The negative effects of natural disasters on human life exist from the foot and did not occur at a specific time but found since the creation of mankind. Humans coexist with extreme events all the time, only when the intensity of the event becomes greater than a certain level there is a resulting disaster. Small earthquakes occur all of the time with no adverse effects. Only large earthquakes cause disasters. Statistical analysis reveals that larger events occur less frequently than small events. Through the large number of seismic events, we find that at the end of the year may have a series of seismic events with different values depending on the strength of activity whether it is high or low on Richter scale and the assessment is only for the greatest value in a year even if recurring this value and the volume of dangerous increases and the frequency of their occurrence according to an ongoing activity, major disasters result from a small number of events and sustained results in a large and devastating event, and can be represented by these results and amounts On a log-scale which points are almost on a straight line and a clear indication of the evaluation event. Through previous data analysis we can understand the following events behavior for coordination and guidance on the development of evacuation plans on the expected future and use a Weibull equation to estimate the frequency of the event and the return again as a percentage for each event and the probability of the occurrence of a particular earthquake to some degree on the Richter scale in the sea during any period. Past records of earthquakes at the West Coast of the Kingdom of Saudi Arabia (Red Sea) for years 1913-2016 are used to predict future conditions concerning the annual frequency, the return period, the percentage probability for each event, and the probability of a certain-magnitude earthquake occurring in the region during any period.

Mitral Valve Commissurotomy: Which One between the Three Techniques Gives a Better Long-Team Outcome?

Objective: we sought to compare long-term results of three techniques: CMC, OMC and PMC in patients with rheumatic mitral stenosis. Patients and Method: Between January 1994 and December 2015, 183 patients underwent mitral valve surgery for rheumatic mitral restenosis. All patients were investigated by echocardiography-Doppler performed by a senior cardiologist. The patients were divided into 3 groups: patients who have previously closed mitral commissurotomy (CMC n = 101), patients with previously open mitral commissurotomy (OMC n = 28) and those treated by Balloon mitral valvuloplasty (PMC = 54). Results: The three groups were comparable in term of major demographic data. Mitral restenosis occurred precociously in groups treated by PMC (7 ± 4 years), followed by group with OMC 11.4 ± 4 years and CMC group but it occurred later CMC 16.8 ± 7.8 years (p Conclusion: CMC produces better long-term outcome than OMC and PMC. However, it would be premature to conclude to its superiority.

An Empirical Study on Aflatoxin Occurrence in Nuts Consumed in Tehran, Iran 2013

By definition, “aflaxions” refer to a group of chemically toxic fungal metabolites, which are generated by specific species of genus Aspergillus. The species flourish on some of raw foods. This research is an attempt to assess aflatoxin contamination in nuts (Fig, Almond, Hazelnut, Walnut, Pistachio and Sunflower) available in Tehran city market in 2013. The assessments were done using ELISA method. To this end, 200 samples including Fig (n = 30), Almond (n = 25), Hazelnut (n = 25), Walnut (n = 40), Pistachio (n = 40), Sunflower (n = 40) were collected and analyzed regarding contamination with aflatoxin. The result of the tests was positive for 96.5% (193 samples) with total average concentration of 1.68 μg/kg (0 - 6 μg/kg). Moreover, in 6% (12 samples) of the cases, contamination exceeded the EU limits (4 μg/kg), while 100% of the samples met the legal limits of National Standard of Iran (15 μg/kg). In spite of contamination of majority of the cases, none was at hazardous level as the condition for growth of mycotoxin fungus or antioxidant activity in the nut was not suitable in Tehran (humidity level).

