MSB ＋ Fe ＋ Zn applications. Soil pH was decreased by the all sulfur treatments in comparison to control; however the most important pH decrease was recorded as 0.28 units in MSB ＋ Fe ＋ Zn treatment. Soil EC values of the all sulfur treatments were higher than the control.

In vitro effect of pantoprazole on lower esophageal sphincter tone in rats

AIM:To investigate the in vitro effects of pantoprazole on rat lower esophageal sphincter(LES)***:Rats weighing 250-300 g,provided by the Yeditepe University Experimental Research Center(Yü-DETAM),were used throughout the *** were anesthetized before *** tissues whose mucosal lining were removed were placed in a stan-dard 30-mL organ bath with a modif ied Krebs solution and continuously aerated with 95% oxygen-5% carbon dioxide gas mixture and kept at room *** tissues were allowed to stabilize for 60 ***-sequently,the contractile response to 10-6 mol/L carba-chol was *** concentrations of freshly prepared pantoprazole were added directly to the tis-sue bath to generate cumulative concentrations of 5×10-6 mol/L,5×10-5 mol/L,and 1.5×10-4 mol/***-ties were recorded on an online computer via a 4-channeltransducer data acquisition system using the software BSL PRO v 3.7,which also analyzed the ***:Pantoprazole at 5×10-6 mol/L caused a small,but statistically insignif icant,relaxation in the car-bachol-contracted LES(2.23% vs 3.95%).The 5×10-5 mol/L concentration,however,caused a signif icant relax-ation of 10.47% compared with the control.1.5×10-4 mol/L concentration of pantoprazol caused a 19.89% relaxation in the carbachol contracted LES(P<0.001).CONCLUSION:This is the fi rst study to demonstrate that pantoprazole has a relaxing effect in isolated *** results might have signif icant clinical implications for the subset of patients using proton pump inhibitors who do not receive full symptomatic alleviation from gastroesophageal reflux disease.

Thiol/disulphide homeostasis in celiac disease

AIM To determine dynamic thiol/disulphide homeostasis in celiac disease and to examine the associate with celiac autoantibodies and gluten-free *** Seventy three patients with celiac disease and 73 healthy volunteers were enrolled in the study. In both groups, thiol/disulphide homeostasis was examined with a new colorimetric method recently developed by Erel and Neselioglu. RESULTS In patients with celiac disease, native thiol(P = 0.027) and total thiol(P = 0.031) levels were lower, while disulphide(P < 0.001) level, disulphide/native thiol(P < 0.001) and disulphide/total thiol(P < 0.001) ratios were higher compared to the control group. In patients who do not comply with a gluten-free diet, disulphide/native thiol ratio was found higher compared to the patients who comply with the diet(P < 0.001). In patients withany autoantibody-positive, disulphide/native thiol ratio was observed higher compared to the patients with autoantibody-negative(P < 0.05). It is found that there is a negative correlation between celiac autoantibodies, and native thiol, total thiol levels and native thiol/total thiol ratio, while a positive correlation is observed between disulphide, disulphide/native thiol and disulphide/total thiol *** This study is first in the literature which found that the patients with celiac disease the dynamic thiol/disulphide balance shifts through disulphide form compared to the control group.