Impact of glucotoxicity induced in vivoand in vitroin Psammomys obesus

Objective: Chronic hyperglycemia characteristic of type diabetes 2 is responsible for the accelerated atherosclerosis with increased cardiovascular risk. In this study, we will propose to analyze the effect of a long-term of glucotoxicity in vivo in Psammomys obesus by addition of sucrose to 30% for 11 months and in vitro study of adventitial fibroblasts in the presence of D-glucose 0.6% for 7 days. Materials and methods: Evaluation of plasma biochemical parameters was carried out at the initial time and at the end of experiment. At autopsy, a morphological study of the aorta was performed after fixation in aqueous Bouin and staining with Masson’s trichrome. The experimental glucotoxicity is induced by incubation of fibroblasts in DMEM enriched with D-glucose at 0.6% for 7 days. The impact of glucotoxicity is assessed in the intracellular compartments through dosage of total nitrite and malondialdehyde, a product of lipid peroxidation, and thanks to a morphological assay after fixation of cells with aqueous bouin and blood staining with May Grünwald Giemsa. The evaluation of cell proliferation is accomplished by cell counting. Collagens I and III of the extracellular compartment are characterized by SDS-PAGE. Results: Animals subjected to sucrose showed hyperglycemia associated with hyperinsulinemia, dyslipidemia, hyperproteinemia, increased CPK and VLDL-LDL and decreased HDL. Histology of aortas revealed endothelial cells hypertrophy, severe disorganization of intima and media. In the presence of glucose, the proliferation of fibroblasts increases very significantly (P = 2.34 × 10-5), the rate of malonaldehyde, nitrite and total density of chains α2 (I) and α1 (I + III) extra-cellular collagens I and III increased significantly. After staining, the cells showed hypertrophy, vacuolation of cytoplasm and chromatin condensation with nuclear fragmentation, indicative of apoptosis. Background: The aim of this study was to evaluate the impact of additional coronary revascularization on the early results in patients submitted to valve surgery. Patients and Methods: A retrospective review of the cardiac surgical database between January 2000 and December 2018 was performed. A total of 1667 patients were included and divided into two groups: Group A isolated valve surgery (IVS n = 1608) and Group B with valve surgery combined to coronary artery bypass grafting (VS + CABG n = 59). Demographic, operative data and postoperative outcomes were compared between groups. Results: Patients with combined procedure were older than patients who underwent isolated valvular surgery (64.9 ± 9.2 years vs 44.4 ± 13.1 years;p = 0.0001) and there was a higher proportion of diabetics (40.7% vs 6.6%;p = 0.0001). The 30 days mortality rate in the combined procedure group was 18.6% versus 6.2% in isolated valve surgery (p = 0.001). Also post-operative complications were more frequent than for patients who underwent IVS. Additionally we noted a high prevalence of coronary artery risk factors in patients with combined procedures. Conclusion: Surgical mortality and morbidity of coexisting coronary and heart valve disease were substantially higher than IVS. More efforts in medical management may reduce the incidence of adverse outcomes.

Does Obesity Affect Early Results after Coronary Artery Bypass Grafting?

Background: The aim of this study was to quantify the effect of obesity on early results after coronary artery bypass grafting (CABG). Methods: A retrospective cohort study of patients underwent isolated CABG from January 2000 through December 2012 was conducted. 586 patients were classified into two groups: Obese (n = 100) BMI ≥ 30 kg/m2 and non-obese (n = 486) BMI ≤ 30kg/m2. Results: The obese patients included more women (p 0.01), hypertensives (p = 0.01) and dyslipidemics (p = 0001). The CPB, aortic champ times and number of bypass graft were similar between the groups (p = 0.35, p = 0.51 and p = 0.59 respectively). Also the composite of in-hospital mortality and postoperative complication didn’t differ between the groups. The incidence of perioperative myocardial infarction, and need for inotropic drugs or IABP were significantly less in obese patients (p = 0.028, p = 0.031 and p 0.01 respectively). Conclusions: The current study showed that obesity is not a risk factor of adverse events after CABG and continuous to give another aspect of the “obesity paradox”.

Langerhans cell histiocytosis is a rare disease involving clonal proliferation of langerhans cells seen in children and young adults. Clinical presentation is variable, ranging from a single location in the bone to severe multivisceral involvement. Moreover, spinal involvement causing myelopathy is even rare and unusual. We report a rare case of adult Langerhans cell histiocytosis in the dorsal spine causing a spinal cord compression associated with a pulmonary process treated by surgery, radiotherapy and systemic therapy with good evolution.
